1.The effects of rosiglitazone on endothelin, nitric oxide and C-reactive protein in patients with impaired glucose tolerance

Objective To observe the changes in endothelin(ET), nitric oxide(NO) and C-reactive protein(CRP) before and after the rosiglitazone therapy in patients with impaired glucose tolerance(IGT). Methods 72 IGT patients were divided randomly into 2 groups: rosiglitazone group(n=45) and placebo group(n=27). 15 weeks later, the efficacy of rosiglitazone was compared. Results The levels of ET, CRP were decreased and NO increased significantly in rosiglitazone group compared with those before treatment (ET 54.8±10.0 pg/ml vs 72.8±24.3 pg/ml, P<0.01;CRP 4.58±0.38 mg/L vs 5.99± 0.85mg/L, P<0.01 ; NO 41.0±10.0 μmol/L. vs 31.3±9.0 μmol/L, P<0. 01)). However, the levels of ET,CRP and NO in placebo group did not change singnificantly (P>0. 05). Conclusions Rosiglitazone can singnificantly improve ET, NO, CRP level in IGT patients, which means an additional effect of rosiglitazone on the correlation of inflammation and endothelial disturbance.
2.Bone bolt for the treatment of the haplo-detached of distal tibiofibular joint

Objective To observe the results of bone bolt crews in treatment of the haplo-detached distal tibiofibular joint.Methods The haplo-detached distal tibiofibular joint were fixed with bone bolt in 7 patients.Their outcomes were evaevaluated according to Mast and Teipner criteria and X-ray.Results All 7 patients were followed up from 12 to 36 months.All were assessed as good,and no fair or poor cases were found.Conclusion Bone bolt fixation was believed to be a good method to treat the haplo-detached distal tibiofibular joint with simple operation,beared shear force and control of brew tightress and was helpful in weight loading and functional exercises in the earlier period after operation,so bone bolt fixation was good in practical value in clinc.
3.Intracranial ependymal tumour: a clinical study of 83 cases

Objective To study the clinical features of patients with intracranial ependymal tumour, and explore the prognostic factors which have.a significant association with the outcome. Methods Eighty-three patients with histologically proven ependymal tumour were retrospectively reviewed. The tumour location, the patients' age, the histological features, imaging characteristics, surgical management and adjuvant therapy were studied. Results In 83 cases of intracranial ependymal tumour,41 cases were infratentorial and 42 cases were supratentorial ependymal tumour, 37 patients age ≤15 years, while 46patients age > 15 years. Fifty-eight patients ( 69.9% ) and 25 patients (30.1% ) had low (WHO Ⅰ - Ⅱ ) and high-grade (WHOⅢ) tumors respectively. Sixty-reven (80.7%) patients had gross total resection and 16patients ( 19.3% ) had subtotal resection. The 5-year survival rate was 81.9%( 68/83 ). The factors that were found to have a statistical significance association with the outcome were the extent of the resection and the pathologic characteristics. Conclusions Gross total resection of these tumors results in a good long-term outcome. Reoperation should be considered, if feasible, to remove residual malignant tissue. Anaplastic ependymomas carries a much poorer prognosis. Radiotherapy may play an important role in their treatment.The role of chemotherapy appears limited but can be attempted, especially in the very young, in order to avoid the deleterious effect of radiotherapy.
4.Application of clinical nursing pathway in perioperative period of patients with hypospadia

Objective To investigate the significance of clinical nursing pathway in perioperative period of children with hypospadia.Methods 63 cases of hypospadia children patients were randomly divided into the control group (30 cases)and the experimental group (33 cases).The traditional nursing method was used in the control group,while the clinical nursing pathway was used in the experimental group.Results The hospital stay,hospital charges and complications of the experimental group were less than the control group.The satisfaction degree of the experimental group was higher than the control group.There was significant difference.Conclusions Clinical nursing pathway is a safe and effective nursing model.It can shorten hospital stay,reduce hospital charges,decrease complications,and improve patients' satisfaction degree.
5.Assessment of safety of the prostate biopsy guided by transrectal ultrasound

Objective To evaluate the safety of biopsy of the prostate guided by transrectal ultrasound.Methods Three hundred and seventeen patients suspected to have prostate cancer were examined and 208 patients were followed up.Results One hundred and eighty-one patients followed up did not feel severe pain during the biopsy of the prostate. Clinical manifestations induded 12 cases of hematuria,4 dases of hematospermia,1 case of fever, 2 cases of pollakisuria and urodymia,2 cases of uroschesis,which disappeared after corresponding treatment.Conclusions Biopsies of the prostate guided by transrectal ultrasound is advantageous in diagnosis of prostatic carcinoma.
6.Investigation of correlation and mechanism between moderate hypoxia and tumor malignant transformation

Objective To investigate the correlation between moderate hypoxia and tumor invasiveness in colon carcinoma cell line HT‐29 and detect the expression of NF‐κB protein in nucleus and activities of MMP‐2/9 ,so as to explore its latent mecha‐nism of malignant transformation .Methods The moderate hypoxia model was established according to the pressure of O2 of tumor in vivo .After different period in moderate hypoxia ,activities of MMP‐2/9 secreted by HT‐29 cells were measured by gelatin zymog‐raphy ,NF‐κB levels in nucleus were assessed by Western blot after extraction of nucleic protein .Results MMP‐2/9 activities under moderate hypoxia gradually up‐regulated in 12 h and reached the peak at 24 h .The expression of NF‐κB protein in nucleus was simi‐lar to that of MMP‐2/9 .Conclusion Moderate hypoxia could induce malignant phenotype in HT‐29 cells through up‐regulating MMP‐2/9 activities ,in which NF‐κB may involve in this progression .
7.Screening protein interacted with enterovirus 71 3A protein by means of T7-phage display system

Objective To screen and identify protein interacted with enterovirus 71 (EV71)3A protein by means of T7-phage display system.Methods The prokaryotic expression vector of 3A protein was constructed for expressing and purifying 3A pro-tein.With 3A protein as the target protein,the T7-phage display system was adopted to screen the human hepatocyte cDNA library. The screened products were performed the DNA sequence analysis and the homologous research.Results 3A protein was expressed and purified,after 4 rounds of biopanning,37 positive clones were selected.The inserted fragments were amplified by PCR using specific T7 primer.The products were sequenced,2 proteins were identified to interact with 3A protein by homologous analysis. Conclusion Trough the T7 phage display system,two proteins interacted with 3A are obtained.The possible function of 3A protein can be speculated by researching the function of these two proteins,which lays the foundation for the further studying the pathogen-ic mechanism of EV71.
8.A survey of sub-health and TCM physical constitution of stroke patients' families

Objective To explore the sub-health and TCM physical constitution of stroke patients’families.Method Four hundred and twelve family members of stroke patients hospitalized during September 2001 to May 2013 were involved in the survey by filling out TCM physical constitution scale(TCMPC)and the sub-health self-rating scale(SSS)for the purpose of investigating the sub-health and conditions of TCM physical constitution.Results Of all the patients,189 were in sub-health,accounting for 45.87%. There were 7 main clinical symptoms related to sub-health,including poor sleep,less activeness,proneness to upsets and irritation, memory loss,depression,tension in interpersonal relations and appetite loss,with the incidences ranging from 64.55%to 48.15%. In TCM,51.85%of the family members were balanced in physical constitution,10.58%of them had Yang-deficiency constitution, 14.29%of them had Yin-deficiency,23.28%of them other kind of constitution.Conclusions The family members of stroke patients have a high rate of sub-health.The members in sub-health hold a balanced constitution,Yang-deficiency or Yin deficiency.In order to eliminate their sub-health,improving the quality of care for patients with stroke,we should guide the families of stroke patients to develop good living habits,strengthen the TCM mental nursing,help them to establish a good social support system.
9.Optimization of immunosuppressive therapy after liver transplantation

The toxic effects of immunosuppressants are often dangerous factors which influence long-term survival of recipients.Individualized immunosuppressive therapy has been a hotspot and difficulty for comprehensive treatment of liver transplantation.Immunosuppressive therapy has been gradually developed to pursue long-term survival for receipients and graft,minimized side effects of immunosuppressants,optimized quality of life of patients,and reduce the economic burden of patients.Through consideration of patient's pathophysiological state,understanding the characteristics and side effects of various immunosuppressive,appropriate choose of the immune inhibitor,correct evaluation of immune state of recipients and graft,minimization of drug doses,individualized dosing regimen could be successfully carried out.
10.Blood gas analysis features of H7N9 influenza patients

Objective To review the blood gas analysis of H7N9 flu patients for better understanding the characteristics and prognosis of the emerging disease.Methods A retrospective review was conducted for the 15 H7N9 influenza patients,whose blood gas profile was tested with a Radiometer 80 Blood Gas Analyzerin our hospital.The profile of blood gas analysis was compared between the 12 surviving patients and the 3 deaths in terms of their outcomes.Results The 3 patients who died of H7N9 influenza averaged 83 years old.The partial pressure of oxygen was <8 kPa in 9 of the 12 surviving patients,which meet the diagnostic criteria of type I respiratory failure.Their partial pressure of oxygen all recovered to normal (11 .033 ± 2.335 kPa,reference range 10-14 kPa)on discharge.The pH value of peripheral arterial bloold averaged 7.335 (range 7.39-7.54)on adission for the 3 patients who died of H7N9 influenza.The worst average pH value was 7.233 (range 7.32-7.35), which was significantly different from the values of the surviving patients.Conclusions Elderly patients are susceptible to H7N9 influenza infection and subsequent respiratory failure.Old age was associated with higher mortality.The partial pressure of ox-ygen is not significantly associated with patient outcomes.Appropriate active treatment may recover the blood gas profile to normal regardless of the initial values.If decreasing peripheral arterial bloold pH value is highly suggestive of poor prognosis, comprehensive measures should be taken to keep acid-base balance,including assisted ventilation to correct hypoxemia.
