1.Anatomical basis and clinical application of "two points" acupotomology surgery program in treating cubital tunnel syndrome.

OBJECTIVETo explore an effective acupotomology surgery program in treating cubital tunnel syndrome.
METHODSAccording to pathogenic factors and elbow anatomy, a "two points" acupotomology surgery program was designed, which could loose the attachment point of arcuate ligament on medial border of olecroanon and medial epicondyle of humerus. Twenty-one cases of cubital tunnel syndrome were treated with acupotmology, then the efficacy was obsered.
RESULTSAfter one year postoperative visit, 21 patients with ulnar nerve area skin numbness were cured, claw hand deformity and medial hand muscle atrophy recovered significantly. Results of function evaluation were excellent in 17 cases, good in 2 cases, fair in 2 cases and poor in 0 cases, the good rate was 90.5%.
CONCLUSIONThe acupotomology surgery program which could cut the starting and ending points of osborne's ligament and solve the problem of ulnar nerve entrapment is an easy, little-traumatic and effective minimally invasive surgery which also conforms to the anatomical structure.

2.Role of organoid in prostate cancer research

As a new model of pre-cancer, organoid is essential for the basic understanding of tumor characteristic and effective tumor treatment. Organoids derived from prostate play an especially important role in the research of fundamental oncology and anticancer drug screen against prostate cancer. Prostate cancer cell lines and xenografts derived directly from primary human tumors are widely used now as models to study prostate cancer and have proven very valuable. But there are some caveats and shortcomes of these two models that have to be accounted for. Here we outline organoid as a third preclini?cal cancer model which may potentially overcome the shortcomes of cancer cell lines and PDTX. This article aims to summa?rizee recent progress of the role of organoid in prostate cancer research.
3.Imaging features of anterior cruciate ligament reconstruction graft insufficiency

ObjectiveTo investigate the imaging features of anterior cruciate ligament(ACL) graft insufficiency.Methods X-Ray and MR imaging examinations in 24 consecutive patients who had ACL reconstructive graft insufficiency were retrospectively evaluated for tunnel position,osteoarthrosis and its related complications.Follow-up arthroscopy showed 16 graft tears and 8 graft laxities.Fisher exact test was used to compare tunnel malpositions,the proportion of graft tear on MRI and osteoarthrosis between graft tear group and graft laxity group.ResultsTwo malpositions of tibial tunnel and 3 malpositions of femoral tunnel were seen in graft tear group.Three-malpositions of tibial tunnel and 4 malpositions of femoral tunnel were seen in graft laxity group.The proportion of tibial or femoral malposition showed no significant difference between the two groups(P =0.289,P =0.167).In graft tear group,15 complete graft tears were diagnosed correctly,1 partial tear was misdiagnosed as normal on MRI.In graft laxity group,4 grafts were diagnosed as normal and 4 were considered as graft tear on MRI.A significant difference was seen between the two groups (P = 0.028) in the proportion of graft tear diagnosed on MRI.Fourteen osteoarthrosis were seen in graft tear group and 5 in graft laxity group.No significant difference was seen between the two groups ( P =0.289) in the proportion of esteoarthrosis.Conclusion The proportions of tunnel malposition and osteoarthrosis showed no significant difference between the graft tear group and graft laxity group.Most graft tears can be diagnosed accurately on MRI,but some cases of graft laxity may be misdiagnosed for graft tear.
4.High-mobility group protein B1 (HMGB1) and its potential in diagnosis and treatment of ovarian cancer

Objective:The objective of this research is to study the serum level of the high-mobility group protein B1 (HMGB1) in human ovarian tumor (OvCa) and in a healthy control. This study also aims to identify different HMGB1 levels before and after sur-gery and to explore the inhibitory effect of HMGB1 gene silencing in the proliferation and invasion ability of OvCa. Methods: En-zyme-linked immunosorbent assay was used to measure the serum level of HMGB1 in OvCa patients and healthy subjects. Lentivirus vector with HMGB1 shRNA was constructed and used to infect OvCa cells. The expressions of HMGB1 mRNA and protein were test-ed by real-time PCR and Western blot. Cell proliferation was detected using the Cell Counting Kit-8 assay, whereas cell invasion and migration were detected by Transwell assay. Results:The serum level of HMGB1 was more elevated in patients with malignant diseas-es compared with individuals with benign diseases and the control groups. In the malignant group, the serum level of HMGB1 de-creased noticeably after therapy. Down-regulation of HMGB1 expression resulted in the inhibition of the biological behavior and metas-tasis of ovarian cancer cells. Conclusion: HMGB1 is closely associated with clinicopathologic features of OvCa. Knockdown of HMGB1 expression can significantly inhibit cell proliferation, cell migration, and cell invasion of OvCa. These findings indicate that HMGB1 can function as a therapeutic target for ovarian neoplasm in the future.
5.Immuno-suppression and mechanism of CD4+CD25+T cells in ascites of ovarian cancer patients

Objective: This research explores the relationship between the immuno-suppression function of regulatory T cells (Treg) in the ascites of ovarian cancer (OC) patients, the clinico-pathologic features of these patients, and the correlation of the function of Treg with initial treatment and relapse status of the patients to further investigate the specific mechanism of immuno-regulatory func-tion of CD4+ CD25+ Treg in the ascites of OC. Methods: Immuno-magnetic activated cell sorting (MACS) was conducted to sort CD4+CD25+Treg and autologous CD4+CD25-Treg from the ascites of 28 OC patients. Carboxyfluorescein-diacetate succinimidyl ester (CFSE) was used to label the autologous CD4+CD25-Treg. These labeled cells were then used as controls and co-cultured with autologous CD4+CD25+Treg at the ratio of 1∶1 or 1∶2. The mean inhibition ratio of Treg in specimens to the proliferation of autolo-gous CD4+ CD25-Treg was calculated after the flow cytometry of the CFSE expression and Modfit software analysis of the CD4+CD25-Treg proliferation index (PI) were performed. Anti-IL-10 and/or anti-TGF-β1 antibodies were neutralized to investigate whether the CD4+CD25+Treg-mediated immuno-suppression escaped through the ascites can produce a marked effect by the inhibitory cyto-kine IL-10 or TGF-β1. Results: The mean inhibition ratio of CD4+ CD25- Treg in the ascites of stage Ⅲ to Ⅳ OC patients was (75.72±17.04)%, which is significantly higher than that of stageⅠtoⅡOC patients (59.61±16.97)%;P<0.05. In addition, Treg in the as-cites of OC patients with recurrent disease showed a significantly higher inhibition ratio than that of patients with primary disease;P<0.001. Moreover, Treg in groups added into neutralizing anti-IL-10 and/or anti-TGF-β1 antibodies displayed significantly lower depres-sant effect than the control group;P<0.05. Conclusion:The immuno-suppression of CD4+CD25+Treg in the ascites of OC patients is correlated with the tumor staging and status of the primary or recurrent diseases. Moreover, Treg may indicate a suppressor function by secreting cytokine IL-10 and TGF-β1.
6.Frequency and suppressor function of CD4+CD25+T cells in ascites and blood of patients with ovarian cancer

Objective:CD4+CD25+regulatory T cells (Treg) may contribute to tumor progression by suppressing antitumor im-munity. The function of Treg in antitumor immunity regulation in the peritoneal microenvironment of ovarian cancer (OC) was investi-gated and compared with the circulating Treg to elucidate OC immune escape. Methods: Flow cytometry was used to determine the proportion of CD4+CD25+T cells in CD4+T cells in ascites of 27 patients with OC and in peripheral blood lymphocytes of 28 patients with OC. The samples were analyzed and classified in three stages:primary disease (PD), after chemotherapy (AC), and recurrence dis-ease (RD), according to the clinical conditions of the OC patients upon donating the samples. The percentage of Treg in the three groups was determined in ascites and blood. CD4+CD25+T cells were isolated from ascites and peripheral blood of patients with OC us-ing magnetic sorting (MACS) system. The cells were then tested for regulatory function through coculture with carboxyfluorescein diac-etate succinimidyl ester-labeled autologous CD4+ CD25- responder cells. Results:The proportion of CD4+ CD25+T cells in CD4+T cells significantly increased in ascites (28.25%± 14.06%) compared with that in blood (14.6%± 4.74%;P<0.0001). The Treg in ascites and blood in AC showed higher proportion (P<0.0001) than those in the PD and RD;the proportion in RD was higher than that in PD (P<0.0001). Moreover, the Treg in ascites mediated a significantly higher suppression compared with the Treg in peripheral blood (P<0.001). Conclusion:The frequency and suppressor function of Treg were significantly higher in ascites than in peripheral blood. This finding suggests more possibility for escape immune surveillance in the peritoneal microenvironment. Moreover, the proportion of Treg in AC was higher than that in PD or RD;the proportion in RD was higher than that in the PD. Chemotherapy may favor the expansion of Treg, which may promote the recurrence of cancer.
7.Correlative analysis between plasma BNP,CRP levels and computed tomographic manifestations in acute moderate or severe traumatic brain injury

Objective To investigate correlative analysis between plasma BNP,CRP levels and computed tomographic manifestations in acute moderate or severe traumatic brain injury.Methods According to the severe traumatic brain injury and mild traumatic brain injury criteria that onset within 24h,patients were devided into the observation group A (73 cases with moderate or severe traumatic brain injury)and observation group B(20 cases with mild traumatic brain injury ).All patients received CT scan,CT image characteristics and Rotterdam CT grading were recorded.,Plasma BNP and CRP levels were detected at the same time,healthy persons with matched age and gender within same period were selected (control group),relationships of plasma BNP and CRP levels with CT manifestations were analyzed.Results Plasma BNP and CRP levels of the observation group were obviously higher than those of the control group (P <0.05).In patients with midline shift >5mm,ventricular pressure≤0.2,Rotterdam CT score >3, the BNP and CRP levels were higher than the corresponding patnents (P <0.05).Patients with high levels of BNP and CRP showed higher midline shift score,Rotterdam CT score and the higher incidence of subarachnoid hemorrhage (P <0.05),and ventricle compression ratio and GSC score less than the low level group (P <0.05 ).Spearman correlation analysis showed that Rotterdam CT score was positively correlated with and plasma BNP and CRP levels (P <0.05),multi -factor Logistic regression analysis showed that plasma BNP levels (OR =1.873,P <0.05),the CRP (OR =1.568,P <0.05)were an independent predictor of Rotterdam CT score >3 points.Conclusion Plasma BNP and CRP levels of patients with acute moderate or severe traumatic brain injury are closely related to various characteristics of CT,and are independent predictors of Rotterdam CT scores,they can be used as a biological marker assisting to evaluate degree of severe craniocerebral injury patients.
8.Current update on combined hepatocellular and cholangiocarcinoma

Combined hepatocellular and cholangiocarcinoma (HCC-CC) is a rare primary hepatic neoplasm (PHN) with features of both hepatocellular and biliary differentiation. Hepatitis B and hepatitis C are the major causes of HCC-CC. Surgical treatment is the main therapeutic method for HCC-CC. For patients with unresectable lesions, curative or palliative locoregional therapy is applied, including radiofrequency ablation (RFA), transarterial chemoembolization (TACE) and supportive treatment. Because of the rare occurrence and ambiguous clinical features of HCC-CC, it is most often misdiagnosed with the other two types of primary liver cancer. Thus, the realization of the current progress of combined hepatocellular and cholangiocarcinoma is particularly important for us. This article aims to summarize the epidemiology and clinical futures, the treatment and prognosis, the progress of genetics and molecular analysis of HCC-CC.
9.Role of Runx2 in Breast Cancer

10.Three cases of orthotopic heart transplantation surviving more than 10 years

Objective To summarize the experience of long-term survival of the patients under- going orthotopic heart transplantation.Methods Heart transplantation was performed on 2 cases of dilated cardiomyopathy and one case of Keshan disease.Before operation,pulmonary artery pressure was 42-53 mm Hg(5.60-7.07 kPa)and pulmonary vascular resistance 5.6-7.0 wood.The body weight difference between donors and receptors was 10%-15%.There were three same antigens in HLA zygosity experiment for all of three patients.Two cases were subjected to standard heart trans- plantation and one case to whole heart transplantation.All the atriums and big vessels were sutured by evting suture method.Cyclosporin A,azathioprine and corticosteroid were used to prevent patients from rejection.Results Survival time of 3 patients was 13 years and 10 months,12 years and 10 years and 3 months.Heart functions of three patients were NYHAⅠand all of 3 patients are living and working commonly.Six,3 and 1 rejection(s)occurred in 3 patients respectively and cured by appro- priate treatment.Electrocardiogram revealed that case 1 and case 2 had two P waves and case 3 sinus rhythm.Ultrasonic cardiogram showed that in case 1 and case 2,the left and right atriums were enlarged and tricuspid valve had slight backstreaming,and in case 3,all of the cardiac chambers were normal and had no backstreaming of tricuspid valve and mitral valve.No abnormal findings were found in 3 cases by 4-9 times of coronary arteriongraphy.Conclusion The important factors for the patients' long-term survival after heart transplantation include the choice of appropriate donors and acceptors, protection of donors' hearts,selection of appropriate operations and suture methods,rational use of im- munosuppressants and prevention of cardiac allograft vasculopathy.
