Dravet syndrome is a rare and catastrophic type of epilepsy in infants. Acute encephalopathy has been sporadically reported in patients with Dravet syndrome; however, the risk factors for this serious complication have not been identified. We report two patients with a clinical diagnosis of Dravet syndrome who experienced acute encephalopathy initiated by refractory status epilepticus. SCN1A mutational analysis revealed a previously reported nonsense mutation in one patient and a novel missense mutation in the other. Analysis of our cases and previously published cases revealed that patients with Dravet syndrome who have a more severe phenotype have an increased likelihood of developing acute encephalopathy compared with patients with less severe phenotypes.

Background: In 24 years, Expanded Programe on Immunization\r\n', u'(EPI) at Thua Thien Hue province achieve high efficiency, reduce remarkably infection rate of children disease in EPI. Objectives: Assessment of instruments and cold chain at City, District Health Center and Commune Health station. Assessment of knowledge and practice EPI of medical officers in districts and communes. Subjects and method: Instrument, cold chain system and officers at City, District Health Center and Commune Health station. Method: Cross-section descriptive study; Observe instruments and cold chain at Health stations and fill in available forms. Interview medical officers, observe practical manipulation and fill in available forms. Results: The rate of good knowledge varied from 61,64% to 94,55% and the rate of appropriate practice was from 45,70% to 80,92%. On average, each commune health station had 0,421 refrigeration; 0,128 ice cabinet; 0,258 cold box; 2,259 thermoses; 6,623 ice packs; 2,826 thermometers and11,321 safe boxes. All commune health stations have vaccine containing thermos; one station has no thermometer; two have no safe box and five have no ice pack. Conclusion: All commune health stations have essential instruments, cold chain. Very few health station lack of one or some types. Medical officers almost have basic knowledge about expandedimmunization, the rate of answering right theoretical questions from 61,64% to 94,55%. Practical manipulation had still many errors, rate of manipulation right only 45,70% to 80,92%. District officers manipulated right higher than commune officers.\r\n', u'

Introduction: Hepatitis C is a dangerous disease and if left untreated can have serious consequences to the patients. While hepatitis C is difficult to treat, its vaccine is still unavailable. Previous studies have showed that there is a low rate of drug users having HCV tests. This means HCV infections does not have any great importance attached to it amongst the drug user group.\r\n', u'Objectives: To discover the status of HCV infection among drug users in Hanoi and to analyse the associated factors. \r\n', u'Subjects and methods: This crossover descriptive study was conducted in Hanoi. Research objects consisted of only men within the 16 - 25 years old age group, who used heroin from 09/2005 to 12/2006. Especially, the research has indicated that 100% of HIV infected cases have also contracted HCV. This co - infectious status can be a challenge to subjects when they experience ARV treatment. Background: Child health care practices in the context of households and community is an important factor to improving the health of children. Some indexes were proposed by the World Health Organization to evaluate these practices. Objectives: To describe the knowledge and practice of pregnant women, under-5-year-old children\u2019s health care and assess the application of the survey toolkit in evaluating various indexes of healthcare practice at family and community levels. Subjects and method: By employing a structured interview, this cross-sectional survey has been conducted in Tu Liem district - Hanoi city and Tien Hai district \u2013 Thai Binh province. 120 mothers of under-5-year-old children participated in the survey. Results:98% of mothers had at least 3 antenatal visits during pregnancy. The rate of complete tetanus vaccination reached 43.3% in Tu Liem and 65% in Tien Hai. Prevalence of infant with low birth weight for age accounted for 8.3% in both districts. The percentage of children given complementary feeding at age of 6 to 9 months as the National Nutrition Program recommendations was only 30%. 65.7% and 68.5% of under-2-year-old children in Tu Liem and Tien Hai were breastfed within the first 30 minutes after delivery. There were a small proportion of mothers who gave complementary food either too early or too late, 8.3% and 8.4% in Tu Liem, 6.7% and 6.6% in Tien Hai, respectively. More than 90% of children were given normal feeding as usual during last their illness, but only 50% of children were given more fluid than usual. 98.2% of mothers knew 2 signs to immediately bring the child to health facilities. The most common injuries in under-5-year-old children were animal bites (62.5%) and falling (31.3%), however only 62.5% of mothers knew at least two measures of injury prevention. Conclusion: The toolkit for collecting data of practical index assessment can be applied at communes.

Background: In recent years, due to the outbreak of new infectious diseases, re-emerging diseases and bio-terrorist threats, the biological safety for laboratories is essential\r\n', u'Objectives: to evaluate knowledge and practices related to biosafety of researchers in microbiological laboratories\r\n', u'Subjects and method: The study was carried out in the period 2006-2007. Questionnaires and checklists were used for the direct interview the knowledge and observe the practices related to biological safety of 97 laboratory technicians from microbiology laboratories of 22 provincial centers for preventive medicine, which represent for all areas in Vietnam.\r\n', u'Results: The percentage of technicians defines correctly the hazardous groups of some common pathogens are 8.2-33%. The percentage of technicians define correctly the transmission routes of Bacillus anthracis, Staphylococus, Streptococcus are 1%, 15% and 19.6%, respectively. The opinion that thay can wear the laboratory blouse out of laboratories, bring personal belongings into the laboratory and pipeting by mouth are 21.6%, 50.5% and 23.7%, respectively. Regarding laboratory practices: The percentage of technicians does not use gloves is 37.8%; pipeting by mouth: 22.6%. Background: Streptococcus pneumonia (S.pneumoniae) is the main cause of acquired pneumonia in the community along with otitis media, sinusitis, septicemia and meningitis. Objectives: The study determined antimicrobial resistance and serotype distribution of Streptococcus pneumonia isolates from hospitalized children at Hai Phong Children's Hospital, Vietnam. Subjects and method: From June 2006 to September 2007, 80 pneumococccal isolates were tested for susceptibility to the 13 antibiotics and 84 pneumococcal isolates were serotyped. Results:Seventy-five percent of strains showed multi-drug resistance. Ninety percent of strains showed resistance to penicillin (48% intermediate and 42% fully resistant). In addition, 100% of isolates were resistant to cotrimoxazole, 74% of isolates were resistant to cephalexin; 71 % of isolates were resistant to erythroomycin and 58% were resistant to cefuroxxime. Almost all the isolates were susceptible to amoxicillin, cefotaxime, ceftriaxone, ceefepime, ofloxacin and 100% of isolates were susceptible to vancomycin. Among the 84 serotypes, 82% were included in the 23 valent pneumococcal polysaccharide vaccine including: 19F (30%), 23F (21 %), 14 (13%) and 6B (13%). Six other serotypes (13, 15C, 18, 11A, 15B and 6A) accounted for 12% of strains and 9 (11%) strains were untypeable. Conclusion: Pneumococcal antibiotics is spreading most rapidly among children in Vietnam, especially strains typs 19F and 23 F. Cytoglobin (Cygb), a stellate cell-specific globin, has recently drawn attention due to its association with liver fibrosis. In the livers of both humans and rodents, Cygb is expressed only in stellate cells and can be utilized as a marker to distinguish stellate cells from hepatic fibroblast-derived myofibroblasts. Loss of Cygb accelerates liver fibrosis and cancer development in mouse models of chronic liver injury including diethylnitrosamine-induced hepatocellular carcinoma, bile duct ligation-induced cholestasis, thioacetamide-induced hepatic fibrosis, and choline-deficient L-amino acid-defined diet-induced non-alcoholic steatohepatitis. This review focuses on the history of research into the role of reactive oxygen species and nitrogen species in liver fibrosis and discusses the current perception of Cygb as a novel radical scavenger with an emphasis on its role in hepatic stellate cell activation and fibrosis.

A number of patients cannot to afford use them due to the high price as they are imported. Meanwhile, Vietnam has high quality input sources for producing gama \u2013 globulin. \r\n', u'Objectives: To study the production process of gama - Globulin from Human Plasma and Standard gama - Globulin with Rich of Anti \u2013 HBs. \r\n', u'Subjects and method: 168 samples of human plasma from voluntary blood donors, which had been screened with for transfusion transmittable infections (TTLs), were chosen as plasma with rich of anti - HBs. The plasma with anti - HBsAg was precipitated with ethanol 25%, pH 6,9 to gain gama - globulin with rich anti - HBs, which was dried by Dutch Ly - 3 - TTE machine. Activation of anti - HBs gama - globulin was identified by a standard method of the degree of antibody specific for anti - HBsAg kit of BIORAD, \r\n', u'Results: The purity of the gama - globulin achieved was 93%, which was almost equal with the results of some foreign researchers (7.8), the activation of anti - HBs was 1:128 dilution degree. The productivity of gama - globulin gaining from 1 litter plasma was 6.0 gram. A 38-year-old female patient, with healthy history, was vaccinated with ChAdOx1 nCoV-19 (Astra Zeneca Cambridge, UK). Five days after the second injection, the patient presented headache, vertigo, then fatigue, nervousness, palpitations, shortness of breath, small amplitude tremors, and sweating episodes. Laboratory investigation revealed a suppressed thyroid-stimulating hormone (TSH), with elevated free thyroxine. However, the TSH receptor antibody and anti-thyroid peroxidase antibody were normal and thyroid-stimulating immunoglobulin negative. The patient was maintained on Metoprolol, and no specific treatment was added. After 3 months of following, the patient now feels comfortable. Our literature review found that 21 cases of subacute thyroiditis (SAT) following coronavirus disease 2019 (COVID-19) vaccines were reported. Most patients were young women who presented neck pain and systemic symptoms, with or without fever. These symptoms can appear as early (3 to 5 days), or later (1 month) after vaccination, regardless of vaccine type and mechanism of action. Laboratory tests showed decreased levels of TSH and elevated thyroid hormone. The mechanism of this event remains unknown. Further study is recommended to investigate the possible predisposing factors to developing SAT after receiving the COVID-19 vaccine.

Introduction: Laboratory capacity is needed in central Viet Nam to provide early warning to public health authorities of respiratory outbreaks of importance to human health, for example the outbreak of influenza A(H1N1) pandemic in 2009. Polymerase chain reaction (PCR) procedures established as part of a capacity-building process were used to conduct prospective respiratory surveillance in a region where few previous studies have been undertaken. Methods: Between October 2008 and September 2010, nose and throat swabs from adults and children (approximately 20 per week) presenting with an acute respiratory illness to the Ninh Hoa General Hospital were collected. Same-day PCR testing and result reporting for 13 respiratory viruses were carried out by locally trained scientists. Results: Of 2144 surveillance samples tested, 1235 (57.6%) were positive for at least one virus. The most common were influenza A strains (17.9%), with pandemic influenza A(H1N1) 2009 and seasonal H3N2 strain accounting for 52% and 43% of these, respectively. Other virus detections included: rhinovirus (12.4%), enterovirus (8.9%), influenza B (8.3%), adenovirus (5.3%), parainfluenza (4.7%), respiratory syncytial virus (RSV) (3.9%), human coronavirus (3.0%) and human metapneumovirus (0.3%). The detection rate was greatest in the 0–5 year age group. Viral co-infections were identified in 148 (6.9%) cases. Discussion: The outbreak in 2009 of the influenza A(H1N1) pandemic strain provided a practical test of the laboratory’s pandemic plan. This study shows that the availability of appropriate equipment and molecular-based testing can contribute to important individual and public health outcomes in geographical locations susceptible to emerging infections.
