2.Two cases of primary renal sarcoma.

Primary renal sarcoma is a rare malignant mesenchymal tumor with a variety of histologic type. The clinical differential diagnosis with renal cell carcinoma may be impossible. The prognosis of primary renal sarcoma is extremely poor and majority of patients die within 2 years. We experienced two cases or primary renal sarcoma, which was diagnosed by immunohistochemical stains.

3.A Clinical Observation in Renal Angiography.

There are many radiodiagnostic methods it renal diseases; intravenous pyelogram, retrograde pyelogram, antegrade pyelogram, nephrotomogram and renal angiogram. In recent years, ultrasonogram and computed axial tomogram were developed. They help for early detection and accurate diagnosis of renal disease Abdominal aortogram was described by dos Santos, a Portugese doctor in 1923. It was greatly advanced by Seldinger through his development of percutaneous method of transfemoral catheterization in 1956. Rentl arteriogram is a highly valuable diagnostic procedure. It appears clearly that selective angiography is a safe and excellent procedure for an early detection and accurate diagnosis of benign and maligtant disease In the past, selective renal angiography has been used mainly in diagnosis of surgical disease rather than those medical disease. But now, many information are available regarding the vascular disease, pyelonephritis, glomerulonephritis and renal tuberculosis, particularly with regard to changes in the small renal arteries And it supplied various information for anatomic vascular feature of donor kidney in renal transplantation. The intent of this report is to emphasize usefulness of this procedure in the diagnosis of certain lesion of kidney which was not clearly defined by conventional methods. Radiologic findings associated with conditions and presenting symptoms or signs were analyzed in 97 cases of renal angiogram, during the period of May 1972 to August 1978.

A retrospective study was conducted on 9 patients (10 kidney) who had undergone anatrophic nephrolithotomy for staghorn calculi. There were some complications which were such as residual stone 3 cases, renovascular hypertension 1 case, urinary fistula 1 case and wound abscess 1 case, but surgical intervention is the treatment of choice for patients with staghorn calculus disease.

6.Penile fracture: experiences with conservative and surgical treatment.

We experienced with 3 cases of penile fractures for recent 2 years, one occurred during masturbation and the others occurred during coitus(one case was associated with urethral injury). While the former was treated with conservative management, the latters were treated with surgical intervention that was immediate removal of hematoma and primary closure of ruptured corpus cavernosum. Penile deformity end decrease in rigidity occurred in conservatively treated case. However, two cases of immediate surgical treatment had a excellent result without complication. Even though we describe only 3 cases and could not compare the degree of injury. our short experiences suggest that immediate surgical intervention is better than conservative management in initial treatment of penile fracture.

7.A Case of Primary Upper Ureteral Polyp.

We report a case of primary ureteral polyp causing UPJ obstruction in l9 year old boy. He presented with intermittent left flank pain and fever for 2 years. RTP, renal ultrasonography, and RGP show a dilated left renal pelvis and linear filling defect in upper ureter the operative findings were as followings : the polyp has five linear polypoid mass, which was originated from left ureteropelvic junction. the base of polyp was protruded like a ureteral tumor with some ecchymosis grossly. We resected left ureteropelvic junction and upper ureter partially, and performed pyeloplasty after confirmation of benign nature of mass by frozen section.

8.Causes or Failure and Complications of Ureteroscopic Removal of Stone.

Ureteroscopic management of ureteral calculi is an established successful technique with low complication. We performed 140 ureteroscopies for stone removal from January 1990 to April 1995. The stone were removed successfully in 131 cases (94%), and complication occurred in 37 cases (27%). In 9 cases, complete removal of stone was failed due to upward migration of stone (3 cases), not mobilization of stone (3 cases), inability to introduce ureteroscope into the intramural ureter (2 cases), inadequate illumination due to bleeding (1 case). Complications of ureteroscopic removal of stone were ureteral perforation (8 cases), tearing of ureteral mucosa (5 cases), gross hematuria (19 cases), infection (3 cases) and fever (2 cases). All complications were treated successfully with conservative treatment. We conclude that ureteroscopic removal of stone is still primary useful treatment of middle and lower ureteral stones and can be done safely if certain guide lines such as proper selection of patient, adequate use of lithotriptor, careful dilatation of ureteral orifice are kept strictly.

9.A Case of Rhabdomyosarcoma of the Bladder Developed in a Child.

Embryonic rhabdomyosarcoma is an extremely malignant tumor of the lower genitourinary tract which arises from striated muscle. It characteristically presents as a lobular grape-like mass. Rhabdomyosarcoma was subclassified by Stout, Horn and Enterline, into 3 basic type; embryonal, alveolar and pleomorphic The onset of irritative, painful and obstructed symptoms predominants in this type of tumor. Combined therapy is the key to proper treatment of this disease utilizing chemotherapy, surgery and radiotherapy, so the survival rate has continued to rise. We report a case of embryonal rhabdomyosarcoma of the bladder developed in a 6 year old boy and the relevant literature has been reviewed.
