1.Diagnosis of anatomic variants of the biliary tree by MRCP

Background: Magnetic resonance pancreatography (MRCP) is a safe noninvasive imaging technique that has proven to be accurate in the diagnosis of biliary tract diseases. Objective: To evaluate the role of MRCP in diagnosis of the anatomic variants of the biliary tree. Subjects and method: A retrospective study (from Augusts 2003 to April 2006, at Gia Dinh People Hospital) was conducted in 147 MRCP. All of anatomic variants of the biliary tract were classified. Results: MRCP had the sensitivity of 85.1%, the specificity of 92.1%, and the accuracy of 89.2%. Anatomic variants of the biliary tract included: type I (65%), type II (33.6%), type III (0%), type IV (1.4%). The anatomic variants of extrahepatic biliary tract that had high risk of damaged bile duct during laparoscopic cholecystectomy such as ectopic of right posterior hepatic duct (18.2%) and abnormal of junction of cystic duct and hepatic commune bile duct (2.9 - 44.6%). Conclusions: In this study, MRCP showed the modality of choices in the evaluation of hepatobiliary diseases and the anatomic variants of the biliary tree. MRCP helped the surgeons to avoid the injury of bile ducts during the operations.

2.Results of the treatment of benign prostatic hyperplasia by ILC Indigo 830e system

Background: Interstitial Laser Coagulation (ILC) Indigo 830e system is the first technique that is used in Viet Nam for the treatment of Benign Prostatic Hyperplasia (BPH) patients. Objectives: (1) To evaluate the effects of treatment of benign prostatic hyperplasia by ILC Indigo 830e system. (2) To describe peri- and post-proceduce complications. Subjects and method: The study consisted of 106 BPH patients who treated by ILC Indigo 830e system. This was a cross-section, descriptive study. Results: Overally, good and moderate outcomes was 94.8%, unsatisfactory was 5.2%. The peri- and post-proceduce complications were: urethral irritability 20.8%, prolonged catheterization 4.7%, urinary tract infections 4.7% and hematuria 0.94%. Conclusion: The treatment for benign prostatic hyperplasia patients by ILC Indigo 830e system was an effective and safe therapy, especially in elderly patient group who had multiple diseases.

3.A study on the infection status and the rate of antibiotic resistance of bacteria at Hospital 103 during the years 2006-2007

Background: Antibiotic resistance is a popular condition, especially in hospitals. The development of multi-resistance strains of bacteria is a really interesting problem for the health branch. In Vietnam, antibiotic resistance surveillance programs are frequency performed, however, the results from them are limited. Objective: To determine the prevalence, component of bacteria species and the rate of antibiotic resistance of bacterial strains, which were isolated in the hospital. Subject and methods: 446 strains of pathogenic bacteria were isolated in Hospital 103 with routine microorganism technique. Antibiogramme was conducted with improved Kirby-Bauer technique. Results and Conclusion: Among these strains isolated in Hospital 103, the majority was the gram positive Cocci with Streptococcus spp (53.14%), S.aureus (19.73%). The gram negative was modest (under 10%). All the isolated bacteria were highly resistant to antibiotics (over 50%), except Augmentin. 29.85% of S.aureus resisted with Vancomycine.

4.Estimation of left ventricular function in ventricular septal defect by tei index

Background: Ventricular Septal Defect (VSD) is one of the most common congenital cardiac diseases, accounting for 20%, and affects the left ventricular function. There is no study on the application of Tei index to evaluate left ventricular function in patients with VSD in Vietnam. Objective:To assess left ventricular function in ventricular septal defect by Tei index. Subject and Method: A cross-sectional descriptive study that involves 35 patients of VSD with an average age of 15.77+/-10.41 (A group: 19 patients of VSD with systolic pulmonary artery pressure (PAPs) < 40 mmHg and B group: 16 patients of VSD with PAPs>=40mmHg and control group: 30 normal volunteers). Results: Tei index for estimation of left ventricular function is higher in the VSD (0.50+/- 0.18) than the control (0.37 +/- 0.03) with p < 0.01, exponentially more in the VSD with PAPs >=40mmHg (0.64 +/- 0.15; p<0.01). Conclusion: Left ventricular function is worse in VSD patients in comparison to the healthy people.

5.Preliminary evaluation of the role of biomarkers in diagnosing dementia

Background: Dementia is a common pathological condition that affects older people. Most causes of dementia are Alzheimer\u2019s disease and vascular dementia. Diagnosing these conditions mostly relied on clinical patterns, but some biomarkers have been mentioned as the indicators of this condition. Objectives: 1) To evaluate the alteration of some biomarkers in cerebrospinal fluid (CSF) samples from Alzheimer\u2019s patients. 2) To compare the concentration of biomarkers in CSF samples from patients with vascular dementia and Alzheimer\u2019s disease. Subjects and method: Case group involved 41 patients who were diagnosed as AD and vascular dementia based on DSM-IV criteria. 31 matched healthy people were included in control group. All subjects were given neuro-psychological tests and thorough clinical examination. Brain CT scan and MRI were done for both groups. CSF samples were taken from patients in the study group to measure levels of some biomarkers. Results. The levels of total taurine (T-tau) and phosphorylated taurine (P-tau) 181 proteins are higher in the dementia group. The concentration of Abeta-42 is significantly different between case and control groups, but similar between vascular dementia and Alzheimer\u2019s disease patients. Conclusion: Changes in biomarkers are valuable in different diagnosis of Alzheimer\u2019s disease and other types of dementia. However, findings of CSF studies have to be considered with findings from imaging studies and clinical examination.

6.Application of techniques of molecular biology in diagnosis of fragile X syndrome

Background: Fragile X Syndrome (FXS) is the second cause of Mental Retardation (MR) and the first cause of familiar MR. This syndrome affects up to 1/4000 men and 1/8000 women. X syndrome is often diagnosed by molecular biology technique such as RCR and Southern blot. Until now there is no study on FXS in Vietnam. Objectives: This study is aimed at: (1) Determine FXS among children with MR by technique of molecular biology. (2) Determine the mutation of FMR1 gen in families having children with FXS. Subject and Method: 214 children between 6 and 16 years of age (136 male and 78 female) with MR were analyzed FMR1 gen by PCR and Southern blot techniques. Families of children with FXS were also analyzed. Result and conclusion: This is the first study on FXS using the techniques of molecular biology in Vietnam. Identified 3 children with FXS, accounting for 1.4% of MR. Children with FXS and members with full mutation and premutation were found.

7.Study of prevalence of mental retardation among children in some quarters and communes of Hue city.

Background: Mental retardation is a common pathological state in children, accounting for about 1 - 3%. Children with mental retardation should have a life of integrity and proper support. The discovery and assessment the children will help us to orient the education, assistance and early intervention for them at each location. Objectives: This study aimed at determining the prevalence of mental retardation (MR) among children in some quarters and communes of Hue city and the degrees of mental retardation in these children. Subjects and method: Screening by WHO questionnaire \u201cTen Question screen for disability\ufffd?and learning results (if possible) of children from 6 to under 16 years old in 5 quarters (urban) and 2 communes (rural) randomly selected in Hue. Diagnostics and evaluations are based on the criteria of ICD \ufffd?10. Results: The prevalence of MR in this study was 0.94% (95%CI = 0.82 \ufffd?1.07), 1.18% in rural area and 0.84% in urban area; 1.16% among boys and 0.70% among girls. Mild MR accounts for 62.67% moderate: 19.36%, severe: 11.52% and profound: 6.45%. Conclusion: The prevalence was statistically significant higher in rural area than in urban area and in boys than in girls. There were predominant percentages of severe and profound MR.

8.Laparoscopic surgery in Quang Tri General Hospital

Background: Laparoscopic cholecystectomy is the first laparoscopic surgery that was performed at Cho Ray hospital in September 1992. In 2005, Quang Tri general hospital has performed successfully the first case of laparoscopic appendectomy. Objectives: To access the preliminary results of application on endoscopic surgery in Quang Tri general hospital. Subjects and method: A prospective study was conducted on 140 patients with laparoscopic surgery or retroperitoneal surgery, was performed in Quang Tri general hospital from September, 2005 to April, 2006. Results:Among 140 patients was operated, there was only one case of postoperative intestinal obstructive complication (accounted for 0.7%), no case of death. Operative aged was between 11 and 70 years old. The average surgical time was 65 minutes for cholecystectomy, 35 minutes for appendectomy, 85 minutes for ureterolithotomy, 80 minutes for gynecological diseases. 2 cases of cholecystectomy and appendectomy changed open surgery. Conclusion: Laparoscopic surgery is a safe and effective method. It can be developed in provincial hospitals. Quang Tri general hospital has successful preliminary developed the new technique.

9.Factors relating to the time of transition from smoking to injecting of heroin among young addicts in Ha Noi

Background: In Vietnam, the HIV/AIDS pandemic is quickly spreading, almost totally in young people and among injecting drug users.The transition phase from smoking to injecting is very important for the life of injecting drug users because it is a risk factor of disease transmission. Objectives: (1) To analyze the transition phase from smoking/sniffing to injecting of the young male heroin users in Ha Noi. (2) To describe some factors related to the transition phase in these people. Subjects and method: A cross-sectional study carried out from September 2004 to September 2005 at 9 districts in Ha Noi city. The subjects were 1115 young men aged 16 to 29 years, who has been living in Ha Noi for over 6 months and smoked/sniffed heroin within the last 30 days. Results and conclusions: The average time of drug user in the 1115 subjects was 4.7 years. The majority started using drug at the age of 18.4 +/- 3.1 years. In which, 71 % consume the drugs by injection. The average time from smoking to injecting is 2.49 +/- 2.2 years. The main reasons for injecting drugs were for being faster \u2018on the height\u2019 (42%) and cheaper (40%). Some factors related to the transition phase included: age of the first heroin use, year of the first heroin use, situation for migrating into Ha Noi, knowledge level, the kind of drug used for the first time. Young people began using heroin in the year 2000 had a quicker transition phase than those using heroin before 1997 (p< 0.01).

10.Results for cardiac intervention in National Hospital of Pediatrics

Background:The progress of the cardiac intervention technique has been changed the doctors' viewpoint in the treatment for congenital cardiac. In the past years, treatment for congenital cardiac needed to surgery but nowadays, the cardiac intervention technique are being applied in many Cardiac Centers with very good results. This technique also helped to prevent the complication in cardiac operation. Objectives:This study aims to report the results for cardiac intervention in National Hospital of Pediatrics. Subjects and method:A prospective study was conducted on 298 children diagnosed with patent ductus arteriosus, atrial septal defect (ASD), ventricular septal defect, coarctation, pulmonary stenosis (PS), aortic stenosis (AS), small patent ductus arteriosus (PDA) infant with pulmonary atresia - ventricular septal defect (VSD) and Dextro- transposition of the great arteries (d- TGA) with intact ventricular septum at National hospital of Pediatric between June 2004and October 2006. Results: Transcatheter closure of PDA, ASD by Amplatzer or Coil was safe and effective. Critically ill children diagnosed with PS, AS and Coarctation may be saved by valvuloplasty and angioplasty procedures. Catheter interventions avoided the needs for surgery without scars. Catheter interventions had low complications. Conclusion: The hospitalized duration was short so the expenditures reduced.
