1.Norovirus P particle and its application

The challenges posed by norovirus infections to global health are increasing accompa-nied by the rapid rate of the genetic and antigenic evolution of circulating noroviruses. Due to lack of in vitro culture cells and small animal models, norovirus vaccine cannot be prepared by using traditional techniques. With the in-depth understanding and study of norovirus, the subunit vaccines against norovirus infection based on P particles have been developed and presented the characteristics of easily expressed, low cost, high immunogenicity, stable structure and so on. In addition, norovirus P particle has been used as a subvi-ral nanoparticle for vaccine development against other viruses and for antibody production against chronic dis-ease ( Alzheimer′s disease) , which benefits from the accommodation of foreign antigens in the three loops of P particle. In this review, we describe the progresses in the field of P particle related vaccines for providing suggestions about the research and development of multivalent vaccines in China.
2.Endovascular repair for aortic dissecting aneurysm combined with aberrant right subclavian arteries

Objective To study the clinical and image features of the aortic dissecting aneurysm(AD) combined with aberrant right subclavian arteries(ARSA),and to discuss the feasibility of endovascular repair for such disorder.Methods The clinical and image data of 4 patients,who suffered from AD combined with ARSA and admitted in the authors' service during 2004 to 2007,were retrospectively reviewed.All the 4 cases complained severe aches in their back,and one of them was companied with aches in upper abdomen.CT-exam showed that the first tear(gap) of AD located near or inside ARSA,the truncus aortae narrowed obviously.Four operational prescriptions were performed respectively for the 4 patients depending on the location of gaps and on the distance between the gap and ARSA.Results All the 4 cases received satisfy operation.A slight early leakage occurred in the first case without steal syndrome,and the lower blood pressure was 80 mmHg on right arm;for the second case,no leakage occurred with normal blood presser on both arms;for the third one a slight early leakage was companied with normal blood presser on both arms and for the fourth one a slight early leakage occurred without steal syndrome,and the lower blood presser was 60mmHg on right arm.Conclusion AD combined with ARSA is a rare case in clinic.The modus operandi of endovascular repair should be designed based on the situation of individual case considering the location of the first tear.Once the target area exposed is not wide enough for operation,subclavian artery bypass would be a right way in reduction of leakage complications and in increasing the opportunity for a successful operation.
3.Efficiency evaluation of Y-type reconstruction plate internal fixation treating the distal humeral comminuted and complicated fracture

[Objective]To evaluate the two plates internal fixation treating the distal humeral comminuted and complicated fracture.[Method]Twenty-eight patient with comminuted distal humeral fracture were treated with the double-plate internal fixation from April 2004 to April 2007.According to AO/ASIF classification method,all fractures were C types.There were 9 C1 types,15 C2 types,4 C3 types.All cases were taken from the middle of posterior elbow incision.The patients were treated with the open reduction and double plate internal fixation.Fifteen cases were done through the triceps tongue-shaped flap,13 cases were olecranon osteotomy (including one case with olecranon fractures with ipsilateral).Surgical steps included:the articular surface reduction of intra-articular fractures,small temporary Kirschner wire fixation,making the complete fractures into simple comminuted fractures of humerl condylar fractures,then resetting metaphysis.Internal fixation method used the restruction plate according to the size of fracture.The restruction plate was fixed in the back of the distal humeral radial column.One-third of a good-shaped tubular plate was used to fix the inner edge of humeral ulnar column,two 90 ? of each other into the plane separately fixed on the inner lateral humeral column.[Result]All cases were followed up for an average of 17 months(ranged,8-33 months).Fracture healing time ranged 8-16 weeks.Myositis ossificans was found in 1 case,joint space narrowing was found in 4 cases.No non-union and ulnar nerve injury were found.The final good-to-excellent rate was 82.1%.[Conclusion]The distal humeral comminuted fracture has complicated structure.The internal fixation with two plates and screws,and early functional exercise could improve the elbow function.It has satisfactory curative effect in clinic.
4.Prevention and treatment of hypertension induced in retroperinoneal paraganglioma operation

ObjectiveTo explore the mechanism and prevention for the hypertension complicated by the retroperinoneal paraganglioma operation.MethodsBlood pressures were monitored during operations on 15 patients with retroperinoneal paraganglioma. Also,the reason and the prevention for the changed blood pressure were studied emphatically during operation.ResultsThe elevated blood pressure of 12 patients was detected during operation,and all dropped to a varied degree after operation,with 2 of them below the normal value.ConclusionThe retroperinoneal paraganglioma operation was liable to cause blood pressure elevated,which was related to the anesthesia induction and the operation stimulation of tumor. After the excision of the tumor,blood pressure of most of the patients dropped to some degree. Therefore,during the retroperinoneal paraganglioma operation,the blood pressure should be monitored closely,and the short-acting antihypertensives should be applied on time. Meantime,occurrence of hypotension was also considered after the excision of the tumor.
5.Sural neuro musculocutaneous flap transplantation plus catheter irrigation for chronic lateral-malleolus Ostoemyelitis

Objective To describe the method and clinical result of super sural neuromusculocutaneous flap grafting plus catheter irrigation in the treatment for chronic lateral malleolus osteomyelitis. Methods From March 2000 to March 2008, 17 cases, male 14, femal 3, 21 to 75 years old (average 43-year-old),were underwent reversed saphenous musculocutaneous island flap after wide excision of lateralmalleolus lesion. The cause of lateral-malleolus lesions was trauma. The smallest flap was 5 cm× 6 cm and the largest was 7 cm × 11 cm. Catheter irrigation was used in all cases. Results Follow-up ranged from 12 to 96 months, average 49 months. After operation, the wounds were irrigated with sensitive antibiotics 1 to 2.5 months(average 49 days), and all flaps were survived. Except 2 cases, the other 15 were healed in 1 month.The 2 cases were not healed at first stage. According to the lab result,we changed the antibiotic, and in 2.5 months, we took off the catheter. Conclusion To deal with the chronic traumatic lateral-malleolus osteomyelitis, super sural neuromusculocutaneous flap grafting plus catheter irrigation is approprite and effective.
7.The effects of lymph after intestinal ischemia/reperfusion on the inflammatory factors and Toll-like receptor 4 ligand high mobility group box-1 in Toll-like receptor 4 deficient mice

Objective To investigate the effects of lymph from ischemic/reperfused intestine on the inflammatory factors and Toll-like receptor 4 (TLR4) ligand high mobility group box-1 (HMGB1) in TLR4 deficient (TLR4-/-) mice.Methods A total of 20 SD rats weighing (300 ±20) g were randomly assigned into two groups:lymph drainage group (group N,lymph drainage for 180 minutes without other treatment) and intestinal ischemia/reperfusion group (group I/R,draining the lymph for 180 minutes while clipping the superiormesenteric artery for 60 minutes followed by 120-minute reperfusion).Thirty-two TLR4-/-mice and thirty-two C57BL/6 wild type (WT) mice were each divided into 4 sub-groups (n =8),injected with different fluids through the caudal vein:group N with normal lymph;group I/R with I/R lymph;group Edt with endotoxin;group HMGB1 with HMGB1 protein.The mice were sacrificed 180 minutes after the injection for sample collection.Results The levels of endotoxin and HMGB1 in the lymph drainage of the group I/R rats were significantly higher than that of the group N rats [(0.034 ± 0.050) Eu/ml vs.(0.017 ± 0.023) Eu/ml,P =0.033;(4.293 ± 0.883) ng/ml vs.(0.509 ± 0.128) ng/ml,P =0.006].In the mice injected with HMGB1,the mucosa thickness and villus height in the ileum of the WT mice were significantly lower than that of the TLR4-/-mice [(335.8±43.2) μmvs.(602.1±37.5) μm,P=0.000;(273.0±31.7) μm vs.(404.5 ± 18.6) μm,P =0.000];in both WT and TLR4-/-mice injected with the I/R lymph drainage,the mucosa thickness and virus height were decreased,but the decrements were significantly lower in TLR4-/-mice;there were no statistically significant differences in the levels of interleukin-6 (IL-6),tumor necrosis factor-α (TNF-α),endotoxin,and HMGB1 between the TLR4-/-and the WT mice injected with normal lymph or endotoxin.In the mice injected with I/R lymph drainage,the levels of inflammatory factors in the TLR4-/-mice were significantly lower than those in the WT mice [TNF-α:(28.637 ±5.166) pg/ml vs.(41.917 ±8.175) pg/ml,P=0.000;IL-6:(60.900 ±24.729) pg/ml vs.(110.265 ±28.545) pg/ml,P =0.000].In the mice injected with HMGB1,the levels of inflammatory factors in the TLR4-/-mice were significantly decreased compared with those in the WT mice [TNF-α:(20.865 ± 6.464) pg/ml vs.(31.059 ± 6.204) pg/ml,P=0.004;IL-6:(36.268 ±8.977) pg/ml vs.(76.677 ± 14.099) pg/ml,P=0.000].Conclusions The concentrations of endotoxin and HMGB1 are significantly increased during intestinal I/R in rats.After injection of I/R lymph drainage,endotoxin,and HMGB1,the levels of inflammatory factors and HMGB1 in the mice injected with I/R lymph drainage are significantly higher than those in the mice injected with normal lymph;the levels of inflammatory factors and local damage of intestinal mucosa are significantly reduced in the TLR4-/-mice than in the WT mice.The gut-lymph pathway may play a key role in the intestinal I/R injury.
8. Optimization of diabetic gastroparesis rat model by orthogonal design

Objective: To optimize the preparation methods of diabetic gastroparesis rat model by orthogonal test. Methods: The diabetes gastroparesis rat model was duplicated with alloxan (ALX) by using four factors and three levels of orthogonal design. The fasting blood glucose, gastric emptying rate, and plasma motilin level were scored as evaluation indexes, and the influence of different factors on diabetic gastroparesis model rats was observed. Results: The effect of feed type on the diabetic gastroparesis rat model was not obvious, but the effects of modeling time and modeling agent ALX dose on diabetic gastroparesis rat model were decisive. The order of the importance was as follows: modeling time > dose of ALX > feed types. Conclusion: The optimized test method can effectively improve the diabetic gastroparesis rat model and provide a technical platform for the study on drug evaluation of diabetic gastroparesis.
9.Role of preoperative hormone therapy in localised prostate cancer:Meta-analysis based on six randomized controlled trials

BACKGROUND:Previous research has demonstrated that preoperative hormone therapy relieved clinicaI and pathological stages of prostatic carcinoma patients,reduced positive ratio of incisaI margin:however,it did not increase disease-free surviva J rate.Worth of preoperative hormone therapy for prostatic carcinoma remains unclear.OBJECTIVE:To assess the role of preoperative hormone therapy in localised prostate cancer.METHODS:Articles were searched from PubMed,Embase.Cochrane Library(No.4,2009),Chinese biomedicaf literature database,Chinese Scientific Journals full_text database,and Chinese Journal full-text database(published before October,2009).Randomized controlled trials which were diagnosed as Iocalised prostate cancer using pathology and cytology were included The sex and nationality were not limited.and patients did not have severe heart and lung diseases Prostatic carcinoma which occurred Later or repeatedly was excluded.Otherwise.randomized controlled trails which were coincidence with the inclusion criteria were also included.Review Manager 5.0 published by Cochrane was used for stafistical analysis.Overall survival rate.disease-free survivaI rate,positive surgical margin rate,positive lymph node rate.and seminal vesicle invasion rate were evaluated.RESULTS AND CONCLUSlON:Six randomized controlled trials totaling 1 027 padicipants were included.The result of meta analysis showed that there was no significant difference in overall surviva J rate[RR=0.94,95%C,(0.86,1.02)],disease-free survival rate[RR=1.02,95%CI(0.89,1.17)],positive lymph node rate IRR=0.86,95%CI(0.47,1.57)],and seminal vesicle invasion rate[RR=1.09,95%CI(0.74,1.59)]between single prostatectomy and preoperative hormone therapy plus prostatectomy,while there was significant difference in positive surgical margin rate[RR=0.46,95%CI(0.32,0.66)]This suggested that preoperative hormonal therapy prior to prostatectomy did not improve survival rate,positive lymph nodes rate and seminaI vesicle invasion rate:however.there was a significant reduction in the positive surgicaI margin rate.
10.Experimental autoimmune myositis in rat

Objective The aim of our study is to establish and characterize the animal model for au- toimmune myositis.Methods Fifty male SD rats were randomly divided into 2 groups:model group(n=40) and control group(n=10).The model group rats were immunized with muscle homogenate every week for 5 weeks and received an injection of 2?g pertussis toxin at the first and second week.As controls,10 SD rats were injected with an equal volume of normal saline.Tissue specimens from limb skeletal muscles were ob- tained at 1,2,3,4,5 weeks after injection.At the same time,the blood samples were collected,and the level of CPK was measured.Results The model group had significantly elevated serum CPK levels.There were multiple inflammatory lesions in the skeletal muscles.Local degeneration and necrosis of muscle fibers with disappeared transverse striation,mononuelear cell infiltration in the interstitial could be observed.The patho- logic grade was mainly 2a.The infiltrating mononuclear cells were predominantly CD8~+T cells that mainly lo- cated in the endnmysium.MHC classⅠantigen expression on muscle fiber membranes in the model group was upregulated.Conclusion The experimental autoimmune myositis induced by syngeneic skeletal muscle ho- mogenate in SD rat is pathologically similar to human myositis.It can be used as a good model for human myositis and provides the basis for the etiopathology and therapeutical studies.
