3.Clinical study of lobaplatin combined with vinorelbine for non-small cell lung cancer

Background and purpose. Chemotherapy is a standard treatment for patients with advanced stage non-small cell lung cancer (NSCLC) However, platinum based chemotherapeutic regimen haves high toxicity profile to normal tissues. Lobaplatin (LBP) is a new third-generation antitumour platinum drug. Studies abroad have shown that Iobaplatin has an anticancer activity similar to cisplatin with better tolerance and is more effective for those who are resistant to cisplatin. This study was aimed to observe the effectiveness and toxicities of lobaplatin combined with vinorelbine (NVB) for the treatment of advanced NSCLC. Methods: Sixty-four patients pathologically diagnosed as clinical stage Ⅲ_B-Ⅳ NSCLC who did not receive treatment before, were randomly assigned to two groups. NL group (NVB+LBP): LBP at a dose of 30 mg/m~2 intravenous infusion on day 1, and NVB at a dose of 25 mg/m~2 intravenous infusion on days 1 and 8. NP group (NVB+DDP): Cisplatin (DDP) at a dose of 30 mg/m~2 intravenous infusion on day 1, 2 and 3 and NVB at a dose of 25 mg/m~2 intravenous infusion on days 1 and 8. Treatments were repeated every 21-28 days for 3 cycles. Results: For 34 patients of NL group, there were CR (1 cases), PR(13 cases), NC(15 cases), and PD(5 cases). The overall response rate (RR) was 41.2%, disease control rate (DCR) was 85.3%. In NP group: RR was 43.3%, DCR was 83.3% (X~2=0.05, P>0.05). Median overall survival time was 8.6 months and 8.9 months for NL group and NP group, respectively. The main toxicities in NL group were myelosuppression. Digestive toxicity such as anorexia, nausea,vomiting were less than those in the NP group (X~2=7.43, P<0.05), Peripheral hour,toxicity, serious liver and renal toxicity were not observed in NL group. Conclusion: Compared with cisplatin plus vinorelbine, domestic lobaplatin with vinorelbine yielded similar efficacies for NSCLC, but had less toxicity and well tolerate.
4.Association between metabolic syndrome and prognosis of endometrioid carcinoma

Objective To study the association between metabolic syndrome (MS) and prognosis of endometrioid carcinoma.Methods A total of 256 patients with endometrioid carcinoma at,Zhejiang Cancer Hospital between January,2001 and December,2008 were chosen.The deadline for follow up was December 2008.The general conditions(including age and body mass index),whether coupled with MS and it's risk factors(including waist circumference,fasting plasma glucose,triglycerides,high-densitylipoprotein,systolic and diastolic blood pressure) were analyzed.The outcome of 256 patients whether coupled with MS were analyzed using Kaplan-Meier curve.Relative risks were estimated using Cox proportional hazards regression model.Results A total of 256 cases were followed-up successfully.Sixtyfour (33.0%) cases coupled with MS among the 194 patients survived,while thirty-two (51.6%)coupled with MS from 62 cases died,there was significant difference between them (x2=6.953,P=0.008).The total fiveyear survival rate was 75.8%,the survival time was (78.0±3.4) months.The rate and the survival time of patients coupled with MS [66.7%,(67.0±2.4) months] were significatly lower than those coupled with no MS [81.3%,(85.0±4.0) months;P<0.05].The Cox proportional hazards regression results showed that coupled with MS,body mass index ≥25 kg/m2,waist circumference>80 cm,abnormol systolic and diastolic blood pressure,abnormal fasting plasma glucose and more than two components of definitions of MS were related to bad prognosis of endometrioid carcinoma(P<0.05).Conclusion Metabolic syndrome may be lead to a bad prognosis of endometrioid carcinoma.
5.Effect of Zinc and Salvia on some whole blood metal ion distribution caused by noise in rats

Objective To observe the protective effect of the separate zinc gluconate oral and combined of Salvia injection on whole blood metal ion of calcium,magnesium,iron,copper and zinc concentration in noise-induced rats.Methods 50 female SD rats were randomly divided into a control group,a noise group,a zinc gluconate noise group (plus zinc group),a Salvia injection noise group (plus Salvia group) and a Zinc gluconate oral liquid and Salvia injection noise group (combined group),with10 rats in each group.Except the control group did not expose to noise,the rest groups were continuously exposed to high frequency steady noise for two weeks.Each group was compared for the concentration differences of whole blood metal ion of calcium,magnesium,iron,copper and zinc after the intervention of noise.Results ① In each group at the comparison of the calcium ion concentration: Calcium ion concentration of the control group(1.25± 0.16)mmol/L and the combination group(1.27 ± 0.10) mmol/L was significantly lower than the noise group (1.42 ± 0.18) mmol/L.The rest groups compared to each other were not statistically significant.②Magnesium ion concentration was highest in the noise group (1.53 ± 0.10)rmtmol/L),and lowest in the control group (130 ± 0.29) mmol/L,and the noise group was significantly higher than that of the control group.The mean of magnesium ion concentration in plus zinc group (1.42± 0.27) mmol/L,plus Salvia group (1.38± 0.15) mmol/L and combined group(l.37±0.11)mmol/L were lower than the noise group,but the difference was not significant (P>0.05).③ The iron ion concentration of the noise group (5.47± 1.29)mmol/L was significantly lower than the other four groups (P<0.05).The control group,plus zinc group,plus Salvia group,the combined group showed no significant differences.④ Whole blood copper ion concentration of the noise group (16.69 ± 4.18) μmol/L was significantly lower than the control group (21.53 ± 3.78) μmol/L and the combination group(19.53± 1.92)μmol/L with a statistical difference; compared with the control group,the concentration of copper ions in plus zinc group(16.19± 1.93)mμol/L was significantly lower (P<0.05).⑤The whole blood zinc ion concentration in the noise group (50.83±7.99)μmol/L was significantly lower than the other groups,zinc ion concentration in the plus Salvia group (53.87±6.77)μmol/L was significantly lower than the control group (63.86± 8.83) μmol/L; the whole blood zinc ion concentration showed no difference between the plus zinc group (54.81 ± 5.90) μmol/L,plus Salvia group and combined group (59.21 ± 3.90) μmol/L.Conclusion Combined zinc gluconate oral solution and Salvia injection had protective effect on whole blood metal ion concentration affected by noise.The protection effect of zinc gluconate oral solution and Salvia injection combination was stronger than any individual.
6.Reasons of cessation of slow-acting antirheumatic drug treatmen in patients with rheumatoid arthritis

Objective To investigate the incidence and possible reasons of patients with rheumatoid arthritis (RA) who stopped taking slow-acting antirheumatic drugs (SAADs).Methods Two hundreds and twenty-four in-or out-patients with RA were prospectively followed up for 4-year,and their demographic and clinical information and reasons of SAADs cessation were recorded.The difference between patients who stuck to SAADs and those who stopped SAADs and the factors that resulting the cessation were analyzed.Results ① Fourty-eight percent of 224 patients with RA ceased taking SAADs during 4-year period.② The main reasons of ceasing SAADs,provided by patients themselves,were side effects of SAADs,ineffectiveness,lack of knowledge about drugs,short of SAADs,concurrent diseases,and intention to pregnancy.③ There was significant difference in low income,poor educational status,number of tender joints,and rheumatoid factor level between the patients who stuck to SAADs and those who did not.Conclusion The high incidence of SAADs cessation in RA patients should rise concern.Both medical and patient factors contribute to the cessation.
7.Clinical efficacy and ultrasonographic evaluation of anti-thyroid drugs in the treatment of primary hyperthyroidism

Objective To investigate the clinical efficacy and ultrasonographic changes treated with anti-thyroid drugs ( ATD ) in patients with primary hyperthyroidism(PHT).Methods 83 cases of PHT patients admitted to the department of ultrasound from February 2013 to August 2015 in Zhuji People's Hospital of Zhejiang Province were selected, the patients were divided into two groups according to the results of ultrasonography, echo nonuniform group 41 cases and echo uniform group 42 cases.ATD therapy was administered to the both groups (were treated with methimazole).The thyroid related indexes and hemodynamics of thyroid right superior diagnosed by color doppler ultrasound, and the curative efficacies of the two groups were analyzed.Results After treatment, the systemic vascular resistance index(RI) of echo uniform group was (0.62 ±0.17), and the echo nonuniform group was (0.65 ±0.18), there was no significant difference between the two groups.The echo uniform group of thyroid artery diameter on the right lobe(D), the peak systolic velocity(Vmax), minimum diastolic blood flow velocity(Vmin), thyroid volume(V) and blood flow volume(Q) were significantly lower than the echo nonuniform group, the difference was statistically significant (P <0.05).The total effective rate of the echo nonuniform group (48.78%) was significantly lower than that in the echo uniform group (85.71%), the difference was statistically significant(P <0.05).Conclusion ATD has a good effect in treating PHT, and can significantly improve the thyroid hemodynamics in patients, it has a positive effect on the regulation of thyroid-related parameters in patients, the clinical treatment is effective, and the effect is better especially in patients with uniform echo.
8.A Miao pedigree of Bardet-Biedl syndrome in Yunnan province and their gene reservation

Objective To observe the features of a rare Miao pedigree of Bardet-Biedl syndrome in Yunnan province and to reserve its gene. Methods Three patients of this pedigree were reported. Their clinical and biochemical features were compared with those of the other pedigree members. Lymphocytes from main members of this pedigree were collected and transformed with cyclosporine A methods. Immortalized B lymphocyte strains were checked by means of chromosome karyotype analysis. Results Patients of this pedigree demonstrated typical clinical characteristics of this syndrome with increased body weight, blood pressure, fasting glucose, and lipoprotein(a)as compared with the other pedigree members(P<0. 05). The chromosome karyotype of the lymphocytes before and after transformation was kept consistent. Conclusions Patients of this Miao pedigree showed typical clinical characteristics of this syndrome as well as abnormal metabolic features. Immortalized B lymphocyte strains with their genetic information were set successfully.
9.The role of regulatory T cells in immune dysfunction during sepsis

BACKGROUND: Although regulatory T cells (Tregs) are key to the maintenance of immunologic homeostasis and tolerance, little is known about Treg-mediated immunosuppression in the stage of sepsis. This article aimed to review the current literature on the role of Tregs in the pathophysiology of septic response, attempting to investigate the role of Tregs in immune dysfunction during sepsis. DATA SOURCES: A literature search was conducted in January 2014 using the China National Knowledge Infrastructure and PubMed. Articles on the role of Tregs in immune dysfunction during sepsis were identified. RESULTS: The identified articles indicated that Treg levels can be used for the assessment of the course of sepsis. The inhibition of Treg activity can promote the recovery of immune function. CONCLUSION: Since the mechanism of Tregs is complex during the sepsis, more studies are needed.
10.Application of Cystatin C-based formulas for prediction of glomerular filtration rate

Objective To investigate the applicability of Cys C-based formulas for prediction of GFR in Chinese patients with CKD.Methods A total of 176 adult patients with CKD including 90 males and 86 females collected from 4 hospitals located in different geographic regions of China (Beijing,Shanghai,Dalian and Changsha) were enrolled in this study from September 2007 to July 2009.The rGFR was measured using 99mTc-DTPA clearance rate two-sample method.Cystantic C was measured by PETIA and PENIA respectively.The results of eGFR in the Larsson formula,Grubb formula,Hoek formula,Filler formula,Stevens formula and Hojs formula were compared with the rGFR to evaluate the calculation coherence,bias,precision,accuracy and the performance of correct phasing of the formulas.Results The mean 99mTc-DTPA clearance was [40.70 ( 19.09 - 86.49)] ml · min-1 · ( 1.73m2 ) -1.Significant difference was witnessed in the evaluation of GFR estimation formulas calculated by PETIA and PENIA (P <0.01).ICC and Spearman correlation analysis revealed a significant correlation between eGFR and rGFR.The ICCs of eGFR and rGFR ranged from 0.874 to 0.938.Compared with rGFR,the 30% accuracy of all the eight evaluation formulas using PETIA and PENIA method were lower than 60%.The percentages of correct phasing in all the 5 stages of CKD were not ideal.With these formulas,percentages of correct phasing from CKD stage 2 to CKD stage 4 were lower than 65%.The eGFRs were underestimated by formulas evaluated by PENIA in CKD stage 1.All the eGFRs were overestimated remarkably by all equations in CKD stage 5.Conclusions None of the eight Cys C based formulas are ideal for estimation of GFR in Chinese CKD patients,and they can not be applied to Chinese patients directly.For this patient population,further studies will be needed to develop a more accurate Cys C-based GFR estimation formula that includes ethnicity,age and other factors.
