3.The relationship between the degree of intracranial and extracranial arterial stenosis and the plasma level of homocysteine in patients with ischemic cerebrovascular disease

Objective To study the relationship between the degree of intracranial and extracranial arterial stenosis and the plasma level of homocysteine ( Hcy ) in patients with ischemic cerebrovascular disease .Methods 96 patients with ischemic cerebrovascular disease were checked by DSA .The distribution of stenosis in the studied cerebral arteries was analyzed .According to the degree of stenosis ,all 96 patients were divided into some groups ,at the same time Hcy level was detected ,and the correlation between Hcy level and intracranial and extracranial artefial stenosis were analyzed .Results 35 of the 96 patients had normal findings in angiography ,and 61 cases had occlusive lesions of different degrees .The mean plasma Hcy concentration in patients with intracranial and extracranial arterial stenosis was 19.26 (14.79~26.13)μmol/L,which was significantly higher than 11.60(8.8~15.11)μmol/L in the patients with no arterial stenosis (P<0.05).There was a significant positive correiation between Hcy level and cerebral artery stenosis degree(r=4.126,P<0.05).Conclusion The plasma Hcy level was positively correlated with intracranial and extracranial arterial stenosis in patients with ischemic cerebrovascular disease ,incidence and nar-row-degree of cerebral artery stenosis are all along with the increasing of Hcy level .
4.Effect of mild hypothermia therapy on coagulatic-fibrinolytic function of head injury patients

Objective To study the effects of mild hypothemia therapy on coagulatic-fibrinolytic functions of the severe head injury patients.Methods 35 severe head injured patients from Jan 1998 to May 2000 were treated by mild hypothermia therapy and compared with past 30 patients.All patients's coagulate and fibrinolytic index was measured.Results All patients have coagulate- fibrinolytic disorders, mild hypothermia therapy can reduce consume of coagulate factors and inhibit secondary fibrinolytic function.Conclusion Mild hypothermia therapy is important to the severe head injury patients.
5.A dosimetric study of hippocampal-avoidance prophylactic cranial irradiation in intensity-modulated radiotherapy and volumetric modulated arc therapy for patients with localized small cell lung cancer achieving complete response after chemoradiotherapy

Objective To investigate the dosimetric characteristics of hippocampal?avoidance prophylactic cranial irradiation ( HA?PCI ) in fixed?field intensity?modulated radiotherapy ( IMRT ) and volumetric modulated arc therapy ( VMAT) and the feasibility and risks of hippocampal avoidance. Methods Prophylactic cranial irradiation (PCI) was performed for 16 patients with localized small cell lung cancer ( SCLC) who were treated in our hospital from January to August, 2014, and achieved complete response ( CR) after chemoradiotherapy, with a prescribed dose of 25 Gy in 10 fractions. CT localization image was fused with brain MRI image to contour the hippocampus on the fused image, and the boundary of the hippocampus was extended 5 mm outward to form the area for reduced dose. IMRT and VMAT plans with hippocampal avoidance were developed separately, and the dose distribution in the whole brain, the hippocampus, and the 5?mm area outside the hippocampus was evaluated for these two plans. Independent?samples t test was applied to evaluate the difference between the two groups. Results The mean hippocampal volume in the 16 patients was 2. 76 cm3 ( range 2. 56 ?3. 01 cm3 ) . The mean radiation dose ( Dmean ) in the hippocampus during IMRT and VMAT was 9. 04± 0. 20 Gy and 10. 32± 0. 28 Gy, respectively, reduced by 66. 0% and 61. 2%, respectively, compared with the prescribed dose ( P=0. 55);Dmean in the area for reduced dose during IMRT and VMAT was 13. 57± 0. 90 Gy and 14. 86± 0. 60 Gy, respectively, reduced by 49. 0% and 44. 1%, respectively, compared with the prescribed dose (P=0. 88). Conclusions HA?PCI in IMRT and VMAT meets the clinical requirements, and can reduce the dose in the hippocampus while ensuring the whole?brain radiation dose, and therefore can be applied in PCI and provide a technical support to protect the patient’ s neurocognitive function.
6.Effects ofⅠ,ⅡCrystal and Amorphous Forms of Lercanidipine Hydrochloride on the Preparation

OBJECTIVE:To study the effects of Ⅰ,Ⅱ crystal and amorphous forms of lercanidipine hydrochloride on the preparation,and provide theoretical basis for its development and consistency evaluation. METHODS:X-ray powder diffraction (XRD),infrared spectrophotometry(IR)and differential scanning calorimetry(DSC)were adopted to identify the 3 crystal forms of lercanidipine hydrochloride. XRD was used to compare the effects of crushing,grinding,pressing technology,wetting granula-tion,adhesive solvents(water,ethanol)and drying temperature(50,60,70℃)on stability of 3 crystal forms of lercanidipine hy-drochloride;the dissolution in vitro in water,hydrochloride,pH 4.5 acetate buffer,pH 6.8 phosphate buffer were compared among 3 crystal forms of Lercanidipine hydrochloride tablet. RESULTS:XRD showed both Ⅰ,Ⅱ crystal forms had characteristic diffrac-tion peak with inconsistent 2 θ values,amorphous had no characteristic diffraction peak;IR showed 3 crystal forms had different absorption intensity and absorption peak number;DSC showed Ⅰ,Ⅱ crystal forms had obvious endothermic peak in 194.6 ℃, 207.3 ℃,respectively,amorphous had obvious endothermic peak in 86.1 ℃ and exothermic peak in 299.8 ℃. Crushing,grinding, pressing and drying temperature had no effects on the stability of 3 crystal forms;water had no effect on the stability of crystal in wetting granulation,ethanol may cause the change of Ⅰcrystal form. Except for the comparison between Ⅰ,Ⅱ crystal forms in hydrochloride (f2=68),the dissolution f2 of 3 crystal forms in 4 kinds of medium were lower than 50. CONCLUSIONS:XRD, IR,DSC methods can identify the 3 crystal forms of Lercanidipine hydrochloride tablet. When preparing lercanidipine hydrochlo-ride by Ⅰcrystal form,wetting granulation should avoid using ethanol as a adhesive solvent,instead of water. Different crystal forms can affect the dissolution in vitro of prepared Lercanidipine hydrochloride tablet.

8.A rapid PCR method to determine the types of cyp2d6~*10 alleles

Aim To establish a new simple, easy and economical PCR method for determining cyp2d6~*10 allele and studying its distributive frequencies in Chinese population. Methods The new PCR are established and compared with the classical PCR-RFLP. The 224 samples have been determined with the new PCR.Results the results of the two methods go on the way. The allele distribution frequencies of cyp2d6~*10 resembles the reports. Conclusion the new method is proved to be accurate and convenient. It provides a genetics basis for the drug therapy with individuals in clinics.
9.Expression of PRMT5 in human gastric cancer tissue and its significance

Objective:To investigate the expression of protein arginine methyltransferase 5 (PRMT5)in the gastric cancer tissue,and to explore the relationship between the PRMT5 expression and the occurrence and the clinicopathologic characteristics of gastric cancer, and to find a new treatment target for gastric cancer.Methods:Forty-three cases of gastric cancer tissue and 15 cases of adjacent normal gastric tissue were used as gastric cancer group and control group,respectively.The immunohistochemical staining was used to detect the expression of PRMT5 in the specimens.Results:Compared with control group (13.3%),the expression rate of PRMT5 in gastric cancer group (72.1%)was significantly increased (P < 0.05).In control group,there was some weak positive expression of PRMT5,the expressions were weak or moderate,the tan particles mainly located in the cytoplasm,less in the nuclei.In high differentiation gastric cancer specimens,the tan particles mainly located in the cytoplasm and more in nuclei, characterized by medium or strong expressions. In poorly differentiated and undifferentiated gastric cancer specimens,the tan particles mainly located in the cytoplasm and nucleus,and strongly expressed in nucleus.The higher expression of PRMT5 was associated with the size of tumor diameter,degree of differentition,depth of invasion,degree of lymph node metastasis,and TNM stage (P <0.05 or P <0.01),but had no correlation with the gender and age of patients (P >0.05).Conclusion:High expression of PRMT5 may play an important role in the occurrence and development of gastric cancer.
10.Clinical analysis of the cerebral DSA in 99 patients with ischemic cerebrovascular disease

Objective To explore the distribution of vascular lesions in the cerebral digital subtraction angi-ography( DSA) and associated diseases in patients with ischemic cerebrovascular disease( ICVD) ,and complications, as well as the value of DSA in ICVD.Methods The DSA data of 99 ICVD cases were analyzed retrospectively, patients treated by ischemia type were divided into the cerebral infarction(CI) and transient ischemic attack(TIA) group.All patients were examined by 3D-TOF and neck vascular ultrasound screening,the final diagnosis by DSA. While observing the perioperative complications and associated diseases.Results 99 patients with ICVD,88 cases of the presence of vascular lesions diagnosed by DSA.In 99 cases,there were 252 lesions,accounting for the top three for the internal carotid artery(ICA),vertebral artery(VA) and the middle cerebral artery(MCA).While there were 121 bifurcation lesions,accounting for the top three for ICA,VA,and the external carotid artery(ECA).TIA in patients with concomitant diseases accounted for the top three for hypertension(HTN),type 2 diabetes mellitus(T2DM) and hyperlipidemia and renal insufficiency;Patients with CI associated diseases accounted for the top three for HTN, T2DM and hyperlipidemia.There were 10 cases of patients with DSA complications,most of the vasovagal reflex in five cases;serious for TIA and CI in 1 case ( not cause any neurological deficits at discharge ) .Conclusion Carotid vascular disease plays an important role in the occurrence of ICVD, especially in the blood vessels of bifurcation lesion.Within ICVD risk factors in HTN in the majority, followed by T2DM, hyperlipidemia.DSA surgery rarely serious complications,blood vessels can provide more comprehensive information,and provide the basis for further development of ICVD prevention and treatment programs,this checking method is safe and reliable.
