Objective To explore the current situation and the relationship of job satisfaction,job performance and career plateau in senior health technical personnel.Methods The career plateau questionnaire,job satisfaction scale,job performance scale were adopted to carry out the questionnaire survey among 410 senior health technical personnel in Wanzhou district of Chongqing.The results underwent statistical description,correlation analysis and regression analysis.Results Senior health technical personnel's career plateau score was 2～5 points,with an average of (3.26±0.58) points,job satisfaction score was 20～60 points,with an average of (44.81±11.46) points,job performance score was 20～75 points,with an average of (35.79 ± 9.55) points.The score of career plateau was significantly negatively correlated with job satisfaction,and was significantly negatively correlated with job performance.The content of plateau was a factor affecting job satisfaction,and the content and the level of plateau were influencing factors of job performance.Conclusions Career plateau in senior health technical personnel was in a moderate level,while job satisfaction and job performance were relatively low.Hospital managers should take in occupation development assistance strategy and reduce negative influence of career plateau to improve job satisfaction and job performance in senior health technical personnel.

Objective To eveluate the effect of tirofiban on TIMI flow in patients with STEMI during primary PCI. Methods Forty-eight patients with STEMI who underwent primary PCI were randomly divided into two groups which were: the tirofiban+PCI group (n=27) and the primary PCI group (n=21). The TIMI flow before and after the operation were reviewed angiographically. BACKGROUND:It is suggested that glucose metabolism of frontal lobes,cingulate gyri and caudate nuclei in cerebral functional area of patients with depression is low,and the blood flow in cerebellum decreases.OBJECTIVE:To observe the effect of scalp acupuncture on cerebral glucose metabolism in depression by positron emission tomography.DESIGN:Randomly controlled and verifying experiment.SETTING:Department of Acupuncture and Aoxibustion in Nanfang Hospital and Center of Positron Emission Tomography in a Guangdong People's hospital.PARTICIPANTS:Twelve patients of primary depression,from Nanfang Hospital,Zhujiang Hospital and Center of Positron Emission Tomography in Guangdong People's Hospital from October 2002 to October 2003,were selected randomly with the diagnostic criteria of [F32] in the Third Edition of the Chinese Classification and Diagnostic Criteria of Mental Disease.Among them,3 were male and 9 were female.Their ages were from 36 to 66 years,with the average of 48.METHODS:Giving scalp acupuncture in middle line of vertex(MS5),middle line of forehead(MS1) and bilateral lateral line 1 of forehead(MS2).All the patients received positron emission tomography examination before and after acupuncture treatment for 6 weeks.Then cerebral Regions of Interest,bilateral frontal lobes,bilateral caudate nuclei,bilateral cingulate gyri and bilateral cerebella,were chosen and their average glucose metabolisms were counted by half-quantifying analysis.MAIN OUTCOME MEASURES:Glucose metabolism of bilateral frontal lobes,bilateral caudate nuclei,bilateral cingulate gyri and bilateral cerebella.RESULTS:According to intension,all 12 patients with primary depression sults of right frontal lobes(2.91 ±0.34),left frontal lobes(2.96±0.26),right cingulate gyri(2.68±0.20),right caudate nuclei (2.61±0.39) and left cerebellum(2.55±0.18)were obviously higher than those of right frontal lobes(2.53±0.31),left frontal lobes(2.53±0.36),right cingulate gyri(2.34±0.31),right caudate nuclei(2.40±0.33) and left cerebellum(2.35±0.25)before treatment(P＜ 0.05-0.01).However, the results of left cingulate gyri (2.69 ±0.38),left caudate nuclei(2.66±0.01) and right cerebellum(2.50±0.27) were similar to those of left cingulate gyri(2.43±0.38),left caudate nuclei(2.45±0.21) and right cerebellum(2.30±0.34) before treatment(P ＞ 0.05).CONCLUSION:Scalp acupuncture on middle line of vertex,middle line of forehead and bilateral lateral line 1 of forehead could improve the glucose metabolism in depressive patients of certain cerebral regions,and it might be a mechanism related to improvement of frontal lobes,caudate nuclei,cingulate gyri and cerebellum.

PurposeTo investigate CT-guided needle aspiration biopsy in segmental bronchial lesions of central lung cancer., and to provide basis for developing therapeutic schedule.Materials and Methods Fifty-five cases of suspected central lung cancer with segmental bronchial lesions were selected. All patients underwent CT-guided needle aspiration biopsy. The puncture complications and the effect of different position and direction of the puncture needle on complications were observed.Results All 55 patients were punctured successfully in one time. Satisfactory biopsy specimens were collected in all patients. Pathological diagnosis rate was 90.91% (50/55). Operative pathology or post-puncture follow-up confirmed the central lung cancer with segmental bronchial lesions in 40 cases with positive rate of 72.73% (40/55), and the negative rate was 18.18% (10/55). As for puncture complications, the needle tract bleeding was seen in 19 cases (34.55%), and some pneumothorax in 4 cases (7.27%). There was statistic difference between the two complications (χ2=6.03,P<0.01). The complications of prone and supine position had no statistical difference (P>0.05). Incidence of complications decreased with performance angle by the order of vertical puncture, horizontal puncture, and 45 degrees puncture. There was statistic difference (χ2=3.68,P<0.05).Conclusion CT-guided percutaneous needle aspiration biopsy is accurate in diagnosis of central lung cancer with segmental bronchial lesions. It has no serious complication and is worthy of general clinical application.

Objective To evaluate the clinical efficacy and toxicity of EPOCH regimen in the treatment of elderly patients with peripheral T-cell lymphoma. Methods Twenty-eight elderly patients with pathologically diagnosed peripheral T-cell lymphoma were treated with EPOCH regimen, including 96-hour continuous infusion of etoposide 50 mg/m2, epirubincin 12 mg/m2 and vincristin 0.4 mg/m2 on daysl through 4,cyclophosphamide 750 mg/m2 given as intravenous bolus on day 5 and prednisone 60 mg/m2 administered orally on daysl through 5. The EPOCH regimen was repeated very 21 days. Clinical efficacy and safety profiles of EPOCH regimen was systemically reviewed and analysed. Results All the 28 patients received a total of 85 cycles of EPOCH regimen. The median cycles was two courses. Fifteen patients achieved complete response, while five cases obtaining partial response. The overall response rate was 71.4 %. The median survival time was 20 months. In newly diagnosed patients, complete response rate (CR) as well as partial response (PR) and overall response rate (OR) reached 64.7 %, 23.5 % and 88.2 %, respectively, which was significantly higher than that in refractory cases, whose CR, PR and OR were 36.4 %, 9.1% and 45.5 %(λ 2 = 5.99, P ＜0.05). In addition, the median survivalduration of newly diagnosed patients was longer than that of refractory cases, whose median survival time was 24 and 13 months, respectively. The major adverse events was myelosuppression with grade 3-4 neutropenia and thrombocytopenia in 53.6 % and 50.0 % cases.Non-hematologic toxicities were moderate and uncommon. The frequency of adverse effects in de novo patients showed little difference in comparison with that in refractory ones (P＞0.05). Conclusion EPOCH regimen was an effective and well tolerated therapeutic schedule for elderly patients with peripheral T-cell lymphoma.

Purpose To observe the effect of acupuncture on gnosia and acetylcholinesterase in patients with senile dementia. Methods Eight patients diagnosed with mild or moderate senile dementia were treated by acupuncture of Sishencong ( Ex-HN 1 ), Shenmen ( HT 7) and Taixi ( KI 3) for I month; gnosia was evaluated by Mini-mental state examination before and after the treatment; plasma acetylcholin esterase activity was measured by flourier before and after the treatment. Results There was a significant difference in gnosia between pre- and post--treatment with acupuncture (P＜0.01); there was no significant difference in acetyl- cholinesterase activity between pre- and posttreatment ( P＞0.05 ). Conclusion Acupuncture has a certain improving effect on gnosia in senile dementia;one month's acupuncture treatment had little effect on plasma acetyl-cholinesterase activity.

Objective To carried out rat as deep inferior epigastic perforator (DIEP) flap necrosis model,to reveal the continuous blood fluid change in process of flap necrosis.Methods Seven SpragueDawley rats of male were used.The designed DIEP flap model was pedicled on the right-sided cranial perforator.Near-infrared fluorescent angiography was performed using SPY imaging system pre-and-aft operation and all angiography videos were compared and analyzed.Results Using SPY imaging system could observe the process of the blood fluid scattered from perforator through choke anastomoses to next vascular territories in living body.The study could clearly observe sequence change of blood fluid pr-and-aft operation,which could give the important information in revealing the reason of necrosis in DIEP flap.Cross-midline traffic vascular sparse and eventually could no longer wear through the contralateral abdominal wall vessel area and open in the region of lateral thoracic vascular occlusion of blood vessels to reach the flap to the far left side,and showed no blood flow in the dark zone.Conclusion The reasons of necrosis of contralateral distal zone of the flap model,are the choke anastomoses across the midline to the contralateral side appeared small,less intensity and the blood supply to contralatral distal zone should through twice choke anastomoses.
