1.CT-guided interstitial implantation of ~(125)I seeds for non-small cell lung cancer

Objective To discuss the technique, therapeutic effect and complications of CT-guided interstitial implantation of ~(125)I seeds for the treatment of non-small cell lung carcinoma (NSCLC). Methods With the help of treatment planning system, CT-guided interstitial implantation of ~(125)I seeds was performed in 21 patients with pathologically-proved NSCLC. Multi-plane implanting technique was adopted and ~(125)I seeds of (0.5 -0.8) mCi were placed at intervals of (1.0-1.5) cm. Checkup CT scan was carried out immediately after the procedure to make sure that the seeds were satisfactorily distributed. Follow-up CT scanning was made (2-6) months after the treatment to survey the distribution of the seeds within the tumor. The curative effect and complications were also evaluated and observed. The patients were followed for 12 months. Results Of 21 cases, complete remission was seen in 7, obvious remission in 12 and partial remission in 2. The one year survival rate was 90.5% (19/21). Complications included pneumothorax (n = 6) and hemoptysis (n = 2). No serious complications or treatment-related radiation injuries occurred. Conclusion CT-guided interstitial implantation of ~(125)I seeds is a safe and effective treatment for NSCLC with reliable short-term therapeutic effect.
2.High risk factors for BK virus viremia in recipients following kidney transplantation

BACKGROUND: One major reason for transplanted kidney failure is BK virus (BKV) associated nephropathy, which leads to renal inadequacy and ureteral obstruction. OBJECTIVE: To identify the incidence of BKV viremia in kidney transplant recipients, as well as to analyze the risk factors of BKV infection. DESIGN, TIME AND SETTING: A retrospective case analysis was performed at the Department of Urology, Affiliated Beijing Friendship Hospital of Capital Medical University from September 2001 to December 2007. PARTICIPANTS: One hundred and twenty-one renal transplant patients, and 20 renal failure patients, who underwent hemodialysis were served as control group. METHODS: The viral load in plasma was quantified with a real-time PCR method. The recipients were divided into the PV+ group (BKV DNA positive blood samples) and PV- group (BKV DNA negative in blood samples) according to the detection outcomes. The gender, age, cold ischemia time, immunosuppressive agent, donor type and other clinical parameters were compared between 2 groups. MAIN OUTCOME MEASURES: Logistic regression was performed to determine risk factors for BKV infection. RESULTS: The incidence rate of BKV viremia accounted for 24.7% (30/121) of kidney recipients, which was higher than the control group (1/20), the difference between them was significant (P = 0.03). Logistic regression found that cold ischemia time (RR 3.34, 95% CI 2.76-5.60) and cadaver donation (RR 2.19, 95% CI 1.32-3.97) were significantly related to BKV viremia. However, ages of recipients, time of collecting samples, and intravenous glucocorticoid therapy was independence to BKV viremia. CONCLUSION: Kidney recipients are the high risk population who are susceptive to BKV infection. Cold ischemia time and cadaver donation may be the risk factors of BKV viremia.

4.Oxidative Damage of Respiratory System in Rats Exposed to Carbon Nanotubes

Objection To explore the oxidative damage of respiratory system in rats exposed to carbon nanotubes(CNTs) . Methods Twelve Wistar rats were randomly divided into two groups(the treated group and the control group) and exposed to CNTs through intratracheal instillation at the dose of 22.5 mg/kg(calculated with CNTs) once a day for 15 days,then the rats were sacrificed,the biochemicaly indexes in serum and bronchoalveolar lavage fluid(BLAF) were measured. Results The results showed that CNTs could cause decrease of body weight gain,even after three days of stoping exposure to CNTs. Compared with the control group,the activity of superoxide dismutase(SOD) and total antioxidant capacity(T-AOC) in serum and BLAF of exposed rats decreased,the level of malondialdehyde(MDA) in serum and BLAF increased significantly. Conclusion CNTs exposure can induce the decrease of the antioxidant capacity and oxidative damage of respiratory system in rats.
5.Descriptive analysis on military training injuries in a special force corps

[Objective]To explore the incidence, causes, training courses at high risk and location of the body of training injury in a special force corps, provide evidence for effective prevention of the injury.[Method]A questionnaire on training injuries and relative situations, and review of medical records from Nov 2005 to Oct 2006 were performed in 823 soldiers and officers in a special force corps at Nov 2006. Data were input into computers after quantization, and descriptive analysis was conducted with SPSS 13.0 software. [Result]Of 823 soldiers and officers, 738 (89.67%) experienced one or more injuries in that year, 525 (63.79%) were affected in normal training, 221 (26.85%) lost more than one training day due to the injuries. Confirmed with the medical records, 213 (25.90%) lost more than one training day due to injuries. A total of 1696 injuries occurred in 823 soldiers and offices in that year, of them, 1263 injuries interfered with normal training. Injury rate was 12.78 per 100 per month. 24.12% of injuries were due to over-exertion, 15.86% due to accident and 13.33% due to environmental field. Injury occurred mainly in runing related training courses including 5km running (22.17%) and 400m obstacle running (20.87%), followed by weight lifting (9.67%). The most frequent site of injury was the lower extremities, accounted for 65.74 % of the inj urie s including the foot ( 18.75 %),the ankle (13.50%), the leg (13.56%) and the knee (14.39%), followed by low back (14.21%). [Conclusion]Persistent over-exertion basic training, the courses at high risk of injury, should be improved in focus. Lower extremities and low back, the most frequent locations of injury, should be taken as key points of prevention and treatment of training injury.
6.Treatment of giant cell tumor bone defect near knee joints with allograft bone grains and autograft bone

[Objective] To investigate the treatment in the use of allograft bone grains combined with autograft bone grafting in giant cell tumor.[Method]From March 1996 to March 2006,24 cases of giant cell tumor closed to knee were verified by pathology in this department,including 11 on thighbone and others on tibia.Before operation allografts was sheared to grains after antigen extracted by repeat freeze thawing and mechanical methods.Giant cell tumor of bone was completely erased in surgical fringe and the defect was thoroughly closed-up with allograft grains combined with cancellous bone autografts.The knee joints exercise was made for 12 weeks with external orthosis fixation.[Result]No severe complication occurred in early stage after operation in the 24 patients.All of them were followed up for 1 to 6 years with an average of 2 years and 8 months.None of them recidivated after the operation,and no insert was taken off for severe rejection,one case showed tolerant pathologic fracture.All the transplantations showed ideal appearance in X-ray photograph and the motion of joints had no significant difference than before.Curative effects were evaluated by Mankin criteria.The outcome was excellent in 20 cases,good in 4 cases.[Conclusion]Bone allograft grains combined with autograft is an effective and few rejection in filling of giant bone defect in surgical operation of giant cell tumor near knee joints.

OBJECTIVE: To study the effect of compound glycyrrhizin on hepatic function and hepatic fibrosis of patients with chronic hepatitis B. 8.Living donor kidney transplantation in child: a report of 14 cases

Objective To explore the benefits of living donor kidney transplantation in child, and to inves-tigate the procedures of renal transplantation, post-operative immunosuppressants regimens and complication management in child. Methods Clinical data, the prognosis of renal transplantation, operative procedure and immunosuppressive agents administrated in 14 children with end-stage renal disease (ESRD) undergo-ing renal transplantation were analyzed. Results The major complications in the early stage after renal transplantation were: 2 cases had acute rejection, 3 cases had severe hypernatremia, 1 case had urological infection, 3 cases had lymphocele. The major complications in the long-term recipients included: 6 cases had hypertension, 3 cases had hyperlipidemia, 4 cases had various kinds of infections, 5 cases had drug-in-duced hepatic injury. The one year survival rates of recipient/graft were all 100%. The average creatinine was 96.43 mmol/L(43-125 mmol/L) for the 14 recipients by the end of this study. All recipients thought their life quality was improved significantly. Conclusions Living donor kidney transplantation is the most ideal method to treat children with ESRD. Adult donor kidneys are suitable to the school age of recipients. The protocol of immunosuppressive therapy (prednisone plus MMF and FKS06) should be applied.
9.Long-term clinical outcome of arthroscopic anterior cruciate ligament reconstruction with remnant preservation versus remnant resection

ObjectiveTo evaluate and compare long-term clinical outcomes of arthroscopic anterior cruciate ligament (ACL) reconstruction with remnant preservation versus remnant resection.MethodsFrom October 1999 to May 2005,standard ACL reconstruction with autogenous quadrupled hamstring tendons under arthroscopy using remnant resection with transtibial techniques was conducted in 87 patients.From June 2005 to May 2010,ACL reconstruction with autogenous quadrupled hamstring tendons using outside-in bone tunnel establishment and remnant preservation was performed in 221 patients.Only the patients who underwent simple ACL reconstruction together with more than three years follow-up records were enrolled in the study,including 66 patients in remnant preservation group and 39in remnant resection group.ResultsPatients of both groups were all followed up for 36-60 months,which showed insignificant difference between groups ( P > 0.05).Compared with the preoperative data,the latest follow up data demonstrated satisfactory outcomes in both groups.The two groups showed no significant differences regarding the preoperative data including age and gender distribution,average time from injury to surgery,joint instability extent and functional score of the affected knee ( P > 0.05 ).There was no statistical difference in regards of the thigh muscle atrophy and Lachman test between groups ( P >0.05).However,the remnant preservation group was superior to the remnant resection group in aspects of the range of motion of the affected knee,Lysholm score,stability in anterior drawer test,stability in pivot shift test and international knee documentation committee (IKDC) grade ( all P < 0.05 ). ConclusionCompared with standard ACL reconstruction using remnant resection with transtibial techniques,the ACL reconstruction using outside-in bone tunnel establishment and remnant preservation has better longterm clinical outcomes including stability and function of the affected knee.
10.Definition of symptomatic units of nose

Objective:To standardize the clinical information of the nose.Methods:Based on the 1034 descriptions on the clinical manifestation of nose in the database of the clinical records of Chinese medicine in the past dynasties,according to the hypothesis of symptomatic unit,the symptomatic units,the second-level clinical characteristics and the limiting factors were summarized in the paper.Results:All the descriptions on the clinical manifestation of nose include 11 symptomatic units,7 second-level clinical characteristics and 1 limiting factor.Conclusion:Comparing with the symptoms in the authoritative works of Chinese medicine,cold-feeling of the nose while breathing was the new fi nding,nares flaring and ozena recorded in the authoritative works were the second-level clinical characteristics of polypnea and rhinorrhoea respectively.Nose diseases,such as nasal eczema,were usually diagnosed by several symptomatic units,so they were not symptomatic units.
