2.Effect of quality control circle on blood pressure and metabolic syndrome in patients with newly diagnosed primary hypertension

Objective To assess the effect of quality control circle on blood pressure and metabolic syndrome in patients with newly diagnosed primary hypertension. Methods Review health examination in 132 cases of newly diagnosed patients with primary hypertension as the control group, routine health education was given. The 128 cases of patients with newly diagnosed primary hypertension as the intervention group and the prospective study was done.The quality control group of quality control circle was set up in order to improve treatment adherence for patients with newly diagnosed primary hypertension, and the nursing intervention was implied for the intervention group for 6 months. Blood pressure and metabolic syndrome were compared between the two groups. Results The compliance rate was improved from 56.3%(72/128) to 90.6%(116/128) after intervention in the intervention group. There was significant difference before and after intervention (χ2=62.57, P<0.01).The systolic blood pressure, diastolic blood pressure, triglyceride (TG), fasting blood glucose (FBG) and waist circumference were (151.6±11.3) mmHg (1 mmHg=0.133 kPa), (98.6±12.1) mmHg, (1.98±0.36) mmol/L, (6.37±1.25) mmol/L, (92.88±6.90) cm respectively before intervention in the intervention group and were (132.6±14.3) mmHg, (82.5 ± 12.5) mmHg, (1.53 ± 0.17) mmol/L, (5.34 ± 1.06) mmol/L, (83.29 ± 5.47) cm respectively after intervention. There was statistically significant difference before and after intervention in the intervention group(t=7.11-12.79, P<0.05). The systolic blood pressure, diastolic blood pressure, TG, FBG and waist circumference were (150.4 ± 10.6) mmHg, (99.4 ± 10.8) mmHg, (1.94 ± 0.39) mmol/L, (6.52 ± 0.29) mmol/L, (91.37±6.03) cm respectively before intervention in the control group and were (145.3±13.7) mmHg, (92.6± 10.9) mmHg, (1.86 ±0.31) mmol/L, (6.02±0.27) mmol/L, (93.86±6.52) cm respectively after intervention. There was significant difference in systolic blood pressure and diastolic blood pressure before and after intervention in the control group (t=3.38, 5.09, P<0.05). But the difference was not statistically significant in TG, FBG and waist circumference (t=1.85, 1.27, 1.93, P>0.05). The difference of systolic blood pressure,diastolic blood pressure,TG, FBG and waist circumference between two groups was statistically significant after the intervention (t=6.77-12.84,P<0.05). The intervention group was significantly better than the control group. Conclusions The quality control circle may improve treatment adherence and it is helpful to improve the metabolic syndrome and it has high clinical application value for the treatment of primary hypertension.
3.Research progress on negative regulation of adaptive immune response by NK cells

Natural killer ( NK) cells are important innate effector cells and play a vital role in maintaining homeostasis through potent cytotoxic activity and cytokine production. Recent findings show that NK cells can also shape adaptive immune responses by in-fluencing a variety of immune cells. In addition to direct interactions with other immune cells,NK cells can indirectly stimulate or inhibit adaptive immune response via influencing infected cells and pathogen load. Abundant studies have highlighted the positive regulatory functions of NK cells, while their negative regulatory functions have increasingly attracted attention in recent years. Here, we review recent findings on negative regulation of adaptive immune response by NK cells, discussing the involved effector cells and function mechanism,and demonstrate how this negative regulation influences the overall outcome of adaptive immunity in infection and tumor disease.

6.Effect of neferine on proliferation and invasion of human hepatocellular carcinoma cell line HepG2 and Bel-7402

Aim To investigate the effect of neferine on proliferation and invasion of human hepatocellular car-cinoma. Methods HepG2 and Bel-7402 cells were cultured in vitro with different concentrations of nefer-ine, then cell proliferation was observed by CCK-8 as-say; cell invasion was observed by transwell invasion assay; the protein expression of RhoA, RhoC and ROCK was detected by Western blot. Results CCK-8 results showed that neferine could significantly inhibit cell proliferation in a dose-dependent manner compared with the control group. Transwell invasion assay showed that cell invasion was significantly decreased with neferine 3 μmol · L-1 . Western blot results showed that RhoA, RhoC and ROCK protein expres-sion was decreased when neferine was co-incubated with hepatocellular carcinoma cells. Conclusion Nef-erine can inhibit proliferation and invasion of HepG2 and Bel-7402 cells, which is mediated mainly by the inhibition of RhoA, RhoC and ROCK protein expres-sion.
7.Tibial intramedullary nailing using a suprapatellar approach for the treatment of proximal tibial fractures.

OBJECTIVETo study the clinical efficacy of semiextended intramedullary nailing of the tibia using a suprapatellar approach for proximal tibial fractures.
METHODSFrom January 2013 to January 2014, a total of 16 patients with unilateral proximal tibial fractures, 14 males and 2 females, underwent closed reduction and internal fixation with TRIGEN™ META-NAIL™ via the suprapatellar approach. The average age was 42.2 years old (ranged, 26 to 57 years old). Radiographic and clinical follow-up examinations were performed at a minimum of 1 year after surgery. Measurements included surgery time, complications, bone healing time, tibial alignment, knee range of motion, pain scoring (visual analogue scale) and functional outcome (Lysholm knee score).
RESULTSThe average surgery time was (75.7±8.3) minutes (ranged, 65 to 95 minutes). No obvious complications were recorded. Average duration of follow-up was (15.6±8.1) months (ranged, 12 to 24 months). Fifteen patients attained radiological bone union for a mean time of (3.6±1.8) months (ranged, 3 to 5 months). At the final follow-up, all tibial alignments were good, and no patients complained of anterior knee pain. Mean arc of knee motion was (124.4±18.8) degrees for the affected extremity compared with (127.5±16.7) degrees for the contra-lateral knee. The total scores of Lysholm knee score ranged from 77 to 92, average 86.4±12.3.
CONCLUSIONFor proximal tibial fractures,the semiextended intramedullary nailing technique via a suprapatellar approach can obtain satisfactory clinical outcomes with simplicity in reduction and fixation, minimal complications, and good recovery of limbs function.

8.Epidemiological and etiological characteristics of hand-foot-mouth disease in Yongchuan District from 2009 to 2011

Objective To provide evidence for control and prevention of hand-foot-mouth disease(HFMD) ,we analyzed the epi-demiological and viral characteristics of HFMD in Yongchuan district from 2009 to 2012 .Methods We collected the clinical dates of HFMD cases from Diseases Infectious Department of the Yongchuan Affiliated Hospital of Chongqing Medical University during 2009 to 2012 ,and analyzed the epidemiological characteristics .Results There were 509 HFMD cases in recruitment ,including 2 se-vere cases(one was dead) .The peak incidence of HFMD occurred from May to July annually .Between 2011 and 2012 ,there was an-other peak incidence ,which occurred from November to December .Most of the patients were under 5 years-old .The number of male cases was larger than that of the female .The number of scattered children cases was larger than that of the nursery children .More cases were reported in rural area than in urban area .Coxsackie virus A16(Cox A16) and other enteroviruses were the predominant strains of HFMD ,however enterovirus 71(EV71) infected cases increased sharply between 2011and 2012 .Enterovirus 71(EV71) inflection is the main cause of severe and death of HFMD .Conclusion The prevalence of HFMD in Yongchuan district had signifi-cant seasonal ,sex and age-specific ,regional differences and etiological characteristics .It was very important to make practical man-agements to prevent occurrence of HFMD according to these epidemiological characteristics .
9.Analysis of risk factors of different locational white matter changes in elderly patients with cerebral infarction

Objective To explore risk factors of different locational leukoaraiosis in elderly patients with cerebral infarction.Methods Patients were collected who aged 60 to 85 with cerebral infarctions came to the Fourth Affiliated Hospital of China Medical University and took cerebral magnetic resonance imaging (MRI) [fluid-attenuated inversion recovery (FLAIR), T1, and T2].The patients were divided into periventricular white matter changes (PVWMCs) and deep white matter changes (DWMCs), and scored by Fazekas scale, respectively.We registered past medical history, medical systems and neurological examination, blood lipids, regular blood test, and resting blood pressure measurements in the morning.Univariate analysis was used to choose risk factors which ~fected white matter changes.After that, rank multivariate logistic regression analysis was used to test risk factors of PVWMCs and DWMCs, respectively.Results There were significant age differences in both PVWMCs and DWMCs, the degree of risk was associated with age.Conclusions PVWMCs and DWMCs risk factors vary, and we should take control of risk factors actively for the elderly in order to reduce the harm arising from WMC.
10.Spectral analysis of nearly 5 years of infectious diseases in a hospital of Chongqing city

Objective To understand epidemic data and provide evidence for prevention and control of infectious diseases in Yongchuan area .Methods We collected the clinical materials of patients hospitalized during January 2008 to December 2012 ,and retrospectively analyzed the spectrum ,classification and epidemic tendency of infectious diseases in a hospital .Results A total of 4 632 patients were admitted to the department of infectious disease .The top two diseases were always hepatitis B virus infection related diseases and tuberculosis .Fatality mainly also appeared in end-stage liver diseases and tuberculosis .Morbidity of measles and mumps increased obviously in recent years .Hand-foot-mouth disease and influenza A(H1N1) were epidemic in this area .Conclusion The main infectious diseases in Yongchuan area are hepatitis B virus infection related diseases and tuberculosis ,which still are the important points for control and prevention .Also we should strengthen prevention of other infectious diseases such as hand-foot-mouth disease ,H1N1 and the mumps in order to decrease the occurrence of these infectious diseases .
