Objective: To report an unusual presentation of thyroglossal duct cyst causing airway obstruction in an elderly man. Methods: Design: Case Report Setting: Tertiary Government Training Hospital Patient: One Results: A 71-year-old man with an anterior neck mass was brought to the Emergency Room due to progressive difficulty of breathing. A smooth, non-ulcerating right supraglottic mass obstructed the airway. Following an emergency high tracheotomy, contrast computed tomography scan of the neck revealed a hypodense mass with peripheral rim enhancement in the right supraglottis and an extralaryngeal component. Intra-operatively, a dumbbell- shaped cystic mass with a tract connected to the hyoid bone led to a Sistrunk procedure. Final histopathology findings were consistent with thyroglossal duct cyst. Conclusion It is possible for an elderly patient with impending upper airway obstruction, dysphonia, and neck mass to still have a benign and congenital thyroglossal duct cyst with intralaryngeal extension.

Thyroglossal duct cyst (TDC) is a developmental anomaly that usually appears in early childhood. The common presentation is midline swelling of the neck, which moves with both tongue protrusion and deglutition. Diagnosis is usually clinical and radiological. Fine needle aspiration cytology (FNAC) can be used as a tool for the exclusion of malignancy in adult patients. In some cases thyroid scan is done to rule out the presence or absence of the normal thyroid gland. A complete work-up is mandatory before cyst removal given that it contains only thyroid tissue. We report the case of a 32-year-old woman with only thyroid tissue in thyroglossal duct cyst.

Objective: To report a case of thyroglossal duct carcinoma with concurrent papillary thyroid carcinoma Methods: Design: Case Report. Setting: Tertiary National University Hospital. Patient: One. Results: A 46-year-old woman was diagnosed with thyroglossal duct carcinoma after undergoing a Sistrunk procedure. Due to presence of thyroid nodules, the patient underwent second stage thyroidectomy with central neck dissection which revealed papillary thyroid carcinoma. Conclusion Thyroglossal duct carcinomas are rare entities and there is no current consensus regarding their management. Difficulties arise in the diagnosis of these tumors as they present similarly to benign thyroglossal duct cysts. Most cases are diagnosed postoperatively. Proper preoperative assessment including head and neck examination, biopsy, and radiologic imaging is necessary to recognize patients who could benefit from more aggressive management.

Thyroglossal duct cyst is the most commonly encountered midline and upper cervical mass that can also be a rare form of malignancy of about <1%. This is a rare case without well-defined management and staging criteria and as such, it has been a cause of debates regarding optimal management as well as the extent of completeness of surgery from Sistrunk procedure only to Sistrunk procedure with total thyroidectomy.
Conclusion: This is a case report of a rare occurrence of a locally advanced papillary thyroid carcinoma of thyroglossal duct cyst presenting as a benign cervical mass. The management dilemma and ultimate surgical approach was carefully drawn with the patient.

OBJECTIVETo study the characteristics of thyroglossal tract cyst epidemiology and summarize how to improve the operation therapeutic effect.

METHODSThe clinic appearance, the pathological characteristics and the therapeutic effect of the 60 thyroglossal tract cyst cases were analyzed.

RESULTSThyroglossal tract cyst mostly occurred in children and teenagers. The incidence of female patients was more than that of male patients. Most of thyroglossal tract cyst located in the middle line of the neck and were explored tubular-objects and fibroropes. The histopathological feature was variform.

CONCLUSIONThe clinical appearance and pathology characteristic of thyroglossal tract cyst are variform. Thyroglossal tract cyst may be recurred easily if it is not operated well.

Thyroglossal duct cyst is the most common congenital malformation of the neck. It is generally with non-specific symptoms. In our hospital, one case of laryngeal obstruction caused by giant thyroglossal duct cyst was cured, and this case was reported for reference.

A 5 years old boy was referred to our department with complaints of a painless midline neck swelling. Clinical feature: there was a painless swelling above the middle of thyroid cartilage, it was about 2.0 cm x 0.5 cm, and it was soft. And its boundary was clear. The skin temperature is normal. B-mode ultrasound features: there is a cystic mass in the front of the neck, it is about 2.3 cm x 1.0 cm, and be apart from the skin is 0.3 cm. It is showed that another cystic mass behind this one, is about 1.9 cm x 0.7 cm. The borders are clear, and no blood flow signal is detected. The thyroid was not abnormal. Pathology results showed the wall of cyst and there were pectin in the cyst. Diagnosis: thyroglossal cyst of double cyst.

Parathyroid cysts are an uncommon cause of neck masses, which are rarely suspected before surgery. Although several theories of their origin have been proposed, none has been proven satisfactorily to the exclusion of the others. We experienced a 33-year-old euthyroid woman presenting with a mass on the jugular notch. She complained of throat discomfort. An ultrasound of the mass in the neek suggested a thyroglossal duct cyst. Treatment consisted of a neck exploration and the removal of a 4x3x3 cm cystic mass. The cystic mass contained a clear fluid with a high level of parathyroid hormones in excess of 11.4 ng/ml. The pathological diagnosis revealed a benign parathyroid cyst. This report reviews current knowledge of parathyroid cysts as well as the relevant literature.
