1.Possible potential impact of Three-gorges Project on population health in Kaixian County of Chongqing

BACKGROUND: In the past, the incidence rates of infectious diseases,natural epidemic diseases and endemic diseases in three gorges areas were always high. Because the flooded areas of Kaixian was enormous, the population of Kaixian was intensive density, the pollution may harm to the population health after the dam has been established.OBJECTIVE: To predict the influence of the Three-gorges dam on population health in Kaixian County of Chongqing and put forward to scientific evidence of preventive measures through analyzing the main health indicators and influence factors in Kaixian.DESIGN: Cross-sectional study.SETTING: Public Health College of Chongqing Medical University and Disease Controlled Center in Kaixian County of Chongqing.PARTICIPANTS: The survey was conducted in Kai County of Chongqing,from February to May 2004. All disease surveillance data of people in this county was investigated and other people lived in three-gorges area or around area were selected as the controls.METHODS: Collect and sort relevant indicators of social development,population health and public health system of Kaixian and related area to go deep into the representative townships to do some filed investigation,collect water samples of local drainage areas to carry out laboratory quantitative analysis; carry out comparative analysis based on the relevant materials; carry out comprehensive predicting and evaluation to the potential impact on population health of the three gorges engineering by combining the quantitative and qualitative analysis methods.general people in Kai county, Chongqing city and all over the nations durKai county, Chongqing city and all over the nations during 1999 and 2003;ty, Chongqing city and all over the nations during 1999 and 2001: Mortality rates of infants and under-five children were also higher than that average levels of whole country and Chongqing respectively; Multi-indicators showed that the general population health status of Kaixian was great bad,which stood a distinct gap among average levels of the whole country and Chongqing maintained about 250/100 000 or so, which was higher than the national level (190/10 000), however, the incidence rate of infectious diseases of Kaixian was rising year after year, even that of the recent two years has been reached up to more than 1.5 times higher than that of the whole of the water-based infectious diseases, especially the virus hepatitis, was severe. Another potential impact on population health of three-gorge dam is the emergence of natural epidemic diseases and insect-based infectious diseases. Malignant tumor such as hepatocarcinoma.CONCLUSION: We should strengthen current public health system, and cognition to the possible threat on disease epidemic and public health.With the construction of three-gorge dam, it is necessary to speed up establishment of effective measures to reduce predictable influence on population health.
2.The expression of caspase-10 in differentiated thyroid carcinoma and association with its development and metastasis

Objective To investigate the expression of caspase-10 in differentiated thyroid carcinoma and association with its development and metastasis. Methods Thyroid samples from 37 patients in a period from January 2006 to December 2007, with differentiated thyroid carcinoma were retrospectively analyzed for caspase-10 by immunohistocbemistry(streptavidin-perosidase, S-P), compared to control group of 46 cases with nodtdar goiter. The relationship between the expression of caspase-10 and the clinical pathologic characteristics of thyroid carcinoma were also explored simultaneously. Results caspase-10 were observed as brown or yellow particles located in the cytoplasm or cell membrane of nodular goiter but there were no significant evidence for its positive expression in thyroid carcinoma, caspase-10 expression was markedly down-regulated in differentiated thyroid carcinoma(29.73%,11/37) compared with benign nodules(71.74%,33/46, χ2=14.528, P<0.01). The positive expression in 18 cases with lymph node metastasis(11.11%,2/18) was significantly lower than those in 19 patients without lymph node metastasis(47.37%,9/19; χ2=4.210, P<0.01). There was no significant correlation(P> 0.05) between the expression of caspase-10 and the clinical pathologic characteristics including male, age, TNM stage and pathologic type. Conclusion Down-regulation of caspase-10 may play a critical role in carcinogenesis and development of differentiated thyroid carcinoma.
4.Therapeutic effect of Tripterygium wilfordii multiglycosides on proteinuria in kidney transplant recipients.

OBJECTIVETo study the effect of multiglycosides of Tripterygium wilfordii (MTW) for treatment of proteinuria in kidney transplant recipients.
METHODForty-five kidney transplant recipients with proternuria were randomized into 3 groups (n=15) and received full daily dose (1 mg/kg) MTW, half dose (0.5 mg/kg) MTW or no MTW (control) in addition to immunosuppressant therapy. The 24-hour urinary protein (24 h Upro), blood urea nitrogen (BUN), serum creatinine (Scr), dose of ciclosporin and the adverse effects of MTW were recorded.
RESULTSMTW at both the full dose and half dose significantly reduced the 24 h Upro as compared to exclusive immunosuppressant therapy (P<0.05). The therapeutic dose of ciclosporin in patients with full and half dose of MTW was significantly lower than that in the control group (P<0.05), and the patients receiving full dose MTW showed greater adverse effects than those having half dose MTW (P<0.05).
CONCLUSIONSMTW can significantly ameliorate proteinuria, reduce the therapeutic dose of ciclosporin and protect the renal function in kidney transplant recipients. While producing similar therapeutic effect to routine full dose, long-term use of half dose MTW may reduce the adverse effect associated with MTW.

6.Impacts of wogonin on cardiac inflammation in pregnant hypertensive rats by regulating the MCP-1/CCR2 signaling pathway

Objective To investigate the impacts of wogonin on cardiac inflammation in pregnant hypertensive rats by regulating the monocyte chemotactic protein-1(MCP-1)/CC chemokine receptor 2(CCR2)signaling pathway.Methods Pregnancy hypertensive rat model was established by intragastric administration of nitroso L-arginine methyl ester.The rats were randomly divided into control group(0.9%NaCl),model group(0.9%NaCl),experimental-L,-H groups(3.33 and 13.32 mg·kg-1 Wogonin),magnesium sulfate group(intrabitoneal injection 100 mg·kg-1magnesium sulfate),recombinant rat MCP-1 protein(rRMCP-1)group(intraperitoneal injected 0.026 mg·kg-1 rRMCP-1),experimental-H+rRMCP-1 group(gavage 13.32 mg·kg-1 Wogonin+0.026 mg·kg-1 rRMCP-1),each group consisted of 12 animals.Each group was administered once a day for 7 days.The serum levels of tumor necrosis factor-α(TNF-α)were detected by enzyme-linked immunosorbent assay;the expression of M1 and M2 macrophages in rat myocardium was detected by immunofluorescence staining;the expression of MCP-1 protein in rat myocardium was detected by Western blot.Results Serum TNF-α levels in control group,model group,experimental-L,-H groups,magnesium sulfate group,rRMCP-1 group,experimental-H+rRMCP-1 group were(10.06±0.41),(23.39±0.57),(19.89±0.62),(13.64±0.51),(12.97±0.48),(28.84±1.05)and(17.15±0.69)pg·mL-1;the fluorescence intensities expressed by M1 type macrophages were 32.26±1.43,58.84±2.11,50.66±1.89,36.69±1.57,35.87±1.59,67.73±2.01 and 48.53±1.86;the fluorescence intensities expressed by M2 type macrophages were 31.15±1.20,46.56±1.03,51.92±1.04,59.11±0.82,59.26±0.83,40.26±1.12 and 45.53±1.65;the expression of MCP-1 protein were 0.58±0.03,1.86±0.11,1.62±0.09,0.76±0.07,0.78±0.06,2.16±0.13 and 1.34±0.12,respectively.The above indicators in the model group were compared with those in the control group,compare the above indicators with the model group in the experimental-L,-H groups,magnesium sulfate group and rRMCP-1 group,the above indicators in the experimental-H group were compared with those in the experimental-H+rRMCP-1 group,the differences were statistically significant(all P<0.05).Conclusion Wogonin may inhibit cardiac inflammatory response in pregnant hypertensive rats by inhibiting the MCP-1/CCR2 signaling pathway.
7.Headless HA of influenza H5N1virus constructs based on baculovirus expression system

Objective To construct recombinant headless HA of avian influenza H5N1virus based on baculovirus expression system.Methods Headless HA gene of an avian H5N1virus,A/Hubei/1/2010 was cloned into pFastBac1vector.The recombinant shuttle vector,headless HA-Bac,was obtained by transforming pFastBac1headless HA into DH10bac cell and then the baculovirus bearing with headless HA gene,headlessHA-Bac,was achieved viaBac-to-Bac baculovirus expression system. The headless HA expression was detected in headless HA-Bac-infected Sf9 cells by Western Blot and HA assay.Results The headless HA has been efficiently expressed and displayed no reaction with turkey red blood cells.Conclusion The constructs provide fundamental work for future studies on universal influenza vaccine.
8.Effects of verbascoside on the proliferation and osteogenic differentiation of rat bone marrow stromal cells

Objective To study the effects of verbascoside on the proliferation and osteogenic differentiation of rat bone marrow stromal cells (BMSCs) and its mechanism.Methods BMSCs were obtained and identified from rat bone marrow by density gradient centrifugation.BMSCs were randomly divided into high,medium,low dose test groups and control group,and treated with 100,50,25,0 μmol · L-1verbascoside.The cell counting kit-8 (CCK-8) method was used to evaluate the BMSCs proliferation at 24,48,72 h after administration the medication.The alkaline phosphatase(ALP) activities were detected at 24,48,96 h after medication.The ALP staining was performed after 72 h of medication.Western blot and real time-polymerase chain reaction (PCR) were used to detect the osteogenic related proteins and genes expression.Results After 72 h,the proliferation of BMSCs in high dose test group was 1.22 ± 0.05,lower than that in control group,which was 1.50 ± 0.09 (P < 0.05).The ALP expression levels in high,medium and low dose test groups were higher than control group at 24,48,96 h.After 72 h,ALP staining showed that the color of culture dishes in high,medium and low dose test groups were deeper than that in control group.Western blot showed that osteogenic related proteins of bone morphogenetic protein-2 (BMP2),Smad1,Smad5,Smad8,Runx2,Osterix in high,medium and low dose test groups were higher than those in control group (P <0.05),but there was no significant difference among those three test groups (P > 0.05).Real time-PCR showed that the osteogenic related genes of BMP2,Smad1,Smad5,Smad8,Runx2,Osterix expression levels in high,medium and low dose test groups were higher than those in control group (P < 0.05).However,there was no significant difference among three test groups(P > 0.05).Conclusion The verbascoside at different concentrations has no obvious effect on BMSCs proliferation and promote osteogenic differentiation of BMSCs through BMP2-Smads -Runx2-Osterix pathway.
9.The synergistic effect of FGF-21 and insulin on regulating glucose metabolism and its mechanism.

Previous studies proposed that the synergistic effect of fibroblast growth factor-21 (FGF-21) and insulin may be due to the improvement of insulin sensitivity by FGF-21. However, there is no experimental evidence to support this. This study was designed to elucidate the mechanism of synergistic effect of FGF-21 and insulin in the regulation of glucose metabolism. The synergistic effect of FGF-21 and insulin on regulating glucose metabolism was demonstrated by investigating the glucose absorption rate by insulin resistance HepG2 cell model and the blood glucose chances in type 2 diabetic db/db mice after treatments with different concentrations of FGF-21 or/and insulin; The synergistic metabolism was revealed through detecting GLUT1 and GLUT4 transcription levels in the liver by real-time PCR method. The experimental results showed that FGF-21 and insulin have a synergistic effect on the regulation of glucose metabolism. The results of real-time PCR showed that the effective dose of FGF-21 could up-regulate the transcription level of GLUT1 in a dose-dependent manner, but had no effect on the transcription level of GLUT4. Insulin (4 u) alone could up-regulate the transcription level of GLUT4, yet had no effect on that of GLUT1. Ineffective dose 0.1 mg kg(-1) FGF-21 alone could not change the transcription level of GLUT1 or GLUT4. However, when the ineffective dose 0.1 mg x kg(-1) FGF-21 was used in combination with insulin (4 u) significantly increased the transcription levels of both GLUT1 and GLUT4, the transcription level of GLUT1 was similar to that treated with 5 time concentration of FGF-21 alone; the transcription level of GLUT4 is higher than that treated with insulin (4 u) alone. In summary, in the presence of FGF-21, insulin increases the sensitivity of FGF-21 through enhancing GLUT1 transcription. Vice versa, FGF-21 increases the sensitivity of insulin by stimulating GLUT4 transcription in the presence of insulin. FGF-21 and insulin exert a synergistic effect on glucose metabolism through mutual sensitization.

10.Efficacy of ibutilide for cardioversion of persistent atrial fibrillation during radiofrequency ablation

Objective The aim of this study was to investigate the efficiency and safety of ibutilide for cardioversion of persistent atrial fibrillation (AF) during radiofrequency ablation.Methods Eighteen patients (16 males) with persistent atrial fibrillation were enrolled in this study.All patients underwent circumferential pulmonary vein ablation guided by a Carto three-dimensional mapping systen.In addition,linear ablation at the top of the left atrium and the isthmus of mitral valves and complex fractionated atrial electrogram (CAFE) ablation were performed.All patients were still in either atrial fibrillation or atrial flutter after ablation,the patients were treated with l mg intravenous ibutilide injection within 10 minutes after unsuccessful ablation.Intravenous injection was stopped in case of sinus rhythm (SR) restoration or occurrence of severe adverse reactions such as ventricular tachycardia.Cardioversion rate within 30 min and adverse reactions within 4 h were observed.Patients were divided into either conversion group or nonconversion group according to whether AF was converted to sinus rhythm within 30 minutes after injection.Results Eleven patients (61.11% ) converted to SR after ibutilide injection.There were no significant differences in gender,age,body mass index,left atrium and left ventricular function between conversion group and non-conversion groups.The average conversion time was ( 13.80 + 7.64 ) min,left atrium scar area ratio was significantly larger in non-conversion group ( 12.40 + 11.03 ) % than in conversion group (5.12 ±3.83)%,P <0.05.Ibutilide significantly prolonged the average wavelength of the AF wave ( 171.8 + 29.5 ) ms vs.(242.0 ± 40.0) ms at baseline,P < 0.01.The QT interval at 30 min after ibutilide injection (0.39 ± 0.21 ) s was significantly longer than before injection (0.51 ± 0.08 ) s,P < 0.05.There was no serious arrhythmias or other adverse reactions post ibutilide injection.Conclusions Ibutilide is highly effective and safe agent for cardioversion in patients underwent unsuccessful ablation.Left atrium scar area ratio is an important determinant for the conversion rate in this cohort.
