1.Effect of epidural morphine for analgesia on the number of interstitial cells of Cajal in rabbit colon

Objective To investigate the effect of epidural morphine for analgesia on the number of interstitial cells of Cajal (ICC) in the rabbit colon.Methods Thirty healthy rabbits of either sex,weighing 2.0-2.5 kg,were randomly divided into 2 groups (n =15 each):control group (group C) and morphine group (group M).The animals were anesthetized with iv 20% urethane 4 ml/kg.The epidural analgesia was performed at L3,4 interspace.In group M morphine 9.2 ug· kg-1 · h-1 was infused into the epidural space for 7 consecutive days,while the equal volume of normal saline was infused instead in group C.The characteristics of excrement and constipation were recorded during morphine infusion.Normal saline or Chinese ink mixture 5 ml was administered via a gastric tube inserted through the mouth 1 day after the end of administration.The rabbits were sacrificed 30 min later and laparotomy was performed.The intestinal segment numbered from the pylorus to the ileocecal region of the bowel was removed and the length of the intestinal segment and propelled distance of the ink within the intestinal tract were measured in the tension-free state.The rate of propelled distance was calculated.The proximal and distal colon tissues were taken for measurement of the expression of c-kit (by immunohistochemisty) and ICC count,and stained with haematoxylin and eosin for examination of pathological changes.Results Compared with group C,the incidence of constipation was significantly increased,and the rate of propelled distance,and expression of c-kit and ICC count in proximal colon tissues were decreased at 3-7 days of morphine infusion (P < 0.05),and no significant change was found in the expression of c-kit and ICC count in distal colon tissues in group M (P > 0.05).Conclusion Epidural morphine for analgesia can inhibit the motor function of the intestinal tract in rabbits,and reduction in the number of ICC count in proximal colon is involved in the mechanism.
2.Study on endothelial dependent vasodilation function of brachial artery in renal allograft recipients

Objective To detect the endothelial dependent vasodilation function of brachial artery in renal allograft recipients. Methods Using high resolution ultrasound, the inner diameter and changes of diameter of brachial artery at rest, after reactive hyperemia and sublingual nitroglycerin were measured in 20 cyclosporine treated allograft recipients, 20 FK 506 treated allograft recipients and 20 controls, reactive hyperemia mediated vasodilation was measured to assess endothelial dependent vasodilation, nitroglycerin mediated vasodilation was measured to assess endothelial independent vasodilation, in order know if the endothelial function was injuried by comparing the endothelial dependent and endothelial independent vasodilation. Results After reactive hyperemia, the absolute and relative values(reactive increase) in brachial artery inner diameter were significantly reduced in renal allograft recipients, nitroglycerin mediated vasodilation was similar between renal allograft recipients and controls. There was no significant difference of the inner diameter of brachial artery in renal allograft recipients treated with cyclosporine and FK 506 after reactive hyperemia. Conclusions The endothelial dependent vasodilation function of brachial artery was injuried in renal allograft recipients.
3.The primary study of fluoxetine in the prevention of post-traumatic stress disorder

Objective To observe the effect of fluoxetine on the single prolonged stress model which mimic the post-traumatic stress disorder (PTSD). Methods Rats receiving single prolonged stress (SPS) (2 h restraint + 20 min FST + anaesthesized to lose consciousness with ethylether) or not were given fluoxetine or tap water for 15 days. Elevated plus maze(EPM),open-field test(OF) and morris water maze(MWM) tests were used to evaluate rats' fear response to environment,high alertness,anxiety & depression behavior,and learning and memory ability. Results In open field test, group of fluoxetine(F1 (8895. 85 ± 599. 78) mm, (40. 23 ±4. 32) s;F2 (8654.07 ±866.05)mm,(41.57 ±4.34)s, P<0.05) showed significant increase in activity times and horizontal motion distance compared with group of SPS (4678.85 ±495.33)mm, (22.15 ±3.43)s, P<0.05). In EPM experiment,group of fluoxetine(F1 (32. 62 ± 4. 57)% , (17. 58 ± 3. 23)% ; F2 (39. 75 ± 4. 46)% , (19. 74 ± 4.44) %) showed significant increase in percentage of the open-arm into the maze and percentage of the open arm pause compared with group of SPS ((23.67 ±2. 87)% ,(12.46 ±2.55)% , P<0.05). In MWM experiment,the escape latency of the SPS group increased significantly in comparison to that in sham group (P<0.01) and fluoxetine group. Fluoxetine significantly reversed the SPS-induced decrease in time spent in the target quadrant (P< 0.05). Conclusion Added fluoxetine can obviously improve rats' fear response to environment ,high alertness ,anxiety & depression behavior as well as learning and memory ability.

Objective: The aim of this study was to identify depression among HIV-
infected patients attending Infectious Disease Clinic in University Malaya
Medical Centre (UMMC). Methods: This is a cross sectional study on HIV-
infected patients attending Infectious Disease Clinic, UMMC. Those who fulfilled inclusion criteria were recruited and interviewed. Socio-
demographic characteristics and clinical conditions such as mode of transmission, year of diagnosis, CD4+ counts, drug treatment and clinical stage were collected. The patients were then subjected to self-administered questionnaires, Patient Health Questionnaire, (PHQ-9) and Hospital Anxiety & Depression Scale (HADS). Results: 89 patients were recruited. Scores from
PHQ9 showed 32% of depression rate while scores from HADS showed 19%
of depression rate among the respondents. Non-self financial supporter, non-
alcoholic drinkers and females were more likely to be depressed (P<0.05). All clinical characteristics showed no statistical differences. Conclusion: The depression rate was lower compared to those from the studies in western countries. The risk factors for depression were different from those found in other studies as well.
6.Cloning and Expression of Esterase Gene to Enantioselective Resolution of (S)-Ketoprofen in NK13

A strain NK13 was screened for a certain extent asymmetric hydrolyze the rac-ketoprofen chloroethyl ester and identified as Bacillus megaterium. For the preparation of gene libraries, a positive clones was obtained from the tributyrin flat. The sequence of this esterase gene had been analysised, and contained the whole ORF of an esterase gene with the length of 933bp. The esterase gene of NK13 was compared with the esterase genes of GenBank and the result showed that the esterase gene of NK13 was a novel gene(GenBank accession nember DQ196347).The new esterase gene was inserted into the plasmid pET21b+, then the recombinant plasmid transformed E.coli BL21. After being induced by IPTG, it was expressed in the host strain. SDS-PAGE analysis showed that the relative molecular mass of the esterase was about 34kDa. The result of TLC and HPLC showed that the recombinant strain had higher conversion ration than templet strain. 47.4%of the rac-ketoprofen Chloroethl ester was hydrolyzed to ketoprofen by the recombinant strain in 45min. The (S)- ketoprofen enantiomeric excess, in the later,was 55.46%, which indicated that the esterase could hydrolyze (S)-ketoprofen chloroethyl ester firstly.

8.Clinical effects of the treatment of gastrointestinal dysfunction after stable thoracolumbar fractures with Simo decoction oral liquid.

OBJECTIVETo evaluate the clinical effects of Simo Decoction Oral Liquid for the treatment of gastrointestinal dysfunction after stable thoracolumbar fractures.
METHODSFrom May 2005 to July 2008, 81 patients with stable thoracolumbar fractures were randomly divided into treatment group (41 cases) and control group (40 cases) according to a random digits table. The treatment group included 32 males and 9 females with an average age of (47.19 +/- 5.18) years old ranging from 21 to 55 years, and the course was from 1 to 45 hours with an average of (7.83 +/- 1.29) hours. The control group included 30 males and 10 females with an average age of (46.31 +/- 3.72) years ranging from 20 to 54 years,and the course was from 1.5 to 43 hours with an average of (8.15 +/- 1.63) hours. The treatment group were dealed with Simo Decoction Oral Liquid,and the control group with neostigmine for acupoint block in bilateral Foot-Three-Li. The recovery of gastrointestinal function and the first passage of gas by anus were compared.
RESULTSThe time of recovery of gastrointestinal function in treatment group (7.27 +/- 3.14) h was shorter than that in control group (10.12 +/- 3.62) h. The time of first passage of gas by anus in treatment group (15.39 +/- 13.70) h was significantly shorter than that in contral group (24.02 +/- 18.11) h. The total effective rate in treatment group was higher than that in control group.
CONCLUSIONBoth the treatment group and the control group have clinical effects in treatment of the restoration of gastrointestinal dysfunction after the stable thoracolumbar fractures, but the treatment group has more remarkable therapeutic effect and less side effects.

9.Diagnosis and treatment of 11 cases with sinonasal neuroendocrine carcinoma.

OBJECTIVETo explore the diagnosis and treatment and prognosis of sinonasal neuroendocrine carcinoma.
METHODSThe clinical data of 11 cases with sinonasal neuroendocrine carcinoma treated in Zhejiang Cancer Hospital from 1998 to 2010 were analyzed retrospectively. Ten cases were small cell type and one case was atypical carcinoid. One case by only operation, one case by only radiotherapy, one case by only chemotherapy, two cases by radiotherapy and chemotherapy, 6 cases by combined treatment based on surgery (surgery combined with radiotherapy and chemotherapy).
RESULTSAll cases have been followed up for 2 months to 12 years. Six cases by combined treatment one case died in 3 years and one case died in 4 years after treatment, one case has survived without tumor for 12 years and two cases have survived without tumor for 8 years, one case was still in treatment. Five cases of other treatment programs, four cases died in 12 months, one case died in 18 months.
CONCLUSIONSThere's no standard treatment plan. Combined treatment based on surgery should be adopted to nasal neuroendocrine carcinoma. Reccurrence is frequent and the prognosis is poor. The key to improving the survival rate of the disease is early accurate diagnosis and combined treatment.

10.Effects of Se-riched soybean peptide on antioxidant function in rats of fatty liver caused by high-fat diet.

OBJECTIVETo explore the effect of Se-riched soybean peptide (SSP) on antioxidant function in rats of fatty liver caused by high-fat diet.
METHODSForty Wistar rats were divided into 4 groups randomly and fed with standard diet and water (NC), high-fat diet and water (HC), high-fat diet and SSP (0.1 g/d) (SeH), standard diet and SSP (0.1 g/d) (SeN) respectively. After 10 weeks, the rats were killed to investigate the pimelosis level in liver tissues by Sudan III staining and the expression of hepatic GRP78 by immunohistochemical analysis. We also analyzed the changes of liver function, blood lipid, the glutathione peroxidase (GSH-Px), superoxide dismutase (SOD) activity and malondialdehyde (MDA) content in livers and serum.
RESULTSThe pimelosis level, total cholesterol (TC), triglyceride (TG), MDA contents and the expression of GRP78 in HC group were significantly higher than those in NC, SeN, SeH groups. The activities of GSH-Px and SOD in liver and serum were markedly up-regulated in SeH (P < 0.01). There was no significant difference between NC and SeN groups.
CONCLUSIONSSP can improve liver cell injury and the antioxidant functions in rats with fatty liver effectively and decrease the expression of GRP78 in liver.
