Extrapelvic endometriosis is a rare disease. The majority of extrapelvic endometriosis cases involve scar tissue following obstetric and gynecologic procedures. We have treated two cases of extrapelvic incisional endometriosis. A 39 year old female patient with cyclic vaginal spotting after laparoscopic assisted vaginal hysterectomy due to uterine myoma and a 35 year old female patient with a painful palpable abdominal mass after cesarean section. Both underwent complete excision and were proven to have endometriosis by pathology. Here we report on both cases and review the medical literatures.

5.A radiologic observation of male infertility by vao-epididymography and vasoseminal vesiculography

The vaso-epididymography and vasoseminal vesiculography is the only diagnositc method for investigating thepatency and abnormal lesion of male genital system radiologically. The author performed 53 cases ofvaso-epididymography and vasoseminal vesiculography in Busan National University Hospital during the 8 years from1973 to 1981. All patients complained of primary infertility more than a year and were confirmed azoospermia onsemen examination, and 23 cases of them underwent testicular biopsy. The results are as follows; 1. The agedistribution were mainly between 26 and 35 years old (83.0%). 2. The infertile duration were almost from 1 to 5years(86.8%). 3. The past history associated with infertility was gonorrhea, tuberculosis and scrotal trauma. 4.Of the 23 cases of confirmed by testicular biopsy, the pathologic findings were normal spermatogenesis in 12 cases(52.2%) and hypospermatogenesis or arrest type in 11 cases (47.8%). 5. Non-filling sites of contrast media onvas-epididymography and vasoseminal vesiculography were epididymis(83.0%), ejaculatory duct (41.5%) and seminalvesicle (24.5%), were commonly bilateral (68.0%) and were more prevalent in the right side than left (1.2:1.0). 6.The abnormal findings of ejaculatory ducts, seminal vesicles, vas ampullas and vas deferens were mainly strictureof obstruction, obstruction of dilatation, beaded appearance or obstruction and obstruction respectively. 7. Inthe epididymis, there was almost obstructive lesion at the tail portion(83.0%).

7.Diagnostic Usefulness of the Multimodality Study with Mammography, Ultrasonography,99mTc-MIBI Scan in Breast Cancer.

PURPOSE: To assess the diagnostic usefulness of multimodality study in the diagnosis of breast cancer. MATERIALS AND METHODS: Eighty-one patients between January 1998 to December 1999 were involved in this study, and who underwent mammography, breast ultrasonography, and 99mTc-MIBI scintimammography in all cases the findings were retrospectively reviewed. Each modality was graded by two physicians specializing in nuclear medicine and two radiologists, all unaware of the pathologic results. A four-grade system(1=definitely benign, 2=probably benign, 3=probably malignant, 4=definitely malignant) was applied to those ofbreast ultrasonography and 99m Tc-MIBI scintimammography and BIRADS was applied to those ofmammography. All breast masses were confirmed by surgery (n=67) or FNA (n=14). Findings of grade 3 or 4 the four-grade system, BIRADS category 4 or 5, or positive coincidence in double and triple combination studies were defined as positive results, and on the basis of the data thus obtained, the sensitivity, specificity, positive and negative predictive value and diagnostic accuracy were calculated for each modality and for multimodality studies. RESULTS: The sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy of 99mTc-MIBI scintimammography were 83.6%, 88.5%, 93.9%, 71.9% and 85.2%, respectively. For ultrasonography, the corresponding figure were 94.5%, 65.3%, 85.2%, 85.0%, and 85.2%, and these for mammography, they were 87.3%, 69.2%, 87.3%, 81.8% and 80.2%. For the ultrasonography and mammography combination, the figures were 83.6%, 50.0%, 93.9%, 100.0% and 95.2%, respectively, and for 99mTc-MIBI scinti-mammography and mammography, the corresponding findings were 72.7%, 69.2%, 95.2%, 100.0% and 96.7%. For the 99mTc-MIBI scintimammography and ultrasonography combination, respective findings of were 80.0%, 61.5%, 95.7%, 94.1% and 95.2%, respectively, and in the triple modality study, respective findings of 70.9%, 50.0%, 97.5%, 100.0% and 98.1% were recorded. CONCLUSION: Among multimodality studies, sensitivity was greatest in the ultrasonography and mammography combination, which is thus extremely suitable for the diagnosis of breast cancer. The findings of two series suggest that in equivocal cases, 99mTc-MIBI scintimammography with its higher specificity and positive predictive value, is a useful adjunctive tool.

8.The effect of Korean mistletoe extract M11C (non-lectin components) on IL-1beta release and expression from macrophages.

BACKGROUND: Korean mistletoe (Viscum album) extract has been found to posses immunostimulatory activity. In this study, Korean mistletoe extract, M11C (non-lectin components), was used to know whether this extract might activate mouse peritoneal macrophages to produce interleukin 1 beta (IL-1 beta). METHODS: Hemagglutination assay was carried out to examine whether M11C contained a lectin or not . To know the effect of M11C on the production of IL-1 beta, the macrophages were treated by the M11C, and then collected the supernatant (M11C stimulated macrophages-conditioned media; MMCM). MMCM was analyzed for the IL-1 beta quantification and mRNA expression by means of ELISA and RT-PCR, respectively. RESULTS: Maximum effective dose and time of M11C on IL-1 beta production from macrophages were 20 micro gram/ml and 8 hours, respectively . This ELISA data was reconfirmed by immunoblotting assay . indicating that M11C is a good candidate for an immunomodulator. The dose and time dependent effect s of M11C on the expression of IL-1 beta mRNA from macrophages was also shown in expression of mRNA detected by RT-PCR. Treatment dose and time for the maximum expression of IL-1 beta mRNA were 20 micro gram/ml and 4 hours, respectively . Maximum gene expression of IL-1 beta was much earlier than maximum production of it. CONCLUSION: As results, Korean mistletoe extract, M11C, may be used for an immunomodulator. This will be able to make up for and solve the problems caused by existent immunoagent with many adverse effects through many other studies in future including one molecule extraction.

9.Wound States in Pediatric Open Heart Surgery with Bilateral Submammary Skin Incision Combined with Vertical Sternotomy.

BACKGROUND: Median sternotomy remains the standard approach used by surgeons for most intracardiacscar we have tried to use alternative skin incision (bilateral subm-ammary skin incision) to median sternal skin incision, In this study we have tried to compare different postoperative results of wound between two different skin incisional methods. MATERIAL AND METHOD: Between June 1997 and June 1998, a bilateral subma-mmary skin incision combined with v operations but the residual scar is cosmetically unsatisfactory. To avoid an unsightly midline ertical sternotomy was performed in 21 pediatric female patients (submammary group)to repair acyanotic congenital heart disease. after the period we carried out a retrograde sion controls in whom there were 23 pediatric patients (control group). RESULT: Patients' age ranged from 14 to 96 months(mean 38.2 months) Mean duration of subcutaneous drains using Hemovac which was used only in the patients of submammary group was 4.2 days and total amount of the drained effusion was 51.1 ml. Postoperative wound complications included wound eruption in one patient wound disruption in one patient and skin necrosis in 3 patients in submammary group and included wound disruption in 4 patients in controls. mean duration required for wound healing was 15,5 days in submammary group versus 10.4 days in controls. The mean scar length was 12.5 cm in submammary group versus 11.3 cm in controls. The average follow-up was 8.2 months in submammary group versus 9.0 months in controls. In submammary group 3 patients parents(14.3%) were pleased with their cosmetic results of wound scar but 8(38.1%) were dissatisfied. Among the 23 patients in control group 8(34.8%) were pleased but 8(34.8%) complained ofunhappiness with the scar.

10.Enrichment of flavonoid aglycones in licorice extract enhanced anti-inflammatory potential,but its hypnotic effect was not altered

OBJECTIVE Licorice is used throughout the world as a traditional herbal remedy. Ac-cording to Chinese traditional medicine licorice alone can be used to treat inflammation.Although there have been some studies investigated the anti-inflammatory ingredients of licorice, but for the potency of flavonoid glycoside and their aglycones on inflammation are not evaluated.This study was designed to assess the contributions of licorice flavonoid glycosides and their aglycons to its anti-inflammatory and hypnotic effects. METHODS For the flavonoid aglycone's enrichment, the extract of licorice (EL) was fermented in submerged culture of the edible fungus Grifola frondosa HB0071 mycelia which can produce β-glucosidase and catalyze the flavonoid glycosides to aglycones.EL and fermented extract of licorice (FEL) were used in this study. The anti-inflammation test was carried out in arachidonic acid (AA)-induced ear edema model and the hypnotic test was performed by using electroencephalogram (EEG)analysis method in normal freely moving SD rats.The chemicals constituents were analyzed by HPLC.RESULTS During fermentation,the falvonoid glycosides of licorice were hydrolyzed by the time process.Along with fermentation time,the concentration of the major flavonoid glycosides,liquiritin and isoliquiritin were decreased obviously, and simultaneously their aglycons, liquiritigenin and isoliquiriti-genin were remarkably increased in FEL.Moreover,the content of another major constituent glycyrrhi-zic acid and glycyrrhetinic acid were not changed after the fermentation. In AA-induced mice ear ede-ma test,after topical application,FEL(effective dose range:5-20 μg·ear-1)showed more potent inhibito-ry activity than EL(effective dose range:25-100 μg·ear-1).On the other hand,oral administration of EL and FEL exhibited the same hypnotic potency and both enhanced the total sleep time including rapid eye movement (REM) sleep and non-REM sleep time. CONCLUSION These results suggested that the enrichment of flavonoid aglycons such as liquiritigenin and isoliquiritigenin enhanced the anti-inflam-matory potency of licorice extract,and this potentiation has nothing to do with glycyrrhizic acid or glycyr-rhetinic acid.In addition,enrichment of flavonoid aglycones did not alter the hypnotic effect of licorice.
