1.An Opinion Poll of Doctors Concerning Working Conditions in Kohseiren Hospitals and the Medical Service System in Japan.

A survey was conducted by sending questionnaires to 466 doctors working at nine hospitals affiliated with the Aichi Prefectural Federation of Agricultural Cooperatives for Health and Welfare (Kohseiren).
The survey found that the length of service was under 5 years in 60% of the respondents and that many of them identified themselves strongly with the department of the teaching hospital where they were trained.
They were found to be little intersted in the present situation of the agricultural cooperatives and Japanese agriculture. However, they said they were satisfied with the present working conditions, because they could show their skills. Most approved of organ transplants after brain death but disapproved of human cloning. They showed a positive attitude toward the allowing of each patient to give access to his or her clinical records and standardization of medical treatment. Some 67% of the doctors admitted they had made medical mistakes and keenly realized the necessity of risk management.
While feeling uneasy about the recent upsurge of lawsuits brought by patients about medical treatment, they said they are endeavoring to provide safe and high quality medical service. Their recent concerns were rearrangement of dispensaries based on the subspecialty, reinforcement of postgraduate courses in medical schools and the postgraduate training system which will be a legal obligation from 2005. They suported the reform plans for the national medical service for aged patients and recognized the necessity of joining hands with local agricultural cooperatives in providing nursing care services. They said they wish to continue their career as a physican at the Kohseiren hospital.
2.Development of the Biological Prognostic Score in patients with advanced cancer and prospective verification of its external validity: Comparison with the Palliative Prognostic Index

Objective: To develop and confirm the validity of a Biological Prognostic Score using only blood test results for prediction of prognosis in patients with advanced cancer. Methods: We conducted parametric survival time analysis using blood test results, age, sex, and primary diagnosis as independent variables, and event of death as a dependent variable, among patients in a palliative care unit (a development cohort). We then developed the Biological Prognostic Score (BPS). Thereafter, we confirmed the accuracy of the BPS and the Palliative Prognostic Index (PPI) prospectively among patients, who withdrew or withheld further curative or life-prolonging therapies, in other facilities (a validation cohort). Results: We developed the BPS, which consists of cholinesterase, blood urea nitrogen, and total iron-binding capacity, from 122 patients in a development cohort. We then examined 195 patients in a validation cohort and found that the area under the receiver operating characteristic curve for 1-9 week survival prediction was BPS=0.76-0.86 and PPI=0.69-0.73. Discussion: Our results suggest that the BPS was valid. It will be necessary to perform further examinations in multiple facilities and to explore more generalized parameters that could replace total iron-binding capacity in our BPS.
4.Effective Treatment of Nightmare with Saikokaryukotsuboreito in a Patient with Advanced Cancer

Nightmares are common in patients with advanced cancer. However, there are no standard treatment of nightmare in patients with cancer. We experienced a case with nightmares improved by Saikokaryukotsuboreito (SRBT). An 82-year-old man with malignant lymphoma experienced insomnia and nightmares. Antidepressants, orexin receptor antagonists, and atypical antipsychotics failed to improve the symptoms, but SRBT immediately eliminated his insomnia and nightmares. SRBT, an herbal medicine, has been used in a variety of clinical situations for treatment of stress-induced psychiatric symptoms, i.e., anxiety, depression, and insomnia, without serious adverse events. Several reports suggest that SRBT improves depression and anxiety with the effects on chronic stress-induced disruption of hypothalamo-pituitary-adrenal axis. The effects of SRBT might have improved nightmares in this case. SRBT may be a drug of choice in the treatment of nightmares in patients with cancer.
5.Angiopoietin-Like Protein 2 Induces Synovial Inflammation in the Facet Joint Leading to Degenerative Changes via Interleukin-6 Secretion

STUDY DESIGN: Experimental human study. PURPOSE: To determine whether angiopoietin-like protein 2 (ANGPTL2) is highly expressed in the hyperplastic facet joint (FJ) synovium and whether it activates interleukin-6 (IL-6) secretion in FJ synoviocytes. OVERVIEW OF LITERATURE: Mechanical stress-induced synovitis is partially, but significantly, responsible for degenerative and subsequently osteoarthritic changes in the FJ tissues in patients with lumbar spinal stenosis (LSS). However, the underlying molecular mechanism remains unclear. IL-6 is highly expressed in degenerative FJ synovial tissue and is responsible for local chronic inflammation. ANGPTL2, an inflammatory and mechanically induced mediator, promotes the expression of IL-6 in many cells. METHODS: FJ tissues were harvested from five patients who had undergone lumbar surgery. Immunohistochemistry for ANGPTL2, IL-6, and cell markers was performed in the FJ tissue samples. After cultured synoviocytes from the FJ tissues were subjected to mechanical stress, ANGPTL2 expression and secretion were measured quantitatively using real-time quantitative reverse-transcription–polymerase chain reaction and enzyme-linked immunosorbent assay (ELISA), respectively. Following ANGPTL2 administration in the FJ synoviocytes, anti-nuclear factor-κB (NF-κB) activation was investigated using immunocytochemistry, and IL-6 expression and secretion were assayed quantitatively with or without NF-κB inhibitor. Moreover, we assessed whether ANGPTL2-induced IL-6 modulates leucocyte recruitment in the degenerative process by focusing on the monocyte chemoattractant protein-1 (MCP-1) expression. RESULTS: ANGPTL2 and IL-6 were highly expressed in the hyperplastic FJ synovium samples. ANGPTL2 was co-expressed in both, fibroblast-like and macrophage-like synoviocytes. Further, the expression and secretion of ANGPTL2 in the FJ synoviocytes increased in response to stimulation by mechanical stretching. ANGPTL2 protein promoted the nuclear translocation of NF-κB and induced IL-6 expression and secretion in the FJ synoviocytes. This effect was reversed following treatment with NF-κB inhibitor. Furthermore, ANGPTL2-induced IL-6 upregulated the MCP-1 expression in the FJ synoviocytes. CONCLUSIONS: Mechanical stress-induced ANGPTL2 promotes chronic inflammation in the FJ synovium by activating IL-6 secretion, leading to FJ degeneration and subsequent LSS.

6.Effect of Acute Rehabilitation Treatment for Patients with Severe COVID-19

Patients with severe coronavirus disease 2019 (COVID-19) have poor prognosis, with many cases being difficult to treat and many reports of neurological complications in patients who have been in the intensive care unit for a long time. Rehabilitation of patients with severe COVID-19 is important;however, no rehabilitation program has shown effectiveness. Thus, this study aimed to investigate the effects of a rehabilitation program developed and used at our hospital for patients with severe COVID-19. We enrolled 28 patients with severe COVID-19 (age 61[23-88]years;female/male, 9/19, body mass index, 25.0[18.0-33.6]kg/m2). We assessed the medical research council scoring (MRC) of the patients at the time of entry and exit from the ordinary ward using Wilcoxon signed-rank tests. MRC was significantly improved after exiting the ordinary ward compared with that on entering the ordinary ward (43 vs. 50 points;P<.001). Moreover, Barthel index improved significantly after exiting the ordinary (32.5 vs. 77.5 points, P<.001). Therefore, our acute rehabilitation treatment for patients with severe COVID-19 may be effective in improving their muscle strength and daily living.

The University of Tokyo Disability Services Office and the University of Tokyo Hospital have striven to advance the inclusion of individuals with disabilities and to encourage the co-production of research as well as mental health services with peer support workers. In convergence with these endeavors, the Center for Diversity in Medical Education and Research (CDMER) was founded in 2021. The Center aims to establish an environment and culture that facilitates the participation and success of medical professionals with disabilities. For this purpose, it is essential to integrate the perspective of the social model of disability into medical education and promote co-production in the medical field, which is among the most challenging areas that can realize co-production. The Center is involved in various educational and research activities, including managing educational programs for medical students and supporting student-led research.
