2.Percutaneous catheter drainage of intraabdominal abscesses and fluid

Percutaneous catheter drainage has been reported to bean effective method in the management of selectedpatients with abscess and fluid collection. Its high sucess rate and relatively low complications make theprocedure an alternative to surgery in the individual cases. During past two years percutaneous catheter dainagein 25 patients with intraabdominal abscesses and fluid collection was performed at the Department of Radiology,Yonsei Universtiy College of medicine. Here the technique and auathor's results were summarized; 1. The total 25patients who had percutaneous catheter drainage are 10 liver abscesses, 3 subphrenic, one subhepatic, 4 renal andperirenal, 2 pelvic, one psoas, one anterior pararenal fluid from acute pancreatitis, one pancreas pseudocyst and2 malignant tumor necrosis. 2. The modified Seldinger technique used for all cases of abscess and fluid draingeunder guidence of ultrasound scan. The used catheters were 10F. Pigtail and 14F. Malecot (Cook c/o) catheters. 3.The abscesses and fluid of 17 patients among 25 were cured by the percutaneous catheter drainage and 4 patientswere clinically improved. The catheter drainage was failed in 2 patients and 3 complications were developed. 4.The success rate of this procefure was 91.3%, failure rate was 8.7% and complication rate was 12%.

3.Percutaneous biliary drainage in acute suppurative cholangitis with biliary sepsis.

Acute suppurative cholangitis is a severe inflammatory process of the bile duct occurred as a result of partial or complete obstruction of the bile duct, and may manifest clinically severe from of disease, rapidly deterioration to life-threatening condition. We analyzed emergency percutaneous transhepatic biliary drainage in 20 patients of acute suppurative cholangitis with biliary sepsis to evaluate the therapeutic effect and complication of the procedure. The underlying causes were 12 benign diseases(stones) and eight malignant tumors and among eight malignant tumors, bile duct stones(n=4) and clonorchiasis (n=1) were combined. Percutaneous transhepatic biliary drainage was performed successfully in 17 of 20 patients improvement of general condition and failed in three patients. The procedures were preterminated due to the patient's condition in two and biliary-porto fistula was developed in one. After biliary decompression by percutaneous transhepatic biliary drainage, effective and successful elective surgery was performed in nine cases, which were seven biliary stones and two biliary cancer with stones. Our experience suggests that emergency percutaneous transhepatic biliary drainage is an initial and effective treatment of choice for acute suppurative cholangitis with sepsis and a safe alternative for nonsurgical treatment.

4.Silver Sulfadiazineb Cream in the Treatment of Herpes Zoster.

Silver sulfadiazine, an antibacterial agent, has been used successfully in the praphylaxis and treatment of burn infection. 42 volunteers with herpes zoster were enrolled in a clinical trial to evaluate the effectiveness of topical silver sulfadiazine cream. 1% silver sulfadiazine cream was applied 4 times daily until crust removal and epithelialization on 21 patients. As the control, wet compress with 1: 5,000-1: 10,000 KMnO4 solution was done 4 times daily on 21 patients. ln treated group with silver sulfadiazine cream, the durations needed for marked reduction of erythema and edema of lesions, striking control of pain and burning sensation, complete drying of vesicles and crust formation, and crust removal and epithelialization were significantly decreased as compared to the control group. The rate of postherpetic neuralgia was also decreased markedly. The sooner the treatment began after the onset of symptoms, the more dramatic the response was. Signs of local or systemic side effects were not observed.

6.Effect of Removal of Internal Limiting Membrane in Macular Hole Surgery.

Tangential vitreoretinal traction around fovea is known to be the principal cause in the pathogenesis of idiopathic macular holes. According to the recent study, the internal limiting membrane(ILM) may provide a scaffold for fibroblast proliferation and contraction, therefore play an important role in the enlargement of macular hole. We removed the ILM during macular hole surgery and assessed the anatomical and functional outcome of the macular hole surgery. Twelve consecutive patients with full-thickness macular hole were enrolled in this study. They were examed for best-corrected vision, fundus photography, fluorescein angiography, Humphrey automated perimetry, focal eletroretinogram, before and after surgery. After surgery, vision was improved in 8 eyes(67%), hole was closed in 10 eyes(83%). Macular thresholds on visual field was increased in 8 eyes and peripheral scotomas developed in 3 eyes postoperatively. The amplitude increased in 7 eyes and the implicit time decreased in 8 eyes on focal electroretinograms postoperatively. Out results suggested that the complete removal of ILM should be attempted to improve vision and to close holes during the full-thickness macular hole surgery.

A 51-year-old female had disseminated, symmetrically distributed, brownish black pigmentation and papillary hypertrophy on the intertriginous and flexural areas for 6 months, and gastric adenocarcinoma was confirmed during the evaluation of internal malignancy, The cutaneous finding of acanthosis nigricans was a significant cutaneous marker of internal malignancy in this case.

9.Cure of severe hypothermia due to loss of consciousness after drug intoxication of inhaled hydrocarbons, Case 1.

A 19-year-old man had a good neurologic recovery from a severe hypothermia(19 degrees C) and a prolonged coma following active infernal rewarming. From inhaling hydrocarbons, he was left unconscious on the cold floor for 24 hours. As soon as the patient was brought into the emergency medical center, he was early evaluated arid treated aggressively. ECG showed Osborn(J) wave on all leads. The temperature o( patient was increased by 2-3 degrees C per hour through active external rewarming (by heating blankets and warm bag) and active infernal rewarming (by airway rewarming, warmed IV fluids, gastrointestinal tract irrigation, and bladder irrigation). The temperature reached 36 degrees C after 6 hours. Active infernal rewarming provides rapid core rewarming with the additional benefit of circulatory support during the period of cardiac instability.

10.Three Cases of Letterer - Siwe Disease.

Letterer-Siwe disease is one of Langerhans cell histiocytosis, with Hand-Schuller-Christian disease and eosinophilic granuloma, characterized by proliferation of Langevha is cell. The clinical course of Letterer-Siwe disease is acute fulmunant of casionally fatal, involving skin and many other internal organs, such as the lymph node, liver, spleen, ung and bone. We present 3 cases of Letterer-Siwe disease with characteristic cutaneous findings and revealed Langerhans granules by imrriunohistochemical stain and electronmicrc scopic examination. All patients died in spite of combined chernotherapy.
