OBJECTIVE:To strengthen people's awareness of pharmacovigilance.METHODS:The pharmacovigilance associated knowledge was introduced,and the status quo about pharmacovigilance working in China and the challenge posed by which were analyzed,then the corresponding countermeasures were presented.RESULTS&CONCLUSIONS:Only through strengthening people's pharmacovigilance can the adverse drugs reactions and the related problems be reduced,and only by so people's level of rational use of drugs could be improved.
2.Comparative study on acute respiratory distress syndrom originating from pulmonary disease and extra-pulmonary disease treated via prone position mechancal ventilation

Objective:To discuss what impact prone position ventilation (PPV) plays in treatment of acute respiratory distress syndrom (ARDS)originating from pulmonary disease(ARDSp) and extra-pulmonary disease(ARDSexp).Methods: Retrospective case-control cohorts study. Sixty patients with ARDS were included, were classified two groups which were originated from pulmonary disease(n=30) and extra-pulmonary disease(n=30), they were ventilated in PPV. And oxygenation of 0,2,4, and 6 hours in PPV, static compliance(Cst), airway resistance(Raw) of were evaluated. The Ultrasonic Cardiac Output Monitor was applied to monitor the cardiac function.Results:At 2 hours after ventilation in ARDSexp group, PaO2/FiO2 have been improved significantly. The longer duration of PPV seemed to be associated with continuous improvement and peaks at 4 hours. In ARDSexp group PaO2/FiO2 after ventilation at 3 hours exceeds the other groups. At 4 hours after ventilation, PaO2/FiO2 of both ARDSp and ARDSexp groups of PPV have been improved than that of supine position. After ventilation for 4 hours, the RAW of ARDSp decreases dramatically and considerably below the other groups.Conclusion: PPV was a safe and effective maneuver for improving oxygenation in patients with ARDSp and ARDSexp. Both ARDSp and ARDSexp clearly appear to benefit from PPV, particularly in ARDSexp group. PPV aslo influenced haemodynamics in patients rarely.
3.PTCD combined with CT-guided microwave ablation for hepatic hilar cholangiocarcinoma:analysis of therapeutic efficacy

Objective To discuss the clinical application of percutaneous transhepatic cholangiopancreatic drainage (PTCD) combined with CT-guided microwave ablation in treating hepatic hilar cholangiocarcinoma. Methods The clinical data of 29 patients with inoperable Ⅲ and Ⅳ type hilar cholangiocarcinoma complicated by obstructive jaundice, who were admitted to authors’ hospital during the period from December 2012 to August 2014, were retrospectively analyzed. The diagnosis of bile duct adenocarcinoma was confirmed by pathology in all patients. Of the 29 patients, both internal and external biliary tract drainage was employed in 19, external biliary tract drainage in 4, and bilateral (both left and right side) bile duct drainage in 6. CT-guided microwave ablation was carried out when the liver function became improved. A total of 46 procedures of microwave ablation were completed in the 29 patients with a mean of 1.5 times per patient. Hepatic function tests, enhanced CT or MR scan were performed 4-8 weeks after treatment. According to mRECIST criteria the therapeutic results were evaluated; the serum bilirubin levels were recorded; the disease progress and the patient’s survival time were followed up. Results One month after the treatment, complete response (CR) was obtained in 15 patients (15/29, 51.7%), and partial remission (PR) in 17 patients (17/29, 58.6%), with the overall efficacy (CR﹢PR) being 82%. After the treatment, one patient developed hepatic metastasis and another one had pulmonary metastasis. Postoperative serum bilirubin levels showed an obvious decrease in all 29 patients. The 6-month, one-year and two-year survival rates were 68.9% (20/29), 31.0% (9/29) and 6.8% (2/29) respectively; the median survival time was 8.9 months and the overall survival time was 11.7 months. Conclusion For the treatment of hepatic hilar cholangiocarcinoma, PTCD combined with CT-guided microwave ablation is minimally invasive, clinically safe and effective.
4.Application of Mutislice CT angiography in cervical vascular diseases

Objective To discuss the clinical value of Mutislice CT angiography (MSCTA)in cervical vascular diseases. Methods 60 patients highly suspected suffering cervical vascular diseases,were implemented 16-slices spiral CT angiography of cervical vascular,and blood vessel reconstruction and analysis were done subsequently. Results 43 cases with cervical vascular abnormality were presented in the 60 patients,who were implemented MSCTA. Positive ratio was 71.7%. 171 pieces vessel stenosis were detected among 240 pieces carotid artery and vertebra artery, and 4 cases carotid artery aneurysm and 1 case AVM were found also. Conclusion MSCTA is a convenient and shortcut modality to inspect cervical vascular diseases, and the blood vessel can be observed 3-dimensionally and in any direction in high quality reconstruction images.
5.Changes of urinary function after nerve-sparing radical hysterectomy for cervical cancer

Objective To retrospectively compare analysis of two kinds of surgery which were nerve-sparing radical hysterectomy (NSRH) and the traditional radical hysterectomy (RH) for cervical cancer was done to find out the effect on urinary function in postoperative patients. Methods Among 57cervical cancer with clinical stage Ⅰ B1- Ⅱ A 31 cases treated with NSRH,26 cases treated with RH. The postoperative bladder function of the two methods were evaluated, and postoperative recovery of urinary function and postoperative recurrence of tumor were compared. Results The period of indwelling catheter for the postoperative patients with NSRH was (8.5 ± 3.2) days, obviously less than that for traditional RH patients which was ( 12.8 ± 3.8) days. The difference was statistically significant(P < 0.01 ). And two methods of postoperative patients were alive without tumor recurrence and metastasis after 1-4 years follow-up.Conclusions NSRH is better than traditional RH in remaining pelvic autonomic nervous system and improving the early urinary function of postoperative patients of cervical cancer. Otherwise, it doesn't effect the radical cure of tumor.
6.The Study of the diffusion tensor imaging in patients with primary trigeminal neuralgia caused by neurovascular compression

Objective Toexplore the application value of diffusion tensor imaging (DTI) in evaluating evaluation offunctional changes of the in patients with primary trigeminal neuralgia caused (PTN) by neurovascular compression. Methods 40 unilateral PTN patients and 40 healthy volunteers were enrolled in ourstudy.They allAll patients underwent the general sequences and DTI ,and then to measured the ADC and FA values of the trigeminal nerves. Results (1) Compared with contralateral side (0.408 ± 0.054)and bilateral sides in control group(0.423 ± 0.057), FA value of the ipsilateral side in PTN group(0.330 ± 0.056) was significant lower (P< 0.05)compared with the contralateral side (0.408 ± 0.054) and bilateral sides in control group (0.423 ± 0.057).The ADC value of ipsilateralside (2.052 ± 0.473)× 10-3 mm2/s was significantly higher (P < 0.05) thancompared withthe contralateral side (1.541 ± 0.266) ×10-3 mm2/s and bilateral sides in control group(1.431 ± 0.308) ×10-3 mm2/s. (2) An There's a nnegative correlation was found (r = -0.613,P < 0.001) between the loss of FA and the increase of ADC (r = -0.613,P < 0.001). Conclusion DTI could be used to evaluate the changes of neuratrophy and demyelination ,so it canmight be used of in diagnosis and treatment of PTNin further way.
7.Preliminary Applicaton of Placing Freka R Trelumina guided by DSA

Objective To evaluate the initial clinical application value of placing the Freka R Trelumina guided DSA. Methods The Freka(R) Trelumina was placed guided by DSA in 10 patients with disease of digestive tract post operation complicated with stomal leak or/and stomal stegnosis or various kinds of patients with gastric emptying disorder. Results The Freka(R) Trelumina could be put 20 cm away from Treize anadesma guided by DSA. The achievement ratio was 90%. The putting time was 1 to 3 weeks. The localization of the Freka(R) Trelumina was well. The process of feeding was successfully. The effectiveness of decompression of stomach intestine was also good. Stomal leak healed up quickly. The appearance of gastric emptying disorder disappeared. Conclusion Placing the Freka(R) Trelumina guided by DSA is a simple, safe and reliable method for jejunum nourishment and gastrointestinal decompression.
8.Clinical Study of Liraglutide Combined with Insulin and Glipizide in the Treatment of Subclinical Hypothy-roidism Complicated with Type 2 Diabetes in the Elderly Patients

OBJECTIVE:To observe the effectiveness and safety of liraglutide combined with insulin and glipizide in the treat-ment of subclinical hypothyroidism(SCH)complicated with type 2 diabetes in the elderly patients. METHODS:Totally 82 elderly patients with SCH complicated with type 2 diabetes were selected from our hospital during Dec. 2013-Dec. 2015,and then divided into trial group(40 cases)and control group(42 cases)according to random number table. Control group was given Insulin in-jection+Glipizide tablets. Trial group was additionally given Liraglutide injection 0.6 mg,sc,qd,on the basis of control group. Treatment courses of 2 groups lasted for 12 weeks. The levels of blood glucose [fasting glucose,postprandial 1 h and 2 h glucose,mean of daily differences(MODD),mean amplitude of glycemic excursions(MAGE)],glycosylated hemoglo-bin,body weight,total cholesterol,blood pressure [systolic blood pressure(SBP),diastolic blood pressure(DBP)],thyroid stimulating hormone(TSH)and homeostasis model assessment(HOMA-B)were observed in 2 groups before and after treat-ment. The occurrence of ADR was recorded. RESULTS:Totally 4 patients of control group withdrew from the study,and no one withdrew from the study in trial group. Before treatment,there was no statistical significance in the levels of blood glu-cose,glycosylated hemoglobin,body weight,total cholesterol,blood pressure,TSH and HOMA-B (P>0.05). After treat-ment,body weight and total cholesterol level of trial groups were significantly decreased and lower than those of control group,with statistical significance (P<0.05). The levels of blood glucose,glycosylated hemoglobin,blood pressure (SBP, DBP)and TSH in 2 groups were decreased significantly,while HOMA-B levels were increased significantly,and trial group was significantly better than control group,with statistical significance(P<0.05). There was no statistical significance in the incidence of ADR between trial group (12.5%) and control group (19.0%)(P>0.05). CONCLUSIONS:Liraglutide com-bined with insulin and glipizide for elderly patients with SCH complicated with type 2 diabetes can effectively reduce blood glucose level,keep blood glucose stable,control the increase of body weight and improve islet B cell function with good safety.
9.Analysis of clinical characteristics and prognosis of young patients with endometrial cancer

Objective To investigate the clinical characteristics and prognosis of young patients with endometrial cancer. Methods Eighty-two cases with endometrial cancer were analyzed retrospectively,31 cases in the young group were ≤ 45 years old, and 51 cases in the old group were > 45 years old. The clinical characteristics, diagnosis, treatment and prognosis between the two groups were compared. Results Twelve cases with primary infertility took up 38.7% in the young group,23 cases with less than 3 times of pregnancy occupied 74.2%, and there was no statistical difference,compared with the old group [3.9%(2/51)and 29.4% (15/51)] (P<0.05). There was statistically significant between the two groups in myometrial invasion (P < 0.05 ); while there was no significance between the two groups in pathological type, histological grade,clinical stage, 5-year survival rate and recurrence period (P>0.05). Conclusion Infertility or less pregnancy are risk factors for endometrial cancer in the young; the degree of endometrial invasion in the young is more shallow than that in the old; there is no significant difference between the young and the old in pathological type, histological grade and clinical stage; but the prognosis is similar between the young and the old patients.
