2.Surgical treatment results of simple thyroid goiter at the Hospital 198

From January 1994 to December 2003, at the Hospital 198, 697 patients (149 males, 548 females) with simple thyroid goiter, aged 16-75 years oldwere treated and operated. The age of highest incidence was 21-40 years old (79.92%). Thyroid cyst accounts for highest incidence 48.21%. According to WHO classification the incidence of surgery was mainly in the goiter of 2nd, 3rd grade. Anapathological analysis found the highest incidence 47.92% in simple colloid goiter. There were 4 cases of thyroid cancer (0.57%). General complications found in 2.44%.

3.Clinical characteristics and surgical treatment in 131 patients with thyroid carcinoma

This study was carried out on 131 patients (101 women and 30 men) with surgery for thyroid cancer at University Hospital No 103 from 1995 though March 2002. 49 patients (37.4%) were operated for the thyroid pathology before hospital admission.
Based on clinical examination and laboratory measurements, the authors found that the rate of thyroid cancer was 1-2% of all cancers. The rate of papillary thyroid carcinoma (PTC) was 62.6%, follicular (FTC): 19.8% medullary (MTC): 5.3%, squamous (STC): 2.3%, and nondifferentiated (NDTC) tumors: 9.9%. All surgical procedures were performed according to principal rules of oncologic surgery. 12 sternotomy and 5 tracheotomy during operation were performed. Accidental injuries of the esophagus (2 cases) and the trachea (3 cases) occur during the resection of tumor. After the surgical resection, the chemotherapy, radiotherapy, hormone therapy have been used in various combinations, one patient died and all survivors have been followed up

5.The variations of some bio-physiological and bio-chemical indices in patients who underwent thyroid surgery under acupuncture anesthesia in associating with supporting medicines

155 patients with thyroid tumors were operated under acupuncture anaesthesia associated with medicaments (group 1) and undergone topical anesthesia in superficiel cervical plexus area (group 2). Observation showed that in the course of vascular, operation, pulse, blood presure and respiratory rhythm increased in both two groups but in group 1, these indices were less increased than in group 2 (in average, mean pulse increased by 4 cycles/minute, max. blood pressure increased by 5mmHg, min. blood pressure 3mmHg, respiratory rythm 0,6 times/minute. After the anesthetic acupuncture, beta endorphine level increased dramatically (45,25pg/ml) versus before that (35,58pg/ml) acetyl choline level increased by 273% and catecholamin by 267% versus before that

6.Surgical management of goiter with intrathoracic extension at the Philippine General Hospital Department of Otolaryngology Head and Neck Surgery

Objective:
To describe the demographic and clinical characteristics of patients diagnosed with intrathoracic thyroid masses managed surgically in our institution, determine associated factors affecting eventual operative approaches for these patients, and assess postoperative outcomes and complications associated with surgical intervention.
Methods:
Design: Retrospective descriptive case series.
Setting:Tertiary National University Hospital.
Participants: 24 patients.
Results:
The mean age of patients diagnosed with intrathoracic goiters was 55.71 years old, with a 1:1.4 male to female ratio; with most having an intrathoracic extent of Huins Grade 1 (67%) compared to others having Huins Grade 2-3. Majority of patients pre-operatively had a Fine Needle Aspiration (FNA) Bethesda Thyroid Nodule Classification of Category II (benign); 79% of total patients underwent excision of thyroid mass utilizing a transcervical approach alone. As Intrathoracic Extension (ITE) grade increased, additional transthoracic approaches were performed; duration of operation, average estimated blood loss, length of hospital stay was also noted to increase. Majority of post operative surgical histopathology results revealed malignant thyroid masses, in contrast to pre-operative FNA. Post-operative transient hypocalcemia was the most reported immediate complication.
Conclusions
Management of intrathoracic goiter is often multidisciplinary. Referral to the thoracic vascular service is warranted for access to the thoracic inlet. Classification by grade of intrathoracic goiters is helpful to determine the most appropriate operative approach and may
be predictive of intraoperative and postoperative outcomes. Postoperative histopathology across all ITE grades mostly yield malignant results; hence, preoperative FNA results should be used with caution.

7.Present and future of robot-assisted endoscopic thyroid surgery.

OBJECTIVERobot-assisted endoscopic surgery has been increasingly accepted because of its unique three-dimensional vision and precise simulation-based technology. However, the utilization of robotic systems in thyroid surgery is limited. We conducted a systematic review to assess the application and development of robot-assisted endoscopic surgical technique in thyroid surgery.
DATA SOURCESArticles published in PubMed before June, 2011 about robot-assisted endoscopic surgery were selected.
STUDY SELECTIONOriginal articles and critical reviews selected were related to robot-assisted (thyroid) surgery or endoscopic thyroid surgery, and a total of 3540 relevant articles were retrieved and 34 were finally cited.
RESULTSRobot-assisted operation of benign thyroid diseases were successfully performed, although the operation time is too long to exhibit its advantages. Nevertheless, the superiority of robot-assisted endoscopic surgical technique compared to conventional endoscopic surgery in the treatment of thyroid carcinoma were obvious, since robotic radical thyroidectomy with central and lateral neck lymph node dissection could be achieved while maintaining operative results and cosmetic outcomes equivalent to or better than conventional endoscopic surgery. Furthermore, the learning curve duration of robot-assisted endoscopic thyroid surgery was shorter than that of conventional endoscopy, especially for the novices without any endoscopic surgical basis.
CONCLUSIONRobot-assisted endoscopic thyroid surgery, with its safety, feasibility, thoroughness, cosmetic benefits, and ability to overcome the limitations of conventional endoscopic surgery, will be further improved and applied, and is worthy of attention.

9.Value of reoperation after local resection of thyroid cancer.

OBJECTIVETo evaluate the rate of residual disease in the thyroid gland and rate of cervical lymph node metastasis after preliminary local resections in thyroid cancer.
METHODSFrom 1994 - 1999, 160 thyroid cancer patients who had previously been treated with nodulectomy or subtotal lobectomy in other hospitals were reoperated.
RESULTSThe rate of residual disease at the primary site was 42.5%. The lymph node metastatic rate at level VI was 28.1%. The rate of lymph node metastasis in those with enlarged lymph nodes in the ipsilateral internal jugular chain was 35.4%. The rate of laryngeal recurrent nerve injury was 12.5% at the first operation which is much higher than that of the second operation (1.9%).
CONCLUSIONBecause of the high rate of local residual disease, nodulectomy or subtotal lobectomy alone is not indicated for thyroid cancer. The exploration to level VI is needed for thyroid cancer. It is of utmost importance to be familiar with laryngeal recurrent nerve anatomy in thyroid surgery.

10.Ectopic thyroid tissue- A case series.

OBJECTIVE: To describe the clinical presentation and course of seven ectopic thyroid patients.
METHODS:
Design: Retrospective Chart Review
Setting: Tertiary Government Teaching Hospital
Patient: Seven patients
RESULTS: Five patients were female and two were male (ratio of 5:2). Three belonged to the 20 to 30 year-old age group, whereas two were below 10 years of age. All seven were biochemically hypothyroid and ectopic thyroid was found to be the only functioning thyroid tissue. Three patients were managed medically with levothyroxine, while ectopic thyroid was excised in four. Ectopic thyroid tissue was autotransplanted in two cases following excision.
CONCLUSION: The ages of presentation in the present series correspond with the increased physiological demand of thyroid hormone. Thyroid substitution therapy is a must in the presence of clinical and/or biochemical hypothyroidism. Surgical excision should be avoided as far as possible especially if the ectopic tissue is the only functioning thyroid in the body. Surgery is required in selected cases presenting with obstructive symptoms or hemorrhage which are unresponsive to substitution therapy. Auto transplantation of the ectopic thyroid may not provide significant benefit to the patient and more research is warranted in this aspect.
