1.The application of one-lung ventilation in open heart surgery through right subaxillary minimally invasive small incision in infants

Objective To investigate theeffect of the one-lung ventilation (OLV) on open heart surgery with right subaxillary minimally invasive small incision in infants. Methods We conducted a retrospective analysis of 316 infants undergoing open heart surgery through right axillary straight mini-thoracotomy in ourhospital from Jan-uary 2014 to December 2014. Ordinary tracheal tube was inserted into left bronchus during OVL in all patients. OLV with pressure-control mode was performed with FiO2 100% and I:E 1:1 ~ 1.5 and their respiratory rate and the pressure ventilation were adjusted to maintain SpO2 and ETCO2 in the normal range. Conventional ultrafiltration was performed after CPB and fluid restrictions were intraoperatively maintained. Blood samples were obtained for deter-mination of PaO2 and PaCO2 after induction of anesthesia (T1), at the time points: OLV 10 min (T2), OLV 30 min (T3), OLV10 min after CPB (T4) and the recovery TVL (T5). The HR, MAP, incidence of pneumothorax and me-chanical ventilation time were recorded and monitored as well. Result All patients lived through perioperative pe-riod, without pneumothorax. There were no significant differences in PaCO2, HR and MAP between time points. But PaO2 at the time point of T1 significantly lowerthan those at other time points of T2, T3, T4 and T5, but its indexes at all time points were in normal range. Conclusion The appropriate skills ofintubation, rational strategy of ventila-tion and intensive prevention of the lung may be good for the safe application of OVL in open heart surgery through right subaxillary minimally invasive small incision in infants.

Distal tibial fractures (Distal tibial fracture) is a common clinical fractures type, more for high-energy damage, fracture type is more complicated. Because of the distal tibia special anatomic structure (soft tissue is less, less blood supply), if the operation method is not suitable or fixed equipment selection is not reasonable, because fracture healing, osteomyelitis, also can damage the function of the ankle joint. Treatment shall be based on the fracture position, type, in the process of the polluted situation and surrounding soft tissue trauma injury and so on many factors that determine the way and the internal fixation methods. This paper reviews recent year different types of distal tibial fracture fixation and fixed ways of literature research, in the treatment of distal tibial fracture clinical practice to provide the reference of objective reasonably select fixation and evaluation standard.

5.Study on cellular deformation of in vitro osteoblasts' shear stress loading experiment and their mechanotransduction region

Objective To study the main mechanotransduction area and to estimate the overall shear deformation of rat osteoblasts in shear stress loading experiment based on data acquired from the in vitro rat osteoblast experimental, and to study the effects of four-point bending medium flow generated shear stress on cells. Methods Viscoelastic mechanics theory was used in the calculation process, the standard viscoelastic model was adopted for cells, and shear force on the cellular surface was simplified to be uniform. Results The cellular deformation caused by shear force was about one-tenth of that from tensile loading experiment which induced equivalent biological response. Conclusion In terms of mechanical stimulus induced biological responds, the mechanical transduction caused by cellular deformation in shear stress loading experiment is negligible, and the main transduction area is in the cellular membranes experiencing shear stress.
6.Content Determination of Chloramphenicol and Salicylic Acid in Acne Tincture by HPLC

OBJECTIVE:To establish a method for the content determination of chloramphenicol and salicylic acid in Acne tinc-ture. METHODS:HPLC method was adopted with the column of Shim-pack CLC-ODS with the mobile phase of methanol-water (50∶50,V/V)at the flow rate of 0.8 ml/min. The detection wavelength was 278 nm,column temperature was 35 ℃,and the vol-ume was 10 μl. RESULTS:The linear range was 2.0-10.0 μg/ml(r=0.999 2)for chloramphenicol and 5.0-25.0 μg/ml(r=0.999 1) for salicylic acid;the RSDs of precision,stability and repeatability tests were lower than 1%;the average recoveries chlorampheni-col and salicylic acid were respectively 99.77%(RSD=0.35%,n=9) and 99.29%(RSD=0.48%,n=9). CONCLUSIONS:The method is simple,rapid,accurate,specific,precise,and can be used for quality control of Acne tincture.
7.Clinical research about liver function of hepatolenticular degeneration combined liver fibrosis by transplantation of bone marrow stem cells

Objective:To evaluate the influence of bone marrow stem cells ( BMSCs ) in the serological indicators of hepatolenticular degeneration combined liver fibrosis.Methods:60 cases were randomly divided into 3 groups: penicillamine group, BMSCs group and BMSCs+penicillamine group.BMSCs(2 ml)were injected into vein with normal saline(100 ml) every 10 days ( 3 times for a period of treatment).Liver tissue pathology biopsy was inspected and TBIL, ALT, ALB, CHE, PDGF-BB, TGF-β1, IL-6 and TNF-αwere detected at 0,4 ,8 and 12 weeks.Results: The level of serological indicators about liver functions were reduced in every group, while the changes in BMSCs+penicillamine group were especially obvious ( P<0.05 ).Conclusion: Penicillamine combined with BMSCs was effective in the improvement of liver functions of hepatolenticular degeneration combined liver fibrosis.
8.Curative efficacy of L -glutamine gung shimron acid sodium in combination with esomeprazole in the treat-ment of pediatric peptic ulcer and its effects on immunological function and intestinal flora

Objective To investigate the clinical effect of L -glutamine gung shimron acid sodium in combi-nation with esomeprazole in the treatment of pediatric peptic ulcer and its effects on immunological function and intes-tinal flora.Methods 120 patients of pediatric peptic ulcer meeting the inclusion criteria were randomly divided into two groups equally.Besides conventional treatments,the control group was treated with esomeprazole,and the observa-tion group was treated with L -glutamine gung shimron acid sodium in combination with esomeprazole.After the course of treatment,the curative efficacy was compared after the eight -week follow -up.Before and after the treat-ment,T lymphocyte subpopulation (CD +4 ,CD +8 ,CD +4 /CD +8 )reflecting cellular immune function,intestinal flora and adverse reactions were compared.Results After the eight -week follow -up,the total effective rate of the observa-tion group was 96.7%,which was significantly higher than 86.7% of the control group (χ2 =3.93,P <0.05).After treatment,levels of CD +4 [(43.5 ±6.6)% vs (40.9 ±6.2)%],CD +4 /CD +8 [(1.72 ±0.38)vs (1.38 ±0.45)] were statistically higher and level of CD +8 [(26.2 ±3.7)% vs (30.1 ±5.0)%]was statistically lower in the obser-vation group in comparison with the control group (t =2.22,P <0.05;t =4.47,t =4.86,P <0.01).As to intestinal flora,the observation group had statistically higher concentration of lactobacillus [(7.5 ±1.3 )LonN /g feces vs (5.8 ±1.2)LonN /g feces],bifidobacterium[(8.3 ±1.1)LonN /g feces vs (6.6 ±1.4)LonN /g feces]in the feces in comparison with the control group after treatment (t =7.44,t =7.40,both P <0.05).During the treatment,there were no severe adverse reactions.Conclusion L -glutamine gung shimron acid sodium in combination with esome-prazole is effective for pediatric peptic ulcer,which can significantly increase curative efficacy,enhance cellular immune function and effectively regulate intestinal flora with minor adverse reactions.
9.Effect of modified postural drainage for treating pulmonary infection of senile patients with gastric cancer

Objective To explore clinical effect of modified postural drainage for treating pulmonary infection of senile patients with gastric cancer.Methods 107 cases of patients with gastric cancer were randomly divided into two groups:control group (n =53) and treatment group (n =54),which were respectively treated by routine postoperative therapy and nursing measures and modified postural drainage based on these routine measures,and compared daily amount of expectoration sputum,positive cases of sputum culture,restore time of postoperative blood test,postoperative fever time,cases of pulmonary infection on the postoperative tenth day and length of stay.Results Daily amount of expectoration sputum of patients in treatment group was significantly higher than that of control group [(185.7 ± 23.7) ml vs.(99.7 ± 17.6) ml] (P < 0.05).However,restore time of postoperative characters of blood test [white blood cell count (2.9±0.9) d vs (5.0±0.7)d,C-reaction protein (35±0.7)d vs.(7.4±0.6) d],postoperative fever time [(1.9±0.5) d vs.(3.6±1.4) d],cases of atelectasis (5 cases vs.19 cases) and length of stay [(1.9±0.5) d vs.(3.6±1.4) d] were respectively significantly less than those of control group (P < 0.05).Conclusions Modified postural drainage could effectively prevent postoperative atelectasis and pulmonary infection,significantly reduced incidence of postoperative atelectasis of senile patients with gastric cancer operation,and so it was worthy of clinical application.

The significance of identification of mild cognitive impairment (MCI) is to intervene as soon as possible and delay or even prevent its progression to dementia.However,due to the limitations of diagnostic methods and standards,the diagnosis of in patients with MCI is more difficult.In recent years,as an emerging functional neuroimaging technique,resting-state functional magnetic resonance imaging (rsfMRI) has developed rapidly.It has been widely used in the studies of MCI.It may be provide objective biomarkers for the diagnosis of MCI.This article reviews the advances in research on rs-fMRI in MCI.
