1.Comparison of Fluid Expansion Effects Between Two Plasma Substitutes and Their Effects on Blood Coagulation Activity

OBJECTIVE:To draw a comparison between 6% Hetastarch(H)and Succinylated Gelatin(G)in fluid expansion and their effects on blood coagulation activity.METHODS:A total of 60 surgery patients who needed volume expansion with colloid fluid were randomized to H group and G group.According to the design,the same dose of H and G were infused into the patients.Before and after infusion,the vital signs were monitored and the venous blood samples were taken for the determination of indexes of coagulation,fibrinolysis and electrolyte.RESULTS:There were no significant differences in the vital signs between two groups.After volume expansion,blood platelets counts in two groups were significantly decreased,much as in H group.And the APTT in H group had been significantly prolonged.In group G,only FIB decreased significantly.After volume expansion,free Ca2+ and total Ca2+ were decreased in both groups.Low level of K+ in group G was noted following diluti-on,and Cl-in group H increased significantly.CONCLUSION:Both group showed good volume expansion effects and stable fi-brinolytic function.Hyperchloremia should be alert in group H,and if necessary,Ca2+ should be promptly added in two groups.
2.Clinical effect of fluticasone salmeterol powder for inhalation in treatment of patients with cough variant asthma

Objective To evaluate the efficacy of seretide (fluticasone/salmeterol) in treatment of patients with cough variant asthma(CVA). Methods 86 CVA patients were treated with fluticasone/salmeterol propionate 8 weeks, after 8 weeks, 86 patients were randomly assigned to treatment group and control group, each group with 43 patients, the treatment group continued medicine for 4 weeks, the control group without any treatment.The two groups of patients FENO measurement values, cough symptom score before treatment and after treatment for 8 weeks, 12 weeks were compared. Results After treatment for 8 weeks, cough symptom scores and FeNO in the two groups values decreased significantly compared with those before treatment(P<0.05), 12 weeks after treatment, treatment group cough product, FeNO values, were lower than in control group(P<0.05).Conclusion Fluticasone salmeterol powder inhalation can effectively improve the symptoms of cough cough variant asthma.Symptom score and FENO value can be used as a monitoring index to evaluate the therapeutic effect of CVA, and the FENO value is more effective, which has the advantages of non-invasive, simple, safe, rapid and repeatable.
3.The role of gastric acid in the esophageal adenocarcinogenesis induced by duodenal juice reflux in rats.

Objective To investigate the role of gastric acid in the esophageal adenocarcinogenesis induced by duodenal juice reflux in the rats. Methods Eight week old male Sprague Dawley rats, 125 in all, weighing approximately 200 g each, were used. Three types of surgical procedures were performed to produce gastroesophageal reflux (GER), duodenoesophageal reflux (DER) and duodenogastroesophageal reflux (DGER), and a group of sham operation (SO). The animals were fed with a standard diet. Esophageal mucosal pathological changes were examined 20 weeks after surgery. Results No pathological changes were found in the 15 SO rats animals. Significant esophagitis were observed in three reflux groups. In 35 DER and 32 DGER animals, the incidence of Barrett's esophagus (BE) was 91.4%, 84.4% respectively and the incidence of esophageal adenocarcinoma (EAC) was 25.7%, 53.1% respectively, significantly higher than those in the 24 GER animals ( P

5.A prospective study of interbody fusion with cage versus corpectomy and fusion with plate in treating adjacent two-level cervical spondylosis

[Objective] To compare the efficacy of interbody fusion with cage versus corpectomy and fusion with plate in treating adjacent two-level cervical spondylosis.[Method] Sixty patients were included in this study.All patients were randomly assigned to anterior interbody fusion with cage (AIFC) group or anterior corpectomy and fusion with plate (ACFP) group. The average follow-up period was 28 months for AIFC group,and 25 months for ACFP group. [Result] The average loss of lordosis angle, the lost correction of segmental height and the scores on the criteria of 40-score method were not statistically significant between these 2 groups. The lordosis correction, operative estimated blood loss, and operative time were statistically better in the AIFC group significantly . [Conclusion] The short-term results of AIFC and ACFP in treating adjacent two-level cervical spondylosis were satisfactory. The advantages of AIFC are the elimination of donor site complications, increasing more segmental lordosis, and reducing blood loss and operative time.
6.Existence of heme oxygenase-carbon monoxide-cyclic guanosine monophosphate pathway in human trabecular meshwork cells in vitro.

To confirm the existence of heme oxygenase (HO)- carbon monoxide (CO)- cyclic guanosine monophosphate (cGMP) pathway in the cultured human trabecular meshwork cells (HT-MCs) in vitro, and to evaluate the inductive role of hemin on this pathway, HTMCs of the third to fourth generation were cultured in vitro. Reverse transcripase-polymerase chain reaction (RT-PCR) was employed for detection of HO-1 and HO-2 mRNA. Immunohistochemical staining was used to detect HO-1 and HO-2 proteins. Hemin was added into the culture solution. The HO-1 mRNA levels were quantified by RT-PCR. The relative amount of carbon monoxide released into the media was measured with the quantifying carbon monoxide hemoglobin (HbCO) by spectrophotometry. Radioimmunoassay was used to determine changes of cGMP in HTMCs. The results showed that cultured cells had the specific characteristics of HTMCs. Both HO-1 and HO-2 genes were expressed in HTMCs, as well as HO-1 and HO-2 proteins in HTMCs. Hemin induced HO-1 mRNA, HbCO and cGMP in a dose-dependent manner. In conclusion, HO-CO-cGMP pathway exists in the cultured HTMCs and can be induced by hemin. Pharmacological stimulation of HO-CO-cGMP pathway may constitute a novel therapeutic approach to rescuing glaucoma.

7.Effect of Intensive Interaction on Teaching of Course of Neurology for Postgraduate for Master

Objective To investigate the effects of intensive interaction on teaching of neurology for postgraduate for master. Methods The postgraduates of grade 2010 study under traditional teaching, and the postgraduates of grade 2012 study under intensive interaction teaching. Results The score of clinical knowledge test and specialized skill test were significantly better in the grade 2012 than in the grade 2010 (P<0.05). The students were satisfied with the intensive interaction teaching. Conclusion The intensive interaction teaching is good for teaching of neurology for postgraduate for master.
8.Autologous skin grafting, vacuum sealing drainage and external fixation for repair of complex and severe open fracture of the elbow joint

BACKGROUND:The incidence of open fracture of the elbow joint is increased rapidly with an increased number of accidents. Fractures are often accompanied by severe soft tissue injuries. At present, a combined therapy of autologous skin grafting, vacuum sealing drainage and external fixation for the treatment of open fractures has been widely reported. OBJECTIVE:To investigate the characteristics of the combined therapy of autologous skin grafting, vacuum sealing drainage, and external fixation for repair of complex and severe open fracture of the elbow joint. METHODS:A total of 41 patients with complex and severe open fracture of the elbow joint who received the combined therapy of autologous skin grafting, vacuum sealing drainage and external fixation during January 2009 to December 2013 in General Hospital of Tianjin Medical University, China were selected. Al patients were treated with routine debridement, external fixation and vacuum sealing drainage to cover wound during the first period. During the second period, autologous skin grafting or flap transfer was used to repair wound when the fresh granulation tissue covered the wound. RESULTS AND CONCLUSION:The postoperative folow-up period of these 41 patients was 8-22 months, with an average period of 13 months. External fixation period was 8-13 months with an average period of 9.8 months. The open wounds were completely healed after autologous skin grafting or skin flaps transfer. Fractures were healed in 31 patients during the first period. There were 7 cases of delayed union, 3 cases of nonunion. Al fractures were completely healed during 8-12 months after iliac bone grafting. The excelent and good rate of limb function recovery was 73.1% among 41 patients with severe open fracture of the elbow joint. The limb function of Gustilo II patients better recovered. These results suggest that the clinical effect of the combined therapy of autologous skin grafting, vacuum sealing drainage and external fixation for the treatment of complex and severe open fractures of the elbow joint is satisfactory.
10.Expression of brain-derived neurotrophic factor and its receptor TrkB in the nasal mucosa of experimental allergic rhinitis animal

OBJECTIVE To investigate the role of brain-derived neurotrophic factor(BDNF) and its receptor TrkB in the development of allergic rhinitis(AR). METHODS SD rats were randomly divided into allergic rhinitis group(AR group, n=20) and control group (Con group, n=10) . A model of allergic rhinitis in health SD rat was established by using ovalbumin intraperitoneal immunization and nasal antigen challenge. The nasal mucosa obtained from 18 out of 20 AR models as well as 10 normal controls were studied with immunohistochemistry (Envision System) method to observe the expression and distribution of BDNF and TrkB. RESULTS Immunohistochemical staining showed that the distribution of BDNF and TrkB in nasal mucosa from normal rats was in accordance with that in the nasal mucosa from rats with allergic rhinitis on the whole. The study showed that immunoreactivity to BDNF (or to TrkB) was present both on the surface and in the cytoplasm of epithelial cells, glandular cells and their duct cells. The statistical analysis of imumunohistochemical staining showed that the IOD of BDNF in rat nasal mucosa with allergic rhinitis (77344.17?8567.02) was obviously higher than that in the normal rat nasal mucosa (54124.82? 18515).
