2.A Case of Neonatal Death due to Group B beta-Hemolytic Streptococcal Sepsis.

Since 1970, the group B streptococcus(GBS) has been a significant cause of neonatal sepsis in the West. Two distinct forms of disease occur in neonates. Early-onset disease which occurs within 7 days after birth is characterized by respiratory distress, apnea, shock, pneumonia, and occasionally meningitis. Late onset disease usually occurs at 3-4 weeks of age(ranging from 7 days to 3 months) and presents occult bacteremia or meningitis. The GBS is thought to be a rare causative agent for neonatal sepsis in Korea. Lately, we experience a case of early-onset GBS sepsis who died at 3rd day of life. We present this case with brief review of literatures.

3.Long Term Follow-up of SAH Patients with Conservative Treatment.

Two hundreds and four consecutive patients who sustained proven spontaneous subarachnoid hemorrhage with conservative treatment were studied retrospectively. The reasons for conservative treatment were poor condition for surgery, difficulties in surgery and operation refusal. Comparative study was done in proven 47 cases of aneurysmal SAH, 27 cases of SAH with negative angiography and 130 cases of SAH without angiography about age, aneurysm site & size, neurologic grade mortality & morbidity, etc. Death had occurred in 2 weeks after attack predominantly. Rebleeding was most common cause of mortality(64%, 68/107). Survivals had good quality of life(73%, 19/26).

4.The Clinical Experience on Orchiectomy in the Postpubertal Unilateral Cryptorchidism.

PURPOSE: The postpubertal males with unilateral cryptorchidism undergo orchiectomy rather than orchiopexy, which is based on the various reports in its effect on fertility and predisposition to testicular cancer. We reviewed retrospectively the clinical features and histopathological findings of orchiectomized testes and investigated the role of orchiectomy in the postpubertal unilateral cryptorchidism. MATERIALS AND METHODS: We reviewed 42 patients with postpubertal unilateral cryptorchidism who underwent orchiectomy from 1986 to 1998, retrospectively. Patient`s age ranged from 15 to 47 years(mean 26.4). The cryptorchid testes were palpable in 33 cases(79%) and impalpable in 9 cases(21%). We classified cryptorchid testes histopathologically into 2 broad groups of specimens with no spermatogenesis(severe atrophy and/or Sertoli cell-only syndrome) and specimens displaying spermatogenesis(maturation arrest, hypospermatogenesis). In 38 cases of all orchiectomized specimens, testicular volume was calculated by Lambert`s formula. RESULTS: Histopathology showed maturation arrest in 8 cases(19%) while 34 cases(81%) had severe atrophy and/or Sertoli cell-only syndrome. No case of carcinoma in situ or cancer was found in testicular tissue. The location of the cryptorchid testes was the prepubic area in 13(31%), inguinal canal in 23(55%), and abdomen in 6(14%). There was a statistically significant difference in the relationship between the presence and absence of spermatogenesis in cryptorchid testes and the anatomical levels of descent(p<0.05). There was no statistically significant difference in the relationship between testicular volume and the location of cryptorchid testes, between testicular volume and patient`s age(p>0.05). CONCLUSIONS: Currently, orchiectomy is mostly advocated for a postpubertal unilateral cryptorchid testis because of the risk of future malignancy. But we consider that orchiopexy is a valuable modality of treatment for postpubertal unilateral cryptorchid testis, especially located at prepubic area, after performing testis biopsy and then periodic follow-up is required.

An en bloc bilateral osteotomy of the orbital roofs and frontal sinus for large or deeply situated anterior skull base tumors offers less brain retraction and a wide exposure. The authors describe en bloc bilateral osteotomy of the orbital roofs and frontal sinus, with skull base reconstruction using inner table of the patient's own frontal bone flap. The approach in this report offers the advantage of wide exposure, one-site operation and short operation time.

6.A Case of Verrucous Carcinoma of Maxillary Sinus.

Verrucous carcinoma is a rare variant of squamous cell carcinoma that is more often seen in the oral cavity and larynx, but extremely rare in the paranasal sinus. In establishing the diagnosis, close cooperation is required between the clinician and pathologist, since the lesion appears obviously malignant on clinical examination but on pathologic study it may be reported as hyperplastic or papillomatous or as hyperkeratotic benign squamous epithelium. Verrucous carcinoma should be treated by surgical excision because there is evidence to suggest that the lesion may become more aggressive after radiotherapy and surgery gives better survival rates than radiotherapy. We experienced a case in which the patient presented with a history of right nasal obstruction and intermittent facial pain for about 4 months and was ultimately diagnosed as having verrucous carcinoma of maxillary sinus. Caldwell-Luc's operation was performed. We report this case with a review of the literature.

7.EAT score according to weight control, depression level and body mass index(BMI).

BACKGROUND: It is reported that abnormal eating behavior is more frequent in the group of weight control than those of no control. Repeated and chronic binge eating behavior has associations with impaired work and social functioning, overconcerns with body shape and weight. The purpose of this study is to investigate the degree of abnormal eating behavior according to weight management, depression level and obesity. METHODS: From June 1999 to July 1999, we collected self record questionnaire including body mass index(BMI), Beck Depression Inventory(BDI), Eating Attitude Test(EAT) from 1292 students of one girl's high school in Seoul. Study subjects were divided by BMI, the set point was 25 kg/m2. EAT score associated with weight control, BMI, BDI, smoking and alcohol was assessed by multiple linear regression. Study subjects were also grouped by the existence of the history of weight management behavoir, then BMI, EAT score and BDI in each subgroups was assessed by t test. RESULTS: Obese students were 42, whereas those of BMI under 25 kg/m2 were 1191. BDI score ranging from 5 to 20 were 940, 21 to 63 were 179. The EAT score has significant relevance to the history of weight control, BMI, and BDI score in order. The EAT score and BMI were higher in the subjects who experienced various modes of weight control (48.41+/-14.47 vs 33.63+/-12.52 in EAT, 20.34+/-2.26 vs 18.72+/-1.89)(p<0.01). CONCLUSION: EAT score was associated with weight control, BMI and BDI. EAT score and BMI were higher in weight control samples.

8.Comparison of Cervical Spine Movement by Airway Equipment during Orotracheal Intubation: A Manikin Pilot Study.

BACKGROUND: In patients with limited cervical spine movement, equipment for orotracheal intubation should achieve sufficient laryngeal exposure with the least cervical spine movement. This study was designed to compare movement of the cervical spine during the orotracheal intubation with various intubating equipment. METHODS: Twelve emergency physicians & residents with a total experience of >50 cases of endotracheal intubation in two emergency centers were assigned to perform orotracheal intubation with four different airway devices, including the Macintosh laryngoscope (ML), DCI video laryngoscope (DCI), Airway Scope (AWS) and Levitan Scope (LS), using the same manikin (Ambu(R) airway management trainer) in random sequences. Movement of the C-spine was examined by measuring the angle formed by two lines which are parallel to the anterior surface of the C2 and C7 vertebrae bodies. The angle was measured when Cormack-Lehane grade II glottis exposure was achieved during intubation. RESULTS: Mean cervical spine movements were 37.6 +/- 9.2degrees, 32.2 +/- 14.2degrees, 32.2 +/- 6.45degrees and 17.4 +/- 10.0degrees with the ML, DCI (p = 0.347), AWS (p = 0.094), and LS (p < 0.001), respectively, compared to that of ML. Cervical spine movement by LS was 54% less than that by ML. LS produced less cervical spine movement in comparison to DCI (p = 0.013) and AWS (p = 0.001). CONCLUSIONS: The Levitan Scope produced less movement of the cervical spine when compared to the Macintosh laryngoscope, DCI video-laryngoscope and Airway Scope during orotracheal intubation in a single airway training manikin model.

9.Expression of claudin-1, claudin-4 and zonula occludens-1 in cervical intraepithelial neoplasia and invasive squamous cell carcinoma.

OBJECTIVE: Cell to cell and cell to extracellular matrix interaction are crucial in tumor development and progression. Tight junction proteins such as claudins and zonula occludens-1 (ZO-1) play an important role in these processes. This study was performed to investigate the difference of expressions of claudin-1, claudin-4 and ZO-1 in low grade squamous intraepithelial lesion (LSIL), high grade squamous intraepithelial lesion (HSIL), and invasive squamous cell carcinoma (ISCC) of the uterine cervix. METHODS: The expressions of claudin-1, claudin-4 and ZO-1 were evaluated using immunohistochemical staining in 78 cervical tissue specimens (LSIL 22 case, HSIL 36 case, and ISCC 20 case). RESULTS: Claudin-1 expression was positive in 40.9% of LSIL, in 94.0% of HSIL and in 20.0% of ISCC. The expression of claudin-1 was significantly high in HSIL (p=0.0001). Claudin-4 expression was positive in 31.8% of LSIL, in 41.7% of HSIL and in 25.0% of ISCC. The expression of claudin-4 was high in HSIL, but it was not statistically different. ZO-1 expression was positive in 13.6% of LSIL, in 41.7% of HSIL, and in 25.5% of ISCC. The expression of ZO-1 was significantly high in HSIL (p=0.011). CONCLUSION: These results indicate increased expressions of claudin-1 and ZO-1 in the HSIL that includes cervical intraepithelial neoplasia (CIN) 2 and 3, which decrease during progression to cervical cancer.
