1.Interaction between serum uric acid and blood glucose and its influence on prehypertension among Chinese Adults

ObjectiveTo explore the relationship between serum uric acid and prehypertension and the effects of age,obesity,fasting glucose and lipids in Chinese adults.Methods14,451 non-hypertensive cases from a community-based health examination survey in Xuzhou,Jiangsu province of China were enrolled in this study.Blood pressure,BMI,and determination of fasting glucose,lipids and serum uric acid were measured in all cases.ResultsThe odds ratios ( OR,95% CI ) of prehypertension across increased serum uric acid after adjusting for age,sex were 1.0,1.20 ( 1.07 - 1.35 ),1.55 ( 1.36 - 1.76),1.82(1.60-2.09),2.33(2.03-2.67) (Pfor trend <0.01).The odds ratios were 1.0,1,04(0.92-1.18),1.21(1.06-1.38),1.26(1.09 - 1.45),1.36(1.17 - 1.58),( Pfor trend <0.01) after adjusting for age,sex,BMI,glucose,and lipids.In addition,fasting glucose significantly interacted with uric acid ( P for interaction < 0.01 ).Conclusions Serum uric acid was associated with prehypertension,which might be an independent metabolic risk factor.Fasting glucose may reinforce the associations.
2.Role of MCPH1 in the DNA damage response

Objective To discover the role of MCPH1 in DNA double-strand damage induced by ionizing radiation and its relationship with H2AX in esophageal cancer cell ECA109. Methods ECA109 cancer cells accepted 8 Gy 1 h after irradiation were collected for protein extraction and immunofluorescence then MCPH1 and H2AX protein expression and nuclear foci changes were observed. A stable low expression of H2AX cell lines was established and MCPH1 and H2AX protein expression and nuclear foci changes induced by ionizing radiation after silence H2AX were detected. Results (1)A stable low expression of H2AX cell lines in ECA109 cells was successfully constructed. (2)Ionizing radiation could cause the increase of r-H2AX and MCPH1 protein expression, as the same as nuclear focus increase of r-H2AX and MCPH1. (3)The protein level and nucleus focus of r-H2AX and MCPH1 were significantly reduced in ECA109 after silence H2AX. Conclusion MCPH1 is the part of DNA damage response triggered by ionizing radiation and is located in damage response downstream and can be regulated by H2AX.

4.Progress on pharmacokinetic study of antibody-drug conjugates.

Antibody-drug conjugate (ADC) is a new class of therapeutics composed of a monoclonal antibody and small cytotoxin moieties conjugated through a chemical linker. ADC molecules bind to the target antigens expressed on the tumor cell surfaces guided by the monoclonal antibody component. The binding ADC molecules can be internalized and subsequently the toxin moieties can be released within the tumor cells via chemical and/or enzymatic reactions to kill the target cells. The conjugation combines the merits of both components, i.e., the high target specificity of the monoclonal antibody and the highly potent cell killing activity of the cytotoxin moieties. However, such complexities make the pharmacokinetic and metabolic studies of ADCs highly challenging. The major challenges should include characterization of absorption, distribution, metabolism and excretion, investigation of underlying mechanisms, assessment of pharmacokinetic- pharmacodynamic relationship, and analytical method development of ADC drugs. This review will discuss common pharmacokinetic issues and considerations, as well as tools and strategies that can be utilized to characterize the pharmacokinetic and metabolic properties of ADCs.

5.Application of damage control orthopedics for the treatment of severe multiple fractures.

OBJECTIVETo investigate the application effect of damage control orthopedics for the treatment of severe multiple fractures.
METHODSFrom January 2014 to December 2016, 23 patients with severe multiple fractures were treated with the damage control orthopedics (DCO), included 14 males and 9 females with an average age of (41.57±8.29) years old ranging from 28 to 60 years old; the NISS averaged(27.70±5.44) points ranging from 18 to 40 points. As the control group, 27 patients with severe multiple fractures were treated by the early total care(ETC) technology from Jan. 2007 to Dec. 2019, included 16 males and 11 females with an average age of (38.33±9.99) years old ranging from 19 to 55 years old, the NISS averaged (31.07±6.46) points ranging from 20 to 43 points. The ICU recovery time, blood transfusion, total operation time, mortality, complication and length of hospital stay were observed and compared between two groups.
RESULTSIn the DCO group, there were 22 cases surviving and 1 case death, 3 cases of postoperative complication contained 2 cases of adult respiratory distress syndrome, 1 case pin of infection in external fixation. In ETC group, there were 25 cases surviving and 2 cases death, 10 cases of postoperative complication contained 4 cases of adult respiratory distress syndrome and 3 cases of pin infection in external fixation, 1 case of wound infection and 2 cases of multiple organ failure. There was statistically significant difference between two groups in blood transfusion in operation, the ICU recovery time, and complications(<0.05). There was no statistically significant difference in total operation time, length of hospital stay and mortality between two groups(>0.05).
CONCLUSIONSFor patients with severe multiple fractures, application of damage control orthopedics can significantly reduce the postoperative complications, ICU recovery time and intraoperative blood transfusion, provide a certain basis for clinical treatment of such patients.
6.Study on inflammatory effect of toxic raphides from Pinellia ternate and its correlation with macrophages.

OBJECTIVETo study the toxic mechanism of toxic raphides from Pinellia ternata.
METHODMouse peritoneal macrophage in vitro culture model was adopted to study dose-dependent and time-dependent curves of toxic raphides, with TNF-alpha, IL-1beta and IL-6 in supernatant as indexes. Scanning electron microscopy was used to observe the changes in surface morphology of raphides-treated macrophages. Macrophages-neutrophils co-cultured the transport model to study the effect of toxic raphides' stimulation of macrophages on neutrophils migration.
RESULTToxic raphides' stimulation of macrophages could cause the increase in the levels of TNF-alpha, IL-1beta and IL-6 released, and showed dose dependence and time dependence. Scanning electron microscopy showed that toxic raphides were swallowed by macrophages, with notable cell membrane creases, increase in the number of pseudopods and decrease in integrity of cell membranes, and could significantly induce migration of neutrophils.
CONCLUSIONThe inflammatory process induced by toxic raphides is mainly mediated by macrophages. The toxic mechanism of toxic raphides from P. ternata is that toxic raphides penetrate into tissues to activate resident macrophages, release phagocytic and inflammatory cytokines, and cause migration of neutrophils, which finally results in acute inflammatory response.

Objective To study the type and corresponding clinical characteristics of primary hemophagocytic lymphohistocytosis (HLH) associated immune gene mutations in the refractory virus infection or HLH of unknown causes. Methods From December 2009 to July 2010, the patients with refractory virus infection or HLH of unknown causes were screened for the primary HLH associated immune genes mutations by DNA sequence analysis, including PRF1, UNC13D, STX11, STXBP2, SH2D1A and XIAP. The clinical characteristics and outcomes were followed up. Results Totally 25 patients with refractory virus infection or HLH of unknown causes were investigated for the 6 genes and 13 cases were found carrying gene mutations, composing of 6 of PRF1 mutation, 3 of UNC13D, and each one of STX11,XIAP, SH2D1A and STXBP2, respectively. Among the 13 cases with gene mutations, 5 suffered from Epstein-Barr virus associated HLH( EBV-HLH), 1 human herpes virus 7 associated HLH (HHV7-HLH),1 HLH without causes, 4 chronic activated EB virus infection (CAEBV) with 1 progressing to Hodgkin's lymphoma carrying abnormal chromosome of t ( 15; 17 ) (q22; q25 ) and hyperdiploid, 2 EBV associated lymphoma. Among the other 12 patients without gene mutation, 4 suffered from EBV-HLH with 1 progressing to peripheral T lymphoma, 8 suffered from CAEBV. Conclusions Primary HLH associated immune gene mutations are critical causes of refractory virus infection of unknown causes, most patients manifest as HLH,some cases appear in CAEBV and EBV associated lymphoma. DNA sequence analysis is helpful to early diagnosis and correct decision-making for treatment.
8.Monitoring the Microtus fuscus plague epidemic in Sichuan province during 2000-2008

Objective To analyze the epidemic tendency of Microtus fascus plague during 2000-2008 in Sichuan province. Methods To investigate the plague each year according to "overall Plan of the Plague in the Whole Nation" and "Surveillance Program of Sichuan Province Plague". Results There were plague epidemic from 2000 to 2008, with the average density as 312.41/ha. 42.57% of the Microtus fuscus were infected by body Fleas. The Fleas Index was 0.88 and the Index for nest Fleas of Microtas fuscus was 55.89. Six kinds of animals were infected by not only the Microtus fuscus but also herd-dog, sand fox, Tibetan sheep, domestic cats and Cricetulus longicaulatus as well. The positive rate of live Microtus fuscus was 0.32% but 22.99% in the dead Microtus fuscus. The overall positive rate on serological test was 6.70%. There were 4 Sections, 11 species and 19 kinds Fleas identified and carrying 3 kinds of fleas, Callopsylla sparsilis, Amphipsylla tntua tutua and Rhadinopsylla dahurica vicina, with the overall infection rate as 0.054%. Conclusion Plague among Microtus fuscus showed a continuous epidemic in Sichuan province during 2000-2008.

Objective To analyzed the variant information on the indices regarding fleas from natural foci of Microtus plague in Sichuan epidemic area during 2000 to 2008.Methods Statistical and analytical methods were used on the surveillance data regarding Microtus fuscus plague.Results There were 19 flea species identified and the share of Callopsylla sparsilis was 62.79 percent while the share of Amphipsylla tuta tuta was 30.90 percent on Microtus fuscus plague.The infection rate of fleas and the flea index were the highest in October and the lowest in December and March.Species as Callopsylla sparsilis,Amphipsylla tuta tuta and Rhadinopsylla dahurica vicina could naturally infect the Yersinia pestis.Conclusion Microtus fuscus plague could become epidemic when animals and flea species were infected.We should emphasis on plague monitoring program so as to prevent the occurrence of the disease.
10.Clinical value of MR diffusion weighted imaging in prediction of pathological complete response of rectal cancer after neoadjuvant therapy.

OBJECTIVETo evaluate the application value of magnetic resonance diffusion-weighted imaging (DWI) combined with routine T2WI sequence in the determination of pathological complete response (pCR) after neoadjuvant therapy for rectal cancer.
METHODSClinical data of 51 cases with locally advanced mid-low rectal cancer undergoing neoadjuvant therapy plus radical resection in the Rectal Cancer Center at The Sixth Affiliated Hospital of Sun Yat-sen University from June 2012 to April 2013 were analyzed retrospectively. Magnetic resonance DWI and T2WI sequences scanning were performed within 1 week before neoadjuvant therapy and within 1 week before operation. Routine single T2WI sequence and DWI combined with T2WI sequence were used separately to predict the residual tumor and to compare with postoperative pathological examination. The prediction values of two methods were compared.
RESULTSOf 51 patients, 12 cases had pathological complete response (pCR). Prediction of DWI combined T2WI sequence was correct in 8 cases of pCR, whose sensitivity and specificity were higher than those of routine single T2WI sequence (66.7%, 94.9% vs. 33.3%, 84.6%). Prediction value of DWI combined T2WI sequence for pCR was significantly higher as compared to routine single T2WI sequence (AUC, 0.808 vs. 0.590, P=0.001).
CONCLUSIONCompared with the routine single T2WI sequence, DWI combined with T2WI sequence can improve the prediction accuracy of pathological complete response.
