Chinese Journal of Endocrinology and Metabolism 2010;26(11):948-953
Objective To study the effects of thyroid hormone (TH) on synaptophysin (syn) expression in rat cortex and hippocampus during developing period. Methods The experimental animals were divided into four groups: (1) Hypothyroidism group: the pregnant maternal rats had been given 0.02% methimazole(MM)water since 5 days, the infant rats suffered from innate hypothyroidism. (2)T4 substitution group: innate 0 days. (4)Normal control group: The pregnant rats were given a normol diet and water, yielding infant rats used as normal matched control. The expression of synaptophysin mRNA were measured by in site hybridization. Results (1) In the normal control group synaptophysin expression showed a characteristics layer distribution in the cortex and hippocampus. The expression was very low at O day, the peak of expression was at 4 days. During the following days, the expression was reduced gradually, reaching nearly the level in the adulthood by 30 days. (2)Synaptophysin expressions in hypothyroidism group were significantly lower than those in normal control group(P<0. 01). In hypothyroidism group the peak of synaptophysin expression was delayed by 3 days, attaining the high peak on 7 days. (3) Synaptophysin expressions in T4 substitution group were significantly higher than those in hypothyroidism group at 4 days, 7 days, 14 days, 21 days, and 30 days, but were still lower than the levels in normal group(P<0.05 or P<0.01, except 7 days synaptophysin expression at hippocampus DG,P>0.05). (4)Synaptophysin expressions in T4 hyperthyroidism group were significantly higher than those in normal control group at 14 days、21 days、30 days(P<0.05 or P<0. 01). Conclusion The results suggested that reduced thyroid hormone, T4 substitution, and T4 hyperthyroidism led to the changes of synaptophysin expression in the rat cortex and hippocampus, resulting in an adverse effect on synapses occurrence and related nerve circuit establishment, with appearance of peerless neuronal place or mistaken nerve thoroughfare, thus hindering the development and function of the brain. Timely T4 substitution therapy is an important way to correct the disturbance in synapses development of the central nervous system due to deficient thyroid hormone.

Objective To investigate the efficiency and safety of stent implantation for severe pulmonary vein stenosis after radiofrequency ablation (RFA)of atrial fibrillation. Methods The clinical date of 231 cases with atrial fibrillation received RFA treatment were analyzed retrospectively.Eight cases were confirmed with severe pulmonary vein stenosis after RFA by CTA.The stents were implanted after angiography. Results All the 1 9 serious pulmonary vein stenosis from the 8 patients were treated stent implantation. The degree of pulmonary vein stenosis,trans-stenotic gradient, mean pulmonary pressure and the level of BNP were significantly decreased after treatment (P <0.01).The minimal diameter of the stenosis was significantly increased compared with preoperative (P <0.01).Six months after stenting,2 patients in 4 pulmonary veins were stenosis. The stenosis was successfully di-lated by repeated interventions with balloon dilation. There was no stenosis appeared in the patients after the repeated interventions with balloon dilation for 3 months. Conclusion Stenting implantation was a feasible and safety method for severe pulmonary vein stenosis after RFA.

Objective To evaluate the diagnostic value of translabial two-dimensional (2D) ultrasound for female anterior pelvic floor dysfunction, and to discuss the ultrasound appearance of anterior pelvic floor dysfunction. Methods Translabial 2D ultrasound was performed on 63 patients who were diagnosed or suspected with anterior pelvic floor dysfunction using pelvic organ prolapse quantitation system, the position and mobility of bladder neck, the retrovesical angle and urethral rotation were measured during maximum Valsalva maneuver.Different types of anterior pelvic floor dysfunction diseases were diagnosed according to the ultrasound findings. Results Translabial 2D ultrasound diagnose different types of anterior pelvic floor dysfunction diseases based on the position and mobility of bladder neck, retrovesical angle and urethral rotation, including 15 cases bladder neck hypermobility,9 cases funneling of the bladder neck and 37 cases cystocele. Conclusions Translabial 2D ultrasound could provide useful information of anterior pelvic floor and is of great value in the diagnosis of female anterior pelvic floor dysfunction.

Objective To dynamically observe and evaluate the position and movement of pelvic floor organs in vaginal delivery primipara with translabial two-dimensional(2D) ultrasound, and to provide the imaging basis for the effects of pregnancy and vaginal delivery on the pelvic floor support structures. Methods Translabial 2D ultrasound was performed on 52 vaginal delivery postpartum women (postpartum group) and 43 normal women as control group to observe the position and movement of pelvic floor organs at rest,on pelvic muscle contraction and during maximum Valsalva maneuver. Results The location of urethrovesical junction and cervix were significantly lower in postpartum group compared with the control group at three stages. The movement of urethrovesical junction and cervix in vertical direction was greater in postpartum group than control group during maximum Valsalva maneuver, while was smaller on pelvic floor contraction. Conclusions Translabial 2D ultrasound could dynamically observe and evaluate the position and movement of pelvic floor organs.Pregnancy and vaginal delivery produce certain effect on pelvic floor support structures, which manifest as weak, defective and dysfunction. The impact was more obvious in anterior and middle compartments.

Chinese Journal of Nephrology 2011;27(9):641-645
Objective To clarify the association of onset and maintenance of metabolic syndrome (MS) with energy metabolism imbalance, especially with dialysate glucose load in peritoneal dialysis (PD) patients. Methods Using retrospective self-controlled study, the changes of MS,dialysate glucose load and dietary energy intake (DEI) in 126 PD patients in about 1 year were collected and analyzed to define the effect of energy intake on MS.Resting energy expenditure (REE) was measured and physical activity level (PAL) was evaluated based on the activity records in PD patients with unchanged MS state and their impacts on MS were analyzed. Results The incidence of changing from non-MS to MS was higher in glucose load increasing group than that of glucose load unchanged or decreasing group. When glucose load increased, patients developing MS had significantly increased serum triglyceride (TG) level (P<0.01) and significantly decreased serum high density lipoprotein cholesterol (HDL-C) level (P<0.05),while the waist circumference and blood glucose level did not alter significantly. In patients changing from MS to non-MS,their serum C reactive protein (CRP) levels significantly decreased during the follow-up (P<0.05).No significant difference was found in DEI in patients changing from MS to non-MS.However,in patients changing from non-MS to MS,their DEI decreased during the follow up (P<0.05).In a subgroup analysis in 36 PD patients who maintained their metabolic status and did not change their glucose load, there was no difference in REE per body surface per day between the MS group and the nonMS group (t=0.840,P>0.05).However,the PAL was lower in the MS group than that of the nonMS group (t=2.358,P<0.05).Conclusions The increase of dialysate glucose load may be an important factor leading to the onset of MS,by altering serum TG and HDL-C level.Inflammation and the sedentary life also contribute to the MS state.
