2.Vaginal Ureterolithotomy: A Case Report.

Herein we report an experience of the vaginal ureterolithotomy. The patient was a 48-year-old woman with a juxtavesical left ureteral stone, which was vaginally palpable. Although this operation has limited indications, it is a safe and easy method of removing stones from the lowermost ureter especially in the women. This method seems to have considerable value to be attempted in the carefully select patients.

3.Effect of Tubercin - 3 on a Case of Chornic Dermatitis with Selective Hereditaty Complete IgA Deficiency and Paritial T - Lymphocyte Inactivation.

A case of a clinical syndrome of selective hereditary complete IgA deficiency with partial T-lymphocyte inactivation is reported. A 14 year old boy was admitted with chjef complaint of persistent wide shallow ulcerated non-vesicular patches on the skin folds of whole body, chronic upper respiatory infections and gastointestinal upsets. General appearance showed us as if the undemourished or malabsorption status had been accompanied. The duration of illness was about 10 years under the incorrect or mistaken diagnosis and skin lesions have incompletely responded to the treatment. Peripheral T-lymphocyte determination was carried out by the method of Wybran to be tumed out active T-cell 14%, total T-cell 48%. Tuberculin test showed negative with 5 TU. Immunoglobulin values were respectively as follows: IgG 325 mg%, IgM 60 mg%, and IgA trace. And we diagnosed this case as selective IgA defiency with partial T-lymphocyte inactivation. We started the immunochemotherapy with tuberculo-protein complex, Tubercin-3 and antibictics for 2 months. After 2 months with above regimen. the skin lesions were markedly improved. T-cell count also retumed to the normal limit(active 35%, total 59%) and levels of TgG and IgM raised(IgG 617.5 mg%, IgM 75mg%), but IgA still showed trace amount, Tuberculin test tumed out positive.

4.Proximal Metatarsal Chevron Osteotomy Combined with Modified McBride Procedures for Hallux Valgus Patients..

Seventy cases of moderate to severe hallux valgus deformities were operated by proximal metatarsal chevron osteotomy combined with modified McBride procedures from September 1994 to September 1996. The hallux valgus angle improved from an average of 35.1 degrees to 12.1 degrees postoperatively. The first intermetatarsal angle improved from an average of 15.4 degrees to 7.9 degrees. The position of sesamoids was normalized and the metatarsal bone shortened minimally. Union occurred in 6 weeks. Subjectively, pain was relieved in 95% of the patients, while walking and standing abilities improved in 90%. The shoewear improved in only 60% of the patients, showing less satisfactory results compared to other factors. In terms of complications, two recurrences occured but were treated without repeat surgery. Two malunions and four superficial wound infections occurred. In conclusion, proximal chevron metatarsal osteotomy combined with modified McBride procedures is a reliable operation as regards stability, technical ease, low complications and satisfactory results for moderate to severe Hallux valgus deformities.

5.Partial Resection of Posterior Calcaneal Tuberosity for Haglund's Disease.

Haglunds deformity is a prominence at posterosuperior portion of the calcaneus. Extrinsic irritation on this prominence can give rise to retrocalcaneal bursitis or Achilles tendinitis. Partial resection of the posterosuperior aspect of the calcaneus was performed on 7 heels with Haglunds disease. All patients were reviewed both by radiographic examination and by questionnaire after an average 14.5 months. All but one patient showed an excellent result in a view point of pain relief. The average passive dorsiflexion angle of the ankle was improved from 5 to 25 degree, postoperatively. There was no complication such as recurrence, wound problem or paresthesia.

6.Seroconversion in Nonresponders to Hepatitis B vaccination after double dose vaccination only once.

BACKGROUND: Korea is, known to be an endemic area of viral hepatitis B, and 5-6% of population are carriers. Immunization can reduce the spread of hepatitis B infection. Hower, not all people respond to hepatitis B immunsation. 5-15% of primary vaccinees fail to be immunized. After revaccination, only 37.5 44% of the primary failures elicit antibody response. Recent studies have reported that it is effective to inject a double dose vaccination in immunocompromsed patients. In some country, they recommend that neonate whode mother is HBV carrier shoud receive double dose vaccination. This study documented the results of double dose vaccintion only once to nonrsponders toward the HBV immunzation. METHODS: Healthy nonresponders to Hepatitis B immunization were recuited from Sep. 1995 to Aug. 1996. Subjects with their AST/ALT level over 40, or over 65 years old were excluded. They were received 2ml of plasma-derived vaccine(Hepavax-B) intramuscularly in the deltoid muscle. Anti-HBs antibody testing were performed at 1 to 3 months after vaccintion. RESULTS: 17 healthy nanreponders to Hepatitis B immunization(male 10, female 7) were included. Their mean ages were 47(32 to64), AST/ALT level 20/19(16 to 28/11 to 35), smoking 4.3pys(1 to 7). Seroconversion occured in 13 of them(76.5%). CONCLUSIONS: Nonresponders to Hepatitis B vaccintion respond well to double dose vaccination once.

8.A Case of Darier' s Disease Improved with Oral Etretinate.

We report a case of Darier's disease occuring in 49-year-old female patient. She had hyperkeratotic crusted eruptions on her forehead, neck and posterior auricular area. The skin lesions aggravated in summer and improved in winter, but never been healed completely. Histopathologic features from neck lesions revealed hyperkeratosis with corps ronds and grains as well as formation of villi and laeunae. We treated her with oral etretinate, and she was almost completely free of disease after 3 months treatment. We suggested that etretinate is an importnat advance in the treatment of Darier's disease.

9.Anesthetic Management for Resection of Pheochromocytoma .

A 51 year old female patient underwent resection of pheochromocytoma under general anesthesia. Thiopental sodium 250mg and succinylcholine 50 mg were used for endotracheal intubation, followed by N2O, O2 and ethrane for induction and maintenance. For the management of blood pressure during surgical amnipulation, we used sodium nitroprusside dripping. After tomor resection, blood pressure was controlled by dopamine dripping and intravenous lactated Ringer's solution. The patient tolerated anesthesia and surgery well despite the episodic hemodynamic changes. Although tolerated anesthesia and surgery well despite the episodic hemodynamic changes. Although specific anesthetic drugs have been recommended, we believe that optimal preoperative preparation, a gentle induction of anesthesia, and good communication between surgeon and anesthesiologist are most important for anesthetic management.
