1.Clinical Analysis of Metachronous Inguinal Hernia in Children.

PURPOSE: The purpose of this study is to identify the incidence of metachronous contralateral inguinal hernia (MCH) and how early patients visit hospital upon discovering the presence of a lump on the contralateral side after initial hernia repair. METHODS: This is a retrospective study of 2,169 patients with inguinal hernia between January 2001 and January 2010 at a single institution. We evaluated the occurrence of MCH among 1,689 consecutive unilateral inguinal hernia (UIH) patients who were treated in our department. We also analyzed and compared the time lag (number of days) between noticing the presence of hernia and hospital visit between UIH and MCH. RESULTS: A total of 102 patients with MCH underwent initial UIH repair during the study period. The incidence of MCH was 6.0% and was slightly higher in males (6.7%) than in females (3.5%). The patients with left inguinal hernia (7.3%) had higher incidence of developing MCH than those with right (5.2%). When we analyzed the patients with MCH, 69.6% of them were under age 5 and 74.6% of MCH occurred within 2 years after initial UIH repair. Seventy-three percent of the patients with MCH, whose time lag was over one month when the first episode of UIH occurred, visited hospital later as MCH occurred. CONCLUSION: The incidence of MCH within the study period is 6.0%. It is slightly higher in males and in patients with left inguinal hernia. Most MCH occurred under age 5 and within 2 years after initial UIH repair.

2.Relationship of workplace violence and perpetrators on sleep disturbance-data from the 4th Korean working conditions survey.

OBJECTIVE: The present study analyzed relationship of workplace violence and perpetrators of violence on sleep disturbance among wage workers in Korea. METHODS: The present study used data from the 4th Korean Working Conditions Survey (KWCS) of 2014 in selecting a total of 25,138wage workers as the study population, which excluded those who failed or refused to respond to questions required for the present study. The workplace violence experience group included people who satisfied at least one of six relevant criteria (verbal abuse, unwanted sexual attention, threatening or humiliating behavior, physical violence, bullying/harassment, and sexual harassment) and the group was divided according to whether the perpetrator of violence was a client or colleague. Presence of sleep disturbance was determined based on subjective symptoms felt within the past 12 months by each individual. A multiple logistic regression analysis was performed to identify the effects on sleep distance according to general, occupational, and psychosocial characteristics, as well as the types of workplace violence and perpetrators of violence. RESULTS: Workplace violence was found as a factor affecting sleep disturbance (OR = 3.773, 95 % CI = 3.058–4.655), and with respect to perpetrators of violence, complaint of sleep disturbance symptoms was higher when the perpetrator was a colleague or boss (OR = 5.688, 95 % CI 4.189–7.723) than a client (OR = 2.992, 95 % CI 2.301–3.890). CONCLUSION: Workplace violence had an effect on occurrence of sleep disturbance and when the perpetrators of violence was a boss or colleague at work, the risk for symptoms such as sleep disturbance increased, which indicated the need for appropriate intervention from a workplace healthcare perspective, including preventive education of workplace violence among employees.

3.Soluble Lectin-Like Oxidized Low-Density Lipoprotein Receptor 1 Is Inversely Correlated with the Activity of ANCA-Associated Vasculitis

Lectin-like oxidized low-density lipoprotein (LDL) receptor 1 (LOX1) binds to oxidized LDL, which is associated with inflammation in various vascular disorders. Here, we aimed to investigate the potential of soluble LOX1 (sLOX1) as an indicator of antineutrophil cytoplasmic antibody-associated vasculitis (AAV) activity. Serum levels of sLOX1 in frozen samples from patients with AAV enrolled in a prospective observational cohort study at the Severance Hospital were measured using enzyme-linked immunosorbent assay. Clinical and laboratory data were collected on the date when the blood sampling was performed. The association between sLOX1 and clinical and laboratory data was assessed using Pearson’s correlation analysis. The median age of the recruited 79 patients was 62.0 years, and 27 (34.2%) patients were men. The median Birmingham vasculitis activity score (BVAS), five-factor score, vasculitis damage index, and sLOX1 level were 6, 1, 3, and 911.9 pg/mL, respectively. Correlation analysis based on BVAS revealed that sLOX1 and total cholesterol were significantly inversely correlated with BVAS (r=-0.224, p=0.047 and r= -0.424, p<0.001, respectively). No significant correlations were observed between continuous variables and sLOX1 except for BVAS, although total cholesterol tended to correlate with sLOX1 (r=0.190, p=0.093). Additionally, sLOX1 was not influenced by sex, hypertension, diabetes mellitus, or the presence of pulmonary, cardiovascular, and renal involvement of AAV. In summary, sLOX1 was inversely correlated with BVAS in AAV patients, which is different from other vascular diseases or inflammatory diseases.
4.Lipid Profiles in Anti-neutrophil Cytoplasmic Antibody-associated Vasculitis: A Cross-sectional Analysis

. Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is an autoimmune disease (AID) characterised by necrotising intravascular inflammation. Growing evidence suggests that immune system triggers altered lipid metabolism in AIDs. We investigated whether changes in lipid profile correlate with severity of disease in AAV. Methods. Seven lipid profiles were evaluated utilizing frozen serum samples from 67 patients registered in the Severance Hospital ANCA-associated VasculitidEs cohort by a chemistry autoanalyzer. The Birmingham Vasculitis Activity Score (BVAS) version 3 was used to measure patient’s assessment of global disease activity. The relationship between the BVAS with continuous variables was calculated by Pearson’s correlation analysis. Results. Thirty-five (52.2%), 19 (28.4%), and 13 (19.4%) patients were diagnosed with microscopic polyangiitis, granulomatosis with polyangiitis, and eosinophilic granulomatosis with polyangiitis, respectively. Patients’ mean age was 60.0 years, and 22 (32.8%) were male. Among the lipid profiles investigated, total cholesterol, high-density lipoprotein, and low-density lipoprotein, and apolipoprotein A1 and B were significantly associated with BVAS; apolipoprotein A1 showed the highest correlation with BVAS (r=−0.521, p<0.001), remaining consistent even in patients with new-onset disease (r=−0.430, p=0.012). Apolipoprotein A1 had the highest association with the renal manifestation score among the clinical scores comprising BVAS (r=−0.457, p<0.001). Conclusion. Decreased lipid levels, especially apolipoprotein A1, are relevant to increased AAV disease activity, and differ according to organ involvement. Measuring lipid profiles could have clinical implications regarding the assessment of global disease activity and organ involvement patterns.
5.Association between masticatory function and cognitive impairment in the elderly

Objectives:
With the surge in the elderly population, a growing interest in the prevention and treatment of geriatric diseases has been observed, along with awareness of the severity of problems associated with dementia, a cognitive impairment. The purpose of this study was to investigate the correlation between chewing ability and cognitive function among elderly people residing in a rural area.
Methods:
A total of 162 elderly individuals, aged between 65 and 97 years, were surveyed. Trained examiners conducted interviews and assessments of chewing ability, on the basis of the number of remaining teeth, denture status, masticatory performance evaluating gum, ShadeEye-NCC measuring overall change in color of the gum (ΔE), and T-Scan® III analyzing distribution of occlusion patterns. Cognitive function was assessed using the Korean version of the Mini-Mental State Examination-Dementia Screening (MMSE-DS) tool.
Results:
Participants with a low score in the MMSE-DS were found to have distinguishably lower denture need, smaller number of remaining teeth, and lesser color change in the masticatory performance evaluating gum. In the cognitive impairment group, a tendency of having unilateral and anterior occlusion led to occlusal discomfort and chewing difficulties.
Conclusions
The study highlights important associations between chewing ability and cognitive function. The finding corroborates that tooth loss may be a predictive risk factor for cognitive impairment.

Methods:
This study included 76 patients with AAV whose serum periostin was measured from sera collected and stored at diagnosis. The correlation of either serum periostin or the Birmingham Vasculitis Activity Score (BVAS) with other variables was evaluated. Cumulative survival rates were compared using Kaplan–Meier survival analysis. The variables at diagnosis were compared between deceased and surviving patients. Hazard ratios were obtained by Cox proportional hazard analysis.
Results:
The median age of the 76 patients was 64.0 years and 60.5% were female. The median BVAS and serum periostin were 5.0 and 10.9 ng/mL, respectively. Five of the 76 patients (6.6%) died. Serum periostin was independently correlated with cross-sectional BVAS, the Vasculitis Damage Index (VDI), white blood cell count, and serum albumin. Patients with serum periostin ≥ 15.9 ng/mL at diagnosis had a significantly lower cumulative survival rate than those without. In addition to high VDI, dyslipidaemia frequency, and C-reactive protein, deceased patients showed higher serum periostin than surviving patients. In multivariable Cox analysis, however, only dyslipidaemia rather than serum periostin was identified as an independent predictor of all-cause mortality.
Conclusions
This study is the first to demonstrate that serum periostin at diagnosis could independently reflect cross-sectional BVAS and further partially contribute to all-cause mortality prediction in patients with AAV.
