1.Study on the effect of circadian rhythm and sleep quality on job burnout among shift nurses in 3A-level hospitals

Objective:To investigate the status of shift nurses′ circadian rhythm, sleep quality and job burnout in 3A-level hospitals, and analyze the influence of circadian rhythm and sleep quality on job burnout, so as to provide theoretical basis for reducing the level of job burnout of shift nurses.Methods:A total of 491 shift nurses were investigated with General Information Questionnaire, Circadian Type Questionnaire(CTI-11), Pittsburgh Sleep Quality Index (PSQI) and Maslach Burnout Inventory General Survey(MBI-GS).Results:The Flexibility or Rigidity score of shift nurses was 10.64±4.14, the Languid or Vigorous score was 17.67±4.80, the PSQI score was 7.47±3.66, the MBI-GS score was 51.14±15.11. The results of multiple regression analysis showed that flexibility or rigidity, languid or vigorous, sleep quality were the influencing factors of emotional exhaustion( t value was 7.415, - 5.281, 7.153, P<0.01); flexibility or rigidity, languid or vigorous, sleep quality were the influencing factors of depersonalization ( t value was 4.828, - 4.079, 4.959, P<0.01); flexibility or rigidity was the influencing factors of professional efficacy( t value was - 3.887, P<0.01). Conclusions:The job burnout of shift nurses in Tianjin 3A-level hospitals were serious. The circadian rhythm and sleep quality are the important factors that affect the job burnout of shift nurses. According to the type of circadian rhythm and sleep quality of nurses, nursing managers can take personalized measures to reduce their level of job burnout.

3.Status and relationship of emergency nurses′ disaster rescue core competence, self-efficacy and disaster preparedness in Tianjin

Objective:To understand the current status of the disaster rescue core capabilities of emergency nurses in Tianjin, analyze the correlation between nurses' core capabilities in disaster rescue and disaster preparedness and self-efficacy, and establish a multiple regression model to explore the impact of emergency nurses' disaster preparedness and self-efficacy on core capabilities of disaster rescue.Methods:A survey of 491 emergency nurses in 12 hospitals of Tianjin was conducted using the general data questionnaire, disaster rescue core competency questionnaire, nurse disaster preparedness scale, and general self-efficacy scale.Results:The total score of disaster rescue core competence of emergency nurses was at a medium level (69.98±17.85) points, the total disaster preparedness score was (186.46±44.24) points, and the general self-efficacy score was (29.04±4.86) points. Disaster readiness ( r=0.783, P<0.001) and general self-efficacy ( r=0.515, P<0.001) were positively related to the core competence of disaster rescue, which together accounted for 61.9% of the total variation in core capacity of disaster rescue. Conclusion:Nursing managers should start with nurses' own factors to improve nurses' self-efficacy. At the same time, they should pay attention to and strengthen nurses' disaster rescue nursing training, improve nurses' disaster preparedness, so as to improve emergency nurses' emergency rescue ability.
4.Effect of implantation of cardiosphere-derived cells combined with rat heart tissue-derived extracellular matrix on acute myocardial infarction in rats.

OBJECTIVETo investigate whether heart tissue-derived extracellular matrix (ECM) promotes the differentiation of cardiosphere-derived cells (CDCs) implanted in rat infracted myocardium to improve the cardiac structure and function.
METHODSRat CDCs were cultured by cardiac explant methods, and ECM was prepared by decelluariztion method. In a Wistar rat model of acute myocardial infarction established by ligating the left anterior descending branch, IMDM solution, ECM suspension, 10CDCs in IMDM solution, or 10CDCs in ECM suspension were injected into the infracted rat myocardium (6 rats in each group). The cardiac function of the rats was evaluated by cardiac ultrasonography, and the percentage of positive heart fibrosis area after infarction was determined with Masson staining. The differentiation of implanted CDCs in the infarcted myocardium was detected using immunofluorescence assay for the markers of cardiac muscle cells (α-SA), vascular endothelial cells (vWF) and smooth muscle cells (α-SMA).
RESULTSThree weeks after acute myocardial infarction, the rats with injection of CDCs in ECM showed the highest left ventricular ejection fraction (LVEF) and percentage of fraction shortening with the lowest percentage of positive heart fibrosis area; implantation of CDCs with ECM resulted in significantly higher rates of CDC differentiation into cardiac muscle cells, vascular endothelial cells and smooth muscle cell (P<0.05).
CONCLUSIONHeart-tissue derived ECM significantly promotes the differentiation of CDCs implanted in the infracted myocardium into cardiac muscle cells, vascular endothelial cells and smooth muscle cells to improve the cardiac structure and cardiac functions in rats.

5.Correlation between U2AF1 Gene Mutation Characteristics and Clinical Manifestations and Prognosis in Patients with Myelodysplastic Syndrome.

OBJECTIVE:
To investigate the correlation between U2AF1 gene mutation and clinical manifestations and prognosis in patients with myelodysplastic syndromes (MDS).
METHODS:
The clinical data of 203 MDS patients who accepted Next Generation Sequencing (NGS) was retrospectively analyzed in Nanfang Hospital, Southern Medical University from December 2012 to October 2019. According to whether the patients had U2AF1 gene mutation, the patients were divided into U2AF1 mutated group and non-mutated group, and the relationship between gene mutation characteristics and clinical manifestations and prognosis was analyzed. Then according to the difference of the mutation site of U2AF1, the patients in U2AF1 mutated group were divided into U2AF1
RESULTS:
The incidence of U2AF1 mutation in MDS patients was approximately 11.3% (23/203), and the mutation frequency of U2AF1 allele was 32.5%. The male ratio in U2AF1 mutated group was significantly higher than that in U2AF1 non-mutated group (P=0.001). There was no patient who had complex karyotypes or TP53 gene mutation in U2AF1 mutated group. There were no significant differences in ages, blood parameters, bone marrow blasts, WHO 2016 classification, IPSS-R category, chromosomal abnormalities like del(5q), -7/del(7q), del(20q), +8, and gene mutation like ASXL1, DNMT3A, RUNX1, SF3B1, and SRSF2 mutation between U2AF1 mutated group and the non-mutated group. Compared with the non-mutated group, there was no significant difference in the overall survival time (P=0.377), the time of acute myeloid leukemia (AML) transformation (P=0.681), and the response rate to hypome- thylating agents in U2AF1 mutated group (P=0.556). Besides, no differences were observed in sex, diagnosis age, WHO 2016 classification, IPSS-R category, blood parameters, overall survival time, and AML transformation time between U2AF1
CONCLUSION
The U2AF1 gene mutation dose not affect the survival time, AML transformation time, and response rate to hypomethylating agents in MDS patients. Besides, there are no statistical differences in the clinical characteristics and prognosis of MDS patients between U2AF1

6.Effects of MiR-221-Mediated Wnt/β- Catenin Signaling Pathway on Biological Activity of Childhood Acute Lymphoblastic Leukemia Cells.

OBJECTIVE:
To study the effects of miR-221 on the biological activity of childhood acute lymphoblastic leukemia cells and its mechanism.
METHODS:
Bone marrow mononuclear cells (BMNC) were isolated from bone marrow samples of ALL children diagnosed in our hospital from May 2018 to November 2018. The cells were divided into control group, miR-221-NC group and miR-221 group. After transfection according to the instructions of Lipofectamine 2000 kit, the levels of miR-221 in each group were detected by RT-PCR. Flow cytometry was used to detect the effects of miR-221 on cell cycle and apoptosis. Transwell assay was used to detect cell migration and invasion. Western blot was used to detect the effects of miR-221 on proliferating cell nuclear antigen (PCNA), Caspase 3, Cyclin D1 and MMP-9 proteins in BMNC. Luciferase reporter gene assay was used to detect the targeting relationship between miR-221 and Wnt gene.
RESULT:
The expression level of miR-221 in the miR-221 group was significantly higher than that in the control group and the miR-221-NC group (P<0.05). MTT assay showed that, after transfection for 2, 3, 4 and 5 days, the cell proliferation level in miR-221 group was significantly lower than that in the control group and the miR-221-NC group (P<0.05). The cell ratio of G/G phase was (73.25±8.1)% in the miR-221 group, which was significantly higher than that in the control group and the miR-221-NC group (P<0.05); moreover, the cell ratio of S phase in the miR-221 group was (12.37±1.6)%,which was significantly lower than that in the control group and the miR-221-NC group (P<0.05). The percentage of apoptotic cells in the miR-221 group was (24.68±3.87)%, which was significantly higher than that in the control group and the miR-221-NC group (P<0.05). Transwell cell invasion experiment showed that the number of invasive cells in the miR-221 group was 23.42±3.62, which was significantly lower than that in the control group and the miR-221-NC group (P<0.05). Transwell cell migration assay showed that the number of migrating cells in the miR-221 group was 34.86±5.32, which was significantly lower than that in the control group and the miR-221-NC group (P<0.05). The relative level of PCNA, Cyclin D1 and MMP-9 in the miR-221 group was 0.26±0.03, 0.17±3.61 and 0.14±0.02, respectively, which was significantly lower than those in the control group and the miR-221-NC group (P<0.05), while the relative level of Caspase-3 in the miR-221 group was 0.37±0.05, which was significantly higher than that in the control group and the miR-221-NC group (P<0.05). Luciferase reporter assay showed that the activity of luciferase in Wnt wild type plasmid was significantly inhibited by miR-221 (P<0.05).
CONCLUSION
miR-221 can inhibit the proliferation, migration and invasion of BMNC, moreover can promote cell apoptosis, which may be related with the inhibition of Wnt/β- catenin signaling pathway.
