1.Critical Discourse Analysis in Doctor-patient Discourse in Chinese Medical Clinic

Critical Discourse Analysis ( CDA) attempts to uncover the inequality in social reality and the way language presents power and ideology through the analysis of language features. Systemic functional linguistics ( SFL) focuses on the dialectical relationship between language and society and has been considered an important approach and theoretical basis to CDA. Doctors in Chinese medical clinic follow the Four Diagnostic Methods, within which the inquiry diagnosis strongly shows the fact that doctor and patient clash over the ideology and strug-gle for the power via language. The study carries out CDA on doctor-patient discourse in Chinese medical clinic within the theoretical framework of SFL from four aspects including nominalization, classification, transitivity and mood, which verifies that the language is the product of asymmetric power between doctor and patient. It is hoped that the paper would provide insight into CDA on doctor-patient discourse and would be of theoretical meaning.
2.Administration of Medical Decision-making in Market-oriented Mechanism---in the View of Medical Ethical Boundary and Stratification

In the circumstance of that the medical system reform follows to the principle of combination of government leading and market mechanism and in view of medical ethics boundary and stratification, this paper pointed out that whether the discussion and introspection of society or the doctors,medical decision-making should focus on the relationship between medical behavior and the patients′health interests, which is ensured by reasona-ble system. Decided by the current situation of medical decision -making stratification, layered construction of medical ethics is beneficial to that the medical behaviors gradually regress to the ethical value, while establishment of diagnosis and treatment norms is the ethical choice of current medical decision-making.

This paper focused on the role conflicts owing to the multiple identity of roles and professional limi -tations in the dispute processing personnel , analyzed the probability in the medical processes and results , appeal of the patients and pattern of medical dispute processing , and made the conclusion that the medical dispute processing personnel is volunteers promoting social harmony and a practitioner of new medical model from the social perspec -tive and should carry forward the dedication , fraternity, mutual assistance and progressive spirit , the concept of the biological -psychology -social medical model to analyze the loss of patients in dispute and appeal , truly safe-guard the health rights and interests of patients , and give some advices to perfect the role ethics dispute processing .

5.Determination of Cefdinir Concentration in Rat Plasma by High Performance Capillary Electrophoresis and Its Pharmacokinetics Research

OBJECTIVE:To determine the concentration of cefdinir in rat plasma,and investigate its pharmacokinetics. METH-ODS:High performance capillary electrophoresis (HPCE) was adopted by using fused-silica capillary (75 μm,total length of 30 cm,effective length of 21.5 cm),buffer solution of 20 mmol/L citric acid,injection voltage of 10 kV for 10 s,separation voltage of -20 kV,and detection wavelength of 214 nm. 6 rats were intragastrically received cefdinir solution(20 mg/kg). 0.2 mL blood sample was taken from the tail vein before and 0.25,0.5,1,1.5,2,3,4,6,12,24 h after administration. BAPP 2.0 software was used to calculate the pharmacokinetics parameters. RESULTS:The linear range of cedinir ranged 0.2-50 μg/mL(r=0.9997), lower limit of quantification was 0.2 μg/mL. The intra-day(n=6)and inter-day RSDs of(n=3)precision test were no more than 12.2%;RSD of stability test was no more than 9.10%(n=6);method recovery rate was 95.4%-114.3%(RSD=9.0%,n=6);matrix effect was 63.5%-70.2%(RSD=10.39%,n=6). The t1/2 of cefdinir in rats in vivo was(0.54±0.01)h,MRT was(1.90± 0.14)h,cmax was(32.92±0.81)μg/mL,tmax was(1.50±0.02)h,AUC0-24 h was(46.65±0.44)μg·h/mL and AUC0-∞was(46.83± 0.44)μg·h/mL. CONCLUSIONS:The method is rapid,accurate and simple,and can be used for the determination of cefdinir con-centration in rat plasma and its pharmacokinetics research.
6.The biomechanical analysis of the etiologies of spontaneous pneumothorax in flat-chest teenagers

Objective To investigate the etiologies of spontaneous pneumothorax in flat-chest teenagers. Methods 139 teenagers with flat chest and spontaneous pneumothorax were designed as the study group, while 152 teenagers without spontaneous pneumothorax during the same period were taken as control. The ratio of thoracic transverse diameters to anterior-posterior diameters, and the ratio of tracheal length to transverse diameters were measured respectively by CT scan. The resected pulmonary tissues in 47 cases with spontaneous pneumonthorax and in 47 cases without spontaneous pneumonthorax underwent various examinations, including tension, compression, and bending tests. Student t-test was employed to perform the statistical analysis. Objective To determine the contents of strychnine and brucine in Xiaobining Spray.Methods A HPLC method was adopted and the chromatogram conditions were as follows:column of Kromasil C18(4.6 mm? 250 mm,5 ? m),mobile phase consisting of acetonitrile-0.01 mol/L sodium heptanesulfonate and 0.02 mol/L potassium dihydrogen phosphate equivalent volume mixture(10 % phosphoric acid adjusting pH to 2.8)(21:79),column temperature of 30 ℃,flow rate of 1.0 mL/min,detection wavelength of 260 nm.Results A good linearity was obtained form 0.109 2 ? g to 0.546 0 ? g for brucine(r=0.999 6)and 0.173 6 ? g to 0.868 0 ? g for strychnine(r=0.999 7).The average recovery of brucine was 100.59 % with RSD being 1.81 %,and of strychnine was 100.83 % with RSD being 1.69 % .Conclusion This method is simple,reproducible,and can be used to determine the strychnine and brucine in Xiaobining Spray.

Objective To determine the concents of palmatine hydrochloride and berberine hydrochloride in the preparation of Fu Yan Xiao Lotion(FYXL).Methods The HPLC method was established for the determination of palmatine hydrochloride and berberine hydrochloride in the preparation of FYXL.The chromatographic column is Agilent Extend C18(4.6mm? 150 mm,5 ? m),mobile phase consists of water-acetonitrile(40 ∶ 60,3.5 g potassium dihydrogen phosphate and 1.5 g sodium dodecyl sufate per 1 000 mL),column tempreture was 30 ℃,and the flow rate was 1.0 mL/min.The detection wavelength was 345 nm.Results A good linearity is obtained in the range of 0.062 5 ? g to 1.250 ? g of berberine hydrochloride,and 0.068 6~ 1.372 ? g of palmatine hydrochloride.The average recovery of berberine was 98.63 %,RSD being 2.01 %.The average recovery of palmatine was 102.34 %,RSD being 1.67 %.Conclusion This method is simple and quick,and can be used for the quality control of FYXL.

Objective To observe the clinical effect of Shiwei Wendan Decoction on coronary heart disease stable angina pectoris of qi deficiency and stasis-phlegm type.
