PURPOSE: To describe chronic rectal mucosal damage after pelvic radiotherapy (RT) for cervical cancer and correlate these findings with clinical symptoms and radiation dose. MATERIALS AND METHODS: Thirty-two patients who underwent pelvic RT were diagnosed with radiation-induced proctitis based on endoscopy findings. The median follow-up period was 35 months after external beam radiotherapy (EBRT) and intracavitary radiotherapy (ICR). The Vienna Rectoscopy Score (VRS) was used to describe the endoscopic findings and compared to the European Organization for Research and Treatment of Cancer (EORTC)/Radiation Therapy Oncology Group (RTOG) morbidity score and the dosimetric parameters of RT (the ratio of rectal dose calculated at the rectal point [RP] to the prescribed dose, biologically effective dose [BED] at the RP in the ICR and EBRT plans, alpha/beta = 3). RESULTS: Rectal symptoms were noted in 28 patients (rectal bleeding in 21 patients, bowel habit changes in 6, mucosal stools in 1), and 4 patients had no symptoms. Endoscopic findings included telangiectasia in 18 patients, congested mucosa in 20, ulceration in 5, and stricture in 1. The RP ratio, BEDICR, BEDICR+EBRT was significantly associated with the VRS (RP ratio, median 76.5%; BEDICR, median 37.1 Gy3; BEDICR+EBRT, median 102.5 Gy3; p < 0.001). The VRS was significantly associated with the EORTC/RTOG score (p = 0.038). CONCLUSION: The most prevalent endoscopic findings of RT-induced proctitis were telangiectasia and congested mucosa. The VRS was significantly associated with the EORTC/RTOG score and RP radiation dose.

Antianginal and untoward effects of fenalcomine hydrochloride were studied in 24 cases of angina pectoris. Following discontinuation of all medications for 2 weeks except for liberal use of sublingual nitroglycerin for the relief of anginal attack, fenalcomine, 150mg a day in 3 divided doses, was given for 8 to 16 weeks. In 20 cases, routine blood counts including platelets, serum electrolytes and cholesterol as well as blood sugar level were checked before and at the completion of the medication. hepatic and renal functions were also studied. The effect of fenalcomine on severity, frequency and duration of anginal attack was excellent to good in 17 cases(70.9%), and was fair in 5 cases(20.8%). In the remaining 2 cases(8.3%), who responded poorly had angina pectoris for more than 3 years. It appeared that fenalcomine was similarly effective in those with or without associated hypertension or diabetes mellitus, or history of previous myocardial infarction. The comparison of the results of post-treatment laboratory tests with pre-treatment data revealed no significant changes. Furthermore, no untoward clinical reactions attributable to the drug was noted in all cases. These facts suggest that fenalcomine is a reliable and well tolerated antianginal agent which can be used singly or in combination with other agents.

For some cyanotic children with deficient pulmonary blood flow, the Blalock-Taussig shunt is a life-saving or temporizing form of palhation. Extensive experience has been gathered, and mortality and morbidity as well as the incidence of shunt stenosis and thrombosis have significantly decreased. However, even nowadays, the most frequently encountered complication excluding mortality in early postoperative course is occlusion of the shunt. We experienced a case of Blalock-Taussig shunt occlusion with thrombus. We diagnosed him as ventricular septal defect with pulmonary atresia by echocardiography and performed a right classic Blalock-Taussig shunt at 7th day of age, Oxygen saturation was gradually decreased since the 6th day postoperative. He was diagnosed as thrombosis of shunt at 12th day postoperative by cardiac angiography, and then was treated with a local low-dose urokinase infusion(1,000U/kg/hr) through Cobra catheter(Cook. In. Co.). After 21 hours, thrombolysis of shunts was comfirmed. We stopped the urokinase and medicated low-dose aspirin(5mg/kg/day). After the eighth day, shunt flow by echocardiography was patent.

PURPOSE: Neuron-specific enolase(NSE) has been established as a reliable marker of neuronal damage in various neurologic disorders. The aim of this study was to evaluate whether febrile seizure cause brain damage, based on the serum and cerebrospinal fluid (CSF) levels of NSE. METHODS: Twenty-one pateints were enrolled. The maximal seizure duration was 90 mins. Blood and CSF samples for the measurement of NSE were obtained immediately after the seizure. NSE was measured using an immunoradiometric assay(IRMA). RESULTS: The CSF NSE level of the febrile seizure group was 11.7+/-2.04 ng/mL and that of the control group was 11.3+/-5.7 ng/mL. The serum NSE level of the febrile seizure group was higher than the serum NSE level of the control group, but there was no significant correlation. The serum NSE level of the febrile seizure group was 19.0+/-7.5 ng/mL and that of the control group was 12.8+/-5.1 ng/mL. The serum NSE level of the febrile seizure group was significantly higher than the serum NSE level of the control group. The CSF/serum ratio of NSE in the febrile seizure group was 0.7+/-0.3 and that of the control group was 1.0+/-0.5. The CSF/serum ratio of NSE in the febrile seizure group was lower than the CSF/serum ratio of NSE in the control group and there was a significant correlation. There was no significant correlation between seizure duration, serum NSE, CSF NSE, and the ratio of the CSF to the serum level of NSE. CONCLUSION: Children with febrile seizure are at relatively low risk for neuronal damage following seizures.
