1.Diagnostic value of Reid Scores under colposcopy in cervical human papiliomavirus infection

Objective To explore the diagnostic value of Reid Scores under colposcopy in cervical human papillomavirus (HPV)infection. Methods Cervical biopsy under colposcopy was carried out for cervical diseases in 528 patients.With pathological findings as criteria,the value of Reid Scores under colposcopy in diagnosis of HPV infection was analysed.Results The coincidence of Reid Scores under colposcopy in diagnosis of HPV infection and cervical intra-epithelial neoplasia(CIN)was 52.66%,with the sensitivity of 51.05%in diagnosis of low-grade cervical diseases(HPV infection and CIN I)and that of 66.67%in diagnosis of high-grade cervical diseases(CIN Ⅱ and CIN Ⅲ).For the patients with diagnosis of HPV infection examined by pathology,the scores of items of boundary,color and vessels were positively related to the total Reid Score,while the scores of items of boundary and color were not related to that of vessels. Conclusion There may exist clinical significance in diagnosis of HPV infection by colposcopy in combination with RCI Scores.
2.Insulin resistance in chronic kidney disease patient treated with glucocorticoids

Glucocorticoids have remained important anti-inflammatory agents in the treatment of chronic kidney disease (CKD).However,the use of glucocorticoids can invoke or enhance insulin resistance,which is closely associated with renal injury and serves as an independent risk of the occurrence and progression of CKD.On the other hand,CKD patients may have insulin resistance,which hampers the use of glucocorticoids in these patients.
3.Laparoscopic appendectomy in 368 cases

Objective To summarize the experience of laparoscopic appendectomy(LA). Methods A retrospective analysis was made on clinical date of 368 cases of appendicitis treated with laparoscopic appendectomy in this hospital from March 1998 to March 2005.There were 35 cases of simple appendicitis,276 cases of suppurative appendicitis,39 cases of appendiceal perforation with peritonitis,and 18 cases of chronic appendicitis. Results The procedure was successfully accomplished in 363 cases,while conversions to open surgery were performed in 5 cases.The operative time was 22~55 min(mean,30.5 min),and the intraoperative blood loss was 2~10 ml.One case of abdominal residual infection and 1 case of adhesive ileus were cured with conservative treatment postoperatively.Pelvic abscess occurred in 1 case and was cured with a re-operation of laparoscopic abscess drainage. Conclusions Laparoscopic appendectomy is safe and effective in skilled hands.
4.Effect of Astilbin on expression of CTLA-4 in activated T cells of mouse heart transplantation model with acute rejection

Objective To investigate the effect of astilbin on expression of CTLA-4 in activated T cells of mouse heart transplantation model with acute rejection.Methods Cardiomyocytes of BALB/ c mouse and spleen cells of C57BL/6 mouse were separated.The cardiomyocytes(2?10~5/ml)as irri- tation cells and spleen cells(1?10~6/ml)as responsers were mixed and cultured.The model of mouse heart transplantation with acute rejection in vitro was established.Three groups were set up:control group,Astilbin(15/?g/ml) group,Astilbin+anti-CTLA-4 mAb 9H10 (30?g/ml)group.Apoptosis of T cells was observed by TUNEL.The expression of CTLA-4 in activated T cells was detected by RT-PCR and Western blot.Results Apoptosis indexes of activated T cells in Astilbin group were sig- nificantly higher than those in the control group(73.4%?12.5% vs 35.1%?9.2%,P<0.01). The expression of CTLA-4 in Astilbin group was significantly higher than control group(P<0.01), but there was no apparently difference between control group and Astilbin+CTLA-4 mAb group(P>0.05).Conclusion Astilbin induces apoptosis of activated T cells in heart transplantation,which may be partially related to its enhanced expression of CTLA-4.
5.Oral-appliance for erectile dysfunction induced by obstructive sleep apnea-hypopnea syndrome.

OBJECTIVETo evaluate the effect of the oral-appliance in the treatment of ED induced by obstructive sleep apnea-hypopnea syndrome (OSAHS).
METHODSThis study included 40 cases of OSAHS-induced ED. Based on the apnea-hyponea index (AHI) and lowest SaO2, we divided the patients into groups A (AHI < or = 30/h and lowest SaO2 > or = 80%) and B (AHI > 30/h and lowest SaO2 < 80%), and treated them with the oral-appliance for 3 months, followed by evaluation of the therapeutic effect using polysomnography, IIEF-5 questionnaire and Sexual Encounter Profile (SEP) diaries.
RESULTSThirty-seven of the patients accomplished the treatment, 16 in group A and 21 in group B. Compared with the baseline, the AHI was significantly decreased in both groups A (21.4 +/- 7.3 vs 14.2 +/- 6.6, P < 0.05) and B (51.4 +/- 9.5 vs 29.7 +/- 8.1, P < 0.05) after treatment, the IIEF-5 scores increased, even more remarkably in B than in A (3.36 +/- 2.48 vs 1.12 +/- 1.27, P < 0.05). The positive rates of SEP2 and SEP3 were markedly lower in A than in B (37.5% vs 76.2% and 25.0% vs 61.9%, P < 0.05).
CONCLUSIONThe oral-appliance can improve the erectile function of OSAHS-induced ED patients, particularly for those with severe OSAHS.

7.Application of rhomboid skin flap in expanded skin flap transfer

Objective To explore the use of rhomboid skin flap in expanded skin flap transfer. Methods A rhomboid skin flap was designed if the top soft part could not be fully utilized after expanded in a rotation skin flap. The flap pedicels were designed near the incision side. It should be ensured that ra-tio of the length to the width of the composite flap, which was composed of the rhomboid skin flap and the rotation skin flap, was 2.5∶1.0. Results Among these 11 patients with re-designed rhomboid skin flaps in the rotation skin flaps, the ratio of the length to the width reached to 3∶1 in some cases, but 2. 5∶1.0 in most cases. All the skin flaps survived, except one patient with disturbance of blood circulation in a small area and one with mild congestion. Conclusion The expanded soft tissue can be fully and rationally utilized to repair the skin defect in this design. Attention should be paid to the ratio of the length to the width of the composited flap, and it is better to select axial flap as the composite flap for safety. This method is safe, and worthy of recommendation.
8.DETERMINATION OF THE COUMATETRALYL IN BIOLOGICAL MATERIALS BY RPHPLC METHOD USING INTERNAL STANDARD

A simple,reliable RPHPLC method on ODS column with UV detector for deter-mination of the anticoagulant rodenticide coumatetralyl in animal tissue was developed.1,1′-Bi-(2-Naphthol)was used as an internal standard to check the process ofexperiments.The mean recoveries from the spiked rabbit liver were around 90% atthe levels of 0.4-16mg/kg.The coeffecint of variation of 5 determination was 3.2%.The results showed that this method has a satisfactory reproducibility.The methodis practical for examining poisons in poisoning cases.

10.Clinical analysis of pure red cell aplasia following major ABO-incompatible allogeneic hematopoletic stem cell transplantation

Objective To study the incidence,risk factors,clinical outcome,management and prevention of pure red cell aplasia (PRCA) following major ABO-incompatible allogeneic hematopoietic stem cell transplantation (allo-HSCT).Methods Forty-two patients underwent major ABO-incompa- tible allo-HSCT,including major ABO-mismatch in 33 patients,major plus minor ABO-mismatch in 9 patients,and 27 recipients with blood group O.Thirteen patients underwent bone marrow transplan- tation,25 peripheral blood stem cell transplantation,and 4 cord blood transplantation.Six patients re- ceived donor-type plasma replacement before transplantation.Cyclosporine A (CsA) and methotrexate (MTX) were used for prophylaxis of graft-versus-host disease (GVHD).Results All 42 patients had sustained engraftment.PRCA occurred in 11/42 patients (26.2%).All the 11 cases of PRCA were in blood group O recipients of grafts from blood group A donor (n=9) or blood group B donor (n= 2);6 patients with blood group O who received donor-type plasma exchange before transplantation did not develop PRCA.PRCA resolved spontaneously in 8 cases with transfusion support.Two patients were treated by donor-type plasma exchange,resulting in the decrease of isoagglutinin titer,followed by complete recovery of erythropoiesis.One patient responded to rituximab and achieved complete re- mission of symptoms of PRCA.Univariate analysis revealed that the most significant risk factors asso- ciated with PRCA were blood group O recipient,blood group A donor,blood group O recipient of graft from blood group A donor;only blood group O/A in recipient/donor pair was identified as being significantly associated with the occurrence of PRCA by multivariate analysis (RR 10.999,95% CI 1.975-61.258,P
