Objective:
Phototherapy is a widely used treatment for neonatal hyperbilirubinemia, but the potential risks in early preterm infants are not well known. So it seems to be necessary to find out which parameters should be carefully observed during phototherapy. In this study, we analyzed blood pressure (BP), heart rate (HR), and body temperature (BT) in preterm infants under 32 weeks of gestational age before and after phototherapy.
Methods:
In this study, we analyzed the medical records of 103 early preterm infants with gestational age <32 weeks and birth weight >1,000 g admitted to the neonatal intensive care unit, treated with and without phototherapy, at Wonju Severance Christian Hospital, a tertiary center in Korea. Changes in BP, HR, and BT were analyzed before and after treatment.
Results:
A total of 91 patients taking phototherapy and 12 control subjects were enrolled. In the phototherapy group (PT group), PT was started on the second day after birth and lasted for 74 hours.In between-group analysis, HR was higher in the PT group after starting phototherapy (at 48 hours;median of differences 8 bpm, P=0.005, at 56 hours; median of differences 9 bpm, P=0.001), while there was no significant difference in BP. The rate of BP increase was lowered and HR was increased after phototherapy, in the PT group analysis.
Conclusion
After starting phototherapy in preterm infants less than 32 weeks of gestational age, the increasing trend in BP was ceased and the HR was increased.

6.Evaluation of nasolabial angle in adult patients with skeletal Class III malocclusion.

OBJECTIVE: The purposes of this study were to evaluate the nasolabial angle changes between closed lip position at centric occlusion and relaxed lip position at which the bite is open so that the lips do not touch and to elucidate the significance of the relaxed lip position for dentofacial diagnosis. METHODS: The subjects consisted of 60 (35 Males, 25 Females) skeletal Class III malocclusion adult patients (mean age 23.3 years) with anterior crossbite. RESULTS: In Class III malocclusion adult patients, there were significant differences in the nasolabial angle changes between closed lip position and relaxed lip position. Using the cluster analysis, the subjects were divided into three groups according to the pattern of nasolabial angle change: Group 1 (N = 27, 45%, -8 ~ 1), Group 2 (N = 30, 50%, 2degrees ~ 17), and Group 3 (N = 3, 5%, over 18). CONCLUSION: The results showed that the pattern of the nasolabial angle change between closed lip position and relaxed lip position varies in skeletal Class III malocclusion patients. Thus, relaxed lip position should be taken into account when diagnostic records are obtained and analyzed to accurately to evaluate the facial soft tissues and predict facial esthetics after surgical-orthodontic treatment.

7.Comparing locus of control in orthodontic patients of a university dental hospital with those of a private dental clinic.

OBJECTIVE: Many factors are considered when a patient chooses the type of dental institution they wish to receive treatment from. Numerous studies have been conducted regarding this issue. However, more research is needed in analyzing the patient's psychological aspects when he/she decides to choose the dental institution they wish to receive treatment from. METHODS: A locus of control test was utilized to analyze the psychological aspects in orthodontic patients of a university dental hospital and a private dental clinic. The locus of control test is known to be useful in predicting a human being's behavior. Both intrinsic and extrinsic locus of control tests were carried out in 934 orthodontic patients attending Seoul National University Dental Hospital and 1466 orthodontic patients from a private dental clinic. RESULTS: The orthodontic patients at the university dental hospital showed a stronger extrinsic locus of control compared with those from the private dental clinic. CONCLUSION: The orthodontic patients at the university dental hospital value the doctor as the authority figure and the treatment quality more than those at the private dental clinic.

8.Early Stage Lymphedema in Breast Cancer Patient Detected by Indocyanine Green Lymphography but not by Lymphoscintigraphy: A Case Report

Secondary upper extremity lymphedema after lymph node dissection in breast cancer patients is a major complication affecting their quality of life. As lymphatic function is compromised before symptoms develop in most cases, early diagnosis and intervention before symptomatic manifestation is known to prevent severe progression of lymphedema. In this case report, we present a patient with secondary lymphedema, whose disease could be diagnosed at an early stage using indocyanine green (ICG) lymphography. The patient had mild edema of the left upper extremity that regressed with elevation, had normal lymph vessel distribution without dermal backflow pattern in lymphoscintigraphy. Her symptom improved after early prescription of complex decongestive physiotherapy. We conclude that ICG lymphography could be an effective diagnostic tool in addition to other imaging methods, for early diagnosis and intervention of lymphedema.

Purpose:
Advances in chemotherapeutic and targeted agents have increased pathologic complete response (pCR) rates after neoadjuvant systemic therapy (NST). Vacuum-assisted biopsy (VAB) has been suggested to accurately evaluate pCR. This study aims to confirm the non-inferiority of the 5-year disease-free survival of patients who omitted breast surgery when predicted to have a pCR based on breast magnetic resonance imaging (MRI) and VAB after NST, compared with patients with a pCR who had undergone breast surgery in previous studies.
Methods
The Omission of breast surgery for PredicTed pCR patients wIth MRI and vacuumassisted bIopsy in breaST cancer after neoadjuvant systemic therapy (OPTIMIST) trial is a prospective, multicenter, single-arm, non-inferiority study enrolling in 17 tertiary care hospitals in the Republic of Korea. Eligible patients must have a clip marker placed in the tumor and meet the MRI criteria suggesting complete clinical response (post-NST MRI size ≤ 1 cm and lesion-to-background signal enhancement ratio ≤ 1.6) after NST. Patients will undergo VAB, and breast surgery will be omitted for those with no residual tumor. Axillary surgery can also be omitted if the patient was clinically node-negative before and after NST and met the stringent criteria of MRI size ≤ 0.5 cm. Survival and efficacy outcomes are evaluated over five years.Discussion: This study seeks to establish evidence for the safe omission of breast surgery in exceptional responders to NST while minimizing patient burden. The trial will address concerns about potential undertreatment due to false-negative results and recurrence as well as improved patient-reported quality of life issues from the omission of surgery. Successful completion of this trial may reshape clinical practice for certain breast cancer subtypes and lead to a safe and less invasive approach for selected patients.
