Objective To investigate the relationship between flow-mediated dilatation of the brachial artery(FMD) and cardiovascular risk factors as well as target organ damage in elder patients with isolated systolic hypertension(ISH).Methods Ninety patients with ISH were divided into two groups: ISH group(n=51) and ISH with hyperlipidemia group(n=39).Color Doppler ultrasound for FMD was performed on all the patients,and carotid artery ultrasound and cardiac ultrasound were also conducted. This study was aimed to evaluate the efficacy of Tiao-Bu Fei-Shen therapies (i.e., Bu-Fei Jian-Pi, Bu-Fei Y i-Shen, Y i-Qi Zi-Shen) on cardiac remodeling of chronic obstructive pulmonary disease (COPD) rats and its mechanisms according to the R-value comprehensive evaluation method. Based on the database of previous experiment of COPD rats, R-value comprehensive evaluation method was used to evaluate the indexes as fellows to discuss efficacy of Tiao-Bu Fei-Shen therapies on cardiac remodeling of COPD rats. ① Indicators of right ventricular morphologic indexes: right ventricular hypertrophy index (RVHI), cardiac muscle sarcomere lengths, bulk density of myocardial mitochondria (Vv), surface area (δ), membrane surface (δm), Vv, δ, δm of heart mitochondria;② Indicators of mechanisms: right ventricular endothelin-1 (ET-1), transforming growth factor-beta ( TGF-β) , vascular endothelial growth factor ( VEGF ) , basic fibroblast growth factor ( bFGF ) , matrix metalloproteinase-9 (MMP-9) and tissue inhibitor of metalloproteinase-1 (TIMP-1). The results showed that the sequence of improving effect of right ventricular remodeling at week 20 was Bu-Fei Jian-Pi, Bu-Fei Y i-Shen, aminophylline, and Y i-Qi Zi-Shen; at week 32 and the integrated week 20 and week 32, the sequence of effect was Bu-Fei Jian-Pi, Bu-Fei Y i-Shen, Y i-Qi Zi-Shen, and aminophylline. At integrated week 20 and week 32, Bu-Fei Jian-Pi had significant better intensity correcting effect than aminophylline (P< 0.01). There was no difference between week 20 and week 32. It showed that each treatment group had good long-term effect. For the mechanism of correcting effect on right ventricular remodeling, at week 20, the sequence of comprehensive effect was Bu-Fei Jian-Pi, Bu-Fei Y i-Shen, aminophylline, and Y i-Qi Zi-Shen. And Bu-Fei Jian-Pi had better effect compared with aminophylline and Y i-Qi Zi-Shen (P< 0.01); Bu-Fei Y i-Shen had better effect than Y i-Qi Zi-Shen (P< 0.05). At week 32, the sequence of effect was Y i-Qi Zi-Shen, aminophylline, Bu-Fei Jian-Pi, and Bu-Fei Y i-Shen. At the integrated week 20 and week 32, the sequence of effect was Bu-Fei Jian-Pi, and Bu-Fei Y i-Shen, Y i-Qi Zi-Shen, and aminophylline. Until week 32, the correcting effect of Tiao-Bu Fei-Shen therapies and aminophylline still maintained the same level as at week 20. It indicated that each treatment plan had good long-term effect. It was concluded that Tiao-Bu Fei-Shen therapies can improve the cardiac remodeling of COPD rats and expression of related factors in the cardiac remodeling through the R-value comprehensive evaluation method. And the effect of Bu-Fei Jian-Pi was obvious with good long-term effect.

4.Role of optical coherence tomography angiography in myopic choroidal neovascularization after intravitreal injections of Ranibizumab

AIM: To investigate the change of myopic choroidal neovascularization treated by ranibizumab and evaluate their value in monitoring the effect of anti-vascular endothelial growth factor ( VEGF) therapy. ·METHODS: The study enrolled 30 patients ( 30 eyes ) diagnosed with myopic choroidal neovascularization. All affected eyes were treated with intravitreal ranibizumab 0. 05mL ( 10mg/mL ). Best corrected visual acuity ( BCVA ) , non-contact tonometer, ophthalmoscope, fundus fluorescein angiograph ( FFA ) and OCTA were evaluated monthly until 6mo. The changes of BCVA and central macular thickness ( CMT) were compared at 1, 3 and 6mo after treatment. ·RESULTS:All patients received an average of 1. 70±0. 65 injections. BCVA was 0. 96 ± 0. 17 ( LogMAR ) before therapy, and BCVA 1, 3 and 6mo after treatment respectively improved by 0. 23 ± 0. 09, 0. 34 ± 0. 07, 0. 38 ± 0. 11. The differences were significant ( t=5. 461, 8. 191, 8. 894; P<0. 05 ). Mean CMT decreased form 281. 07 ± 13. 72μm to 261. 33 ± 13. 13μm, 243. 47 ± 16. 65μm, 234. 73 ± 17. 52μm respectively 1, 3 and 6mo after treatment, showing significant differences (t=12. 007, 13. 360, 9. 531;P<0. 05). OCTA revealed a progressively smaller vascular lesion and reduction in capillary density. · CONCLUSION: Intravitreal ranibizumab for CNV secondary to pathologic myopia is effective and safe;OCTA is a noninvasive and time-saving new technology, and it also is a promising tool for clinicians to make preliminary diagnosis and assess treatment efficacy in the follow-up visits.
5.Assessment of the left thoracic artery graft late postoperative patency by ultrasound contrast with angiography

Objective To access the left internal thoracic artery (LITA) graft late postoperative patency after coronary artery bypass grafting (CABG) by peripheral blood vessel ultrasound combined with color Doppler coronary flow imaging (CDCFI).In contrast with angiography,try to find available flow parameter to access graft patency.MethodsForty-six patients with CABG more than 1 year postoperatively followed-by angiography were detected by ultrasound.The LITA graft and left anterior descending artery were examined.Systolic and diastolic peak velocity(Smax,Dmax),velocity time integral(VTIs,VTId)of each segment were measured separately.The ratio of diastolic and systolic peak velocity (D/S),and diastolic velocity time integral fraction(DVTIF) were calculated.All patients were divided into groups according to angiography results.ResultsThirty -one LITA grafts were patent,11 were dysfunctional,4 were occlusive.According to the angiography results,the flow parameters of the proximal segment of LITA graft were significant.The D/S and DVTIF of patent group was higher than that of dysfunctional group.The diastolic peak velocity of distal segment of LAD of patent group was higher than that of dysfunctional group.ConclusionsPeripheral blood vessel ultrasound combined with CDCFI could provide the evidence to access the patency of the graft.It was an effective method for the clinical follow-up.
6.Transfection of rat myoblasts with leuflvirus carrying autocrine motility factor gene

Objective To explore a safe and high efficiency way of gene transfection of autocrine motility factor(AMF) in order to provide experimental basis for transplantation of myoblasts carrying AMF gone. Methods Sprague Dawley rat myoblasts were cultured, purified, proliferated and immunohisto-chemically verified. Then, the myoblasts were transfected with AMF and eGFP (enhanced green fluores-cent protein) gene by FIV (feline immunodeficiency virus). Fluorescence microscope and laser scanning confocal microscope were employed to detect eGFP so as to verify positive transfection rate. Expression of AMF was detected by immunohistochemical method. Results Myoblasts with 98% purity could he ob-tained after two weeks of primary culture and purification. Positive transfection rate reached 90.4% when MOI (multiplicity of infection) was 100 (P <0.01). The transfected AMF gene could express normally. Conclusions Explant culture is a proper way in rat myoblast culture. Meanwhile, AMF gene can he effectively transfected into rat myoblast and well expressed via FIV.

Information System) according to first management unit, and the laboratory test results of sputum smear, sputum culture and molecular tests were collected to statistically analyzed the positive rate of etiology, sputum smear, sputum culture, molecular
biology testing rate, and indicators of positive testing rate of tuberculosis patients. Results From 2016 to 2020, the etiology�positive rate of tuberculosis in Shaanxi province were 13.49% (2 664/19 754), 22.68% (5 081/22 401), 35.99% (8 232/22 876),
48.14% (10 438 / 21 682), 52.65% (9 332 / 17 724), respectively, with an increasing trend (χ2
trend=9 473.12, P<0.001) year by
year; the proportion of molecular tests positive only in etiology-positive pulmonary tuberculosis (PTB) were 0 (0/2 664), 0.16%
(8/5 081), 15.44% (1 271/8 232), 27.58% (2 879/10 438), 31.52% (2 941/9 332), respectively, with an increasing trend year by
year (χ2
trend=2 971.44, P<0.001); the molecular test rates of the 5 years were 0.01% (2 / 19 754), 0.38%(85 / 22 401), 21.11%
(4 828/22 876), 52.42%(11 365/21 682), 55.18%(9 780/17 724), respectively, with an increasing trend year by year (χ2

28 269.23, P<0.001). The rate of molecular test in sputum smear-negative was 22.72%(17 976 / 79 130). The proportion of
patients with only molecular test-positive was 33.43% (4 032/12 062) in municipal designated hospitals, and 11.99%(2 279/
19 014) in county-level designated hospitals, the difference was statistically significant (χ2
=2 096.46, P<0.001).
Conclusions The rate of molecular biology testing in Shaanxi Province from 2016 to 2020 showed a year-on-year increase.
Through the application of rapid molecular tests, the etiology-positive rates of tuberculosis have been increased significantly,but the current molecular test detection rate is not high compared with other provinces, especially in county-level designated
hospitals and smear-negative patients, so we should make a big promotion in application of rapid molecular technique.
9. Urolithin A activates autophagy to improve liver insulin resistance in diabetic mice

Objective: To investigate the effect of urolithin A (UA) on liver insulin signaling pathway in type 2 diabetes model mice and its relationship with autophagy. Methods: C57BL/6 mice were randomly divided into four groups according to body weight, namely control group, model group, UA (50 mg/kg) group, UA (50 mg/kg) combined with chloroquine (50 mg/kg) group. After 6 weeks of high-fat diet, a type 2 diabetes model was established by ip streptozotocin (STZ). The mice in each group were ig administrated for 7 weeks, and their body weight, water intake, blood lipids, fasting blood glucose (FBG), and fasting insulin (FINS) levels were measured; HE staining was used to observe pathological changes in mouse liver tissue; Western blotting was used to detect mouse phosphorylated protein kinase B (p-Akt), glucose transporter 2 (Glut2), phosphorylated glycogen synthase kinase-3β(p-GSK3β) and autophagy-related protein microtubule-related protein 1 light chain 3 II/I (LC3II/I) and selective autophagy linker protein (p62) expression levels. Results: Compared with the model group, UA significantly improved liver tissue steatosis and edema in diabetic model mice, significantly reduced plasma triacylglycerol, free fatty acids, low-density lipoprotein-cholesterol, FBG, FINS levels, and increased high-density lipoprotein-cholesterol level (P < 0.01); UA significantly reduced HOMA-IR and increased ISI (P < 0.01), up-regulated the protein expression of p-Akt, Glut2, p-GSK3β, and LC3II/I in liver tissues, and inhibited the expression of p62 protein (P < 0.01). Combined with chloroquine, FBG, FINS, and HOMA-IR in mice were increased, and ISI was decreased (P < 0.05); Liver tissue edema and steatosis were significantly aggravated; The expression levels of p-Akt, Glut2, LC3II/I protein in liver tissue were decreased, and p62 protein expression levels were increased (P < 0.05), indicating that the autophagy inhibitor chloroquine significantly weakened the effect of UA. Conclusion: UA may improve liver insulin resistance in diabetic mice by activating liver autophagy.
