1.Evaluating the acceptance of the community on field training of medical students

Background: Training community-oriented general practitioners with the aims to provide the medical knowledge and skills of working in the community for medical students is appropriate with the socioeconomic context in Vietnam. Objectives: To evaluate the acceptance of the community in two districts Kim Bang (Ha Nam province) and Dong Hy (Thai Nguyen province) on field training for medical students. Subjects and method: The cross - sectional study was carried out on 300 households that were chosen randomly from 2 rural districts on April 2006. The heads of households were interviewed by open - ended questionnaires. Another 120 key informants from these two districts were invited to participate in a focus group discussion. Results: 81% of households voluntarily participated in the organization of field training for the medical students. More than 97% households paid attention to the medical students\u2019 information about health care and environmental hygiene. With medical community-based education, the people from communities receives\u2019 improvement in their knowledge of child care and personal hygiene in order to prevent diseases. Conclusion: Being exposed to health problems in communities, medical students can enhance their working and communication skills. People in the communities had a chance to improve their knowledge of disease preventions.

2.Observation of Nam Dinh virus multiplication on Aedes albopictus cell line clone C6/36 by electron microscopy

Background: Virus is one of main causes of children acute encephalitis syndrome in countries of Asia south-east. In Vietnam, apart from Japanese Encephalitis Virus which is considered as main cause of children acute encephalitis syndrome, there are other viral pathologies, of which is Nam Dinh virus. Nam Dinh virus \ufffd?a novel Arbovirus was isolated from acute encephalitis syndrome patient in northern Vietnam, 2002. The circulation of this virus has been recognized in the north, central, and highland regions. Objective: To observe the multiplication of Nam Dinh virus on the Aedes albopictus cell line clone C6/36. Materials and method: In this study the ultra-thin section method was used to observe the multiplication of the Nam Dinh virus on the Aedes albopictus cell line clone C6/36, 48 hours post-infection. Results and Conclusion: Nam Dinh encephalitis virus got used to Aedes albopictus cell line clone C6/36 and damaged cells, 24-48 hours post-infection. Its multiplication is taking place in the cytoplasm, a typical characteristic of RNA virus. Nucleocapsids of the virus were found in vacuoles of the cell. Proteincapsid of the virus was synthesized in a rough endoplasmic reticulum (rER). After assembling in the cytoplasm, the virus is released from the cell by budding and used the cell membrane as its envelope.

3.Appication of chemo- immunotherapy using M. Vaccae for newly diagnosed pulmonary tuberculosis AFB (+) in Ha Tay and Ha Noi.

Of which,the immunotherapy with M vaccea an environmental saprophyte, combined with the antituberculosis medicines all over the world has been applied since 1985, aimed to enhance the host immune responses.\r\n', u'Objectives:\r\n', u'To evaluate the results after 6 months of shortened course anittuberculosis chemotherapy by combining the immunotherapy withM vaccae.\r\n', u'Subjects and method: \r\n', u'233 newly diagnosed pulmonary tuberculosis patients with AFB smear (+) selected from Centers for Tuberculosis Control in some districts of Ha Tay and Ha Noi. They are 15 above, and not the pregnant, breastfeeding, diabetes, HIV infections, and hepatitis B (Unnecessary) \r\n', u'Dividing randomly into 2 groups, group A was in treatment of the standard 8 month chemotherapy regimen, and group B was treated in a shortned 6 month chemotherapy regimen in combination with immunotherapy by using M. vaccae vaccine. \r\n', u'Results:\r\n', u'The rates of AFB (-) sputum of group B were 98,2% and 100% respectively, the rates of the group A were 97,4% and 99% respectively after 2 and 6 months of treatment. There are 88.5% and 100% for group B, 85.6% and 98.8% for group A at the same time to achieve a negative sputum culture for M. tuberculosis. IgM responses against M. tuberculosis sonicated antigen before and after the follow-up time in Group A are not changed, whereas this level of the immunotherapy group had reduced significantly at the 6th moth (P<0.05). There is no responses in IgC to M. tuberculosis of both groups during the same follow-up period. The mean weight of patients in group B was higher than that of group A after 2 and 6 months of treatment.\r\n', u'Conclusion:\r\n', u'M. vaccae vaccine brings good benefits when be combined with the chemo-immunotherapy for newly diagnosed pulmonary tuberculosis patients with AFB smear (+). The negative sputum indicators and clinic improvements in the chemo- immunotherapy group of treatment are better than that of chemotherapy only. Background: The method of immunoelectron microscopy has been found more than 20 years. 5.Evaluation of microbial contamination in shrimp paste

Background: Food-born bacteria can be present in raw materials or contaminated foods during process and storage. Shrimp paste is a popular food in Viet Nam, but there are no studies on the hygiene and safety of this food. Objectives: To identify the microbial contamination of commercial shrimp paste available in Ha Noi City. Materials and method: A total of 50 shrimp paste samples were collected randomly from markets around Ha Noi City. Enumeration and isolation methods were used to determine the microbial contamination in these samples. Results: 100% of the samples were contaminated with Clostridium perfringens and Candida albicans. 10% of samples were contaminated with Coliform. Other pathogenic bacteria, including Escherichia coli, Salmonella, Staphylococcus aureus, Vibrio parahaemolyticus and Vibrio cholerae were not found in shrimp paste samples. Conclusion: Evaluation of microbial contamination in popular foods such as shrimp paste should be done regularly to prevent food-born diseases in the community.

6.Major enteropathogenic bacteria isolated in Hai Phong pediatric hospital from diarrhea patients

Background: Diarrhea is a leading cause of death in children under 5 years old, especially in developing countries. About 12.600 children die because of diarrhea everyday in Asia, Africa and Latin-America. Objective: To identify the main organism that causes diarrhea in children under 5 years old. Subject and Method: The etiology agents of diarrheal children under 5 years old admitted to the Pediatric Hospital in Hai Phong city were studied in the period from September 2006 to August 2007. A total of 968 children were examined for diarrheagenic Escherichia coli (DEC), Salmonella, Shigella, Vibrio, Aeromonas, Campylobacter, these pathogens as being significant bacteria associated with diarrhea. A total of 212 out of 968 cases were positive for bacteria. Result: The main pathogens were diarrheagenic Eschierichiacoli 153 (15.7%), Salmonella 12 (1.24%), Shigella 32 (3.3%). Vibirio paraheamolyticus 1(0.1%), Aeromonas 8 (0.8%), Campylobacter 6(0.6%) from rectal swabs and no Vibrio cholerae was found. The multiplex PCR assays for the identification of DEC was developed. DEC was classified into 6 categories with frequencies of EPEC 3.9%, ETEC 4.4%, EIEC 0.6%, EAggEC 6.7%, DAEC 0.1%, no EHEC was identified. Conclusion: An analysis of incidence of enteropathogens with respect to seasonal variant demonstrated that the frequencies of isolation of etiology agents mainly in July, August and September. This study also showed that diarrheagenic- Escherichia coli is the main organism causing diarrhea in children under 5 years old.

7.Classification of vibriophages isolated from environment surface water samples in Thai Binh

Background: Classification of vibriophages has been applied for epidemiological surveillance of cholera, but in Vietnam, there was virtually no knowledge about it. It is very important to determine whether the strains have common origin or different origins.Objectives: To classify vibriophages isolated from environmental surface water samples in Thai Binh in order to monitor cholera outbreaks. Subjects and method: Three vibriophages (84, 87, and 811) were collected from surface water samples in Thai Binh in 2006. Electronic microscopy was used to compare the size and morphology of Vibrio phages. Also, the structures of genomic DNA of vibriophages were compared by hind III digestion pattern and Southern blotting. Results: Vibriophages S4 and S11 isolated from Thai Binh were similar to kappa phage (a phage produced by V. cholerae 01 EI Tor strain), phage IV (a phage produced by V. cholerae 01 classical strain) in term of morphology and genome and were the same shape and DNA dimension of phage 1823 isolated from V. cholerae O139 strain in India and Bangladesh. Conclusion: Phage 84, 81 isolated from Thai Binh were new kinds of vibriophage isolated in Viet Nam. These findings were very useful for supervising and controlling cholera outbreaks in Vietnam.

8.HCV Infection in Young male drug users in Hanoi in 2007

Introduction: Hepatitis C is a dangerous disease and if left untreated can have serious consequences to the patients. While hepatitis C is difficult to treat, its vaccine is still unavailable. Previous studies have showed that there is a low rate of drug users having HCV tests. This means HCV infections does not have any great importance attached to it amongst the drug user group.\r\n', u'Objectives: To discover the status of HCV infection among drug users in Hanoi and to analyse the associated factors. \r\n', u'Subjects and methods: This crossover descriptive study was conducted in Hanoi. Research objects consisted of only men within the 16 - 25 years old age group, who used heroin from 09/2005 to 12/2006. These participants were interviewed and tested for HIV, HCV and HBV.\r\n', u'Results: HCV prevalence is 35.1 %. Especially, the research has indicated that 100% of HIV infected cases have also contracted HCV. This co - infectious status can be a challenge to subjects when they experience ARV treatment. The mean age of surveyed participants is 19. 83.6% of them have attended secondary school. 30.1 % have taken part in illegal earning activities. \r\n', u'Conclusion: HCV infection was discovered in the early drug injection stage. 9.Factors relating to the time of transition from smoking to injecting of heroin among young addicts in Ha Noi

Background: In Vietnam, the HIV/AIDS pandemic is quickly spreading, almost totally in young people and among injecting drug users.The transition phase from smoking to injecting is very important for the life of injecting drug users because it is a risk factor of disease transmission. Objectives: (1) To analyze the transition phase from smoking/sniffing to injecting of the young male heroin users in Ha Noi. (2) To describe some factors related to the transition phase in these people. Subjects and method: A cross-sectional study carried out from September 2004 to September 2005 at 9 districts in Ha Noi city. The subjects were 1115 young men aged 16 to 29 years, who has been living in Ha Noi for over 6 months and smoked/sniffed heroin within the last 30 days. Results and conclusions: The average time of drug user in the 1115 subjects was 4.7 years. The majority started using drug at the age of 18.4 +/- 3.1 years. In which, 71 % consume the drugs by injection. The average time from smoking to injecting is 2.49 +/- 2.2 years. The main reasons for injecting drugs were for being faster \u2018on the height\u2019 (42%) and cheaper (40%). Some factors related to the transition phase included: age of the first heroin use, year of the first heroin use, situation for migrating into Ha Noi, knowledge level, the kind of drug used for the first time. Young people began using heroin in the year 2000 had a quicker transition phase than those using heroin before 1997 (p< 0.01).

10.Estimating the effectiveness of nimodipin in treatment of children with intracranial hemorrhage

Background:Cerebral vasospasm is a proved reaction and nimodipin revolutionised the treatment of subarrachnoid hemorrhage in adult. However, the effectiveness of Nimodipin in cerebral vasospasm remained controversial in intracranial hemorrhage (lCH) in children. Objectives:This study aims to estimate the effectiveness of nimodipin in treatment of children with intracranial hemorrhage.Subjects and method:A descriptive, prospective study was conducted on all children patients diagnosed intracranial hemorrhage and treated at Neurology department of National Hospital for Pediatric, Vietnam from 2004 to June 2007. They applied the Transcranial Doppler sonography (TCD) in diagnosis of vasospasm in intracranial hemorrhage in children and follow-up the evolution after treatment with nimodipin. Results:The results showed that cerebral vasospasm with hight resistance index (RI) on TCD (RI = 0.78 \xb1 0.08) and cerebral ischemie with low systolic velocity (Vs = 68 \xb1 12.2 crn/s) and hight pulsality index (PI = 1.4 \xb1 0.13) were observed in 100% of cases after 3 days with ICH. Nimodipine improved clinical status (71.2%) and CT scan (52%), also RI, PI, Vs returned to normal when used nimodipin within 7 days of ICH. Conclusion: The treatment method for intracranial hemorrhage with nimodipin supplement had proved to have initial effecacy in comparison with traditional method.
