1.Cutaneous Metastasis from Prostatic Cancer.

Cutaneous or subcutaneous metastases occur in 2% to 9% of visceral malignancies. Skin metastases are seen most commonly from carcinomas of the breast, lung, large intestine. Skin metastases of prostatic origin are quite uncommon and preferentially localized to the lower abdomen and genital area. Two cases of cutaneous metastases of prostatic origin have been reported in the Korean dermatological literature in our review. We report a case of cutaneous metastases from prostatic cancer

2.Nevus Lipomatosus Superficialis on the Dorsal Foot.

Nevus lipomatosus superficialis is a fairly uncommon disease characterized by ectopic fat tissue in the dermis. Lesions most commonly occur on the lower trunk, especially on the back, buttocks or abdomen. Rarely, lesions also occur on the knee, axilla, arm, ear and scalp. To our knowledge this is the first case of nevus lipomatosus superficialis developing on the dorsal foot.

3.A Case of Primary Carcinoma of the Fallopian Tube.

Primary carcinoma of the uterine tube is one of the least common gynecologic malignancies with a reported incidence of approximately 0.3%. As a result of it, the experience of any one physian is limited. Almost all cases are adenocarcinoma and the cilinical presentation is generally nonspecific, of which the most common symptom is postmenopausal vaginal bleeding. Primary fallopian tube carcinima is infrequently diagnosed before explolatory laparotomy and the majority of patients have extensive disease at diagnosis. A case of vaginal and cervical adenocarcinoma, mostly of clear cell type, in young women have been associated with intrauterine exposure to diethystillbestrol(DES) or other nonsteroidal estrogenic substances and vaginal adenosis. We have encountered a case of clear-cell adenocarcinoma of the cervix uteri of 27years young house wife, in which there was a history of intrauterine exposure to DES. We presented a case with a brief review of related literature.

5.Perforating Lichen Nitidus.

A healthy 18-year-old woman presented with an asymptomatic tiny papular eruption on the hands, forearms, legs and trunk. A skin biopsy from the left forearm revealed a typical lichen nitidus lesion associated with an adjacent transepidermal perforating lesion. We report a case of perforating lichen nitidus, and briefly review the relevant literature of perforating lichen nitidus.

6.Immunohistochemical Expression of P-Glycoprotein in Gynecologic Malignancies.

The expression of P-glycoprotein in gynecological tissues was studied by immunohistochemical staining methods. Aspects of study included the expression of P-glycoprotein in different tissues throughout the clinical treatment regimen, the relationship between the expression of P-glycoprotein and the degree of pathologic malignancy, and the expression of P-glycoprotein in cancerous tissue before and after chemotherapy. Studies were based on patients who were admitted to the Department of Obstetrics and Gynecology of Chungnam National University Hospital from January 1988 to December 1993. Tissue samples collected prior to chemotherapy included 34 ovarian cancers, 73 cervical cancers, and 11 endometrial cancers. Pre and post-chemotherapy tissue samples included 11 ovarian cancers and 15 cervical cancers. Normal tissue samples included 12 from the ovaries, 15 from the cervix, and 10 from the endometrium. RESULTS ARE AS FOLLOWS:1. p-glycoprotein was mainly found in the cytoplasm of both normal tissue cells and cells of tissues prior to chemotherapy. After chemotherapy it was found more intensely in the cell membrane than in the cytoplasm. 2. For normal tissue, p-glycoprotein was found in 25% of ovarian tissues, 33.3% of uterine cervical tissues, and 40.0% of endometrial tissues. 3. For cancerous tissues prior to chemotherapy, p-glycoprotein was found in 45.5% of ovarian cancer cases, 47.9% of uterine cervical cancer cases, and 45.5% of endometrial cancer cases. There was no statistically meaningful difference in these rates in cancerous versus normal tissues. 4. The expression of P-glycoprotein in cancerous tissues prior to chemotherapy was not related to histologic type. 5. For ovarian cancer tissue, p-glycoprotien was expressed in 45.5% of cases prior to chemotherapy, and 54.4% of cases subsequent to chemotherapy. For uterine cervical cancer tissue, p-glycoprotein expression rates before and after chemotherapy was 46.7% and 60.0% respectively and there was a statistically meaningful difference(p<0.05). 6. There was no relationship between P-glycoprotein expression in cancer tissues after chermotherapy and the presence of cisplatin in chemotherapeutic drugs. 7. For uterine cervical cancer tissues prior to chemotherapy, there was no relationship between the degree histologic differentiation and the expression of P-glycoprotein. 8. For cancerous tissues there was no relationship between clinical stage and the expression of P-glycoprotein. In conclusion, the expression of P-glycoprotein was identified in the tissues before the drug exposure. However, there was no relationship between the expression of P-glycoprotein and hlstologic type, clinical stage, and effectiveness of chemotherapy, This may be related to P-glycoprotein inducing a cellular resistance to chemotherapeutic agents, although the importance of this resistance is thought to be small. Further studies of P-glycoprotein are needed to delineate its role in cellular anticancer drug resistance.

7.Effect of Unilateral Diaphragmatic Palsy on Lung Perfusion in Rabbit Model.

BACKGROUND: In congenital heart disease, the lung perfusion through stenosed pulmonary artery is usually decreased. And this decrement of lung perfusion also occurs with diaphragmatic palsy after the operation of congenital heart disease. It is difficult to delineate the amount of lung perfusion in case of combination of pulmonary artery stenosis and diaphragmatic palsy. We examined the change of lung perfusion after the induction of diaphragmatic palsy in rabbits. METHODS: We dissected left phrenic nerves in 20 rabbits to induce left diaphragmatic palsy. The lung perfusion scan was performed with 99mTc-MAA and the movement of diaphragm was examined with fluoroscopy. They were performed as baseline data and on 3rd and 10th day postoperatively. The amount of left lung pefusion before and after diaphragmatic palsy was compared and analysed in 12 rabbits which definitely had diaphragmatic palsy. RESULTS: Weight of the rabbits was 1.65+/-0.26 kg. Left lung perfusion percent was 45.93+/-6.42% before operation and these were 32.48+/-6.09% and 37.62+/-3.39% on the 3rd and 10th postoperative day, respectively. Left lung perfusion was significantly decreased just after diaphragmatic palsy but it was not changed thereafter. The decrement of lung perfusion was not affected by the body weight. The decreased amount of left lung perfusion was reciprocally correlated with the body weight of the rabbits on the postoperative 3rd day but not 10th day. CONCLUSION: Left lung perfusion percent of the rabbits was decreased 7% with the induction of diaphragmatic palsy and the decreased amount was reciprocally correlated with the body weight just after the diaphragmatic palsy was induced.

10.Epidural Morphine Does not Reduce Mean Inspired Concentration of Isoflurane in Humans during General Anesthesia.

The effect of epidural morphine on the mean inspired concentration of isoflurane was inves- tigated in 34 patients undergoing hepatoma surgery, The patients were randomly assigned to the control(CTRL), epidural morphine(MORP) or epidural bupivacaine(BUPI) groups. Before induction of anesthesia, epidural catheter was inserted beween T6-10 intervertebral space in the right lateral decubitus position. MORP group(n=13) received morphine sulfate 4 mg in 4 ml normal saline and BUPI group(n=10) received 0.5% bupivacame 10 md through epidural eatheter. CTRL group(n=11) wss treated in an identical fashion except that, after placement of epidural catheter, nothing was injected. After endotracheal intubation, general anesthesia was maintained during surgery with oxygen-nitrous oxide(1:1 ratio) and isoflurane. Inspired concentration of isoflurane was adjusted to maintain blood pressure in the range of blood pressure at the ward during surgery. Mean inspired concentration of isoflurane was monitored from incision to last skin suture. Mean inspired concentration of isoflurane(+/-SD) of CTRL, MORP and BUPI group was 1.41+/-0.36, 1.52+/-0.39 and 0.37+/-0.16 vol%, respectively. There was no difference in mean inspired concentration of isoflurane between CTRL and MORP group. Under the condition of this study, epidural morphine did not reduce inspired concentration of isoflurane during surgery even though epidural morphine is effective for post operative pain control.
