1.Analysis of Re-treatment for Cervical Lymph Node Recurrence in Nasopharyngeal Carcinoma Patients after Radiotherapy

Objective:To investigate the efficacy of re-treatment for cervical lymph node recurrence in nasopharyngeal carcinoma patients after radiotherapy and to explore the prognostic factors.Methods:Eighty-two patients who received therapy due to cervical lymph node recurrence after deftnitive radiotherapy were reviewed.The relevant factors including sex.the interval between radiotherapy and recurrence,rN stage,recurrence site,treatment modalities,and treatment efficacy were analyzed.Subgroup analysis was performed to review the relevant factors including the involvement in the adjacent tissues,the number of positive nodes and surgical range.Kaplan-Meier method,Log-rank test and Cox method were used for statistical analysis.Results:The 1-,3-and 5-year local control rates were 58%,39.0%,and 39.0%,respectively.The 1-,3-and 5-year overall survival rates were 82.9%,47.6%.and 25.0%,respectively.The 1-,3-and 5-year overall survival rates in the radiotherapy-based treatment group were 80.8%,37.1%and 19.1%,respectively.The 1-,3-and 5-year overall survival rates in the surgery-based treatment group were 86.7%,66% and 34.2%,respectively.The efficacy of surgery-based treatment was superior to that of radiotherapy-based treatment.In the multivariate analysis,tumor recurrence site was an independent prognostic factor.Conclusion:Radical neck dissection-based treatment can achieve satisfactory efficacy for cervical lymph node recurrence after radiotherapy in nasopharyngeal carcinoma patients.
2.The enhancement of the renal cortex and medulla and the renal function: a correlative study of conventional CT scan with glomerular filtration rate

Objective To study the relationship between the enhancement of renal cortex and medulla during the conventional CT scans with and without contrast enhancement and the renal glornerular filtration rate (GFR). Methods Thirty five cases with unilateral posternal obstructive hydronephrosis According to GFR, the 70 kidneys were divided into 4 groups: normal, slightly impaired, moderately impaired and severely impaired. In the cortical phase of enhanced CT scan, the enhancing intensity of the renal cortex and medullar was measured and following measures were calculated: CTce - CTcp, CTmc -CTmp,ratio of enhancement of affected and unaffected renal cortex and medullar, and the ratio of those of each case: CTac/CTnc, CTam/CTum, CTac/CTam, CTnc/CTnm, (CTac/CTam) / (CTuc/CTum). The differences of these measures in different groups were analyzed with one-way ANOVA and their correlation with GFR was studied with Pearson test. The best indicator to reflect enhancement of renal cortex was selected from these measures, and all the kidneys were divided into 4 groups accordingly: strong, less strong, medium and weak. Then its consistency with GFR groups were calculated. ( 2 ) Positive correlation was found between GFR and CTce-CTcp ( r = 0. 887, P <0. 01 ), CTac/CTuc ( r = 0. 872, P < 0. 01 ), CTam/CTum ( r = 0. 504, P < 0. 01 ), CTac/CTam ( r =0. 772, P < 0. 01 ), (CTac/CTam) / (CTuc/CTum) ( r = 0. 663, P < 0. 01 ). ( 3 ) Selecting CTac/CTam to reflect enhancement of renal cortex, the diseased kidneys can be divided into groups of strong ( CTac/CTam ≥2. 60); less strong (2. 20 ≤CTac/CTam <2. 60); medium ( 1.80 ≤CTac/CTam <2. 20); and weak ( CTac/CTam < 1. 80). The consistency with GFR grouping was 80. 0%. Conclusions There is significant correlation between renal cortex-related indicators and GFR, especially CTac/CTam, which can be used as the indicator of renal function in patients with unilateral or bilateral postrenal obstructive hydronephrosis. The conventional contrast enhanced CT scan can be taken as a semi-quantitative method of GFR measuring with relative low X-ray dosage compared with CT perfusion.

4.A Medical Devices Management Information System Supporting Full Life-Cycle Process Management.

6.The research, diagnosis, and treatment of thrombotic disease in China

The International Society on Thrombosis and Haemostasis has proposed to hold World Thrombosis Day on October 13 annually from 2014 to improve awareness and education of thrombosis. Thrombotic diseases are multidisciplinary disorders involving multiple systems.Thus, multidisciplinary team treatment for thrombotic diseases should be developed in China.A comprehensive study of genetic factors for thrombotic diseases is expected to achieve early diagnosis and risk prediction for this type of disease, and thus precision diagnosis and individualized treatment can be achieved.
7.Strategy on improving the status of Chinese hernia and abdominal wall surgery in international academic circles

Tension-free repair for inguinal hernias has been led into China since 1997 and being promoted quick development of hernia and abdominal wall surgery in China,meanwhile,there are still many problems in the recurrence,chronic pain and patch infection due to irregular operations.In order to solve these problems and improve the diagnosis,treatment and research levels of hernia and abdominal wall surgery,surgeons must pay attention to the basic and clinical technologies,quality control and so on.The studies on etiology of hernia will bring innovative therapy,registration system will contribute to summarize the clinical experiences,and establishment and implementation of quality control of hernia could ensure the effects of diagnosis and treatment.However,there is still a controversy about whether laparoscopic technology on inguinal hernias is minimally invasive surgery or not,though it is safe and effective.

10.Study of the individualized health management of senile mental disorders

Objective To evaluate the effect of the individualized health management on senile mental disorders.Methods 421 patients with senile mental disorders were randomized into two groups:the intervention group (n =213) received routine antipsychotic drugs treatment,health education and individual health management,and the control group(n=208) received only population health education and routine drug antipsychiatric treatment.Results The Brief Psychiatric Rating Scale (BPRS) scores in two group were significantly higher after treatment than before treatment (all P<0.01).The BPRS score in each period after treatment were significantly higher in intervention group than in control group (all P<0.01).The mini mental state examination (MMSE) score in two treatment groups were significantly higher after treatment than before treatment (all P<0.01).The MMSE score in each period after treatment were significantly higher in intervention group than in control group (all P<0.05).Conclusions The individualized health management in senile mental disorders is not only feasible,but also effective.
