1.Primary Glioblastoma of the Cerebellopontine Angle: Case Report and Review of the Literature.

Glioblastoma multiforme (GBM) is located most frequently in the cerebral hemispheres. Glioblastoma presenting as an extraaxial mass of cerebellopontine angle (CPA) is very rare in adults. We report a rare case of GBM arising in the CPA. The patient was a 71-year-old female, who complained of progressive gait disturbance and poor memory. Initial magnetic resonance imaging (MRI) revealed a 1.4×1.3 cm mass in the left CPA, with broad base to the petrous bone, showing homogenous enhancement. Follow-up MRI showed a rapid increase in size of mass (2.7×2.2 cm) with a necrotic portion. A stereotactic biopsy was done under the guidance of navigation system, and the histopathologic diagnosis was GBM, World Heath Organization grade IV. Further surgical resection was not performed considering her general condition, and the patient underwent concurrent chemotherapy with radiation therapy. Although rare, the possibility of glioblastoma should be included in the differential diagnosis of atypical CPA tumor.

3.Prediction of Type of Proximal Femur Fracture by Analysis of Serum Makers

PURPOSE: To analyze serum markers in proximal femur fracture patients, which are known to be accompanied with osteophorosis or metabolic diseases, and consider it as predictive factor and to study how it affects on the fracture form. MATERIALS AND METHODS: Vit. D3 known as serum marker of Vit. D, 25(OH)D, and 1,25(OH)2D of 152 patients from April, 2013 to March, 2014 who visited our hospital and diagnosed as proximal femur fracture(Femur neck fracture 73, Intertrochanteric fracture 79) were analyzed. Also, serum calcium level corrected by serum protein, phosphate, creatinine, alkaline phosphatase, estradiol, IGF-1, and PTH were compared and analyzed. RESULTS: Femur neck fracture group showed significantly low value of D3, 25(OH)D, and 1,25(OH)2D (P-value<0.05) as the values were 37.72+/-7.21 pg/mL, 27.05+/-8.43 ng/mL, 25.05+/-6.78 pg/mL in femur neck fracture group, and 40.36+/-6.97 pg/mL, 29.54+/-9.12 ng/mL, 28.87+/-7.43 pg/mL in intertrochanteric fracture group respectively. The number of patients who were diagnosed as hypovitaminosis D. due to serum 25(OH)D value lower than 30ng/ml were significantly high in femur neck fracture group as the numbers were 56 in femur neck fracture, and 44 in intertrochanteric fracture. Serum calcium level corrected by serum protein, phosphate, creatinine, alkaline phosphatase, estradiol, IGF-1 and PTH level showed no significant difference between two groups. CONCLUSION: Serum 25(OH)D level was decreased in proximal femur fracture patiens, and was lower as the patient was older and BMD was lower. Therefore it is thought that proximal femur fracture is affected by vitamin D metabolism.

8.Correlation between the Penile Size and Prostatic Volume.

PURPOSE: To evaluate the clinical correlation between penile size and prostatic volume. MATERIALS AND METHODS: A total of 196 men were included in this study, 96 patients diagnosed as benign prostataic hyperpalsia(BPH, mean age: 67.2+/-18.8) by transurethral ultrasonography(TRUS), and 100 healthy volunteers(mean age: 48+/-16.2). The stretched penile length and penile circumference were measured by one examiner. We compared penile size with prostatic volume. BPH patients were classified as group I(30~39 ml), group II(40~49 ml) and group III(above 50 ml) by prostatic volume. RESULTS: For penile length and penile circumference, the mean values were 12.3+/-2.0 cm and 9.1+/-1.1 cm in group I, 12.6+/-2.2 cm and 9.2+/-1.0 cm in group II, 13.9+/-2.7 cm and 9.8+/-1.2 cm in group III, and 12.3+/-1.3 cm and 9.0+/-1.2 cm in control group, respectively. Compared to the control group, penile size was significantly higher in group III(p <0.05). CONCLUSIONS: Larger prostatic volume, especially above 50 ml, is associated with a significant increase in penile size.

9.Change of Serum Electrolyte Level Following Succinycholine Administration.

Succinylcholine(SCC) is well known to elevate the serum potassium level and to threaten life in some cases. In order to minimize the elevation of serum potassium following SCC, several attempts have been made such as SCC tamming and pretreatment with pancuronium or lidocaine or diazepam. Our study was undertaken to evaluate the change of serum potassium following induction in 60 patients divided into 7 different groups; group l. SCC 1mg/kg; group ll. SCC 2 mg/kg, grouplll, SCC 1mg/kg after SCC 10mg; group IV, SCC 1mg/kg after pancuronium 0.015mg/kg, group V. Pnacuronium 0.1mg/kg; group VI, SCC 1mg/kg after Lidocaine 1mg/kg; grou VII, SCC 1mg/kg after Diazepam 0.2mg/kg. The results were as follows: 1) Serum potassium were increased 0.14mEq/L in group l and 0.17 mEq/L in group ll. 2) Serum potassium were decreased in group lll, VI and VII than their control values but they were not statistically significant. 3) The decrease in serum potassium was most significant in group IV and V.

10.Outer Retinal Tubulation in Chronic Central Serous Chorioretinopathy.

PURPOSE: To evaluate outer retinal tubulation (ORT) found in chronic central serous chorioretinopathy (CSC) using color fundus photography and spectral domain optical coherence tomography (SD-OCT). METHODS: ORT identified in patients with chronic CSC was examined using color fundus photography, fundus autofluorescence image, fluorescein angiography, indocyanin green angiography, and SD-OCT. The images were analyzed for morphological features, location, and size of ORT in the retinal layers. RESULTS: ORT was detected in 3 of 342 (0.88%) chronic CSC patients. Color fundus photography revealed circular or ovoid shape with hollow lumen and deep yellowish border mainly appearing at the affected retinal pigment epithelium layer in the macular area. SD-OCT B-scan revealed hyperreflective material observed inside the hyporeflective internal space with hyperreflective border. ORT had circular or ovoid shape on the SD-OCT C-scan. ORT was primarily located on the outer nuclear layer in the retina, emanating to the inner nuclear layer and was not greater than 170 x 170 microm in size when measured with SD-OCT B-scan image. CONCLUSIONS: ORT was identified in patients with chronic CSC which was observed using color fundus photography and circular or ovoid structure was observed using a SD-OCT C-scan.
