1.An Idiopathic Pulmonary Artery Aneurysm, Treated by Aneurysmectomy and Graft Replacement

A 61-year old man was referred to our institute under a diagnosis of pulmonary aneurysm. Contrast computed tomography revealed a huge pulmonary aneurysm of 70 mm in maximal dimension at the main pulmonary trunk. No congenital heart disorders were identified on trans-thoracic or trans-esophageal echocardiography. No significant signs of pulmonary hypertension were demonstrated on right heart catheterization. Laboratory findings on admission included positive results for syphilitic antibodies. T-shaped graft replacement of the pulmonary arteries using a cardiopulmonary bypass was scheduled. The main and left pulmonary arteries were replaced with a J-Graft 26 mm in size (Japan Lifeline, Tokyo, Japan). Then, the right pulmonary artery was reconstructed with the rest of the J-Graft, and anastomosed to the side of the newly reconstructed main and left pulmonary arteries. His postoperative course was generally uneventful. Pathological findings of the excised aneurysmal walls revealed true aneurysmal formation with no specific inflammatory changes. This case was considered to be an idiopathic pulmonary aneurysm without congenital heart disorders, pulmonary hypertension, and pathologically inflammatory reactions of aneurysmal walls.
2.Two Cases of Unilateral Pulmonary Edema after Heart Surgery : Successful Strategy Using Veno-venous Extracorporeal Membrane Oxygenation

We report 2 patients with unilateral pulmonary edema after heart surgery who were successfully treated using venovenous extracorporeal membrane oxygenation (VV ECMO). Case 1 : A 35-year-old woman presented with dyspnea. Echocardiography showed severe mitral regurgitation (MR) and tricuspid regurgitation (TR) and therefore, mitral valve plasty (MVP) and tricuspid annular plasty (TAP) were performed via right thoracotomy. After weaning from cardiopulmonary bypass, respiratory failure occurred with expectoration of foamy sputum and it was difficult to maintain oxygenation. Therefore, we performed VV ECMO to maintain oxygenation. A chest X-ray film after surgery showed ipsilateral pulmonary edema. After weaning from VV ECMO, deep venous thrombosis occurred and therefore we inserted an IVC filter. Case 2 : A 67-year-old man, who had previously received aortic valve replacement experienced dyspnea and visited our hospital. Echocardiography showed an aortic root abscess, and therefore Bentall operation was performed. After weaning from cardiopulmonary bypass, oxygenation was difficult to maintain, and therefore we performed VV ECMO. A chest X-ray film post operatively showed right ipsilateral pulmonary edema. The patient was weaned from VV ECMO 5 days post operatively and was discharged 60 days post operatively. We believe that VV ECMO can be beneficial for patients with respiratory failure after heart surgery, but complications related to this approach such as DVT should also be considered.
3.Trial of the Treatment of Kampo Medicines for Thalamic Pain

Thalamic pain is known as intractable central pain caused by thalamic bleeding and infarction. Although some physical and surgical therapies have been tried, there is no established method for its improvement. We attempted the treatment of 6 cases of thalamic pain, and 4 of them showed improvements in their symptoms. Our patients were aged 27-70 years, 4 men and 2 women. Their diagnoses were 3 cases of right thalamic bleedings, 1case of left thalamic bleeding, and 2 cases of right thalamic infarctions. The periods from onset to consulting our department were from6months to 12 years. In the 4 cases whose symptoms improved, the symptoms had almost disappeared or were decreased by at least 40%. These improved cases used formulas containing uzu or bushi. Yokukansankachimpihange was effective for two cases with mental disturbances. One case showing major improvement was treated by only the formula against oketsu. The cases whose symptoms had continued for a long time were more intractable, with the symptoms remaining to some degree in spite of their decreasing tendency. Finally, in intractable cases, their symptoms remained fixed and they had severe paralysis.
4.Development of a Novel Database That Rapidly Provides Valuable Information Efficiently Aimed at Improving the Safety of the Simple Suspension Method

Objective: Many patients in Kagawa University Hospital are administered medicines prepared by the simple suspension method. Pharmacists in charge of these patients receive inquiries from doctors and nurses regarding the suitability of medicines for the simple suspension method. Answering these inquiries is complicated and time-consuming as multiple data sources need to be searched. In order to simplify these complicated procedures, we herein attempted to develop a novel database to provide valuable information that could contribute to the safe performance of the simple suspension method, and evaluated its usefulness.
Method: The specifications of the database were determined by analyzing previously answered inquiries. To evaluate the usefulness of the database, we used test prescriptions and compared the amount of time required to gather information using the database and the conventional method, i.e., using books alone. We also analyzed previous prescriptions with the database in order to determine what kinds of problems could be detected.
Results: The investigation of previous prescriptions indicated that some medicines needed to be examined not only for their suitability for the simple suspension method, but also their incompatibility. Therefore, we added a feature regarding the incompatibility of medicines to the database. The time required to gather the information needed to answer the test prescription was shorter with our database than with the conventional method. Furthermore, the database improved the detection of medicines that require particular attention for their properties including incompatibility. An analysis of previous prescriptions using our database indicated the possibility of incompatibility in half of the previous prescriptions examined.
Conclusion: Our database could rapidly provide information related to the simple suspension method, including the incompatibility of medicines.
5.Impact of Spinal Correction Surgeries with Osteotomy and Pelvic Fixation in Patients with Kyphosis Due to Osteoporotic Vertebral Fractures

Combination of retrospective and prospective study. We aimed to compare the clinical outcomes between local fixation surgery and spinopelvic fixation surgery for the treatment of kyphosis secondary to osteoporotic vertebral fractures with spinopelvic malalignment. The clinical characteristics of patients with rigid kyphosis due to osteoporotic vertebral fracture differ from that of middle-aged patients with vertebral fractures in terms of bone fragility and presence of spinopelvic malalignment. Little is known about the surgical strategies for these deformities, most especially the extent of fusion of vertebra involved. We analyzed 24 patients with vertebral osteotomy at the level of the fracture and spinal fixation without pelvic fixation (local group), and 22 patients with vertebral osteotomy and pelvic fixation (pelvic group). Radiographic parameters, the incidence of proximal junctional kyphosis (PJK), distal junctional kyphosis (DJK), rod fractures, and the Oswestry Disability Index (ODI) were compared between the two groups over a 2-year follow-up period. In the pelvic group, postoperative spinopelvic parameters significantly improved, with the improvements maintained. No remarkable changes in spinopelvic parameters were seen in the local group. The mean ODI scores 2 years after surgery were 45.3 and 33.0 in the local and pelvic group, respectively ( For patients with rigid kyphosis due to osteoporotic vertebral fractures, better spinopelvic alignment and health-related quality of life can be achieved through extensive corrective surgery with pelvic fixation.

8.Selective Angiography to Detect Anterior Spinal Artery Stenosis in Thoracic Ossification of the Posterior Longitudinal Ligament

Methods:
This study prospectively examined nine T-OPLL patients who underwent posterior thoracic decompression with kyphosis correction and instrumented fusion at Hamamatsu University School of Medicine between 2017 and 2019. All underwent preoperative selective angiography to detect and evaluate the Adamkiewicz artery and ASA. Intraoperative neuromonitoring and Doppler ultrasonography were performed to analyze neurological complications and spinal cord blood flow.
Results:
All nine patients showed ASA stenosis in the area of T-OPLL. In all patients, the Adamkiewicz artery was located between T7 and L2 and the area of ASA stenosis corresponded to the level of T-OPLL and greatest spinal cord compression; intraoperative Doppler ultrasonography confirmed the ASA defect at the same spinal level. The number of spinal levels from the Adamkiewicz artery to the most compressive OPLL lesion was greater in the two patients who developed postoperative neurological deficit compared to those who did not (5.5 vs. 2.3, p=0.014).
Conclusions
This is the first study to report detection of ASA stenosis in patients with T-OPLL. Maintaining spinal cord blood flow is important in these patients to avoid neurological deterioration.
9.Association between Pelvic Parameters and Vaginal Delivery

Methods:
As part of a 2016 health screening, 320 female volunteers underwent whole-spine radiographs. Age-matched healthy women were grouped according to the number of vaginal deliveries (0, 1–2, or ≥3). Demographic variables and spinopelvic parameters were compared among the three groups.
Results:
Of the 320 volunteers, 213 were enrolled (mean age, 71.1±7.2 years). The mean number of vaginal deliveries was 2.2. The average pelvic incidence (PI) was 55.6°±11.1° and was significantly higher in the 90 women with three or more vaginal deliveries than in the other two groups (p<0.001). The average sacral slope was 33.4°±11.1° and was significantly higher in the women with three or more vaginal deliveries than in the 18 who did not deliver vaginally (p<0.001). The 105 women with one or two vaginal deliveries had significantly higher PIs and sacral slopes than did those who did not deliver vaginally (p<0.001).
Conclusions
This is the first study documenting an association between vaginal delivery and pelvic parameters. Bony birth canal realignment during vaginal delivery can affect postnatal PI. Our study helps in understanding the PI changes over a woman’s life span.
10.Pruritus Caused by the Change in Hydromorphone Formulation Disappeared after Switching to the Fentanyl Patch: A Case Report

Systemic administration of opioids rarely causes pruritus, although its mechanism is still not clearly understood. We report an intractable pruritus induced by a change in the dosage form of opioids with the same dose by the conversion ratio, which promptly disapeared with opioid switching. A 80-year-old female experienced worsening dorsal pain and abdominal pain due to recurrent pancreatic cancer. The relief of pain was insufficient with the administration of oral hydromorphone 10 mg/day, changing to continuous intravenous hydromorphone 3 mg/day considering adjustability. The next day, her pain was reduced, but the pruritus appeared. Administering oral and topical antihistamines was ineffective. Her pruritus tended to worsen with continued administration of continuous intravenous hydromorphone at the same dose. Hence we switched to fentanyl patch 0.6 mg/day. The following day, her pruritus significantly decreased, and two days later, her pruritus almost disappeared. This case suggests that opioids switching to fentanyl patch may be effective to relieve pruritus caused by hydromorphone.
