1.Analysis on correlation between vessel cancer embolus with prognosis in patients with colorectal cancer

Objective To investigate the relationship between vessel cancer embolus with other clinicopathological features and tumor tissue KRAS gene mutation in the patients with colorectal cancer and its significance to colorectal cancer prognosis.Methods The postoperative clinicopathological data in 182 patients with colorectal cancer during 2011-2014 were retrospectively analyzed and the patients were followed up.Results Forty-five cases showed vessel cancer embolus positive of colorectal cancer,which had the rrelationship with the lymph node metastasis,pathological differentiation degree,liver metastasis,nerve infiltration and increase of CEA and CA199 levels,while had no relationship with the age,sex,tumor size,infiltration depth and pathological type.The 3-year survival rate was 51.1% in the vessel cancer embolus positive group,which was significantly lower than 61.3% in the negative group (P<0.05).However,the 3-year survival rate had no statistically significant difference between the vessel cancer embolus positive complicating KRAS gene mutation group and the vessel cancer embolus positive group(P>0.05).Conclusion Vessel cancer embolus decreases the 3-year survival rate and affects the prognosis in the patients with colorectal cancer.The survival rate in complicating KRAS gene mutation is similar.
2.Long-term versus short-term introvesical chemotherapy in patients with non-muscle-invasive bladder cancer: A systematic review and meta-analysis of the published results of randomized clinical trials.

In order to assess the effect of long-term versus short-term intravesical chemotherapy in preventing the recurrence of patients with non-muscle-invasive bladder cancer, we searched several databases with words as mesh terms and free text words to find all eligible randomized clinical trials (RCTs) for the comparison of the two strategies of instillation durations. "Observed-Expected events research (O-E)" and "Variance (V)" for calculating hazard ratio (HR) were used in Revman 5.2 software recommended by Cochrane Collabration for data analysis. Sensitivity and subgroup analysis were selected to minish heterogeneity. GRADEpro 3.6 profile recommended by Cochrane Collabration was employed for quality assessment of analyses. Finally, 13 eligible RCTs with 4216 patients were included in this review and 16 comparisons from 13 trials were involved for analysis. The pooled analysis revealed no significant difference between long-term and short-term duration [HR=0.99, 95% CI (0.89, 1.11), P=0.89]. Within the subgroup analysis, patients benefited from long-term instillations with a start regimen of one immediate instillation [HR=0.83, 95% CI (0.69, 1.00), P=0.05]. But patients were not suitable to receive long-term instillations with epirubicin (EPI) [HR=1.01, 95% CI (0.91, 1.13), P=0.78]. The progression rate was not reduced after long-term instillations [HR=0.96, 95% CI (0.66, 1.39), P=0.82]. From our results, patients should not receive introvesical chemotherapy more than half a year. In contrast, patients with one immediate instillation are preferred to have a long-term duration at least one year. Long-term instillations can not reduce the progression rate.

Objective To explore the impact of modified vacuum sealing drainage (VSD) on prevention of tension blisters during treatment of opening fracture wounds.Methods From March 2015 to February 2016,53 patients with 60 open fracture wounds were treated after surgery by VSD at our department.They were 28 males (33 wounds) and 25 females (27 wounds),aged from 24 to 65 years (average,41.1 years).The patients were divided into 2 groups.Group A (33 wounds) were subjected to traditional VSD after surgery while group B (27 wounds) to modified VSD.The mean time from injury to surgery was 5.2 hours (from 2.0 to 8.3 hours).The VSD dressing was first removed to observe whether any tension blisters would appear surrounding the dressing 5 days after surgery.Results Tension blisters appeared around 9 wounds in group A (with an incidence of 27.3%) but around one wound in group B (with an incidence of 3.7%).The difference between the 2 groups was statistically significant (P =0.037).Conclusion In treatment of open fracture wounds,the modified VSD can effectively prevent formation of tension blisters surrounding the dressing while it is used to cover a wound.
5.Clinical study of pleural effusion controlled by interrupted drainage via central venous catheter

Objective To explore the therapeutic effect,advantages and shortcomings of pleural effusion controlled by interrupted drainage via central venous catheter.Methods The research group had 52 cases with pleural effusion controHed by interrupted drainage via central venous catheter,and the control group had 50 cases with pleural effusion controlled by conventional pleuracentesis.Pleural effusion disappearing time,average hospitalization time,pleuracentesis frequency,related therapy expenses and complications of two groups were observed and compared.Results The pleuracentesis frequency,pleural effusion disappearing time,average hospitalization time,related therapy expenses and per capita incidence of complications of the research group were(1.06±1.30)times,(4.31±2.20)days,(9.87±2.30)days,(264.77±37.20)yuan and 9.62%and in the control group were(4.20±2.60)times,(9.92±3.70)days,(15.08±5.80)days,(487.62±55.56)yuan and 38.00%.The differences between two groups were particularly remarkable.Conclusion The method of pleural effusion controlled by interruptod drainage via central venous catheter is safe,eonvenient,economical and microtraumatic,and it is worth using more widely.

Objective To evaluate the clinical application of our self-designed ultradistal locking tool in the intramedullary nailing for tibial fractures.Methods From January 2014 to May 2016,175 patients with tibial fracture were treated at our department.They were 119 men and 56 women,from 19 to 73 years of age (average,46.3 years).They were divided into 2 groups according to the different targeting devices used in the intramedullary nailing.Conventional locking tools were used in the 83 patients from January 2014 to January 2015 and our self-designed new ultradistal locking tools in the 92 patients from February 2015 to May 2016.The 2 groups were compared in terms of operation time,frequency of intraoperative fluoroscopy,and successful rate of one-time locking.Results There were no significant differences between the 2 groups in general clinical data(P > 0.05),showing similarities of the 2 groups.The operation time(59.8 ±4.3 min),frequency of intraoperative fluoroscopy(11.0 ± 2.1 times),and rate of one-time successful locking[94.4% (238/252)] in the ultradistal locking group were significantly better than those in the conventional locking group [73.6 ± 5.3 min,23.0 ± 3.8 times and 85.7% (180/210),respectively] (P < 0.05).Conclusions Our new ultradistal locking tools are superior to the conventional ones in that they lead to shorter operation time,less intraoperative fluoroscopy and higher successful rate of one-time locking.Additionally,the new locking tools are easy to handle and incur no extra costs.
8.Protection and mechanism of total flavone of Hippophae rhamnoides on vascular endothelial cells.

OBJECTIVETo observe the protection of total flavones of Hippophae Rhamnoides (TFH) on vascular endothelial cells (VECs).
METHODSHuman umbilical VECs (ECV304) were used. The vascular endothelial injured cell model was prepared using hydrogen dioxide (H2O2). The cell apoptosis rate and changes of mean fluorescence intensity were detected using flow cytometry (FCM). The Caspase-3 activity in VECs was detected by Western blot.
RESULTSVEC apoptosis was induced by 200 micromol/L H2O2. TFH in different concentrations (400, 200, and 100 microg/mL) could significantly lower the cell apoptosis rate induced by H2O2 respectively (all P < 0.05), and obviously inhibit Caspase-3 activities (all P < 0.01).
CONCLUSIONSTFH could fight against H2O2 injured VECs apoptosis. Lowering the Caspase-3 expression was one of its mechanisms in protecting VECs.

9.Three-dimensional observation of the vasculature in the anterolateral thigh adipofascial flap and its clinical applications.

OBJECTIVETo investigate the blood supply patterns of the anteriorlateral thigh adipofascial flap for clinical applications.
METHODSTo investigate the blood supply patterns of the anteriorlateral thigh (ALT) adipofascial flap, 16 fresh cadavers were anatomically examined by intraarterial injection of colored latex. Three-dimensional analysis of the vasculature of the subcutaneous adipofascial tissue was performed. Sixteen patients underwent microsurgical correction with the ALT adipofascial flap. Among them, there were fifteen with hemifacial atrophy, one with micromastia.
RESULTSThe three-dimensional arterial structures of the ALT adipofascial flap were the same in all the cadaver specimens. Each layer of the adipofascial tissue was supplied by several blood vessels of the axial pattern, especially in the deep layer. Sixteen patients were successfully treated with this flap. The postoperative follow-up ranged from six months to eleven years. There was not postoperative flap necrosis or absorption of the fatty tissue. Stable restoration of the facial contour or the breast was achieved. The donor-site morbidity was minimal.
CONCLUSIONThe blood supply of the ALT adipofascial flap is reliable. A considerable amount of the fatty layer of the flap can be removed primarily. It is a preferable procedure for reconstructing soft tissue defects.
