Acute suppurative thyroiditis is a rare and potentially fatal condition. We present a case of an 18-year-old Malay female who presented with one-week history of painful right sided neck swelling, fever and odynophagia. Neck CT confirms ruptured multiloculated abscess with posterosuperior extension into prevertebral space. Pus and tissue cultured Streptococcus anginosus and Eikenella corrodens with positive TB PCR. She responded well to ampicillin/sulbactam and anti-tuberculosis treatment with no evidence of residual collection from ultrasound.

Acute suppurative thyroiditis is uncommon in children. The relative rarity of thyroid infection is thought to reflect its complete fibrous encapsulation, generous vascular and lymphatic supply and high local iodine level. The bacterial involvement include the normal oropharyngeal microflora, and broad spectrum antibiotic therapy including anaerobic coverage should be utilized initially. We have experienced three cases of acute suppurative throiditis. All of them had painfull swelling on anterior cervical area and diagnosed by clinical and radiological features. Since two of them occured repeatedly, we report with brief review of literatures.

Acute suppurative thyroiditis (AST) is a rare condition, as the thyroid gland is relatively resistant to infection. Thyroid function tests are usually normal in AST. A few cases of AST associated with thyrotoxicosis have been reported in adults. We report a case of AST that was associated with thyrotoxicosis in a 70-year-old woman. We diagnosed AST with thyroid ultrasonography and fine needle aspiration of pus. The patient improved after surgical intervention and had no anatomical abnormality. Fine needle aspiration is the best method for the difficult task of differentiating malignancy and subacute thyroiditis from AST with thyrotoxicosis. Earlier diagnosis and proper treatment for AST might improve the outcome.

7.Recurrent Acute Suppurative Thyroiditis due to Piriform Sinus Fistula.
Journal of the Korean Surgical Society 2006;70(6):470-474
Acute suppurative thyroiditis is an uncommon condition because the thyroid gland is remarkably resistant to infections. In children, anatomic defects, such as a left piriform sinus fistula, predispose subjects to this infection. The number of reports on acute suppurative thyroiditis and an anterior neck abscess due to piriform sinus fistula have recently increased. The sinus may be overlooked for years, with the patient receiving multiple procedures before a proper diagnosis is made and the condition correctly treated. Herein, we report our experience of a case of recurrent suppurative thyroiditis associated with piriform sinus fistula, and reviews the embryological development of the sinus, as well as clinical symptoms and treatment of acute suppurative thyroiditis.

Korean Journal of Otolaryngology - Head and Neck Surgery 2003;46(3):246-249
The fourth branchial pouch sinus is a developmental anomaly which most freqently manifests itself by recurrent episodes of neck abscess or acute suppurative thyroiditis. The most common developmental anomalies of the branchial apparatus are second cleft remnant, which constitute almost 95% of all anomalies. The remaining 5% are comprised almost exclusively of first and third arch remnants. So the fourth arch remnants are exceedingly rare. A high index of suspicion, combined with barium swallow pharyngogram and computed tomography scan can aid in diagnosis. Complete excision of the entire epithelial tract combined with ipsilateral thyroid lobectomy remains the mainstay of treatment. Authors experienced a case of lateral neck mass which was pathologically presumed to be the fourth branchial pouch sinus. We report this case with the related literature.

This is the first case report about acute suppurative thyroiditis with thyrotoxicosis and pyriform sinus in Korea. A female patient, who was previously healthy, visited our hospital for the treatment of common cold symptoms and neck pain which developed 2 weeks before the visit. The condition of the patient did not improve through the use of antipyretics and even worsened. The patient was admitted to the hospital and was diagnosed with acute suppurative thyroiditis. The culture result of the drained fluid revealed group D Streptococcus. The patients condition was improved after using antibiotics and drainage. After recovery from acute thyroiditis, esophagography was performed and the pyriform sinus was found to be obliterated by chemical cauterization. It is very important yet difficult to differentiate acute thyroiditis with thyrotoxicosis from subacute thyroiditis. In this study, we discussed the differential diagnosis between acute thyroiditis and subacute thyroiditis with a review of literature.
