1.Heart diseases and pregnancy at the Institute of Protection Mother and Newborn (IPMN)

During 1 year (2001), there were 86 cardiac patients treated at IPM. Rate of acquired heart diseases is 81.79%. Out of these, the most frequent ones are mitral stenosis and regurgitation (37.02%), followed by pure mitral stenosis (12.79%). Congenital cardiopathies accounted for 18.58%. The most frequent complications are heart failure (91.67%). There was not any death. The main obstetric management is cesarean section (55.81%) and forceps (32.56%).

3.Atrophic gastritis and gastric cancer

In many cancers the chronic inflammation caused by microorganisms can be observed, espe-cially in gastrointestinal system. Atrophic gastritis is thought to be a precancerous lesion. Studies have approved that Helicobacter pylori plays an important role in the genesis and development of gastric caner. It is now known to be responsible for inducing chronic gastric inflammation that progresses to atrophy, metaplasia, dysplasia and gastric cancer.
5.Tumor immunotherapy:progresses and trends

New tumor antigens are continuously being discovered due to the improvement of immunologic techniques.Whether dendritic cells induce immune activation or immune inhibition after they capture tumor antigens depends on the danger signals(GM-CSF,MCP1,and HSP) or the inhibitory signals(TGF-?,IDO,and iNOS) released by tumor cells.Under the regulation of danger signals,dendritic cells activate Th1 immune response and eliminate tumors;under the regulation of inhibitory signals,they activate Th2 immune response and can not effectively eliminate tumors.Progress in tumor immunotherapy mainly is manifested by antibody-based therapy,T cell-based therapy,and tumor vaccine-based therapy.To day at least 7 antibodies have been confirmed effective when combined with chemotherapeutic agents in treatment of tumors.Despite of the progress made in antibody therapy,discovery of new targets,development of new antibodies,and expanding of the application scope to more tumors still need intensive research efforts.The clinical effects of T cell-based therapy have not been satisfactory;most tumor vaccine-based therapies are in phase Ⅰ and Ⅱ clinical trial,and the outcomes of few phase Ⅲ clinical trials are not satisfactory,leaving more work to be done for improvement.As we understand more about the roles of antibody in immune surveillance,it will help to make immunotherapy of tumors a promising strategy.
6.Analysis and comparison of theories of circulation of blood and qi in both eastern and western ancient medicines.

In ancient Egypt, the theory of circulation of blood and qi was preliminarily established on the basis of cadaver dissection, gradually revised and improved following the progress of anatomy and physiology. However, the "Meridian Theory" of circulation of blood and qi in Chinese medicine had never been established on the basis of anatomical and physiological studies. Thus a suspicion was aroused, "whether the meridians really exist, or artificially assumed by imagination"? Therefore, in recent one half of a century, numerous scholars at home and abroad spent countless financial capacities and human power to study the essence of Meridians in order to find their material or substantial evidence, but with no breakthrough obtained. Nevertheless, in the 70th of the last century, studies on mechanisms of acupuncture analgesia and acupuncture anesthesia won an undoubtedly good result. Hence, the therapeutic effect of acupuncture on pain control was admitted by the "Consensus Statement on Acupuncture" at an international meeting held by NIH in Dec. 1997. Then the old Chinese acupuncture won an international acceptance and application. Therefore, the investigation of mechanism of acupuncture analgesia set up a positive model for future scientific researches on acupuncture mechanisms and essence of Meridians, as well as on other Chinese medical theories.

8.Retrospective Study on the Microbial Drug Resistance Profile and Predictive Risk Factors in Early Infection of Open Fractures

Introduction: This study was conducted to evaluate the susceptibility of microorganisms involved in
early infection of open fractures in orthopaedics to current prophylactic antibiotics practice and determine
the association of various parameters related to the causative organism, patient, injury and management to
the resistance of such microorganisms. Methods: This is a retrospective study that reviewed cases collected
from the surgical operations record from January 2015 to December 2017 in Hospital Tengku Ampuan Afzan,
Kuantan. A total of 179 cases were collected with 110 cases fulfilling the proposed inclusion / exclusion
criteria. They all underwent routine debridement followed by culture and sensitivity. Previous clinical
records were traced to identify the various risk factors for infection and evaluate these against the
resistance of the microorganisms to prophylactic antimicrobial drugs. Results: Admission to the intensive
care unit, blood transfusion, antibiotic regime and duration, fracture grade, injury severity score and
duration of admission were all significantly associated with resistance of microorganism. The study also
demonstrated the predominance of Gram-negative microorganisms constituting 65.9% of isolates. Gramnegative microorganisms were seen more in higher fracture grades and Injury Severity Scale, and longer
duration of admission and antibiotic treatment. The overall susceptibility of microorganisms to antibiotics
was also demonstrated with up to 68.6% resistance to cefuroxime and 36.9% to Gentamicin. Staphylococcus
aureus and MRSA were the commonest Gram-positive organisms while Pseudomonas and Acinetobacter spp.
the commonest Gram-negatives. Conclusion: Admission to Intensive Care Unit, blood transfusion, antibiotic
regime and duration, fracture grade, injury severity score and duration of antibiotics have a significant
positive predictive value to the development of resistance of microorganisms. The shift to predominantly
Gram-negative microorganisms indicate an urgency to modify management of open fracture to achieve
optimal results. This will also prevent the emergence of new multi-drug resistance microorganism which is
already a worldwide public health problem.
9.Prognosis and effect of rectovaginal fistula operation on patients' life quality

ObjectiveTo investigate prognosis and effect of rectovaginal fistula operation on patients' life quality.MethodsMedical data of 18 cases with rectovaginal fistula were analyzed retrospectively,and a follow-up was also made.ResultsAmong 18 cases,types were anal canal (1 case),low rectum (5 cases) and high rectum (12 cases). Three different kinds of surgical repair procedures were performed on these patients respectively,with 13 cases obtaining the first surgical repair successfully and 2 cases having the second surgical repair because of the fistula relapse. After operation,the general life quality and sexual life of patients improved.ConclusionThe rectovaginal fistula operation can deliver a good therapeutic effect,and evidently enhanced the life quality of patients.
