1.The clinical application of ultrasonic scalpel in laparoscopic cholecystectomy

Objective To investigate the applicetion value of combined ultrasonic scalpel in laparoscopic cholecystectomy(LC).Methods 140 patients with acute and chronic cholecystitis,gallbladder stone patients,received intraoperative ultrasound combined electrosurgical knife,suction devices auxiliary red line LC.The clinical effects were observed.Results 138 cases were completed cholecystectomy.Blood loss ( 8.16 ± 1.85 ) ml,operation time(38.13 ±4.35) min,Postoperative hospital stay(3.25 ± 1.18)d,4 cases of intraoperative rupture of the gallbladder,Hepatobiliary burns in 1 case,2 cases because of dense adhesions around the gallbladder and gallbladder more extensive anatomical triangle unclear laparotomy.Conclusion The rational use of combined high-frequency ultrasonic scalpel knife,suction devices auxiliary red laparoscopic cholecystectomy,surgical conditions that meet medicalrecords,are safe and reliable,quicker recovery can be achieved by minimally invasive medical treatments.
2.Therapeutic effect of two-stage delayed open reduction and internal fixation on Pilon fracture

Objective To investigate the therapeutic effect of two-stage delayed open reduction and internal fixation on Pilon fractures.Methods The clinical data of 36 patients(26 males and 10 females;aged 18-57 years with a mean of 38.5 years;9 patients suffered from open fractures and 27 with closed fractures;25 combined with fibular fractures) with monolateral Pilon fracture hospitalized in succession from Jan.2001 to Oct.2006 were analyzed retrospectively.According to the Ruedi-Allgower classification,there were 6 cases with type Ⅰ,21 with type Ⅱ and 9 with type Ⅲ fracture.All patients underwent temporary fixation with calcaneal traction or plaster slab for 7 to 14 days(mean 9.8d),and then they were treated with delayed surgery when the soft tissue injuries subsided.Ankle functions were evaluated to assess the clinical outcomes,and complications were documented for further analysis.Results Follow-up was performed for 19 to 34 months(in mean of 28 months).Based on the Mazur classification,the ankle function in 13 cases was excellent,17 as good,4 as fair and 2 as poor,with a satisfactory rate of 83.3%.Postoperative complications included 2 cases of superficial incision infections,2 of skin slough and 19 of articular degeneration.Conclusion Two-stage delayed open reduction and internal fixation is a reasonable and effective procedure in treating Pilon fractures with advantages of few complications and satisfactory ankle function.
3.Microfluidic chip digital PCR technology and its clinical application

Microfluidics-based digital PCR depends on microfluidic chip to split PCR reaction mixture into many tiny equal-volume units.Quantitative assessment of target DNA template can be obtained by counting the number of fluorescence-positive units after thermocycling.Microfluidics-based digital PCR exhibits many advantages including absolute quantification, high sensitivity and accuracy, and shows great promise in a variety of applications, such as infectious diseases diagnose, early cancer detection and prenatal diagnose. There are already several microfludics-based digital PCR products produced from sereval companies.It is believed that as the technology improves, microfluidics-based digital PCR will find broader applications and become the next-generation tool for genetic tests.

6.Curative Effect of Local Application of Minocycline-HCl Ointments and Influence on High Sensitivity C Re-action Protein (hs-CRP) and Interleukin-10 (IL-10) Levels in Gingival Crevicular Fluid of Patients with Chronic Periodontitis

Objective:To observe the curative effect of local application of minocycline-HCl ointments and the influence on high sensitivity C reaction protein (hs-CRP) and interleukin-10 (IL-10) levels in gingival crevicular fluid of patients with chronic periodon-titis. Methods:Totally 72 cases of patients with chronic periodontitis (96 teeth) were selected and divided into the observation group (36 cases, 47 teeth) and the control group (36 cases, 49 teeth) by a random number table. The patients in the two groups were given the routine periodental non-surgical treatment, such as supragingival scaling, subgingival scaling, scaling and root planning etc. The patients in the observation group were given minocycline-HCl ointments filled in the periodontal pockets, and the patients in the control group were given compound iodine glycerol liquids filled in the periodontal pockets once a week for 4 weeks. The changes in hs-CRP and IL-10 levels in gingival crevicular fluid of the patients before and after the medical treatment were observed, and the clinical cura-tive effect was compared as well. Results:After the 4-week medical treatment, the hs-CRP levels in gingival crevicular fluid of the pa-tients in the two groups were obviously declined, while the IL-10 levels were obviously increased(P<0. 01 or P<0. 05), and the de-clining or increasing rate in the observation group was much higher than that in the control group (P<0. 05). The total clinical effi-ciency in the observation group was 94. 44%, which was much higher than that in the control group (77. 78%, P<0. 05). Conclu-sion:The local application of minocycline-HCl has favorable curative effect on chronic periodontitis, which can reduce the hs-CRP lev-els in gingival crevicular fluid, increase the IL-10 levels, control the inflammatory damage in periodontium effectively and improve the local inflammatory reaction in periodontium.
7.Effect of rosuvastatin calcium combined with clopidogrel and aspirin on prevention of cerebral infarction in elderly patients with transient ischemic attack and the effects on blood lipids and platelets

Objective To study the effect of rosuvastatin calcium combined with clopidogrel and aspirin on preventing cerebral infarction in elderly patients with transient ischemic attack, and the influence on blood lipid and platelet.Methods 80 elderly patients with TIA were treated in Haiyan people's hospital from September 2014 to May 2017.All the patients were randomly divided into the observation group and the control group with 40 cases in each group.The control group were treated with clopidogrel and aspirin, and the observation group was treated with rosuvastatin calcium combined with clopidogrel and aspirin.The effect of cerebral infarction prevention, blood lipid and platelet change were compared between the two groups.Results Cerebral infarction incidence was 5% in the observation group, the incidence of adverse reactions was 10%, which were significantly lower than 22.5% and 17.5% in the control group, the differences were statistically significant (P<0.05).Compared two groups of patients with blood lipid levels after the treatment, the indicators of the observation groupTC (4.89±1.13) mmol/L、TG (1.04±1.02) mmol/L、LDL-C (2.62±1.22) mmol/L was significantly lower than the control group (5.57±1.20) mmol/L、(1.58±1.06) mmol/L、(3.39±1.24) mmol/L, with statistical significance (P<0.05).After treatment, PT (16.88±1.97), APTT (46.23±4.22) in the observation group were longer than (14.01±2.02) (38.21±3.99) in the control group, PLT (150.44±9.87) in the observation group is lower (165.82±9.71) in the control group, the differences were statistically significant (P<0.05).Conclusion Rosuvastatin calcium combined with clopidogrel and aspirin has a significant effect on the prevention of elderly patients with transient ischemic attack of cerebral infarction, which can reduce the blood lipid level, prolong PT, reduce PLT, APTT, and has high value in clinical application.
9.Clinical features of 11 cases of renal cortical abscess

Objective To explore methods of diagnostic and treatment for renal cortical abscess to reduce its misdiagnosis.Methods Eleven cases of renal cortical abscess diagnosed and treated at Beijing International Airport Hospital during 1991 to 2003 were reviewed and analyzed.Results Among the 11 patients with renal cortical abscess,diagnosis was established in nine(82%)before treatment,seven were cured by incision and drainage,one was cured by incision and drainage only after failure in percutaneous nephropuncture,and one was cured by intravenous injection of antibiotics.Two cases(18%)were misdiagnosed as renal cancer and radical nephrectomy were performed for them.Conclusions Diagnosis for renal cortical abscess should be based on combination of patients clinical manifestation,laboratory examinations and imaging findings.Incision and drainage is the choice of treatment for renal cortical abscess.
10.Observation on clinical efficacy of warm needling moxibustion plus Mulligan dynamic joint mobilization for cervical radiculopathy

Objective: To observe the clinical efficacy of warm needling moxibustion plus Mulligan dynamic joint mobilization for cervical radiculopathy. Methods: A total of 60 cases with cervical radiculopathy were randomly divided into group A, group B and group C by random digital table, 20 cases in each group. The patients in three groups were treated with similar warm needling moxibustion. Group A only received warm needling moxibustion; group B was treated with warm needling moxibustion plus Mulligan dynamic joint mobilization; group C was treated with warm needling moxibustion plus cervical traction. Before the treatment and after 12 sessions of treatment, the patients in the three groups were assessed for score of pain and range of motion (ROM). Results: By 12 sessions of the treatments, the scores of pain were obviously decreased in the three groups, with statistical significances (all P<0.05), and the pain scores of group B and group C were significantly different from the score of group A (both P<0.05). ROM of the neck was increased than before the treatment in the three groups, with statistical significances (all P<0.05), and the ROM of the neck of group B was significantly different from those of group A and group C (both P<0.05). Conclusion: Warm needling moxibustion plus Mulligan dynamic joint mobilization can effectively improve the neck ROM and relieve pain in patients with cervical radiculopathy.
