1.A study on evaluation of heart function after percutaneous coronary intervention by real-time three-di-mensional echocardiography

Objective:To explore application value of real-time three-dimensional echocardiography (RT-3DE ) for heart function evaluation after percutaneous coronary intervention (PCI) .Methods:Clinical data of 45 patients with coronary heart disease (CHD) collected from Jan 2013 to Jan 2014 in our hospital were enrolled .All patients received selective coro-nary angiography (CAG) and PCI .Another 45 healthy cases undergoing physical examination during the same period were selected as normal control group .End-diastolic volume (EDV) ,end-systolic volume (ESV) ,left ventricular ejection frac-tion (LVEF) ,peak ejection rate (PER) and peak filling rate (PFR) were measured by RT-3DE and two-dimensional echo-cardiography (2DE) respectively and compared between two groups .Results:In normal control group ,there were no sig-nificant differences in all indexes between two measuring methods ,P>0.05 all;in CHD group ,compared with 2DE meas-ured value ,there were significant rise in EDV [ (130.6 ± 30.3) ml vs .(152.5 ± 35.2) ml] ,ESV [ (75.2 ± 21.8) ml vs . (94.1 ± 22.5) ml] ,PER [(2.41 ± 0.47) vs .(2.81 ± 0.39)] and PFR [(1.85 ± 0.25) vs .(2.13 ± 0.31)] ,and significant reduction in LVEF [ (45.8 ± 5.2)% vs .(41.6 ± 4.9)% ] in RT-3DE ,P<0.01 all;on one week ,one month and three months after PCI ,EDV and ESV gradually reduced ,LVEF ,PER and PFR gradually rose in CHD group with significant difference ,P<0.05 or <0.01 .Conclusion:RT-3DE can perform direct ,accurate and quantitative evaluation on heart form and function before and after PCI ,which is a reliable ,fast ,simple and new method evaluating PCI therapeutic effect in CHD patients .
2. Population pharmacokinetics of doxapram in five different Chinese ethnic groups

OBJECTIVE: To construct a population pharmacokinetic (PPK) model of doxapram in five different ethnic groups including Han, Mongolian, Korean, Hui and Uigur by NONMEM. METHODS: Fifty healthy volunteers were given a single dose of 50 mg doxapram intravenously. Blood samples were collected and determined by HPLC with a UV detector. Population pharmacokinetic model was constructed by NONMEM software. The influences of fixed-effect factors on the PK parameters were investigated. RESULTS: The following population parameters were estimated with a three-compartment model: CL1 of 17.8 L · h-1, CL2 of 10.5 L · h-1, CL3 of 85.2 L · h-1, V1 of 17.4 L, V2 of 62.6 L, and V3 of 16.8 L. Ethnic group has a significant effect on V2, CL3 and CL1, while gender has a significant effect on CL2. CONCLUSION: The final PPK model of doxapram established by NONMEM is robust and reliable. 3. Pharmacokinetcs of single dose cefepime injection in Chinese Han, Mongolia, Korean, Uigur, and Hui healthy volunteers

OBJECTIVE: To investigate and compare the pharmacokinetics of cefepime in healthy volunteers of different Chinese nationalities (Han, Mongolia, Uigur, Korea, and Hui) and different genders, thus to provide instruction and help for using cefepime safely, rationaly and effectively in both ordinary clinic and remedy of battle wound. METHODS: A single dose of 1.0 g cefepime was given by intravenous infusion under fasting condition to ten healthy volunteers (five males and five females) of each Chinese nationality. Plasma samples were obtained before dosing and at various predetermined time points after the dosing, and the plasma concentrations were determined by a validated HPLC-UV method. The pharmacokinetic parameters were calculated by DAS2.0 software and were compared by SPSS11.5 software. RESULTS: The PK parameters of cefepime in Han, Mongolia, Uigur, Korea and Hui nationalities after a single intravenous administration of 1.0 g were as follows: t1/2β (1.97 ± 0.20), (2.10 ± 0.33), (2.06 ± 0.33), (2.40 ± 0.65), and (1.94 ± 0.24) h; AUC0-β (139.69 ± 23.78), (153.92 ± 22.79), (165.58 ± 24.49), (142.11 ± 29.77), and (132.78 ± 23.79) mg · h · L-1;ρmax(68.75 ± 10.41), (78.22 ± 13.87), (83.19 ± 10.31), (73.82 ± 9.59), and (62.91 ± 10.09) mg · L-1 , respectively. The PK parameters of cefepime in 25 male volunteers and 25 female volunteers after a single intravenous administration of 1.0 g were as follows: t1/2β (2.06 ± 0.54) and (2.13 ± 0.44)h; AUC0-β (134.06 ± 22.33) and (158.68 ± 24.85)mg · h · L-1;ρmax (66.94 ± 9.14) and (78.95 ± 12.71)mg · L-1, respectively. Results of ANOVA and Kruskal-Wallis rank test showed that there were no significant differences in the PK parameters Vd, CL, InAUC, and t1/2β of different nationalities and in Vd and t1/2β of different genders, but there were significant differences in lnρmax of different nationalities and CL, InAUC, and lnρmax of different genders. Then, it was found there was high linear correlation( P < 0.01) between body weight and CL, InAUC and lnρmax. The differences of the above PK parameters became insignificant after normalization by body weight. CONCLUSION: When cefepime is given in doses based on body weight, its pharmacokinetics has no significant differences among five nationalities and between genders. 4.Preparation and Application of Internal Quality Control Substance of Free Hemoglobin

Objective To prepare an internal quality control substance of free hemoglobin (FHb),and evaluate its perform-ance.Methods ① 5 normal whole human blood samples with known hemoglobin (Hb)concentration was selected and dilu-ted with distilled water in the ratio of 1∶500,1∶1 000,1∶2 000,1∶4 000 respectively.After 20 FHb solutions was fully mixed,the concentration of FHb solutions was tested and compared with its theoretical data.②One of the highest value and the lowest value solutions were chosen as qualitye control substance,packed and stored in -20℃ fridge.Each group was an-alyzed 20 days continuously in the routine conditions,where average (-x )of the groups,standards deviation (s )and coeffi-cients of variation (CV)were calculated and Levey-Jennings control chart was conducted.Then,the test was repeated every week,and the data on the chart was updated accordingly until (CV)of the reagents were changed.The average (-x)of the-groups,standards deviation (s)and coefficients of variation uality control material was calculated during the whole test peri-od.Results ①The theoretical value (-x ±s)of the concentration of free hemoglobin was 125.44±93.04 mg/L,and the actu-al value was 125.22±93.08 mg/L,there was no statistic significance (t value was 0.706,P >0.05).② During 20 continu-ously daystest,the detected values (-x ±s)of the highest quality control material was 303.55±3.70 mg/L;the values of the lowest quality control material was 69.29 ± 1.88 mg/L.The coefficients were 1.22% and 2.68% respectively.Both the highest and the lowest quality control material were under control during the whole detecting period.The (-x ±s )values of the highest was 302.56±3.99 mg/L,the CV value of it was 1.32%;The (-x ±s)values of the lowest was 69.04±1.88 mg/L,the CV value of it was 2.71%.Conclusion The preparation method of internal quality control substance of free hemoglo-bin is convenient and stable,and was applicable in blood station’s and clinical laboratory.

Objective:To study the difference of upper airway form and hyoid position of the adults with different vertical facial types.Methods:Cephalograms of 120 adults with different vertical facial types were retrospectively investigated by computerized cephalometric analysis.Upper airway depths and hyoid position were measured,and differences were compared between male and female,also compared among different vertical facial type groups.Results:The sagittal depth of upper airway in male was larger than that in female,but there was no statistical difference except V-LPW.Among different vertical facial types,the sagittal depths of upper airway in high angle group was smaller than that in average angle group and low angle group.There was statistical difference in hyoid position between genders,AH-SN,AH-FH and AH-C3 in male were larger than in female(P
10.Evaluation of Pemberton osteotomy for the treatment of developmental dysplasia of the hip in older children

[Objective]To evaluate the outcome of the Pemberton osteotomy for the treatment of developmental dysplasia of the hip (DDH) in older children. [Method]There were 21 children (24 hips) older than 7 years in our study , who received the Pemberton osteotomy, and combined with femoral shortening, derotational or varus osteotomies according to the level of dislocation, anteversion and neck-shalf angle of the femur. The average age at operation were 9 years and 7 months (range: 7~13.5 years).[Result]The mean follow-up were 5 years and 5 months. Eighteen hips achieved the excellent or good clinical results according to the McKay's clinical criteria. All of the hips got improvements of the Acetabular Index, Sharp Acetabular Angle, Center-Edge Angle and Acetabular Head Index. The average results were in the normal range. Eighteen hips got ⅠorⅡclass of the Severin’s radiographic classification.However, 9 hips were limited in the range of movement. Four hips got subluxation.Six hips developed avascular necrosis of the femoral head. Three hips got improved osteoarthritis.[Conclusion]The Pemberton osteotomy for the treatment of developmental dysplasia of the hip in older children can mostly achieve well-used, contained or concentric reduction hips, meanwhile need to reduce the complications.
