1.A New Modified FOLFIRINOX(mFOLFIRINOX)Regimen as First-Line Chemotherapy for Patient with Metastatic Pancreatic Adenocarcinoma(MPC)

[Objective]To investigate efficacy and toxicity of a new modified FOLFIRINOX regimen(mFOLFIRI-NOX)as first-line chemotherapy for the patients with metastatic pancreatic adenocarcinoma(MPC).[Methods]20 patients with metastatic pancreatic adenocarcinoma(MPC)accepted mFOLFIRINOX arm(oxaliplatin 60 mg/m2,irinotecan 150 mg/m2,bolus of 5-FU 400 mg/m2,continuous infusion of 5-FU 2 400 mg/m2)first-line treatment and evaluated the curative effect and side effect after 3 cycles by imaging and laboratory at Sun Yat-sen Memorial Hospital from December 2012 to December 2016.The primary endpoint was overall survival(OS).The second endpoint was response rate(ORR) and toxicity.[Results]Four patients were excluded according to exclusion criteria. A total of 16 patients were enrolled including 7 males and 9 females.The median age is 55 years(ranging from 43 to 67 years).The median cycle of chemo-therapy was 4.5(3-15). The ORR was 31.3% including 5 patients with partial response,9 patients with stable disease and 2 patients with progression disease.The median follow-up duration was 14.1(2.7-24.5)months.The median OS was 16.6 months(95%CI,11.36-21.84). Overall survival rates at 6,12months were 80%and 55.6%,respectively. Three patients were presented grade 3-4 adverse events.[Conclusion]The new mFOLFIRINOX regimen improved survival of patients with MPC with tolerated toxicity as first line treatment.
2.Origin of acoustically evoked short latency negative response in guinea pigs

Objective To establish a model of acoustically evoked short latency negative response (ASNR) in guinea pigs, a model of profound hearing loss with normal saccular functions, and verify the correlation between ASNR and vestibular evoked myogenic potential (VEMP). Methods Thirty-two healthy guinea pigs were employed in the experiment, which were randomly divided into control group ( 16subjects) and deafened group (16 subjects). Each animal experienced auditory and vestibular tests including auditory brainstem response ( ABR), VEMP and caloric test. A quick treatment was employed for deafened group consisting of a subcutaneous injection of kanamycin at a dose of 400 mg/kg followed by a jugular vein injection of ethacrynic acid at a dose of 40 mg/kg one hour later. The animals were received ABR, VEMP and caloric test 7 - 10 days following the drug administration. The deafened group was further divided into ASNR group and non-ASNR group, based on the presence of ASNR. Results In deafened group, five subjects died postoperatively, 11 subjects (22 ears) provided full data, ASNR was elicited in eight ears (36.4%), the threshold was 120- 130 dB SPL with mean of (124.4 ±4.96) dB SPL. Its latency range was 1.75 - 2. 60 ms with mean of ( 2. 15 ± 0. 27 ) ms. The mean latency of threshold was (2. 34 ±0. 18) ms. All eight ASNR ears presented with VEMP. The VEMP threshold, positive and negative potential latencies proved no statistical difference (P > 0. 05 ) between ASNR group and control group.Significant difference was detected between the VEMP presence of ASNR group and non-ASNR group ( P =0. 002). There was no statistically significant correlation between VEMP and caloric test neither between ASNR and caloric test in deafened group. Conclusions This study evoked ASNR in an ototoxicity guinea pig model which has profound hearing loss with normal saccular functions. The presence of ASNR correlated with VEMP, however, not correlated with caloric test, suggesting that ASNR and VEMP are both originated from the saccule.
3.Inner ear morphological study of guinea pigs with acoustically evoked short latency negative response

Objective To establish a model of ototoxicity in guinea pigs with acoustically evoked short latency negative response (ASNR) and verify the responsible organ of ASNR based on microscopic characteristics of basal membranes,saccules,utricles and ampulla canalis semicircularis of the inner ear.Methods Total of 45 guinea pigs were employed in the experiment,which were randomly divided into the control group (15 subjects,30 ears) and the deafened group (30 subjects,60 ears). Each animal experienced auditory brainstem response ( ABR ). A quick treatment was employed for deafened group consisting of a subcutaneous injection of kanamycin at a dose of 400 mg/kg followed by jugular vein injection of ethacrynic acid at a dose of 40 mg/kg one hour later.The animals were performed ABR test from 7 to 10 days after the drug administration.The deafened group was further divided into ASNR group and non-ASNR group based on the presence of ASNR.All the guinea pigs were sacrificed after ABR tests.The Corti organ,macula sacculi,macula utriculi and crista ampullaris were observed by light microscope.Results In the deafened group (60 ears),3 subjects died postoperatively,27 subjects (54 ears) provided full data.ASNR was elicited in 19 ears (35.2%,19/54 ),the thresholds of ASNR were from 110 to 125 dBSPL with average of ( 121.7 ±4.5)dBSPL ASNR latency ranges were 1.80-2.08 ms,the average latency of thresholds were ( 1.93 ±0.07) ms.The stretched preparation results:overall hair-cell density of macula saccule,macula utriculi and crista ampullaris decreased in order of normal eontrol group,ASNR group and non-ASNR group.There was no difference between the normal group and ASNR group for cell density of macula saccule.Apart from this,statistical differences were found among other groups.Conclusions The present study evoked ASNR in an olotoxicity guinea pig model which was profound hearing loss with normal saccular function and normal saccular hair cell density.It suggested that ASNR originates from the saccule and have no relation with cochlear,utricle and semicircular canal according to morphological study.
4.Study on the ARIMA model application to predict echinococcosis cases in China

Objective To predict the monthly reported echinococcosis cases in China with the autoregressive integrated mov-ing average(ARIMA)model,so as to provide a reference for prevention and control of echinococcosis. Methods SPSS 24.0 software was used to construct the ARIMA models based on the monthly reported echinococcosis cases of time series from 2007 to 2015 and 2007 to 2014,respectively,and the accuracies of the two ARIMA models were compared. Results The model based on the data of the monthly reported cases of echinococcosis in China from 2007 to 2015 was ARIMA(1,0,0)(1,1, 0)12,the relative error among reported cases and predicted cases was-13.97%,AR(1)=0.367(t=3.816,P<0.001),SAR (1)=-0.328(t=-3.361,P=0.001),and Ljung-Box Q=14.119(df=16,P=0.590).The model based on the data of the monthly reported cases of echinococcosis in China from 2007 to 2014 was ARIMA(1,0,0)(1,0,1)12,the relative error among reported cases and predicted cases was 0.56%,AR(1)=0.413(t=4.244,P<0.001),SAR(1)=0.809(t=9.584, P<0.001),SMA(1)=0.356(t=2.278,P=0.025),and Ljung-Box Q=18.924(df=15,P=0.217).Conclusions The different time series may have different ARIMA models as for the same infectious diseases.It is needed to be further verified that the more data are accumulated,the shorter time of predication is,and the smaller the average of the relative error is.The estab-lishment and prediction of an ARIMA model is a dynamic process that needs to be adjusted and optimized continuously accord-ing to the accumulated data,meantime,we should give full consideration to the intensity of the work related to infectious diseas-es reported(such as disease census and special investigation).
5.Upholding Chinese spirit on schistosomiasis control in the new era to accelerate the progress towards schistosomiasis elimination in China

China is one of the schistosomiasis-endemic countries with the highest burden of disease across the world. Following the control efforts for over 60 years, great successes have been achieved in schistosomiasis control in the country, and the control program is moving towards transmission interruption and elimination. To commemorate the 60th anniversary of publishing Chairman Mao Zedong’s two poems entitled “Farewell to the God of Plague”, a series of activities that disseminate schistosomiasis control achievements have been conducted in China throughout 2018, including the development of Chinese spirit on schistosomiasis control in the new era. After extensive discussion, collection and screening, and “Integration of all efforts, scientific control, willingness to dedication and swearing to wipe out the 'God of Plague' “ was proposed as Chinese spirit on schistosomiasis control in the new era. Integration of all efforts is a summary of administrative policy-making and population participation in Chinese schistosomiasis control programs; scientific control is the refinement of the Chinese national schistosmiasis control strategy that is developed and implemented tailoring to time and circumstances; willingness to dedication is a valuable spiritual wealth and inexhaustible source of power for Chinese schistosomiasis control professionals in the new era; and swearing to wipe out the “God of Plague” is a sacred mission assigned to Chinese professionals participating in the national schistosomiasis control program in the new era. Chinese spirit on schistosomiasis control in the new era will further strengthen our belief in achieving the goal of schistosomiasis elimination in China eventually.
6.Effectiveness of different waist circumference cut-off values in predicting metabolic syndrome prevalence and risk factors in adults in China.

OBJECTIVETo study the effectiveness of waist circumference cut-off values in predicting the prevalence of metabolic syndrome (MetS) and risk factors in adults in China.
METHODSA cross-sectional survey was condcuted in 14 provinces (autonomous region, municipality) in China. A total of 47,325 adults aged⋝20 years were selected by multistage stratified sampling, and questionnaire survey and physical and clinical examination were conducted among them. MetS was defined according to the International Diabetes Federation (IDF) criteria and modified IDF criteria.
RESULTSThe age-standardized prevalence of MetS was 24.2% (22.1% in men and 25.8% in women) and 19.5% (22.1% in men and 18.0% in women) according to the IDF criteria and modified IDF criteria respectively. The age-standardized prevalence of pre-MetS was 8.1% (8.6% in men and 7.8% in women) according to the modified IDF criteria. The prevalence of MetS was higher in urban residents than rural residents and in northern China residents than in southern China residents. The prevalence of central obesity was about 30% in both men and women according to the ethnicity-specific cut-off values of waist circumference for central obesity (90 cm for men and 85 cm for women). Multivariate regression analysis revealed no significant difference in risk factors between the two MetS definitions.
CONCLUSIONUsing both the modified IDF criteria and ethnicity-specific cut-off values of waist circumference can provide more useful information about the prevalence of MetS in China. Since 2015 when the transmission of schistosomiasis was controlled in China, the country has been moving towards elimination of schistosomiasis, with the surveillance-response as the main interventions for schistosomiasis control. During the period of the 13th Five-Year Plan, the transmission of schistosomiasis had been interrupted in four provinces of Sichuan, Jiangsu, Yunnan and Hubei and the prevalence of schistosomiasis has been at the historically lowest level in China. As a consequence, the goal set in The 13th Five-Year National Schistosomiasis Control Program in China is almost achieved. However, there are multiple challenges during the stage moving towards elimination of schistosomiasis in China, including the widespread distribution of intermediate host snails and complicated snail habitats, many types of sources of Schistosoma japonicum infections and difficulty in management of bovines and sheep, unmet requirements for the current schistosomiasis control program with the currently available tools, and vulnerable control achievements. During the 14th Five-Year period, it is crucial to consolidate the schistosomiasis control achievements and gradually solve the above difficulties, and critical to provide the basis for achieving the ultimate goal of elimination of schistosomiasis in China. Based on the past experiences from the national schistosomiasis control program and the challenges for schistosomiasis elimination in China, an expert consensus has been reached pertaining to the objectives, control strategy and measures for The 14th Five-Year National Schistosomiasis Control Program in China, so as to provide insights in to the development of The 14th Five-Year National Schistosomiasis Control Program in China.
8.Analysis of the new WHO guideline to accelerate the progress towards elimination of schistosomiasis in China.

On February 2022, WHO released the evidence-based guideline on control and elimination of human schistosomiasis, with aims to guide the elimination of schistosomiasis as a public health problem in disease-endemic countries by 2030 and promote the interruption of schistosomiasis transmission across the world. Based on the One Health concept, six evidence-based recommendations were proposed in this guideline. This article aims to analyze the feasibility of key aspects of this guideline in Chinese national schistosomiasis control program and illustrate the significance to guide the future actions for Chinese national schistosomiasis control program. Currently, the One Health concept has been embodied in the Chinese national schistosomiasis control program. Based on this new WHO guideline, the following recommendations are proposed for the national schistosomiasis control program of China: (1) improving the systematic framework building, facilitating the agreement of the cross-sectoral consensus, and building a high-level leadership group; (2) optimizing the current human and livestock treatments in the national schistosomiasis control program of China; (3) developing highly sensitive and specific diagnostics and the framework for verifying elimination of schistosomiasis; (4) accelerating the progress towards elimination of schistosomiasis and other parasitic diseases through integrating the national control programs for other parasitic diseases.
