1.Clinical Observation of the Acute Osteomyelitis in Children

The clincal experience of thirteen patients with proven acute osteomyelitis under twelve years of age treated by surgical exprolation such as immediate decompression followed by continuous suction-irrigation. Five patients of them who had short duration from the onset improved while three patients who had long duration from the onset had persisting inflammatory change of the bone in spite of the regimen. The causating organisms were mostly staphylococcus aureus in bacterial cultures. The combination of gentamycin, minocin and penclox were the antibiotics of choice. Early diagnosis and early operation is advocated to attain good results in the treatment of acute osteomyelitis in the children.

2.Molecular docking study of "detoxification" mechanism of Chebulae fructus- Margarita intervention in stroke

Objective:To analyze the material basis, target and pathway of Chebulae fructus and Margarita in Mongolian medicine Garidi-13 Pill on stroke with the method of network pharmacology and molecular docking technology, so as to better understand its "detoxification" mechanism. Methods:TCMIP and BATMAN-TCM were used to predict the target of Chebulae fructus and Margarita composition, and GeneCards were used to search for the target of stroke. The overlapping targets of the two platforms were imported into the Metascape software for GO biological analysis and KEGG pathway analysis. The molecular docking of key molecules and targets was carried out with LEDOCK. Results:A total of 22 active components were collected and 217 targets related to stroke were predicted. Among them, 1-O-galloyl-glucose, cuprum, ellagicacid, arjungenin and corilagin were the key substances playing the role of "detoxification" of the Chebulae fructus and Margarita; IL6, TNF, HSP90AA1, PTGS2, CASP3, NR1I2, VKORC1 and ATP1A1 were the key targets playing the role of "detoxification" . These targets were significantly enriched in cell response, humoral level regulation, hemostasis, response to steroid hormones, steroid metabolism, coagulation and other biological processes, as well as nitrogen metabolism, IL-17 signaling pathway and other pathways. Molecular docking verified the accuracy of previous prediction results from computer simulation level. Conclusion:The process of Chebulae fructus and Margarita intervening strok is closely related to the elimination of harmful metabolites and calmingthe inflammatory reaction, which was not only consistent with the modern medicine on the pathological process of stroke, but also consistent with the interpretation of "evil and poison" with Mongolian medicine theory.
3.Real-time Analysis of Organic Composition of Oral and Nasal Breath Air by High Resolution Mass Spectrometry

Human exhaled breath has great application prospects,e.g.,monitoring pharmacokinetics,disease diagnosis,due to its advantages such as non-invasive and high-frequency sampling.Breath samples can be collected from the oral and nasal cavity.However,the oral and nasal environment affect the chemical composition of breath sample.Therefore,the investigation on the chemical composition of mouth-exhaled breath and nose-exhaled breath is crucial for selection of appropriate sampling strategy for individual studies.In this work,secondary electrospray ionization-high resolution mass spectrometry(SESI-HRMS)was applied to analysis of respiratory metabolomics in real time.A quantitative analysis approach was established for 9 kinds of volatile organic compounds(VOCs)e.g.2-butanone,2-pentanone,ethyl acetate,methyl methacrylate,toluene,styrene,mesitylene,isoprene and limonene.The limit of detection was 2.3?240.8 ng/m3.The intra-day(n=6)and inter-day(n=18)relative standard deviations were 0.6%?4.6%and 4.3%?12.2%,respectively.Nine healthy subjects were recruited to investigate the chemical composition of mouth-exhaled and nose-exhaled breath.The results showed the good performance in quantitative analysis of 9 VOCs in breath air.It was found that the number of unique component(m/z)detected in mouth-exhaled breath(167)was 2.2 times greater than that detected in nose-exhaled breath(76),which might result from the complex environment in oral cavity.The signal intensity of commun component(163)was significantly different between mouth-exhaled breath and nose-exhaled breath.Additionally,the elemental composition analysis showed that the proportion of polar compounds detected in nose-exhaled breath was higher than that in mouth-exhaled breath.This study demonstrated that there was significant differences in the chemical composition between mouth-exhaled and nose-exhaled breath,which provided a theoretical basis for selection of exhalation mode.
4.Optimization of antibiotics in combination.

OBJECTIVETo evaluate the antibacterial activity of Ciprofloxacin, Amikacin in combination with beta-lactams against Pseudomonas aeruginosa strains in vitro, to optimize treatment regime for antibiotics on the basis of pharmacokinetics (PK)/pharmacodynamics (PD) and drug sensitivity tests. Methods With checkerboard titration method, the minimal inhibitory concentrations (MIC) of a combination of antibiotics in different concentrations for 33 clinically isolated Pseudomonas aeruginosa strains were determined by broth dilution. Fractional inhibitory concentrations (FIC) were calculated for judging synergic effect of antibiotics.
RESULTSThe combination of Amikacin and Ceftazidime showed synergic effects (accounting for 57.6%). The combinations of Ciprofloxacin with Ceftazidime, Cefepime, Imipenem/Cilastatin, Meropenem showed synergic or additive effect. In the study with PK/PD, C(max)/MIC was the principal parameters for evaluation of aminoglycoside and fluoroquinolone antibiotics, while T > MIC was the principal parameter to be used to evaluate beta-lactams antibiotics.
CONCLUSIONWhen antibiotics are used in combination, MICs can be reduced significantly and antibacterial activities are enhanced remarkably. The combination of antibiotics results mainly in synergic or additive effect, and no inhibitory effect is observed. PK/PD analysis plays an important role in planning optimal combination regime to raise clinical efficacy.

5.Research review on pharmacological actions of Cartthamus tinctorius L

Introduction:
To collate the pharmacological literature of safflower and lay a foundation for its later development.
Methods:
Collect and sort out the literature of modern periodicals, and the study the pharmacology of Safflower in academic websites.
Conclusion
The pharmacological study and clinical medication of Mongolian medicine Safflower were studied in this paper. Safflower is one of the clinical medicines of Mongolian medicine, which is often used to treat liver diseases and is known as "the best of the liver". It has the functions of blocking blood vessels, irregular menstruation, clearing liver heat, nourishing, relieving pain and detumescence. Through collecting and sorting out the pharmacological studies of Safflower in modern periodicals and academic websites, we conclude that safflower can protect liver, expand blood vessels, anticoagulation, ischemic injury, lowering blood lipids, stimulating uterus, analgesic and sedative effects on nervous system, nourishing, anti-inflammatory, anti-oxidation, anti-cancer and anti-aging.
6.Repair of defects in lower extremities with peroneal perforator-based sural neurofasciocutaneous flaps.

OBJECTIVETo explore the operative technique and clinical results of using peroneal perforator-based sural neurofasciocutaneous flaps to repair skin and soft tissue defects in lower extremities.
METHODSFrom January 2009 to March 2012, 26 patients with skin and soft tissue defects at distal region of leg and foot were hospitalized. Among them, 9 patients suffered from tendon or bone exposure at the distal region of leg after injury due to traffic accident; 4 patients suffered from skin defects in the ankle as a result of electric injury; 8 patients suffered from chronic ulcer at the distal part of leg and foot; 5 patients suffered from plantar pressure ulcer. After debridement, soft tissue defect sizes ranged from 11 cm×5 cm to 18 cm×13 cm. According to the position and size of the defects, peroneal perforator-based sural neurofasciocutaneous flaps were designed and procured to repair the skin and soft tissue defects. The size of flaps ranged from 12 cm×6 cm to 20 cm×15 cm. Flap donor sites were closed by direct suture or skin grafting.
RESULTSTwenty-five flaps survived completely. Only one flap suffered partial margin necrosis in the size of 2 cm×1 cm, which was healed after dressing change. Patients were followed up for 6 to 12 months. The appearance and sensation of flaps were satisfactory; no ulcer occurred; the movement of lower extremities was normal.
CONCLUSIONSIt is suitable to repair the skin and soft tissue defects at distal region of leg and foot with the peroneal perforator-based sural neurofasciocutaneous flap, as it possesses reliable blood supply, long and non-bulky pedicle, and sufficient available size. The operation is also easy to perform.
