1.Expression and significance of CD24 in colorectal cancer

Objective To determine the expression of CD24 in colorectal carcinoma, and to explore the relationship between CD24 and the clinicopathological features of colorectal carcinoma.Methods The expression of CD24 in 62 cases of colorectal carcinoma, 47 cases of adenomas, 15 cases of colorectal polyps and 30 cases of the adjacent non-cancerous tissues were observed by immunohistochemical assay.The relationship between CD24 and the clinicopathological features was analyzed.Results The positive rates of CD24 in colorectal carcinoma 72.6% and adenomas 63.8% were significantly higher than those in colorectal hyperplastic polyps 13.3% (x2 =17.83, P =0.00;x2 =11.61, P =0.00) and adjacent non-cancerous tissues 6.7% (x2 =35.15, P =0.00;x2 =24.64, P =0.00).The expression of CD24 in colorectal carcinoma had a significant correlation with the tumor diameter (x2 =5.48, P =0.02), tumor differentiation (x2 =8.86, P =0.00), Duke staging (x2 =11.47, P =0.00) and lymph node metastasis (x2 =8.92, P =0.00).Conclusion The expression of CD24 is high in colorectal carcinoma, having a significant correlation with the size of tumor, degree of differentiation, Duke stage and lymph node metastasis.
2.Distance between brain metastases of non-small cell lung cancer and the hippocampus and its correlation with PTV low-dose regions in prophylactic cranial irradiation with hippocampal avoidance: an analysis of 56 patients

Objective To investigate the feasibility of prophylactic cranial irradiation with hippocampal avoidance (HA-PCI) in non-small cell lung cancer (NSCLC).Methods The clinical data of 56 patients with brain metastases of NSCLC who were treated from 2011 to 2014 were collected.Brain metastases and the hippocampus were delineated on T1 W1 contrast-enhanced MRI,and the distance between brain metastases and the hippocampus was analyzed;an HA-PCI regimen was also developed,and the distribution of the metastases in planning target volume (PTV) low-dose regions around the hippocampus was analyzed.Results None of the 139 metastases involved the hippocampus.There were 6(4.3%) and 18 (12.9%) metastases within 5 mm and 10 mm,respectively,outside the hippocampus.In the HA-PCI regimen,the D50% and D2% of PTV were 25.6 Gy and 27.1 Gy,respectively.Dmean and D2% for the hippocampus were 7.4 Gy and 9.9 Gy,respectively;D50% within 0-5.0 mm,5.1-10.0 mm,and 10.1-15.0 mm outside the hippocampus was 10.3 Gy,15.1 Gy,and 20.5 Gy,respectively.Conclusions HA-PCI may be feasible theoretically,but this needs to be confirmed by the intracranial failure pattern in patients with long-term survival.
3.Situation and effect of vertical integrated of rural health services:A case study in Dafeng Coun-ty of Jiangsu Province

Objective: To analyze the current situation and effect of vertical integrated of rural health services in Dafeng County. Method: Policy documents and data are collected and interviews are implemented for studying the integrated situation. Results: The vertical integration in Dafeng County is virtual joint form with forming 6 city-level medical volunteer service teams and 52 health management teams. With the imple-mentation of fully integrated management, signing service with rural doctor mode is carried out, which pro-mote the establishment of rural unified service standards and regulations. Using health card of residents which promote the regional shared of treatment information. Conclusions: Vertically integrated mode had connection in health management teams, supported by fully integration, developed by signing service with rural doctor and carried by residents’ health card, which could provide reference for implementing vertical integration of rural health service. But strategies needed to be strengthened in improving health services’ ability of township hospitals, clarifying interest relationship as well as integrating health service regulations and strengthen the health information system for sharing and docking.
4.Curative effect of Ganciclovir combined with interferon-α1b inhalation in treating children with infectious mononucleosis

Objective To investigate the clinical efficacy and safety of Ganciclovir combined with interferon-α1 b inhalation for children with infectious mononucleosis(IM).Methods A total of 177 childhood cases of IM were selected,and they were divided into 3 groups,59 cases in each group according to the random number table.Three therapeutic methods were applied in different groups for 5-7 days in different groups:Ganciclovir (group A),Ganciclovir + interferon-α1 b inhalation (group B) and Ganciclovir + interferon-α1b intramuscularly (group C).The time of post-drug recovery from isthmitis,less than 0.05 of heterotypic lymphocytes,shrink of cervical lymph nodes shrink,liver retraction,spleen retraction among groups were compared.The Epstein-Barr virus (EBV)-DNA copy number and T lymphocyte subsets were compared before and after treatment.Adverse reactions were observed in each group.Results Compared with group A,the time to defervescence [(3.20 ± 1.81) d,(3.17 ± 1.76) d vs.(4.01 ± 2.34) d],duration of isthmitis was [(3.15 ± 1.33) d,(3.09 ± 1.37) d vs.(3.98 ± 1.31) d],and the time of heterotypic lymphocytes less than 0.05 [(3.12 ± 1.55) d,(3.10 ± 1.33) d vs.(3.95 ± 1.26) d] in group B and group C,were obvious shorter,and there were significant differences(F =4.150,4.580,4.060,all P < 0.05).EBV-DNA negative conversion rate of group B and group C were higher than that of group A [53 cases(89.8%),52 cases (88.1%) vs.41 cases (69.5%),x2 =10.403,P < 0.05],and the cellular immune function was improved significantly than that of group A after treatment for 7 days [CD3 +:(63.00 ±4.39)%,(62.75 ±4.84)% vs.(68.70 ± 7.70)%;CD4+:34.08(30.21,41.70)%,33.94(29.17,45.17)% vs.32.34(28.16,43.53)%;CD8+:30.59 (27.14,40.22)%,30.09(27.54,40.48)% vs.32.57(28.68,41.17)%;CD4+/CD8+:1.12(1.03,1.31),1.11 (0.99,1.64) vs.0.94 (0.87,1.59),F/x2 =11.020,1.217,1.121,6.728,all P < 0.05].The differences in indexes between B group and C group were not significant,and there was no statistical significance (all P > 0.05).There were 2 cases with fever in the group C,and 2 cases of granulocytopenia in all group.Conclusions Ganciclovir combined with interferon-α1 b inhalation or intramuscular injection is effective and safe in treating children with IM.It can improve clinical symptoms,cellular immune function and EBV-DNA negative conversion rate.Since inhalation is of less side effects and no pain,it can be accepted by children and their parents easily.Therefore,it is recommended that Ganciclovir be used together with interferon-α1 b inhalation in the treatment of children with IM.
5.The value of multi-source RF-transmission in balanced fast field echo cardiac cine imaging at 3.0 T magnetic resonance imaging

Objective To assess the advantages of multi-source RF transmission(MT) for balanced fast field echo(BFFE) cardiac cine imaging.Methods Fifteen volunteers were scanned on a clinical 3.0 T MR system equipped with MT.Acquisition of B1 maps with and without MT were followed by axial and four chambers BFFE cine imaging for all subjects with different transmission modes[single source RF transmission (ST),MT and MT with allowable shortest TR(MTS)].The B1 field uniformity and contrast to noise ratio (CNR) were quantitatively analyzed.The B1 field uniformity was evaluated by the pixel values within the ROI and was compared with Student t test.CNR was defined as follows:(SIblood-SImyocardium)/ 0.5 ×(SDblood + SDmyocardium).CNR were tested with one-way ANOVA for three groups comparision and with LSD for inter-group comparison.Image qualities were blindly assessed by 2 readers with a 4-score scale.Global image quality scores were tested for statistical significance by using the nonparametric paired sample Friedman rank test for three groups and the Wilcoxon signed rank test for further inter-group comparison.The Kappa statistics was used to assess interreader agreement.Results B1 homogeneity was significantly improved for images with MT compared with ST under the condition of other parameters unchanged (t =21.632,P <0.01).In left ventricular,CNR of blood and myocardium was improved from 10.8 ±2.2 for ST to 14.4 ± 2.4 for MT and 14.1 ± 2.2 for MTS.In fight ventricular,it was improved from 7.7 ± 1.2 for ST to 12.2 ± 1.4 for MT and 12.0 ± 1.2 for MTS.The differences were statistically significant (F =11.617,61.472,P <0.01).The images of MT and MTS groups demonstrated greater CNR versus the ST images.There was no statistical difference between latter two groups.The image scores of Reader 1 and 2 were 4.60 ± 1.18 and 4.73 ± 1.10 for ST,6.53 ± 1.19 and 6.67 ± 1.29 for MT and 6.73 ± 1.03 and 6.73 ± 0.88 for MTS respectively.There were statistically differences among three groups (x2 =23.577,24.275,P < 0.01).The image quality was improved on BFFE images using MT and MTS technology compared to ST technology,and there were also significant differences.The interreader agreement between two readers was good [K=0.643,0.722 and 0.814(P <0.05) for ST,MT,and MTS group respectively].Conclusions MT technology significantly improves B1 field uniformity,increases CNR and reduces artifact on BFFE cardiac cine images.The TR is decreased within the normal SAR ranges,and thus scanning speed is increased.

7.Spatio-temporal process and the influencing factors on influenza A (H1N1) pandemic in Changsha

Objective To analyze the spatio-temporal process on 2009 influenza A (HlNl) pandemic in Changsha and the influencing factors during the diffusion process. Methods Data were from the following 5 sources, influenza A (HlNl) pandemic gathered in 2009, Geographic Information System (GIS) of Changsha, the broad range of theorems and techniques of hot spot analysis, spatio-temporal process analysis and Spearman correlation analysis. Results Hot spot areas appeared to be more in the economically developed areas, such as cities and townships. The cluster of spatial-temporal distribution of influenza A (HlNl) pandemic was most likely appearing in Liuyang city (RR=22.70,P<0.01). The secondary cluster would include districts as Yuelu (RR=6A9,P< 0.01) , Yuhua (RR=81.63, P<0.01). Xingsha township appeared as the center in the Changsha county (RR=2.90, P<0.01) while townships as Yutangping (RR=19.31, P<0.01) , Chengjiao (RR=73.14,P<0.01) and Longtian appeared as the center in the west of Ningxiang county (RR= 14.43,P<0.01) and Wushan as the center in the Wangcheng county (RR= 13.84,P<0.01). As time went on, the epidemic moved towards the eastern and more developed regions. Regarding factor analysis, population, the amount of students, geographic relationship and business activities etc. appeared to be the key elements influencing the transmission of influenza A (H1N1) pandemic. At the beginning of the epidemic, population density served as the main factor (r=0.477, P<0.05) but during the initial and fast growing stages, it was replaced by the size of students to serve as the important indicator (r=0.831, P<0.01; r=0.518, P<0.01). However, during the peak of the epidemics, the business activities played an important role (r=-0.676, P<0.01). Conclusion Groups under high risk and districts with high incidence rates were shifting, along with the temporal process of influenza A(H1N1) pandemic, suggesting that the protection measures need to be adjusted, according to the significance of influencing factors at different stages.
8.Research of water contact and labor practice of residents in plateau moun-tain schistosomiasis endemic areas in Yunnan Province

Objective To understand the status of water contact and labor practice of residents in plateau mountain schisto?somiasis endemic areas in Yunnan Province so as to provide the evidence for formulating the strategy of prevention and control of schistosomiasis in the next step of work. Methods A total of 412 residents aged 6-65 years old in 2 villages were randomly sampled and investigated with questionnaires for their water contacts and labor practices in plateau valley schistosomiasis endem?ic areas. Then the schistosome infection status of the residents was surveyed with the indirect hemagglutination assay IHA . Re?sults Among the 412 residents investigated the rate of water contact was 88.35% and the main causes of water contact were watering and swimming and playing. The percentages of residents who had schistosomiasis history were 1.16% 0.00%30.80% 3.85% and 0 in them who had swimming and playing water?contacts bathing watering washing hands and others respectively χ2=38.96 P<0.01 . The positives of IHA in the above?mentioned residents were 18.60% 0.00% 37.60%23.08% and 0 respectively χ2=12.61 P<0.05 . Conclusions The productive infested water contact is the main way of schistosome infection. Therefore the changes of labor practices and water contact of the residents are very important for the ef?fective prevention and control of schistosomiasis transmission.
9.Effect of comprehensive schistosomiasis control measures based on infection source control in mountainous areas of Yunnan Province

Objective To evaluate the effect of comprehensive schistosomiasis control measures based on infection source control in plateau mountain areas of Yunnan Province. Methods From 2006 to 2004,four administrative villages were selected as test areas from plateau canyon and plateau basin endemic areas in Jindun Town,Heqing County,two villages each type,and the comprehensive control measures were implemented,including the examination and treatment of schistosomiasis,Oncomela?nia hupensis snail survey and control,health education,improving drinking water and lavatories,banning grazing,constructing sanitary pen of livestock,replacing cattle with machine,etc. The schistosome infection state and snail status in 2006 were treat?ed as the baseline information,and the effect of the comprehensive measures were evaluated. Results The infection rate of hu?man in plateau canyon areas decreased from 4.94%in 2006 to 0.06%in 2014,and that of livestock decreased from 1.11%to 0. In plateau basin areas,there was only 1 case of schistosomiasis found in Xiaolian Village in 2007,and no any other cases found in the other years,the infection rates of livestock dropped from 7.38%to 0. Compared with 2006,the snail areas in the two type areas decreased by 74.89%and 75.30%,respectively,meanwhile,the percentage of snail area,the occurrence rate of frames with snails,as well as the average density of living snails also decreased,and no infected snails were found since 2008. Xidian and Xinzhuang villages in plateau canyon area reached the criteria of schistosomiasis transmission controlled in 2009,and Xiao?lian and Kangfu villages in plateau basin reached the criteria of transmission interrupted in 2014. Conclusions The comprehen?sive schistosomiasis control measures based on infection source control can effectively control the endemic situation of schistoso? miasis in plateau areas of Yunnan Province. In the future,we should pay an equal attention to the infection sources control and snail control to consolidate and amplify the achievement of schistosomiasis control.
