1.A Study on the Catecholamine under the Room Temperature and 5 degrees C Refrigerator Environment in Rat.

Under the extreme change of the environment, animals react physiologically to adapt to the stress and secrete catecholamines. Cold exposure is a kind of the environmental stress. Author tried to determine the amount of catecholamines in rat urine as a parameter of physiological response to cold stress. Urinary catecholamine was measured by using HPLC with fluorescence detector, coation exchange column prepacked with Bio-Rex 70 and ammonium pentaborate as catecholamine eluent. The amount of dopamine in normal state rat urine was 42.0 ng, but under the low temperature of 5 degrees C, the dopamine amount was increased to 221.25 ng/5 ml. Above findings are suggesting that catecholamine secretion, especially dopamine, increase in the stressful condition such as cold exposure.

2.Reconstruction of Rheumatoid Forefoot Deformities.

Rheumatoid arthritis can result in progressive deformity of the forefoot, which leads to marked limitation in shoe wear, significant pain, and substantial diminution in standing and walking ability. Twenty feet of seventeen patients with severe forefoot deformities underwent surgical correction hy means of first metatarso-phalangeal arthrodesis and resection arthroplasties of the lesser M-P joints from May, I 994 to March, l996. The results were as follows; l. Suhjective improvement in pain, standing and walking ability, and shoe wear was excellent in fifteen (7~5%) feet, good in three (l5%), fair in one (5%), and poor in one (5%) foot. 2. The average hallux valgus angle (HVA), 1st intermetatarsal angle (IMA) were improved from 46.7 and 13.7 to 11. 1 and 8.2 degree individuaIly. 3. The average angle of dorsiflexion after the first metatarsophalangeal (MP) joint arthrodesis was 12.9 degree. 4. Complications of the procedures were minimal; three delayed wound healing, one non-displaced fracture of fusion site due to trauma, one malunion of the 1st MTP joint, one recurrence of the fibular deviation of the lesser toes.

3.Computed tomographic evaluation of adrenal gland tumors

CT findings of twenty-four patients with adrenal gland tumors were analyzed to evaluate their location, size,shape, density etc. The post-surgical confirmation was made in 17 cases. The findings were as follows: 1. 4-10cmsized tumor messes were most common (42%). 2. Round or oval shaped forme were most common (59%). 3. Functionaltomors were 71% (17/24) including Cushing's syndrome of 10 cases, pheochromocytomas of 6 cases and aldosteronismof 1 case. 4. Abnormal adrenal glands were delineated in 88%(21/24), most effectively by CT. We concluded that CTwas the valuable study in the evaluation of the adrenal gland tumors and in the determination of surgicaloperation.

4.Clinical Evaluation and Patch Test with Vegetables in Housewife' s Hang Eczema.

The importance of vegetables as a allergen for the development of hand eczema has been a matter of debate. The aim of this study was to evaluate the clinical manifestations of hand eczema and to investigate the role of vegetables in the hand eczema of Korean housewives. In this study, clinical evaluatons and patch tests with Hollister-Stier standard allergens and 22 vegetable allergens wer performed in 32 patients with housewife's hand eczema. The results are summarized as follows : 1. The prevalence was highest in fourth(59.4%) and third(25.0%) decades of life and the peak age of onset was third(53.1%) and fourth(31.3%) decades. 2. Only 3 of 32 patients(9.4%) had past histories of atopic dermatitis ; however, childrens of 12 patients(37.5%) had atopic dermatitis. 3. The common skin signs were fissuring(90.6%), scaling(78.1%), erythema(75.0%), hyperkeratosis(71.9%), lichenification(62.5%), and vesicle(43.8%) in descending order. 4. The common aggravating factors were soap or detergent(93.8%), water(90.6%) and rubber glove(25.0%). 5. The most common involved areas of hand eczema were fingers including fingertips and fingertips only. 6. The results of patch test using vegetable allergens with 10 normal controls were all negative. Of the 32 patients with hand eczema, 13(37.5%) showed positive patch test with standard allergens and 5(15.6%) showed positive with vegetable allergens. 7. Among 30 standard allergens, cinnamic alcohol(9.4%). potassium dichromate(9.4%), nickel sulfate(6.3%), and wool wax alcohol(6.3%) were the common allergens which showed positive reactions. Of the 22 vegetable allergens, garlic, Chinese cabbage juice, Chinese cabbage extract, cucumber juice, spinach extract, potato juice showed weak positive reactions in only one patient(3.1%) respectively. From the above results contact sensitivity to vegetables is not significant in Korean housewives with hand eczema.

5.A Clinical Review of the Ureteroneocystostomy.

Primary vesicoureteral reflux, distal ureteral obstruction or stricture and injuries are the main indications for the ureteroneocystostomy There are several operative techniques in ureteroneocystostomy and these should be antireflux to preserve renal function. To prevent the reflux, sufficient submucosal length, minimum ureteral manipulation, adequate butters and making the new orifice near the trigone are stressed. We performed ureteroneocystostomy in 19 patients(20 ureters) in last 10 years. We evaluate the preoperative state, operative technique, postoperative course and follow up of these cases and report with the review of the articles Results were 1. 19 cases included 5 VUR, 6 ureteral injuries by transabdominal hysterectomy, 3 bladder tumors involving ureteral orifices, 2 ureteroceles with obstruction and 3 chronic inflammatory strictures. 2. Ureteroneocystostomy were done by Cohen in 4, Glenn-Anderson in 2, Politano-Leadbetter in 3, Boari-Kuss in 5, Paquin in 2 and fish-mouth in 3 cases 3. Ureteral stent and suprapubic cystostomy were made in all cases but one. Duration of stenting was 4-14 days and cystostomy was 6-14 days. 4. There were improvements in 16 cases, but failed in 2 ureters of reflux cases.

6.A Case of Proliferating Trichilemmal Tumor.

Proliferating trichilernmal tumor is a rare benign solitary tumor that may be histopathologically misinterpreted as squarnous cell carcinoma. It affects frequently the scalp of elderly women. We present herein a case of proliferating trichilemmal tumor in an 87-year-old male who had had a slowly growing tumor on the scalp for 30 years.

7.A Case of Paroxysmal Atrioventricular Block Complicating Takayasu's Arteritis.

Takayasu's arteritis is briefly as an inflammatory process of unknown etiology occuring dominantly in young female most commonly involving the great vessels arising from the aortic arch. The disease process results in stenosis and obliteration of involved vessels. Syncope in Takayasu's arteritis is known to result from decreased blood flow to brain because of stenosing blood vessels. We herein describe a case of paroxysmal atrioventricular block in a 40-year old man with Takayasu's arteritis. The patient presented with spontaneous recurrent syncope associated with paroxysmal AV block. Carotid sinus pressure repeatedly induced paroxysmal AV block and syncope. Carotid sinus pressure after pretreatment of atropine failed to induce AV block or syncope. Coronary angiogram was normal. We concluded that syncope in this patients was attributed to carotid hypersensitivity rather than decreased blood flow to the brain due to vessel stenosis.

8.The Ultrasonographic Diagnosis of Interdigital Neuroma and Result of Surgical Excision.

PURPOSE: TThe purpose of this retrospective study was to demonstrate the characteristic findings of ultrasonogram and the functional result in the surgical excision of the interdigital neuroma. MATERIALS AND METHODS: Forty feet of thirty-five patients with clinical sign and symptoms of interdigital neuromas were examined by ultrasound with a 7.5MHz transducer, In 23 of these patients, surgical excision and pathological analysis of their interdigital masses were subsequently performed. RESULTS: Interdigital neuromas were seen sonographically as well-defined, ovoid, hypoechoic masses located just proximal to the metatarsal heads in the intermetatarsal space. The diameter of neuroma on average was 5.6 mm in the 2nd interspace, 5.4 mm in the 3rd and 4 mm in the 1st. Postoperatively, the satisfactory result was exellent in five (22%), good in twelve (52%), fair in four (17%) and poor in two (9%). CONCLUSIONS: These results suggest that ultrasonographic examination is an accurate diagnostic technique, and surgical excision is most effective method of treatment in the view of pain management than other therapeutic modalities in patients with clinically suspected interdigital neuroma.
