ObjectiveTo investigate the related risk factors ot the portal vein thrombosis (PVT) in patients with liver cirrhosis.MethodsThirty-seven liver cirrhosis patients with PVT (PVT group) and 100 liver cirrhosis patients without PVT (control group) from Jan.2007 to Dec.2009 were enrolled.The clinical data of these patients were collected and the related risk factors of PVT were analyzed.ResultsThe increase of D-dimer,severe esophageal varices(EV) and portal hypertension gastropathy(PHG) were the risk factors of PVT in liver cirrhosis patients(OR =12.872,3.916 and 2.851).No significant correlation was found between sex,platelets,albumin and PVT.ConclusionsThe increase of D-dimer,severe EV and PHG are the main risk factors of PVT.The above examinations should be used to monitor patients with liver cirrhosis for early prevention and diagnosis of PVT.

Objective The changes of CECs,EPCs and CPCs in the old type 2 diabetes mellitus retinopathy(DR)patients and the clinical purpose were discussed.Methods 30 cases of T2DM(including 10 DM cases without complications and 20 DM cases with retinopathy)and 10 cases of health controls were enrolled in this study.Results(1)The changes of CECs in DR Patients were significantly higher while the levels of EPCs and CPCs were significantly lower than those in the patients of the control group.(2)There were some corrlation beteen the level of CECs,EPCs and CPCs and the degree of diabetic retinopathy.(3)The serum EPCs levels in type 2 diabetics were negativey associated with body mass index,waist-hip ratio,abdominal circumference,FPG,triglyceride,UAER,creatinine.Conclusion The levels of EPCs and CPCs in T2DM-DR patients may be the important detected targets for early discovery and monitoring of DR.

In this study, we constructed a yeast consortium surface-display expression system by using Flo1 as an anchor protein. Endoglucanase II (EGII) and cellobiohydrolase II (CBHII) from Trichoderma reesei, and β3-glucosidase 1 (BGLI) from Aspergillus aculeatus were immobilized on Saccharomyces cerevisiae Y5. We constructed the cellulose-displaying expression yeast consortium (Y5/fEGII:Y5/fCBHII:Y5/fBGLI = 1:1:1) and investigated the enzymatic ability and ethanol fermentation. The displayed cellulolytic enzymes was stabile during the 96-h fermentation. The yeast consortium produced 0.77 g/L ethanol from 10 g/L phosphoric acid swollen cellulose (PASC) within 96 h. The yield (in grams of ethanol produced per gram of carbohydrate consumed) was 0.35 g/g, which correspond to 68.6% of the theoretical yield.

BACKGROUND: Bones are currently considered as an immune organ. A variety of immune cells that originate from the bone marrow can interact with the cells of the skeletal system to jointly regulate bone metabolism. Explorations on the pathogenesis of postmenopausal osteoporosis as well as treatment-related molecular targets and signal pathways can help prevention and treatment of the disease. OBJECTIVE: To investigate the expression profiles of immune-related genes in peripheral blood leukocytes of postmenopausal osteoporosis patients using RNA-Seq technology. METHODS: Forty female patients who had experienced menopause for 0 to 20 years and were hospitalized due to fractures were enrolled. They were divided into normal bone mass group (T >-1) and osteoporosis group (T <-2.5) by scanning with Lunar Prodigy dual-energy X-ray bone densitometer. Then, the total RNA of leukocytes in the peripheral blood samples from five patients of each group was extracted. Differentially expressed genes between two groups were detected by RNA-Seq technology followed by GO enrichment analysis. The immune-related genes were screened and analyzed by KEGG signal pathway. Peripheral blood samples from 20 patients of each group were collected and the results of KEGG PATHWAY bioinformatics analysis were verified by real-time PCR. The implementation of the study plan complied with the relevant ethical requirements of the Pudong New Area Gongli Hospital, Shanghai (approval No. 20170301). RESULTS AND CONCLUSION: Compared with the normal bone mass group, 187 genes were significantly changed in the osteoporosis group (fold change > 2), and 131 genes were up-regulated and 56 genes were down-regulated. We identified in total 29 differentially expressed immune-related genes including 25 up-regulated and 4 down-regulated ones. There was significant difference in expression between the osteoporosis and normal bone mass groups for genes, including KIR3DL1, KIR3DL2, KIR2DL4, KLRD1 and HSPA6 (P < 0.05). These differentially expressed genes are potentially important for the natural killer cell-mediated cytotoxicity by the KEGG pathway analysis. KIR3DL1, KIR3DL2, KIR2DL4, KLRD1 and HSPA6 may be closely related to the natural killer cell-mediated cytotoxicity during the occurrence of postmenopausal osteoporosis.

Objective This review was aimed to provide reference for production, management and use of laboratory animals by analyzing the test results on intestinal parasitic infections of mice and rats in different provinces from 1989 to 2013 in China.The results showed that the infection rates in clean and SPF mice and rats were reduced to 10%, being better than that in the past years, but the situation was still not optimistic for the control of flagellate parasites infections.

Objective:To optimize the extraction process of Shenqi Lixin capsules through pharmacodynamic evaluation combined with orthogonal experiments with multi-index comprehensive evaluation.Methods:The congestive heart failure(CHF) model was established by intraperitoneal injection of doxorubicin in rats,and the LVEDD,LVESD,FS and LVEF of CHF rats were used as the indicators to screen the extraction process of the samples (process A was with the whole decoction of herbs and process B was with heterophylla powder mixed with the other herbs after boiling).The single factor tests and orthogonal tests were used to optimize the extraction process by taking the contents of astragaloside and tanshinone ⅡA and the quality of the decocted material as the indices,and adding water amount,decocting times and duration as the influencing factors.Results:Pharmacodynamic experiments indicated that the improvement effects of the samples from process B on cardiac symptoms and cardiac function in CHF rats were better than that of the samples from process A.The other medicinal materials were decocted by 12-fold amount of adding water,and repeated for 12 times with one hour for each time.The average extraction rate of astragaloside and tanshinone ⅡA was 61.82% and 50.07%,respectively,which was proven by the verification experiments.The average weight of the decoction was 6.02 g.Conclusion:The optimized extraction process of Shenq Lixin capsules is scientific,reasonable,stable and reliable.

Objective To explore the clinical efficacy of laparoscope-assisted transanal total mesorectal excision (La-TaTME) for middle-low rectal cancer.Methods The retrospective cross-sectional study was conducted.The clinical data of 16 patients with middle-low rectal cancer who underwent La-TaTME in the Peking University Third Hospital from August 2015 to August 2016 were collected.Sequential surgery of La-TaTME was applied to patients in the same team,with laparoscopic surgery first and then transanal surgery.Observation indicators:(1) operation and postoperative recovery situations:conversion to open surgery,anastomosis method,operation time,volume of intraoperative blood loss,intraoperative complications,time for out-of-bed activity,time for liquid diet intake,postoperative complications and duration of postoperative hospital stay.(2) postoperative pathological situations:length of surgical specimen,tumor diameter,distance from tumor to resected distant intestinal canal,complete degree of mesorectum,circumferential resection margin,pathological T stage,pathological N stage,number of lymph node detected and tumor cell differentiation.(3) follow-up.Patients in stage Ⅲ-ⅣV of TNM stage of RC underwent postoperative adjuvant chemotherapy.Follow-up using outpatient examination was performed once every 3 months postoperatively to detect the patients' survival and tumor recurrence up to December 2016.Measurement data were represented as M (range).Results (1) Operation and postoperative recovery situations:all the 16 patients underwent successful La-TaTME without conversion to open surgery,including 10 with colorectal anastomosis,3 with colon-canalis analis anastomosis and 3 with permanent colostomy.Operation time and volume of intraoperative blood loss were 290 minutes (range,215-420 minutes) and 50 mL (range,30-100 mL),respectively.One patient had intraoperative complication,showing broken ends ischemia of sigmoid colon after dragging out resected rectum from the anus,following free splenic flexure of colon,about 5 cm ischemic sigmoid colon were resected,and descending colon-rectum anastomosis was performed.Time for out-of-bed activity and time for liquid diet intake were 1 days (range,1-3 days) and 2 days (range,1-9 days),respectively.Among 3 patients with postoperative complications (Ⅱ stage of ClavienDindo),2 with incomplete intestinal obstruction were improved by gastrointestinal decompression and total parenteral nutrition,and 1 with presacral infection was improved by drainage and antibiotic therapy.Duration of postoperative hospital stay was 7 days (range,5-21 days).(2) Postoperative pathological situations:length of surgecal specimen,tumor diameter and distance from tumor to resected distant intestinal canal were respectively 18.0 cm (range,12.0-24.0 cm),3.5 cm (range,0.5-6.8 cm) and 2.5 cm (range,1.0-5.0 cm).Evaluation of mesorectum of surgical specimen:14 patients had complete mesorectum of surgical specimen and 2 had nearly complete mesorectum.There was no residual tumor at circumferential resection margin,proximal and distal ends.Pathological T stage of 16 patients:T0 (pathological complete response after neoadjuvant therapy),T1,T2 and T3 stages were found in 1,1,4 and 10 patients,respectively.Pathological N stage:12,2 and 2 patients were detected in N0,N1 and N2 stages,respectively.Number of lymph node detected was 16 (range,6-32).Tumor cell differentiation:no tumor cell (pathological complete response after neoadjuvant therapy),high-,moderateand low-differentiated tumors were respectively detected in 1,2,7 and 6 patients.(3) Follow-up.All the patients were followed up for 12 months (range,4-16 months).There were no local tumor recurrence or distant metastasis and death.Conclusion La-TaTME may be a new,safe and effective resection for middle-low rectal cancer.

Objective To investigate the gastrointestinal imaging (GI)performance of herniation of pure abdominal omental fat (PAOF)into the esophagus hiatus(EH).Methods 7 cases of PAOF herniated into EH found by GI and MSCT were collected.The performance of GI was analyzed and compared with MSCT.Results 4 cases with large soft tissue shadow around lower segment esophagus,its density are lower,esophageal mucosa was showed coarse disorderly in the range of 2-4 cm of lower segment esophageal in the mucous membrane phase,of which 1 case with the mucosal line of esophagus at the j unction of esophagus and the superior border of the soft tissue slung up.Mild stenosis lumen of flexible wall was displayed in the filling phase,the upper bound of the lesions was often visible.3 cases with obtuse His angle,of which 1 case its change was shown with position.A more larger cystic fat density shadow was showed in MSCT right side of lower segment esophagus.3 cases were almost normal GI performance,among them 1 case of esophageal diaphragmatic ampulla lasting and a smaller cystic fat density shadow was showed in MSCT right side lower segment esophagus.The connection of the lower part of cystic fat density shadow to abdominal fat was showed all in 7 cases by MSCT MPR,and left gastric artery was shown to point to or protruded into EH by arcuate form.Conclusion A slight change of mucous membrane and lumen of lower segment esophagus which bounded above with larger and fade soft tissue density shadow and His angle obtuse variable were the special GI performance of the herniation of PAOF into EH,and the diagnose depended on MSCT.

Objective To investigate the abnormal region of gray matter and distribution in patients with Parkinson’s disease (PD),and explore the brain structural changes closely related to mild cognitive impairment(MCI).Methods 20 cases of PD-MCI, 1 7 cases of PD-nMCI and 20 normal controls (NC)were examined by 3D-T1 WI brain volume sequence.The data of three groups were analyzed using voxel-based morphometry(VBM)based on SPM8 to generate gray matter map.Results Compared to NC,decreased gray matter volume in PD-nMCI was observed in the right temporal lobe,including the middle temporal gyrus,inferior temporal gyrus,and right fusiform gyrus,uncus and left superior temporal gyrus.Compared to PD-nMCI,decreased gray matter volume in PD-MCI was observed in the bilateral midfrontal gyrus,inferior frontal gyrus,left precentral gyrus,right midtemporal gyrus,superior temporal gyrus,bilateral insular,and right cuneus,lingual gyrus,posterior cingutate.Conclusion Decreased gray volume in PD-nMCI is mainly located in the right temporal lobe and right uncus.With occurrence of MCI,abnormal brain regions are gradually extended.Gray matter atrophy in the bilateral midfrontal gyrus,inferior frontal gyrus,and left precentral gyrus are related to the MCI.

OBJECTIVETo investigate the influence of in vitro low-temperature degradation (LTD) treatment on the structural stability of 5 kinds of Y2O3 stabilized tetragonal zirconia polycrystals (Y-TZP) dental ceramics.

METHODSTZ-3YS powder was compacted at 200 MPa using cold isostatic pressure and pre-sintered at 1050 degrees C for 2 h forming presintered blocks. Specimens were sectioned into 15 mm x 15 mm x 1.5 mm slices from blocks of TZ-3YS, Vita In-Ceram YZ, Ivoclar, Cercon Smart, and Kavo Y-TZP presintered blocks, 18 slices for each brand, and then densely sintered. Specimens were divided into 6 groups and subjected to an accelerated aging test carried out in an autoclave in steam at 134 degrees C, 0.2 MPa, for 0, 1, 2, 3, 4, and 5 h. X-ray diffraction (XRD) was used to identify crystal phases and relative content of monoclinic phase was calculated. Specimens for three-point bending test were fabricated using TZ-3YS ceramics according to the ISO 6872 standard and bending strength was tested before and after aging. The polished and aging specimens of TZ-3YS and Cercon Smart zirconia ceramics were observed by atomic force microscopy (AFM) to evaluate surface microstructure.

RESULTSTetragonal-to-monoclinic phase transformation was detected for specimens of TZ-3YS, Vita In-Ceram YZ, Ivoclar, and Kavo zirconia ceramics except for Cercon Smart ceramics after aging, and the relative content of monoclinic phase was increasing with the prolonged aging time. TZ-3YS was the most affected material, Kavo took the second, and Vita and Ivoclar were similar. Aging had no significant negative effects on flexural strength of TZ-3YS with average bending strength being over 1100 MPa. The nucleation and growth of monoclinic phase were detected by AFM in surface of Cercon Smart zirconia in which monoclinic phase was not detected by XRD.

CONCLUSIONSThe results suggest that LTD of dental Y-TZP is time dependent, but the aging test does not reduce the flexural strength of TZ-3YS. The long-term clinical serviceability of dental Y-TZP needs further observation.
