2.Tetracycline versus combination of terracycline and metronidazole in the treatment of male patients with nogonococcal urethriris.

BACKGROUND: The number of nongonococcal urethritis(NGU) cases is exceeding more and more that of Gonorrhea in developeing nations and some of the developing countr es such as Korea. The best evidence for an etiologic role for Trichomonas Vaginalis is in nongonococcal urethritis. OBJECTIVE: We compared the effects of Tetracycline to Tetracycine and Metronidazole in the treatment of male patients with nongonococcal urethritis and wished to find out the incidence of Trichomon-as Vaginalis in male patients with nongonocaccal urethritis indirectly. METHOD: From January, ],989 to December, 1992, three hundred male patients with nongococcal urethritis(NGU) at the Venreal Disease Clinic of Choong-Ku Publi Health Center in Seoul were allocated randomly into one of two treatment regimens. and one hundred sixty eight patients were followed. RESULTS: Eighty(83.4%) of ninty six patients treated with Tetrac; cline, 500mg, PO, q.i.d. for 14 days were cured. Sixty two(86.2%) of seventy two patients treatec with Tetracycline, 500mg, PO, q.i. d. for 14 days and Metronidaole. 2.0gm, PO, for 1 day were cured. There is no significant different between the 2 groups in treated rates(P<0.05). CONCLUSION: It suggests that male patients with nongonococcal ure1hritis due to trichomoniasis are rare in Korea. This may be die to the fact that the incidence of trichorioniasis has been declining since the 1960s. It also suggests not only the need for further studies on tool is disease but also more studies on other causes of nongonocoqeal urethritis, its treatment and epidernio.ogical study.

3.A case of apocrine hidrocystoma.

A 26 year old female presented complaining of a pea-sized nodule on her left lower eyelid for 10 years duration. Histopathological study showed several cysts in the dermis surrounded by secretory cells. The cysts were filled with decapitatory secretions. Positive staining with carcinoembrynic an tigen(CEA) was detected by immunoperoxidase technique. These clinical and histopathological findings suggest apocrine hidrocystoma developed on the left lower eyelid.

4.A CASE OF RECONSTRUCTION IN UPPER LIP NECROSIS AFTER TCA INJECTION.

There are some effective methods for removing layers of skin to improve aging face and dermatologic defects : chemabrasion (chemical peeling), dermabrasion(surgical removal), laserabrasion. Chemabrasion, generally performed with a chemical solution, is most useful for removal of fine facial wrinkles and abnormal pigmentation. The application of chemical caustics, especially TCA (trichloroacetic acid), is known to be an effective and is now commonly practiced to improve the surface of the facial skin that has been blemished by pigmentation, wrinkles, solar damage and certain scars. Trichloroacetic acid is a colorless melting crystal and derivatives of acetic acid which das protein precipitating properties. It has specific odor but there is not systemic toxic effect like a phenol. It has heavy caustic effect on skin and mucosa and cause the coagulation necrosis of the skin and therefore, it must be handled carefully and stored in the proper condition. Especially, moderate to high concentrated TCA solution must be treated by well-trained persons. A 46-year-old woman visited a private clinic for removal of fine wrinkles around the lip. She was refered to our department because of acute upper lip coagulation necrosis which was caused by injection of 35% TCA solution mistaken for lidocaine, which was supposed to be used for anesthesia. At her arrival, cental half of upper lip showed severe coagulation necrosis. The lesion was well discriminated in a few days, then debridement and immediate reconstruction was done using an Abbe flap. Chemical peeling is a relatively effective procedure for improvement of aging skin lesion in a lower cost with a short time operation, if the practitioner has much experience and optimal indication is properly selected. But not so as, side effect and complication may be often ocurred in a mistake and as a result irreversible scars are remained. We experienced a rare case of upper lip necrosis which was caused by TCA injection mistaked for lidocaine in a private clinic and so, We report this case with the results of experiment of rabbit model which was observation of macroscopic and microscopic changes of the abdominal skin of the rabbit injected with serially diluted TCA solution.

5.Subendocardial Ischemic Necrosis in Hypertrophied Rabbit Heart Induced by Chronic Aoric.

BACKGROUND: Cardiac hypertrophy is the compensatory response of the myocadium to increased workload. Compensatory mechanisms come into play when the hypertrophied heart can no longer accommodate the increased demand or persistent stimuli. Although it has been reported that the molecular changes in hypertrophied hearts that initially mediate enhanced function may contribute to the development of heart failure, the structural/biochemical/molecular basis for myocardial contractile failure is still obscure. This study was aimed to clarify the structural basis for relation between hypertrophy and failure. METHOD: Nine pairs of rabbits were sacrificed at 8,12,24,48 hours and 1,2,4,6,8 weeks after experiment aortic constriction. There hearts were studied with routine histopathology. Each heart was weighed and compared with total body weight. Multiple sections were embedded in paraffin, sectioned at 5um, and stained with hematoxylin and cosin and Masson's trichrome and analysed. RESULTS: The heart weight to body weight ratio(g/Kg) increased progressively with time after aortic banding. Banding of the aorta in the rabbit resulted in multifocal areas of myofiber degeneration, necrosis and fibrosis through the wall of the left ventricle, in the papillary muscles of the left ventricle and in the left ventricular portions of the interventricular septum in rabbit of 6 and 8 weeks after aortic banding. By 4 weeks after banding, the foci of necrosis were not observed. CONCLUSION: There findings suggest that the increased necrosis, fibrosis in animals with cardiac hypertrophy induced by banding the aorta may play a role in progression to heart failure.

6.Isolation of a Partial Human cDNA Encoding a Factor Binding to the Perfect Palindrome of Enhancer A of HLA Class I Promoter, Homologous to NF-kB2.

Genes encoding sequence-specific DNA binding proteins have been isolated by screening cDNA libraries constructed in rgt11 expression vector with recognition site DNAs. We isolated a rgt11 recombinant human cDNA clone, designated to C2, using a DNA probe consisted of heptamer of the perfect palindrome (PP; GGGGATTCCCC) of enhancer A (Enh A) of HLA dass I promoter. Sequencing analysis showed that this clone contained a partial cDNA homologous to NF-kB2. Lysogenic E. coli containing the C2 was generated and crude cell extract was prepared. Immunoblot using anti-B-galactosidase antibody showed that this lysogenic E. coli expressed B-galactosidase fusion protein. Electrophoretic mobility shift assay (EMSA) and DNase I footprinting assay were done using crude cell extract and their patterns were compared with nuclear protein extracted from an EBV transformed B lymphoblastoid cell line (BLCL). EMSA showed that crude cell extract prepared from E. coli lysogen speci5cally bound to the PP of Enh A region of HLA class I gene. DNase I footprinting assay showed that the binding sequence of this recombinant B-galactosidase fusion protein was identical to that of nuclear protein extracted from a BLCL. Our data indicate that a Agt11 recombinant cDNA clone was isolated from a human cDNA library using the PP of Enh A of the HLA class I promoter and this clone encoded a B-galactosidase fusion protein capable of binding to the PP and belongs to a NF-xB subunit.

7.The Effects of Leuprolide Acetate to the Testicular Histopathology of Male Rat.

We performed this study to observe the effects of chronic administration of leuprolide acetate(potent synthetic LHRH analog) to the testicular histopathology of prostatic cancer patient and to the spermatogenesis in cryptochism treatment. Ten out of fifty healthy male rats whose body weight ranged 250-300gm were selected as a control group and the other forty as a study group. Two hundred micro-gram of leuprolide acetate was injected daily into the peritoneal cavity of study group, and ten rats were sacrificed at each first, second, third and sixth week, respectively. Then the changes of testicular weight and histopathological readings under light microscope of both control and study group were recorded and analyzed. The results of experimental data can be summarized as follows : 1. The average testicular weight of control group increased progressively until sixth week to 1. 1.43gm from the initial value of 1.26gm, while the weight of study group decreased to 0.96gm which was 67.1 % of age-matched control group. 2. There were almost no remarkable changes between both groups in terms of number, size and cellularity of seminiferous tubules. We observed the degenerative and regenerative changes simultaneously in spermatogonia and spermatocytes. During the first three weeks, degenerative changes took place while the regenerative changes started from the second week and have fully recovered at the sixth week. And, the number of spermatids and spermatozoa slightly decreased at the first week, but have recovered to normal value from the second week. 3. There were noticeable changes neither in the interstitium of testes except the minimal edema nor in terms of size and number of Leydig cell. The results using rats showed the discordance with those of human study of prostatic cancer patient. This conflict can be attributed to the functional differences of leuprolide action between two species, age of sacrificed rats and insufficiency of dose and time of administration. However, we can draw another conclusion that the leuprolide acetate is expected to have no significant adverse effects to the spermatogenesis in cryptorchism treatment.

9.Microvessel Quantification, Expression of p53 Protein and MIB-1 in Colorectal Adenoma and Carcinoma.

Angiogenesis is a crucial step in tumor growth and progression. Scarce data is available on angiogensis in gastrointestinal tumors. We studied 16 normal colon, 44 adenomas and 29 carcinomas to evaluate angiogenesis in colorectal tumors and to assess the correlation among p53 protein, proliferative activity and other clinical prognostic parameters. Endothelial cells were immunostained with an anti-Factor VIII mAb; in each case three microscopic fields(x 200) were counted: average number of the three fields was defined as microvessel density (MVD). p53 protein expression was 45.5%(20/44) in adenomas, and 79.3%(23/29) in carcinomas (p<0.01). p53 protein expression of carcinomas was 57.1%(4/7) in diploid tumors, 100%(8/8) in aneuploid tumors (p=0.07), 100%(8/8) in well differentiated tumors, and 50%(2/4) in poorly differentiated tumors (p=0.09). MIB-1 score was 2.3+/-0.7(38) in adenomas, 3.4+/-0.5(29) in carcinomas (p<0.01). There was no significant correlation between p53 protein and MIB-1 score. MVD was 10.4+/-4.1(16) in the normal mucosa, 21.5+/-7.9(39) in the adenomas, 35.3+/-9.7(26) in carcinomas (normal versus adenomas, p<0.01; adenomas versus carcinomas, p<0.01). MVD was 25.8+/-5.4(2) in carcinomas confined to mucosa, and 36.1+/-9.6(24) in carcinomas with transmural invasion. The higher MIB-1 score was in carcinomas the more MVD increased but there was no statistical significance (r=0.38, p=0.055). MVD of carcinomas was not associated with nodal metastasis, p53 expression, and DNA ploidy. p53 protein and MIB-1 expression are useful methods for the evaluation of malignancy, and tumor angiogenesis is an early event in a colorectal tumor but MVD does not correlate with prognostic parameters except for the tumor depth.
