1.Analysis of antibiotic resistance of Pseudomonas aeruginosa and integron Ⅰ and insertion sequences common area survey

Objective To investigate antibiotic resistance of Pseudomonas aeruginosa isolated from the patients'specimens in the hospital ,the carrying rate of integronⅠ and insertion sequences common area(ISCR)and provide reference for clinical treatment . Methods 106 isolates of Pseudomonas aeruginosa were collected and their bacterial antibiotic resistance and clinical distribution were analyzed by using WHONET5 .6 software .PCR and electrophoresis were used to screen integronⅠ and ISCR carried by the target strains .Results Clinical isolates of Pseudomonas aeruginosa mainly collected from department of pulmonary disease which accounted for 48 .11% ,and sputum specimens were the major source of the 106 isolates ,which accounted for 73 .58% .The sensitiv‐ities of Pseudomonas aeruginosa to tobramycin ,amikacin ,gentamicin were about 85% ,while to imipenem only 65 .09% .The carry‐ing rate of integronⅠ and ISCR in Pseudomonas aeruginosa was 84 .91% and 76 .42% ,respectively .Conclusion Pseudomonas aeruginosa distribution of hospital departments mainly concentrate in pulmonary department and the major type of specimen is spu‐tum .The sensitivity of Pseudomonas aeruginosa to aminoglycoside antibiotic is relatively good .IntegronⅠand ISCR in Pseudomonas aeruginosa could be associated with antibiotic resistance .
2.Analysis on pathogenic distribution and bacterial drug resistance of clinical blood infections

Objective To understand the pathogenic distribution and drug resistance in the patients with blood infections in our hospital to provide reference for the empirical medication of blood infections .Methods The cases of blood infections in our hospital during the period 2012-2014 were performed the retrospective investigation .The BD Bactec blood culture system was adopted to conduct the blood culture .The bacterial strain identification and susceptibility test was conducted by using the Microscan Walkaway 40 and the data were analyzed by adopting the WHONET5 .6 software .Results The top four of department distribution in blood infections were the digestive system department ,lung diseases department ,orthopedic department and surgery department ,account‐ing for 25 .20% ,19 .60% ,14 .70% and 10 .50% respectively .The pathogens were mainly Gram negative bacteria ,and the top three were Escherichia coli ,Klebsiella pneumoniae and non fermenting bacteria ,accounting for 44 .10% ,13 .30% and 7 .69% respective‐ly .The top three of Gram positive bacteria were coagulase negative staphylococci ,Staphylococcus aureus and Enterococcus ,account‐ing for 12 .58% ,9 .09% and 7 .69% respectively .The positive rates of ESBL in Escherichia coli and Klebsiella pneumoniae were 45 .5% and 60 .8% respectively .The detection rate of methicillin resistant coagulase negative staphylococci (MRCNS) was 55 .5% , which of methicillin-resistant staphylococcus aureus (MRSA) was 58 .0% .Conclusion The cases of blood infections in our hospi‐tal come from different wards areas ,and the digestive system department is in the majority mostly .The pathogenic bacteria are dom‐inated by Gram negative bacteria ,and the treatment should rationally use the antibacterial drugs according to the bacteria drug re‐sistance situation .
3.Status survey on control of glycolated hemoglobin A1c in type 2 diabetes mellitus of Baoding community

ObjectiveTo investigate the control rate and influence factors of glycolated hemoglobin A1C(HbA1c) of type 2 diabetes in Baoding city community.MethodsA cluster-randomized study was conducted and three communities were selected randomly.The study involved all of people aged 45 years old and above in the three communities.The type 2 diabetic patients diagnosed before,were recorded through the cross section study.Then the questionnaire was finished.All diabetic plasma HbA1c was determined by HPLC method.ResultsEighty-seven patients with HbA1c≤7% were only 18.6% in all diabetic patients.As the plasma level of HbA1c increased,occurrences of macroangiopathy and microangiopathy were both increased,by the trend test.Logistic regression analysis showed that therapeutic measures and knowledge about diabetes were main influence factors of achieving the hemoglobin Alc target of <7% in type 2 diabetes.ConclusionThere were low HbA1c control in diabetic patients of Baoding community,and knowing diabetes well and receiving insulin treatment might decrease HbA1c level apparently.
4.Manufacture and experimental study of the categorizing pedestal of multi-function apparatus in the supply room

Objective To control the nosocomial infection and avoid the second pollution of thereclaimed medical apparatus to the washing area of supply room. Methods Count, classification and place of the reclaimed medical apparatus went along at the same self-designed categorizing pedestal of the muhi-function apparatus.No absorbing cloth was needed on the table-board, the dirty water flew into the outfall of the sewer directly and automatically, the discreteness could be unpicked, washed and lis-terized after work. Results It avoided the collision of the apparatuses, shortened the time of counting effectively and reduced the chance of pollution.It also avoided the waste of the manpower, the material resources, the time and the second environmental pollution. The effects of the two table-boards after disinfection had statistical difference (P<0.01). Conclusions The categorizing pedestal of the mul-ti-function apparatus makes the reclaim and handling of the polluted apparatus more standard and to re-move the pollution more thoroughly.
5. Xinjiang sambucus sibirica Nakai bark extract promotes fracture healing

BACKGROUND: Studies have found that Sambucus sibirica Nakai can promote osteoblast division, proliferation, and secretion of alkaline phosphatase to accelerate bone matrix calcification, promote new bone formation, and promote the absorption of necrotic bone. OBJECTIVE: To observe the effects of two extracts of Xinjiang Sambucus sibirica Nakai bark in promoting fracture healing in rats. METHODS: A total of 108 male Sprague-Dawley rats, 3 months old, were randomly divided into sham-operated group, model group, Zhong Hua Die Da Wan group (0.54 g/kg, negative control group), high-, middle- and low-dose ethanol extract groups (0.314, 0.157, 0.078 g/kg), high-, middle, and low-dose water extract groups (0.340, 0.170, 0.085 g/kg). Except for the sham-operated group, the middle segment of the femoral shaft was cut off to make a model of open femoral fracture. In the sham-operated group, only the femoral shaft was exposed and no osteotomy was performed. Intragastric administration began at 1 day after modeling and continued for 6 weeks (1 time/day). At 2, 4, 6 weeks after modeling, the effects of two extracts of Sambucus sibirica Nakai bark on serum-related biochemical indicators, X-ray imaging, biomechanical indicators and hematoxylin-eosin staining were observed. RESULTS AND CONCLUSION: (1) Serum biochemical indicators: The serum calcium level of the high-dose ethanol extract group in each phase was lower than that of the sham-operated group and the model group, indicating that a faster osteogenesis in the high-dose ethanol extract group, followed by middle-dose ethanol extract group, low-dose ethanol extract group, and high-dose water extract group. The serum phosphorus level of ethanol extract groups was higher than that of sham-operated group, model group, and Zhong Hua Die Da Wan group; the serum alkaline phosphatase level of high-dose ethanol extract group at each stage was significantly higher than the other groups, indicating that a stronger osteoblast activity in the high-dose ethanol extract group, followed by the high-dose water extract group. (2) X-ray film: Fracture healing was significant in the high-dose ethanol extract group, fair in the Zhong Hua Die Da Wan group, high-dose water extract, middle-dose water extract group, middle-dose ethanol extract group, and poor in the low-dose ethanol extract group and low-dose water extract group. (3) Three-point bending test: At 6 weeks, the maximum load and maximum deflection of the femurs of the two extract groups were higher than those in the model group. The maximum load and maximum deflection of the femurs increased significantly in the high-dose ethanol extract group followed by the high-dose water extract group, middle-dose water extract group and middle-dose ethanol extract group. (4) Hematoxylin-eosin staining: Fibrous epiphysis gradually formed in the fracture area, and the bone marrow cavity was communicated. Bone healing was better in the high-dose ethanol extract and high-dose water extract groups than in the other groups. To conclude, these two extracts of Xinjiang Sambucus sibirica Nakai bark are capable of improving fracture healing. Moreover, different extraction methods and dosages of Sambucus sibirica Nakai bark demonstrate various effects on fracture healing. Compared with water extracts, ethanol extracts are better to promote fracture healing, and high-dose extracts have better effects than middle- and low-dose extracts.
6.Electrocardiogram Minnesota codings from 30 000 adult cases with Kazakh ethnicity in Xinjiang,China

Objective To observe the abnormal Minnesota code (MC) distribution and interrelated characteristic on electrocardiograms (ECGs) of the adult Kazakh population.Methods Resting ECGs and blood press of randomly sampled 30 000 adult Kazakh people in three Northern regions of Xinjiang were continuously examined and analyzed,using Minnesota code recommended by WHO as the classification of ECG.Results The overall rate of abnormal ECG findings was 248.60‰,and the main abnormality in males was 146.83‰,compared to 157.71‰ in females.The prevalence rates of abnormal ST-T changes,the total arrhythmia and atrial fibrillation (AF) were 100.03‰,71.17‰ and 2.83‰ respectively.There were statistically significant differences among the main abnormities from the three regions.Conclusion The ECGs abnormalities of adult Kazakh people were high.There was significant relation found between the main abnormalities and hypertension.The prevalence of AF was different from the domestically reported literature that calls for further study.
7.Temporal trend and spatial distribution of premature mortality burden due to chronic obstructive pulmonary disease in urban Kunming for the years 1998-2003.

OBJECTIVETo study the temporal trend and spatial distribution of premature mortality burden due to chronic obstructive pulmonary disease (COPD) in terms of years of life lost (YLL) in an urban region of Kunming for the years 1998-2003.
METHODSPan Long district was selected as the study region. YLL was used to measure premature mortality burden due to COPD. The tate of YLL per 1000 residents was calculated without age-weighting but with 3% discounting rate and broken down by year. Geographic information system (GIS) technique was used to display the spatial distribution of premature mortality burden due to COPD during the six years of study.
RESULTSDuring the years from 1998 to 2003, the premature mortality burden due to COPD decreased by years while males seemed to have a higher YLL rate of COPD than females. Central areas remained to have higher YLL/1000 population values of COPD for the six years, whereas remote areas tended to have a decreasing trend of YLL/1000 population values of COPD in the same period.
CONCLUSIONCentral areas of the city should be emphasized in further control of COPD in the urban region of Kunming.

8.The changes of vascular active substances in pulmonary embolism rats and a comparative study of anticoagulant drugs.

OBJECTIVETo establish the rat model of acute pulmonary embolism, and study the changes of vascular active substances in pulmonary embolism rats, and investigate the interventive effect of anticoagulant drugs on vascular active substances.
METHODSOne hundred and twenty-eight rats were randomly divided into four groups: control group, model group, low-molecular-weight heparin and warfarin treated group and rivaroxaban-treated group (n = 32 in each group). The method of autologous thrombosis was used to establish the animal model of acute pulmonary embolism. The animals were treated with saline or different anticoagulant drugs. The physiological and biochemical parameters were detected at different time points after embolization. The rats were killed after embolism of 24 h, 3 d, 5 d or 1 week respectively and the pathologic samples of lung tissues were collected to analyze the pulmonary pathological changes in different groups.
RESULTSRats in embolization group after blood clots injection showed shortness of breath, oral cyanosis; quicken heart rates and other symptoms. All embolization groups had pulmonary hypertension, the levels of type B natriuretic peptide (BNP) were increased significantly. The ratio of endothelin-1 (ET-1)/NO and thromboxane (TXB2) and prostacyclin (6-k-PGFla) were abnormal. After treated with effective anticoagulant drugs, the levels of BNP, ET-1, NO, TXB2 and 6-k-PGF1a were tended to the normal levels in the control group. The pulmonary hypertensions were gradually decreased. The efficacy of rivaroxaban on pulmonary embolism was the same as that of the low molecular weight heparin or warfarin.
CONCLUSIONAnticoagulation therapy can effectively improve endothelial function after pulmonary embolism, reduce pulmonary hypertension, and revise the increased BNP levels to normal levels. The efficacy of rivaroxaban is not inferior to that of low molecular weight heparin and warfarin.

9.Study on serum erythropoietin levels in patients with hematologic malignancies.

OBJECTIVETo study whether hematologic malignancy patients with anemia have a lower erythropoietin (EPO) response.
METHODSSerum EPO levels were detected by ELISA in patients with hematologic malignancies and with iron deficiency anemia (IDA). Eighty patients with hematologic malignancies, including 13 multiple myeloma (MM), 7 chronic lymphocytic leukemia (CLL) and 60 non-Hodgkin's lymphoma (NHL) were studied. Thirty of them had anemia(21 NHL,6 MM and 3 CLL). Twenty patients with IDA were studied as the control.
RESULTSHematologic malignancy patients with anemia had higher EPO levels [(97.8 +/- 183.9) IU/L] than those with normal Hb values [(27.8 +/- 85.4) IU/L; P <0.01]. In patients with IDA, serum EPO response was inversely correlated with Hb level (r= -0.5, P <0.05) , but no such inverse correlation was found in the hematologic malignancy patients with anemia (r = -0.14). After corrected for Hb level, the serum EPO levels were significantly lower in anemic patients with hematologic malignancies than in IDA patients (P = 0.032) , indicating a decreased EPO response in the former group.
CONCLUSIONAnemia associated with hematologic malignancy might result from an inappropriately low EPO response. EPO treatment for these patients may be beneficial.

10.Naloxone′s inhibition on the effects of sulfentanil on percentage ofβ1-band power of quantitative pharmaco-electroencephalography in rabbits

Objective To explore the effects of sulfentanil on the per-centage of β1 -band power of quantitative pharmaco-electroencephalo-graphy ( QPEEG ) in rabbits, and its relationship with opioid recep-tor.Methods Thirty-six healthy rabbits were divided randomly into six groups(n=6):normal group (0.9%NaCl 1 mL? kg-1), low, medium, high doses sulfentanil groups ( 1.5 , 3 , 6 μg? kg -1 ) , naloxone group (400 μg? kg-1 ) and naloxone and sulfentanil group ( naloxone 400μg? kg -1 and sulfentanil 3 μg? kg-1 ) .The percentage of each band power was respectively noted down 30 s before administration and 1, 3, 5, 10, 20, 25 min after administration.Results Compared with the baseline, the percentage ofβ1 -band power was decreased in the groups of medium, high doses sulfentanil ( P <0.05 ) , which had a negative correlation with sulfentanil dose ( P <0.05 ) . There was no obvious change in the percentage of β1 -band power in the group of naloxone compared with the baseline ( P >0.05 ) .Conclusion Sulfentanil de-creases the percentage ofβ1 -band power in a dose-dependent manner, and this effect is mediated by the opioid receptor.
