3.The Diagnosis Distribution in Health Examination of Old Aged People and Associated Sociodemographic Factors.

BACKGROUND: According as old aged people have an increased interest in early detection of disease and health promotion, old aged people who take health examinations are increased. So this study is aimed to survey common problems about geriatric health in community and their frequency by way of investigation on diagnosis in health examination data and to evaluate the effects of health risk factors on each disease patterns. METHODS: The medical records of the clients who have taken health examinations for the first time from March 1995 to February 2000 at a health examination center of a university hospital in Seoul were reviewed. The category and number of judged diagnosis, sociodemographic factors are compared and analyzed. RESULTS: The selected medical records accounted to 22,393. The study subjects consist of 11,544(51.6%) male. Mean age is 52.0+/-10.5 years(range 16~90). 65 years or more old aged people of them are 2,612(11.7%), the number of judged diagnosis per one old person is 4.84+/-2.22(range 0~9) and young people took less diagnosis is 35.7%, functional diagnosis is 7.9%. The common diagnoses are liver disease(7.07%), decreased physical strength(6.68%), hyperlipidemin(6.53%),obesity(5.90%), osteoporosis or postmenopausal syndrome(4.72%), need for immunization against viral hepatitis(4.69%), hypertension(4.20%) functional gastrointestinal disease(3.93%). The number of total diagnosis and organic diagnosis is larger in older age, no exercise, female,smoker, no-spouse, lower education(p<0.05). The number of other diagnosis is larger in no exercise, smoker, female, younger age(p<0.05). CONCLUSION: According to health examination, total diagnosis and organic diagnosis are more in old aged group than in young. Functioinal diagnosis and other diagnosis is less in old aged group than in young.

4.Morphometric Study on the Sigmoid Sinus of the Korean Adults.

The variations of the sigmoid sinus were studied in 50 Korean adults (average age 67). The classification of the shape and a measurement of the diameters of the sinus were done and the morpological. 1. The shape was classified into 4 types, and the round type was the most common in both sides. S shaped type was below 10% in both sides. 2. The breadth was regular in 54% of the right and in 70% of the left sinuses. According to the breadth change, the shape was classified into 4 shapes. 3. The average diameter was 8.6mm on the right side and 7.2mm on the left side. 4. Symmetry was present in 16% of all. 5. The right transverse sinus was absent in 1 case (2%) and the superior petrosal sinus did not open into the sigmoid sinus in 2 cases (4%). 6. Petrosqamous sinus was observed in 16%.

5.Experimental Study for Comparison of the Ablation Effects of Extracted Bovid Liver by Percutaneous Procedures with Ethanol, Microwave, and Radiofrequency.

PURPOSE: The purpose of this study was to evaluate the differences in the ablation effects of percutaneous ethanol injection therapy (PEIT), percutaneous microwave coagulation therapy (PMCT), and radiofrequency (RF) ablation therapy in extracted bovid livers, and to assess the appropriate therapeutic method for treating hepatic tumors according to their shape, size and location. MATERIALS AND METHODS: PEIT, PMCT and RF ablation were performed at 1 cm and 5 cm depth in bovid livers. PEIT was performed with 1, 2, 3 and 4 ml of ethanol, PMCT with 60 watt for a 30 th, 60 th and 120 th of a second, and RF ablation for 6, 8, 10, 12 and 14 minutes. Ultrasonography was used for the detection of the tissue necrosis that followed the different therapeutic modalities. RESULTS: For PEIT, the extent of the necrosis was smaller than that of the other modalities. For PMCT, the necrosis was elliptical and longer in length. For RF, the necrosis was oval and larger in size. At 1 cm depth for PMCT and RF ablation, the liver capsule was injured. CONCLUSION:We concluded that these different findings in tissue necrosis would aid in selecting the appropriate therapeutic method for hepatic tumors according to the tumors' shape, size and location.

9.A Comparative Observation on Success Rates of Stone Dislodgers.

A comparative observation was done on 139 patients of the lower ureteral stone, those were applied cystoscopic manipulation with 3 different types of stone dislodger at the Department of Urology, Seoul Adventist Hospital, during the periods from Jan. 1982 to June 1985. During that periods, we applied end-loop catheter in 48 patients, blunt-tip Dormia basket in 38 patients. The results were as follows; 1. The success rate of passage of dislodger beyond the stone was the highest in filiform-tip Dormia basket (97.4%) and loop catheter was the lowest (8l.3%). 2. The success rate of the primary stone extraction with filiform-tip Dormia basket was the highest (86.8%). 3. Indwelling of the dislodger in the ureter loop catheter was the most successful method among them(62.5%). 4. Of the cases of passage beyond the stone, the success rates of the stone extraction were very high and similar in their results, as in 92.3% with loop catheter, in 90.9% with blunt-tip Dormia basket, in 91.9% with filiform-tip Dormia basket. 5. The overall success rate of stone extraction was 75.0% in loop catheter, 75.5% in blunt-tip Dormia basket 89.5% in filiform-tip Dormia basket.

10.Femoral Neck Fractures in Patients with Cerebrovascular Accident (CVA)

To assess the prognostic factors of the femoral neck fractures in patients with CVA, we compared the survival rates in 12 cases of CVA group with 12 cases of non-CVA group and evaluated the various clinical factors of CVA group affect the mortality. All of the patients both CVA group and non- CVA group were treated hemiarthroplasty for femoral neck fractures between June 1990 and June 1992. The mean age was 68.9 years old in the CVA group and 69.3 years old in the non-CVA group. The mean follow-up periods were 32.8 months in the CVA group and 33.6 months in the non-CVA group. The results were as follows; 1. The incidence of the femoral neck fracture with CVA was 15%(12 cases among SO femoral neck fractures). 2. The survival rates in the CVA group at l-year, 2-year, and 3-year were 83.3%, 66.7%, and 58.3% respectively. In the non-CVA group, the survival rates were 83.3%, 75.0%, and 62.5%. 3. In the CVA group, serum total protein, nutritional index, hypertension, lung disease, and cardiac disease may be valuable prognostic factors. In conclusion, the pretraumatic patient's general physical condition could affect the mortality of femoral neck fracture in the CVA patients rather than CVA itself or postoperative status.
